Opening a database with checkpointWorker: true fails at once with database is locked if another process holds the database at that moment. startCheckpointWorker runs while the client is being created, so its SELECT sqlite_version() is the writer's first statement. The PRAGMA busy_timeout = 5000 in Sqlite.ts only runs after the client exists, so that first statement has no busy timeout.

We hit this in CI on apps/server CLI tests, in runProjectMutation, with errcode: 261. The stack ended at the sqlite_version() line in startCheckpointWorker.

Change

Set the same 5 second busy timeout on the writer at the start of startCheckpointWorker, before its first statement. The checkpoint worker's own connections already use this value.

• With the fix: passes, along with the rest of nodeSqliteClient.test.ts (19 of 19).
• packages/shared typecheck, vp lint and vp fmt --check are clean.

I ran these on macOS only. I did not run the test on Windows or Linux locally.

On bdcdedf, the new PRAGMA busy_timeout = 5000 runs after the setAuthorizer check in startCheckpointWorker. On Node 22.x and 23.x, which the server's engines range still allows, setAuthorizer doesn't exist (it arrived in 24.10), so the function returns early and the writer's first query still fails with database is locked straight away.

I ran the PR's open sequence (constructor options from make(), then the same guard) on Node v22.23.4, with another process holding an exclusive lock for 300 ms:

same + constructor timeout: 5000: ok after 304ms

On Node 24.21 both succeed. Passing timeout: 5000 to the DatabaseSync constructor (available since 22.16) would cover every path, including checkpointWorker: false.

verified as-is at bdcdedf8790ce3a163676c4069f9c56588f7664c on a linux boat vm. this fresh run used node 24.22.1, repo playwright, and system google chrome.

• reproduced on github/main (955e2bfbb5) in a separate worktree. the real t3 project rename offline path failed with database is locked at SELECT sqlite_version() before the lock was released.
• the exact pr head waited and succeeded through the same path. a further run completed an actual project rename under contention. the holder released one second after strace observed an actual fcntl lock conflict, so slow startup could not bypass contention.
• a lock held beyond five seconds failed at the busy timeout as expected. node 22.23.2 also waited and succeeded through the real cli initialization path, with its checkpoint fallback warning. the node 22 concern did not reproduce on that application path.
• added a real git project through the web ui and completed real codex gpt-6.1-sol turns through both running dev servers. after restarting the pr server, chrome loaded the persisted conversation and a follow-up submitted through the actual composer returned sqlite follow-up verified. a live cli project rename also propagated to the web client during the recording.
• blacksmith: vp test run packages/shared/src/nodeSqliteClient.test.ts, 19/19 passed. existing pr checks: 30 successful, none failing or pending. no source changes, commits, or pushes. task dev servers stopped.
