Problem

PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.

Change

Keep watches active after provider failures reported as failed. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL FORBIDDEN responses now use a distinct forbidden reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.

The provider's failed category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.

Scope and approval

This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side forbidden reason does not change RPC contracts or add settings.

Verification

vp run check:changed passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.

Model: GPT 6.1 Sol. Harness: Codex.

- Retry failed reads so watches recover when the host returns
- Stop watching when the pull request is not found

Approvability

Verdict: Approved at 793be57

Macroscope's review found this PR approvable — PR watches now survive host outages while retaining termination for persistent access failures. Existing polling and rate-limit controls limit the impact of continued retries.

Walkthrough

The GitHub provider now classifies API response errors by HTTP status and GraphQL error types. The watch reactor handles read failures according to provider reason. Tests cover provider classification, watch failure handling, and recovery.

PullRequestProviderError now accepts forbidden. GitHub API response errors can carry GraphQL error types. The provider maps HTTP 403 or qualifying FORBIDDEN errors to forbidden; other response errors map to failed. Tests cover classification and extraction of missing GraphQL error types.
Watch read failure handling
apps/server/src/orchestration-v2/PullRequestWatchReactor.ts, apps/server/src/orchestration-v2/runtimeLayer.test.ts
The reactor extracts provider failure reasons and combines detail and activity read results, with detail failures taking precedence. Rate-limited reads do not count as failures. Reads with reason failed clear the accumulated count and propagate; other failures use the existing give-up handling. Tests cover not-found, failed, and forbidden errors, along with recovery.

Nitpick Cover reset from an existing failure count.

3176-3304: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick win

Cover reset from an existing failure count.

The "failed" case starts with an empty counter. It does not create a generic read failure before the provider "failed" read. Therefore, it does not prove that an existing nonzero counter is cleared.

Add one generic failure, one provider "failed" failure, and seven more generic failures. Assert that the watch remains active. Without the reset, the later failures reach READ_FAILURE_LIMIT.

Extend the “failed” case in the repeated host read failures test to verify that
a provider “failed” read resets an existing generic read-failure count: make the

Two questions on the new failed branch, separate from CodeRabbit's activity/detail ordering note:

  1. failed is also the catch-all. In gitHubProviderFailure (GitHubPullRequestProvider.ts#L127-L128), every error that isn't missing-tool, unauthenticated, rate-limited or not-found becomes failed. So a persistent 5xx or 422 for one PR, or a response that no longer decodes, now also resets the counter on every sweep (PullRequestWatchReactor.ts#L492-L495). Those watches would never reach READ_FAILURE_LIMIT, and the agent would never get the "stopped watching, could not read it" note it gets today. Is that intended? A time bound (for example, give up after some hours of consecutive failed reads) might keep the outage recovery without making non-transient failures permanent.

  2. Cost while the host is failing. The failure path deletes lastReads (L488), so planRead returns { detail: true, activity: true } on the next sweep (L143-L144), and the per-group catchCause logs pull request watch check failed at Warning with the cause (L632-L635). Before this change that stopped after 8 sweeps. Now it's a full detail and activity read plus a Warning every 2 minutes per watched PR for as long as the failure lasts. Given the SWEEP_MINUTES note about the shared host quota, would a backoff for consecutive failed reads, and logging only the first one after a success, be worth adding here?

@aravhawk Retrying unclassified failed errors is intentional here. That category does include persistent host or decode errors; it does not give the watcher enough information to distinguish them from an outage. A fixed deadline would reproduce the lost-watch bug for outages longer than that deadline. Known permanent detail failures still reach the eight-read stop notification, and the latest commit prevents an activity failure from masking them.

The cost concern is valid: an unclassified failure continues detail/activity reads and warning logs at the existing two-minute cadence. Reads remain grouped per project/PR, with four groups read concurrently, and rate-limited reads still pause. Backoff and log deduplication could reduce that cost, but add recovery latency and retry policy beyond this patch. I am keeping this change focused on preserving explicitly requested watches through outages. The PR description now states this tradeoff.

* fix(web): play completion sounds without delay (pingdotgg#29146)

Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.

Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.

This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.

Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)

* fix(server): keep PR watches active after host failures (pingdotgg#29450)

## Problem

PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.

## Change

Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.

The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.

## Scope and approval

This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.

## Verification

`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.
