fix(relay): T3 Connect repairs tunnel ingress that drifted from a matching origin - #27961
On every boot registration the relay's reconcileOrigin returned ready as soon as the allocation's recorded origin matched the host's, without reading the tunnel's live Cloudflare ingress. If the ingress had drifted to another port (in the report: ingress http://127.0.0.1:4711, allocation and server 127.0.0.1:3773), the tunnel stayed healthy but proxied to a dead port, clients saw 502 / origin_unreachable, and restarting T3 Code changed nothing because the restart takes the same matching-origin path. Reproduced on main as a provider test: provision at 3773, move the tunnel's ingress to 4711 behind the relay's back, call reconcileOrigin with 3773, and it returns ready with the ingress still on 4711.
Change
reconcileOrigin now reads the tunnel's configuration when the recorded origin matches. If the hostname's ingress rule already serves the origin, it returns ready with no write. Otherwise it falls through to the existing claimed putConfiguration + markReady path that already repairs a changed origin, so every boot registration converges the ingress.
The tunnel client gains a getConfiguration operation wired to the Cloudflare binding. Cloudflare reports a tunnel with no remote configuration as TunnelConfigurationNotFound (code 1055, not a 404), so the binding reads that as empty ingress and the rewrite runs. A missing tunnel still returns recovery_required.
The cost is one gated Cloudflare read per registration whose origin matches. The recovery path (reuseReadyTunnel) is unchanged.
Focused tests in infra/relay/src/environments/ManagedEndpointProvider.test.ts:
restores Cloudflare ingress that drifted from the recorded origin: a registration whose ingress already serves the origin only reads (get, getConfiguration), then the ingress drifts to 4711 and the next registration rewrites it to 3773.
reads a tunnel without remote configuration as having no ingress: Cloudflare's code-1055 response through the real binding.

✓ reads a tunnel without remote configuration as having no ingress
✓ restores Cloudflare ingress that drifted from the recorded origin
Tests 34 passed (34) (ManagedEndpointProvider.test.ts)
vp run check:changed: format, lint, knip, typecheck (infra/relay) and tests pass.
Server-only (relay) change with no rendered surface, so there are no captures. Not checked: a run against the live Cloudflare API. Whether Cloudflare echoes the service string back exactly as the relay writes it is assumed, not observed; if it normalised it, a matching registration would rewrite on each boot rather than fail.
Made with Claude Opus 5.5 (Claude Code, run from T3 Code); reviewed by GPT-6.1 Sol (Codex) and Claude Opus 5.5.
These tests reproduce #27952: reconcileOrigin returns ready when the
allocation origin matches, without checking that the tunnel's live
Cloudflare ingress still points at that origin.

reconcileOrigin returned ready as soon as the allocation's recorded origin
matched the host's, without reading the tunnel's live Cloudflare ingress.
An ingress that drifted to another port stayed broken across restarts.
On a matching origin the relay now reads the tunnel configuration and
returns ready only when the hostname's ingress rule already serves the
origin; otherwise it takes the existing claimed rewrite path. A missing
configuration asks for recovery, like a missing tunnel.

Cloudflare reports a missing tunnel configuration as TunnelConfigurationNotFound
(code 1055) rather than a 404, so the config read failed registration instead
of falling through to the rewrite. Read it as empty ingress at the binding.

Macroscope's review found this PR approvable — Tunnel reconciliation now checks live ingress and repairs drift even when the recorded origin already matches. Repairs remain scoped to the validated managed allocation and use the existing guarded configuration-update path.

Walkthrough
The tunnel client now reads Cloudflare ingress configuration. When the recorded origin matches, reconciliation checks live ingress and proceeds to repair if the matching rule does not serve the requested origin or has a path restriction.
The tunnel client adds a configuration-read operation. The Cloudflare binding returns ingress rules, treats missing configuration as empty ingress, and maps other failures to client errors. Tests add configuration-aware mocks and cover the missing-configuration response.
Ingress verification and repair infra/relay/src/environments/ManagedEndpointProvider.ts, infra/relay/src/environments/ManagedEndpointProvider.test.ts, infra/relay/src/environments/ManagedEndpointReaper.test.ts
When the recorded origin matches, reconciliation checks the first matching exact-host, wildcard, or catch-all rule. It returns ready only if the service matches and the rule has no path; otherwise, it continues to configuration repair. Tests cover service drift, wildcard ordering, and path removal. The reaper test client adds a getConfiguration stub.

Explanation
The title names the main change: reconcileOrigin now repairs tunnel ingress that drifted from a matching origin. It also names the relay scope. It follows the conventional commit form used in the repository template, and the title is 81 characters long, which is longer than ideal but still readable.
Full details: Description check
Explanation
The description covers the required template sections. Problem explains the drift bug, the failing behavior, and a reproduction. Change explains the fix, the new getConfiguration operation, and the code-1055 handling. Scope and approval links issue #27952 and states the triage path. Verification lists the focused tests, a red/green transcript, the check command results, and what was not checked, including the live Cloudflare API run. The description does not show an explicit maintainer approval comment, so the Scope and approval section relies on a triaged issue reference. That is a gap the reviewer should confirm, but the section is otherwise complete.
Full details: Linked Issues check
Explanation
Issue #27952 requires live ingress verification when recorded and host-reported origins match, plus repair when ingress differs. reconcileOrigin now calls getConfiguration, selects the managed hostname, wildcard, or catch-all rule, and returns ready only when the service matches and the rule has no path restriction. Otherwise it uses the existing guarded configuration update and markReady path. The Cloudflare binding treats code 1055 as empty ingress, so reconciliation repairs the configuration; missing tunnels retain recovery_required. Focused tests cover matching reads, drift repair, path-restricted rules, and code-1055 handling.
Full details: Out of Scope Changes check
Explanation
The changes remain within issue #27952. The getConfiguration client operation and code-1055 mapping support live ingress verification. The path field and path-restricted-rule test prevent a false ready result. Test-fixture updates support the new client method and behavior. No unrelated product surface or administrative changes are present.

Check the effective ingress rule before returning ready.
The managed hostname is a subdomain of t3code.test, so an earlier *.t3code.test rule can match it. reconcileOrigin currently selects a later exact-hostname rule and can report ready even when Cloudflare routes the request through the earlier wildcard rule.
Select the first matching rule, including wildcard hostnames, before skipping repair.

* fix(web): play completion sounds without delay (pingdotgg#29146)
Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.
Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.
This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.
Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)
* fix(server): keep PR watches active after host failures (pingdotgg#29450)
## Problem
PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.
## Change
Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.
The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.
## Scope and approval
This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.
## Verification
`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.

Problem
On every boot registration the relay's
reconcileOriginreturnedreadyas soon as the allocation's recorded origin matched the host's, without reading the tunnel's live Cloudflare ingress. If the ingress had drifted to another port (in the report: ingresshttp://127.0.0.1:4711, allocation and server127.0.0.1:3773), the tunnel stayed healthy but proxied to a dead port, clients saw502/origin_unreachable, and restarting T3 Code changed nothing because the restart takes the same matching-origin path. Reproduced on main as a provider test: provision at3773, move the tunnel's ingress to4711behind the relay's back, callreconcileOriginwith3773, and it returnsreadywith the ingress still on4711.Change
reconcileOriginnow reads the tunnel's configuration when the recorded origin matches. If the hostname's ingress rule already serves the origin, it returnsreadywith no write. Otherwise it falls through to the existing claimedputConfiguration+markReadypath that already repairs a changed origin, so every boot registration converges the ingress.getConfigurationoperation wired to the Cloudflare binding. Cloudflare reports a tunnel with no remote configuration asTunnelConfigurationNotFound(code 1055, not a 404), so the binding reads that as empty ingress and the rewrite runs. A missing tunnel still returnsrecovery_required.reuseReadyTunnel) is unchanged.Scope and approval

Verification
Focused tests in
infra/relay/src/environments/ManagedEndpointProvider.test.ts:restores Cloudflare ingress that drifted from the recorded origin: a registration whose ingress already serves the origin only reads (get, getConfiguration), then the ingress drifts to4711and the next registration rewrites it to3773.reads a tunnel without remote configuration as having no ingress: Cloudflare's code-1055 response through the real binding.Red/green transcript (provider source at main
80a2351fcbwith this branch's tests, then this branch atdd118a035d):vp run check:changed: format, lint, knip, typecheck (infra/relay) and tests pass.Server-only (relay) change with no rendered surface, so there are no captures. Not checked: a run against the live Cloudflare API. Whether Cloudflare echoes the
servicestring back exactly as the relay writes it is assumed, not observed; if it normalised it, a matching registration would rewrite on each boot rather than fail.Made with Claude Opus 5.5 (Claude Code, run from T3 Code); reviewed by GPT-6.1 Sol (Codex) and Claude Opus 5.5.
