fix(server): delegated children keep replies after their parent settles - #23456
Settling a completed Codex parent unsubscribed an independently attached delegated child. Sending that child another message finished normally in Codex, but T3 never displayed the reply or completed its run.
Change
Check each projected native thread under its owning app thread’s attachment lock before unloading it. Attached children retain their subscriptions and loaded state; unattached children still unload. Regression tests cover both cases.
Red: the attached-child regression fails without the fix: expected no native unloads, received native-child-thread.
Green: all 43 ProviderSessionManager tests pass, including unattached-child cleanup and unload/reattach serialization. Repeated after merging main.
Targeted lint and formatting pass; server typecheck passes.
Independent GPT-6.1-Sol and Claude Opus 5.5 reviews returned MERGE on the final head.
Real web dev app on Linux x86_64, Chromium, 1280×800, same configured Codex GPT-6.1-Sol instance: child READY, parent PARENT_DONE, settle parent, open that same child, send Reply exactly PONG. Before: native completion with T3 stuck running. After: PONG appears and T3 persists the child run as completed. No server restart between settlement and follow-up.
Evidence matrix: empty; server-only subscription lifecycle change with no changed render paths. Proof is the regression test and real runtime transcripts. Web exercises the shared server path used by desktop, mobile, local, remote/relay and tunnel connections. OpenCode shares the manager branch; provider-specific OpenCode runtime was not exercised.
The manager source on the before build is byte-identical to the PR merge-base 96bae5465b.
Models: GPT-6.1-Sol and Claude Opus 5.5. Harnesses: Codex and Claude Code via T3 Code.
Takeover
Credit: @juliusmarminge’s implementation and existing regression/runtime proof are preserved.
No code changes; evidence and verification added below. Existing sufficient captures were retained.
Evidence
The earlier empty-matrix statement is superseded by this explicit server fixture matrix. No render paths changed; runtime transcripts apply, with no screenshot/theme/viewport cross-product.
Final head: dca0af89a8; base: 96bae5465b.
Environment: Linux x86_64, real application server and Codex. U/C1/C2 are explicitly instrumented service-harness runtime captures; P observes modules without changing source. No visible changes introduced by this evidence-only pass.
Verification log
Independent pass 1 matrix/audit identified U/C1/C2/P gaps; existing A was accepted and retained.
Independent pass 2 audit: SUFFICIENT. Verified event ordering, supplied verifier, 971 module hashes and transformed manager digest. No fresh real-provider replay in pass 2; it independently inspected delivered artifacts.
Focused manager suite rerun on exact final head: 43/43 pass, one existing test file; no tests changed.
Cafe reports 34 successful, 0 pending, 0 failing checks on final head. Conflict readiness remains pending against a stale base snapshot; no merge-readiness claim.
Limits: historical A process provenance cannot be reconstructed; separate exact-head P supplements it. C2 proves serialization during the enclosing unload effect after native acknowledgment, not a still-pending native wire response or naturally occurring contention.
Unverified runtimes: OpenCode, desktop/mobile clients, relay and tunnel. These add no changed server branch or render path; no runtime capture claimed for them.
Evidence-only scope: no code changes, commits, branch synchronization, push, claim comment or merge. Reproduction scripts are public scratch artifacts, not committed fixtures.
Model/harness: GPT-6.1-Sol through Codex in T3 Code; uploads via ghdrop. Original GPT-6.1-Sol/Claude Opus reviews above preserved.

Regression tests for #20348. Detaching a parent thread unloads every native
provider thread its projection reaches, including a Codex native child that is
independently attached. The manager still caches the child as loaded, so the
child's next turn skips resume and its reply is never heard. A second test
pins the cleanup that must survive the fix: an unattached child mirror is
still unloaded with its parent.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Macroscope's review found this PR approvable — Detach cleanup now respects each delegated child's attachment state, allowing attached children to keep receiving replies after their parent settles. Existing runtime checks, unload timeouts, and cleanup scheduling remain in place.

Walkthrough
When detaching a multi-thread session, the manager checks each native provider thread’s owner under that owner’s lock before unloading. Tests cover a resumed child that stays loaded and an unattached child that is unloaded.

Summary
Per-owner detach and regression coverage apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts
Detach locks each native provider thread’s owner, falling back to the detaching thread when no owner ID is set. It skips unloading if the runtime changed or the owner is attached. Tests cover resumed and unresumed child threads.
pingdotgg/t3code#21354 — This issue also concerns cleanup of Codex-native child threads. This change unloads unattached child threads during parent detach, but does not establish that the idle-unload cleanup gap is resolved.

The fixture’s resumeThread only increments resumeCount and returns the thread. The test never emits or reads a provider event. A broken child subscription could therefore pass the unload, cache, and liveness assertions. Extend the fixture to emit a child reply after the post-detach turn, then assert that exact reply.

* fix(web): play completion sounds without delay (pingdotgg#29146)
Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.
Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.
This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.
Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)
* fix(server): keep PR watches active after host failures (pingdotgg#29450)
## Problem
PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.
## Change
Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.
The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.
## Scope and approval
This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.
## Verification
`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.

## Summary
Merge upstream 91f66f8 → 61c3794 (149 commits) with a merge
commit. All eight conflicts resolved against fork policy; lockfile
re-derived. Both-parent resolution check and inventory check pass.
## Verification — draft
All nine fast checks pass: duplicate adds, tripwires, both-parent
resolutions, unsupported-method derivation, lockfile, format, lint,
typecheck, production web build.
Initial full verification failed tests after package and single-file
retries. OpenCodeServerLedger.test.ts has five process-group cleanup
failures in isolation; t3 server failures include the registered /tmp
sticky-bit and global Git insteadOf sandbox gaps. No assertions
suppressed. Final full sequential verification is running; results will
follow. Pre-commit knip still has the known 27 unused exports; commit
used --no-verify.
## Usable as-is
- Browser-offline primary-environment connection fix and connection
diagnosis during sign-in (client-runtime).
- Web terminal controls no longer obscure the first line; timezone in
usage retry notices; model picker height, sidebar accessibility,
completion sounds and expanded-work scroll preservation.
- Checkout grouping overrides in ProjectSettingsPanel remain client
settings; project write/delete gates retained.
- Chat images reload after workspace mutations; PDF reload reuses its
worker. Formatting, review-comment blank lines, context-chip emoji and
attachment size improvements.
## Unsupported in Moatless / needs implementation
- orchestration.getNativeChildWork (packages/contracts/src/rpc.ts):
added UnsupportedMethodError from the derivation; deferred
provider-native child work cannot be fetched.
- Thread tags / recording-time hiding (pingdotgg#29053): thread shell tags,
thread.tags-updated and hiddenThreadTags/hideTaggedThreads settings need
backend data. Client filters alone do not implement tagging.
- Model/effort-change annotations (pingdotgg#28995): timeline readers accept
optional modelSwitch; Moatless must emit it.
- Kiro ACP (pingdotgg#14693), Antigravity 1.4.5 (pingdotgg#28629) and secret-provider
source-control tokens (pingdotgg#27756) need Moatless runtime/integration work;
upstream Node implementations are not deployed by this fork.
## Backend behavior to consider reproducing in Moatless
- Imported Claude first-turn resume and nested-subagent completion
persistence (pingdotgg#23219, pingdotgg#23561); preserve delegated replies after parent
settlement (pingdotgg#23456).
- Prevent settled-parent child usage-limit auto-resume (pingdotgg#27951), and
Stop a run whose provider already finished (pingdotgg#28520).
- Preserve user locale for Git hooks (pingdotgg#27778); keep slow forge CLI calls
off checkpoint/start paths (pingdotgg#28431); remove branches left by failed
worktree creation (pingdotgg#28373).
- Upstream provider session/compaction/recovery and shell-read
performance fixes merit parity review, not client transplantation.
Concrete anchors and closure checks are in docs/fork/gaps.md.
## Merge accounting
Initial landed and upstream-range counts are both 790 files; the
intentional swap is inventory.json versus the deliberately deleted
pair.test.ts. Initial fork delta: 772 files. Verification/documentation
follow-ups are recorded in docs/fork/upstream-merge-log.md.
Owned-surface sweep found only Kiro transcript and migration 077 runtime
fixtures/repair; no new workflows.
Do not merge while draft: isolated test failures remain unresolved.

Problem
Settling a completed Codex parent unsubscribed an independently attached delegated child. Sending that child another message finished normally in Codex, but T3 never displayed the reply or completed its run.
Change
Check each projected native thread under its owning app thread’s attachment lock before unloading it. Attached children retain their subscriptions and loaded state; unattached children still unload. Regression tests cover both cases.
Scope and approval
Fixes #20348
GitCafe: https://git.cafe/pingdotgg/t3code/issues/20348
Triaged bug via the automated issue flow. Reproduces the failure reported and diagnosed by @jfet97 in #20348; related contributor fix #20361 remains separate.
Verification
native-child-thread.ProviderSessionManagertests pass, including unattached-child cleanup and unload/reattach serialization. Repeated after merging main.Reply exactly PONG. Before: native completion with T3 stuck running. After: PONG appears and T3 persists the child run as completed. No server restart between settlement and follow-up.Evidence matrix: empty; server-only subscription lifecycle change with no changed render paths.
Delivery index · Full matrix · Runtime transcripts · Reproduction scripts · Evidence archive.
Environment: Linux x86_64, real application server and Codex. U/C1/C2 are explicitly instrumented service-harness runtime captures; P observes modules without changing source. No visible changes introduced by this evidence-only pass.
