The server's state grew without bound in two places. The orchestration effect outbox kept every finished row forever (10,258 rows on a real install). The pull request read cache left one file per PR read on disk long after the entry expired (739 files since Sep 15), because the persisted cache only drops an expired entry when the same key is read again.

Fix

  • Effect outbox. A new EffectOutboxV2.pruneSettled deletes succeeded and cancelled rows completed more than 7 days ago. It deletes 500 rows per statement, so a backlog never holds the write lock for long. failed rows stay, because storage cleanup keeps a deleted thread's worktree while any of its effects failed. Every other reader (claiming, recovery, the orchestrator's restart-continuation check, storage cleanup) only asks whether a row is pending, running or failed. The only use of an old settled row is deterministic-id dedupe, and that spans one command retry or one restart. The window is justified in a comment on SETTLED_EFFECT_RETENTION. A layer-owned fiber (pruneWorkerLive) runs the prune after server activation and then hourly via Schedule.spaced. It is wired into OrchestrationV2ProductionLayerLive only.
  • PR read cache. The cache layer forks the same kind of hourly sweep, but only when the filesystem store is in use. The sweep deletes pr-v2* entry files whose mtime is more than a day old. Entries expire one minute after they are written, so the day is only slack for clock changes, and pruning a live entry would cost one refetch. The revisions scope file and unrelated files are left alone, and one failing file does not stop the sweep.

Not changed: ProjectionMaintenance.compactEventStore stays unwired. Running it on a live server is unsafe for three reasons:

  • Idempotent command retries replay their events by command id, so they would get an incomplete list.
  • Client catch-up from a cursor would see messages and nodes reorder, and could miss a superseded thread.deleted.
  • Its synchronous pages stall the event loop, and without a vacuum no disk space comes back.

A safe version needs an age cutoff, must keep thread.deleted, and needs a vacuum, so it is a separate design.

Verification

From apps/server:

  • vp test run src/orchestration-v2/FoundationPersistence.test.ts: 42 passed
  • vp test run src/pullRequest/PullRequestReadCache.test.ts: 11 passed
  • vp test run src/pullRequest/PullRequestService.test.ts: 181 passed
  • vp test run src/orchestration-v2/runtimeLayer.test.ts: 67 passed
  • vp exec tsc --noEmit -p .: exit 0, 0 errors, 0 warnings

From the repo root:

  • vp lint on the 5 changed files: exit 0
  • vp exec knip --workspace apps/server --exports --preprocessor ./scripts/knip-schemas.ts --no-config-hints: exit 0

There are four new tests. They drive time with TestClock and wait for observed sweep runs, not sleeps:

  • Outbox prune: 1,203 expired settled rows go away across three batches. Recent settled, old failed, pending and running rows stay.
  • Outbox worker: the first run keeps a settled row that is still inside retention, and the run one hour later removes it. The pending row stays.
  • PR cache prune: an entry file past the max age goes. A file exactly at the max age, the revisions file, and an unrelated file stay.
  • PR cache layer: the production layer keeps a fresh entry at its first two sweeps and removes it at the sweep where it passes the max age.

All four fail when the delete or remove is disabled. The hourly PR cache test then hangs waiting for the removal.

Event-store compaction (`compactEventStore`) stays unwired: command
retries replay their events, clients catch up from a cursor, and it
needs a vacuum to return space, so it needs its own design.
