Problem

makeDrainableWorker ran every item inside its worker loop. If the processor failed or died on one item, the error escaped the loop and the worker fiber exited. Later items were never processed, and every later drain hung forever because the outstanding count for those items was never decremented.

A probe test against main shows it. It enqueued a dying item and then a normal item. The worker fiber exited with the boom defect, processed stayed [], and drain never resolved.

Fix

packages/shared/src/DrainableWorker.ts now runs each item as Effect.suspend(() => process(a)), then applies Effect.catchCause to log the cause with Effect.logError("DrainableWorker item failed", cause). Effect.ensuring still decrements the outstanding count. A failed or defective item is logged and skipped, and the loop moves on to the next one. An item that only interrupted itself is skipped without a log, because every caller treats an interrupt-only cause as a cancellation. The suspend also catches a process that throws while building its effect.

Interrupts are not swallowed. In Effect 4.0.1, interrupting a fiber skips catchCause handlers on the way out (exitFailCause unwinds past them while the fiber is interruptible, in internal/core.ts). So when the worker's scope closes, the fiber still stops. A probe confirmed this: an interrupted fiber ran its ensuring but not its catchCause handler, and its exit had interrupts. A new test checks that closing the scope stops the worker, logs nothing, and never runs the queued item.

Closing the scope used to leave drain waiting forever too: the queue's shutdown dropped queued items without uncounting them, and enqueue counted items the shut-down queue refused. The shutdown finalizer now uncounts the dropped items in the same transaction as the shutdown (an item still running uncounts itself, so this also holds when a parallel scope runs the finalizers concurrently), and enqueue only counts items the queue accepted. drain also returns void now, instead of leaking the count through Effect.tap.

Verification

  • vp test run src/DrainableWorker.test.ts in packages/shared: 3 passed (3). This includes two new tests:
    • logs a failed or defective item and keeps processing later items: a typed failure, a defect, a self-interrupting item, then a normal item. The normal item is processed, drain resolves, and only the failure and the defect are logged at Error level. The test races drain against the worker fiber's exit, so the old code fails it (expected [] to deeply equal [ 'ok' ]) instead of hanging.
    • stops quietly when its scope closes during an item: closing the scope while an item runs leaves the worker fiber with an interrupted exit, logs nothing, never runs the queued item, and a drain after an enqueue on the closed worker completes at once. It fails on main (drain never completes).
  • vp test run src/relay/AgentAwarenessRelay.test.ts src/storageCleanup.test.ts src/orchestration-v2/ThreadPullRequestService.test.ts src/orchestration-v2/ThreadSettlementService.test.ts src/orchestration-v2/PullRequestSyncReactor.test.ts in apps/server: 5 files, 77 passed (77).
  • vp exec tsc --noEmit -p . in packages/shared: exit 0. The only diagnostic is an existing TS377114 suggestion in src/symlink.ts, which this PR does not touch.
  • vp lint packages/shared/src/DrainableWorker.ts packages/shared/src/DrainableWorker.test.ts: exit 0, no warnings.

worker loop and its fiber exited. Later items were never processed, and
anything waiting on `drain` hung forever because the outstanding count
of the queued items was never decremented. AgentAwarenessRelay's publish
worker and ThreadSettlementService's sweep worker pass `process`
functions that can still die, so one bad item stopped them for the rest
of the server's life.

The worker now catches each item's cause, logs it with
`Effect.logError`, and keeps going; the outstanding count is still
decremented in `Effect.ensuring`. Closing the worker's scope still stops
it: in Effect 4.0.1 an interrupt of the worker fiber skips `catchCause`
handlers, so only the item's own failure, defect, or self-interruption
is caught.

caller treats an interrupt-only cause as a cancellation. It is now skipped
quietly.

Closing the scope dropped queued items without uncounting them, and enqueue
counted items the shut-down queue refused, so a later drain waited forever.
The shutdown finalizer now clears the count and enqueue only counts accepted
items. drain also returns void instead of leaking the count.

Zeroing the count on shutdown assumed the worker fiber had already exited.
Under a parallel scope the in-flight item's own decrement could then run
afterwards and leave the count at -1. Only the dropped items are uncounted
now; the running item still uncounts itself.
