Problem

Four V2 lifecycle paths could leave a run stuck or drop live events:

  • A rollback retry could restore files under a new turn. The outbox only held a thread's later effects behind a running effect. If a rollback failed once, a turn started during its retry backoff ran first, then the retry restored checkpoint files while that turn worked.
  • A failed state read looked like "the run moved on". The turn start guards turned any projection read error into false. One SQL error made the start skip the provider turn or the terminal write, the run stayed active, and the worker marked the effect done.
  • A failed release left a ready session and live approvals. Release removes the live session first, then writes the stopped status and resolves pending requests under Effect.ignore. If a write failed, nothing logged it, and a later release found nothing to clean up.
  • Live events could reach clients out of order. Writers publish after their transaction with no ordering. If writer A was descheduled between commit and publish, writer B's later event went out first, and clients drop anything at or below their cursor. A new test reproduced this: subscribers got sequences [2, 1].

Fix

  • Outbox: a thread's effects run one at a time in enqueue (rowid) order. An earlier effect that waits out a backoff still blocks later ones. Title generation keeps its own lane.
  • Guards: read failures propagate. A read failure right before the provider call goes through the existing start-failure path. That path now records the failed terminal with the transactional writeIfRunCurrent guard instead of a second read that can fail the same way.
  • Release: the status write and the request cleanup each run even if the other fails, and failures are logged. Failed cleanup retries in the background (capped backoff, defects included) until it lands. Each retry holds the session's open lock. If a replacement session with the same id is live, the retry skips the status write and settles only the requests created before the release. Each released session keeps its own retry.
  • Event sink: each writer takes a one-permit publish lane as the last step inside its transaction and releases it after publishing. The lane is taken inside the transaction because LegacyV1ThreadImporter writes inside an outer transaction, so taking it outside could deadlock.

Each fix has a focused test that fails without it.

Report items (tech debt hunt, 2026-10-02)

  • S3: a failed state read leaves a run active
  • S4: a rollback retry can restore files under a new turn
  • S5: a failed provider release leaves a ready session and live approvals
  • S15: live events can reach clients out of order (reproduced by a test first)
  • S2: dropped. Open #14424 already fails the run on the last start or restart attempt, including interrupt and detach failures. Its failStartingRun guard checks the run's current attempt, not the attempt the restart effect was created for.

Notes

  • #14424 also edits isCurrentAttemptInStatus. This PR makes that guard propagate errors at the source, so its separate read helper is no longer needed. Whichever merges second rebases.
  • Not covered: if the guarded terminal write itself fails, the run still stays active, because start is a no-op once a run is running. That is the same class #14856 handles for the event consumer.

  while a failed rollback is about to restore files.
- Turn start guards: a failed state read is an error, not "superseded".
  A read failure right before the provider call fails the run through the
  guarded terminal write.
- Provider session release: the stopped-session write and the runtime
  request cleanup run independently, failures are logged, and failed
  cleanup retries until it lands. Retries hold the session's open lock,
  leave a same-id replacement and its newer requests alone, and survive
  defects.
- Event sink: writers publish in commit order, so clients do not drop an
  event that a later commit overtook.

- The ingestion failure fallback records the failed terminal with the
  transactional run guard, not the ownership read that may have failed.
- Release reads its cutoff clock right after the entry leaves the live
  map, so requests the old runtime created just before are settled.

The event pump persists runtime requests under the session's request
permit. Release now removes the live entry and reads its cleanup cutoff
under the same permit, so a request the pump is persisting lands before
the cutoff and is settled.
