fix(server): track resumed subagent follow-ups as separate tasks

Problem

Messages sent to a completed subagent thread could overwrite or reuse the original delegated task, leaving follow-up results without their own status and completion delivery. Follow-up results could also be confused with results from later runs in the same child thread.

Change

Create a fresh task for each requested follow-up in the existing child conversation, and associate restart continuations with their original task. Track each follow-up's status, result transfer, acknowledgment, and completion notification independently while preserving the original task result. Match MCP reads and status responses to the run belonging to each task.

Scope and approval

This is a focused fix to delegated-task lifecycle behavior: follow-up messages in a subagent thread need the same independent result and delivery tracking as new delegated work, without creating a new child conversation.

Verification

Added focused orchestration and MCP coverage for completed, failed, interrupted, restarted, and stopped follow-ups; independent results and notifications; recovery; and result acknowledgment on thread read. Verification results were not included in the supplied change details.
