refactor(usage): Antigravity usage is a reader on its driver

The last provider branch in UsageService was Antigravity: its protobuf/SQLite reader, a module-level database cache, and profile-root resolution.

• The reader becomes a scan reader on AntigravityDriver. Its cache and root resolution live in an AntigravityUsage service, so there is no module state.
• Profile roots come from the configured instances the aggregator passes in (ProviderUsageInstance.configured).
• Scan readers may fail with UsageReadError, which keeps the previous profile-resolution failure.

UsageService no longer names any provider.

One small behavior change: concurrent identical usage requests now share a scan only when the whole settings snapshot matches, not just the three usage fields. This is needed because readers now read settings of their own; in practice it only reduces sharing if unrelated settings change mid-request.
