fix(server): title regeneration works after a thread's worktree is cleaned up - #29606
Storage cleanup removes a settled thread's worktree but leaves worktreePath on the thread. Title generation then uses that missing directory as the provider CLI's working directory. Node cannot spawn a process in a missing directory and fails with spawn codex ENOENT. T3 Code reports that as "Codex CLI (codex) is required but not available on PATH."
As a result, Regenerate title does nothing on any thread whose worktree has been cleaned up while text generation uses Codex, which is the default. The title stays as it was, and the server logs only Thread title generation failed. Claude is not affected because it already generates titles in a temporary directory.
To reproduce: start a worktree thread, let worktree cleanup remove it (or delete the folder yourself), then choose Regenerate title.
Change
ThreadTitleRegenerationService now checks that the worktree exists before using it. If it is missing, it uses the project root. ThreadSettlementService and ThreadPullRequestService already handle a missing worktreePath the same way.
Scope and approval
This is a very small, focused fix for an obvious bug, so it has no prior issue. A thread whose worktree was removed by cleanup should still be able to regenerate its title, and the fix uses a pattern that two sibling services already have. The fix itself is 7 lines in one service. The two test harnesses that build this layer now also provide NodeServices.layer for FileSystem.
Overlap search: the closest open PR is #26979, which falls back to another provider when the selected provider's CLI is missing. That fixes a different problem and does not touch the cwd line. #29082 and #26758 handle deleted worktrees in git status and checkout, not title generation.
Verification
Red on main: the existing case lands the regenerated title from the conversation digest now gives the thread a nonexistent worktree path and failed with AssertionError: expected '/repo-worktrees/removed-by-cleanup' to equal '/repo'.
Green: vp test run src/orchestration-v2/ThreadTitleRegenerationService.test.ts src/orchestration-v2/ThreadLaunchService.test.ts passed 52/52 after rebasing on current main.
vp run check:changed passed: format, lint, knip, graphql, typecheck (3 packages), and tests (3 files, 53 passed).
Macroscope's review found this PR approvable — Title generation now falls back to the project workspace when a thread's worktree no longer exists. Existing worktrees and generation settings are unchanged, keeping the repair narrowly scoped.

Walkthrough
Title generation uses a thread’s worktree as its working directory only when that path exists. If the path is null, missing, or its existence check fails, generation uses the project workspace root.

The service checks whether a non-null worktree path exists before using it for title generation. Tests provide NodeServices.layer and the Effect filesystem. The title-landing test covers an existing temporary worktree and a removed path, and verifies the selected working directory and generated title.
Title regeneration falls back to the project root after cleanup. The test may fail on a host where its fixed fixture path exists; otherwise the change is mergeable.

The title names the main change: title regeneration now works after a thread's worktree is cleaned up. The raw summary confirms that ThreadTitleRegenerationService falls back to the project workspace root when the worktree path is missing, and the tests cover the removed-worktree case. The title follows conventional commit style and is specific enough for a teammate scanning history. It is 77 characters, which is long but acceptable.

Explanation
The description follows the repository template. It has Problem, Change, Scope and approval, and Verification sections. The Problem section gives the cause, the error text, and reproduction steps. The Change section explains the fix and the reason it matches sibling services. The Scope section explains why no prior issue exists and lists the overlapping PRs. The Verification section gives a red-then-green test result, the commands run, the spawn behaviour check, and an explicit statement of what was not checked. The description does not name a conventional commit title beyond the PR title, and it does not include the agent model line in the template's required form, but the description still meets the requirements for a PASS.

The current test only verifies the missing-worktree fallback. Add a focused case where FileSystem.exists returns true and assert that generateThreadTitle receives the worktree path. Without this case, an implementation that always selects project.value.workspaceRoot would still pass the new regression test.
As per path instructions, backend behavior changes should ship with focused tests at the lowest failing layer.

Verified unchanged head f3bb0caccf8f60765f78486c412eaa1631235b86 in Linux Chrome, driven with the repository Playwright dependency and /usr/bin/google-chrome. Installed with vp i and ran one background vp run dev server at a time.
Created a real git project through the app, selected Codex gpt-6.1-sol, and completed real provider turns. No mocks, fixtures, replay provider, or database edits.
On github/main (71646e031a), created a worktree thread and completed a basil-watering conversation. Regeneration worked with the worktree present. Removed that worktree with git worktree remove, renamed the thread to TITLE REGENERATION CHECK, then clicked Regenerate title. The title stayed unchanged and the server logged the missing-directory failure below.
Stopped main and started the exact PR head with the same isolated dev state. The same conversation and missing worktree regenerated successfully to Garden Watering Schedule. Repeated the interaction for the recording; it generated Basil Watering Schedule.
On the PR head, created two more threads with real Codex turns. Regeneration also worked for an existing worktree (Grow Rosemary in Well-Drained Soil) and a project-root thread (Grow Mint in a Pot). Manual rename worked before every regeneration.
blacksmith-check ran vp run check:changed 60aec28a9c6fdb7417a7382ec094ccc4984d0738: format, lint, knip, graphql, typechecks for 3 packages, and 53 tests across 3 files passed. The explicit base supplies the PR merge base in the worker shallow checkout. GitHub checks were also green.

Native iOS/Android clients and Electron-specific behavior were not exercised on this Linux VM. Other providers, remote/relay connections, and the automatic storage-cleanup scheduler were not exercised. Worktree removal used the real git command to reproduce the missing-directory result of cleanup. No code changes were needed.
