fix(pi): extension errors name folder extensions instead of "index" - #29768
When a Pi extension throws, T3 Code adds an error item to the turn, titled with the extension's name. Pi's documented layout for multi-file extensions is a folder with an index.ts entry (~/.pi/agent/extensions/<name>/index.ts, .pi/extensions/<name>/index.ts). Pi resolves that folder to its index.ts and reports the file in extension_error.extensionPath. T3 Code named the extension after the file, so every folder extension showed up as "index", for example "index failed during tool_call.", and the user can't tell which extension failed.
Change
piExtensionDisplayName now uses the parent folder name when the entry file is index.*. Single-file extensions such as /ext/a.ts still show "a", and a missing path still falls back to "Pi extension".
Scope and approval
Very small, focused fix for an obvious bug, so no prior issue: a 4-line change to one label helper, plus one assertion added to the existing extension-error test. Overlap search: the local PR index and GitHub search for Pi extension error PRs turned up nothing that touches piExtensionDisplayName. #27556 and #24207 touch extensionPath elsewhere in the Pi adapter, but neither changes how the error item is titled.
Verification
Red on main: vp test run src/server/adapter.test.ts -t "distinct ids" (packages/provider-pi) failed with AssertionError: expected 'index' to equal 'a'.
Green: vp test run src/server/adapter.test.ts: 93/93 passed.
vp run check:changed: format, lint, knip, typecheck (9 packages) and tests (94/94) all pass.
Checked Pi's own loader (resolveExtensionEntries in pi-coding-agent dist/core/extensions/loader.js) to confirm that a folder extension reports <dir>/index.ts as its path.
Not covered: package-layout extensions whose entry is something like <pkg>/src/index.ts now show "src" instead of "index". That is no worse than before, but those packages still don't get a good name. I did not run a live Pi turn.

Macroscope's review found this PR approvable — Extension errors now identify directory-based extensions by their folder name rather than “index.” The change is limited to error titles and messages, leaving extension execution and failure handling unchanged.

Walkthrough
The adapter derives extension display names from normalized path segments. For an index file, it uses the parent directory name. The test checks that an extension error for /ext/a/index.ts has the title a.

Summary

The adapter uses the parent directory name when the final path segment is index, and retains the fallback name for empty results. The test pairs thread IDs with extension paths and checks the extension-error item title.

This test now covers only /ext/a/index.ts. The helper also preserves /ext/a.ts, but the test no longer checks that non-index branch. Add a single-file assertion in the existing test or table.
As per path instructions, backend behavior changes need focused tests for behavior that can break.

Extend the existing extension-path test to cover both the `/ext/a/index.ts` and

as the index-file path.

Addressed the CodeRabbit nitpick about losing single-file coverage in d823fa4. The existing test "gives notify and extension-error items distinct ids in each thread" now sends a different extension path in each of its two threads: /ext/a.ts (single file) and /ext/a/index.ts (folder index). It checks that both error items are titled a, so both branches of piExtensionDisplayName are covered. No new it was added.

merged current main into the contributor branch without rebasing or conflicts (e828766a9d). this brings in main's fix for the failing Astro check:changed assertion. no additional source changes; the pr remains two files, 12 additions and 4 deletions. the contributor's single-file and folder-index coverage is preserved.
blacksmith verification passed: vp run check:changed github/main (format, lint, knip, nine package typechecks, 100 tests), plus vp test run scripts/check-changed.test.ts (15 passed). two independent reviews found no issues at this head. all 31 reported checks pass, github reports clean/mergeable, and no unresolved review threads remain.
live pi provider/client behavior was not exercised; adapter event-path coverage passed. no ui files changed.

reproduced on github/main (61c3794c32), in a separate installed worktree. added a project pointing at a real git repo through the app, created threads, selected providers, and submitted real turns. the project has two actual pi extensions that throw from agent_start: .pi/extensions/folder-check/index.ts and .pi/extensions/single-check.ts. pi v1.5.0 used gpt-6.1-sol through the local model proxy. no synthetic provider events or replay data.
main rendered the folder error as index failed during agent_start.. the exact pr head rendered folder-check failed during agent_start. with the same extension, project, prompt, and viewport. single-check retained its correct name. both errors remained separate and preserved their details. pi read README.md and completed PI_PATH_OK despite the hook errors. a second turn reported both correctly named errors again and completed PI_FOLLOWUP_OK; reloading preserved the conversation. real codex gpt-6.1-sol turns also read the file and completed on both main and the pr.

native ios/android and the electron shell were not exercised on this linux browser run. windows path separators, missing extension paths, and package entries such as src/index.ts were not exercised through a live provider. the changed path was verified with real pi; codex does not emit pi extension errors. an initial pi attempt returned 401 because the temporary custom-provider config used a literal environment-variable name; after correcting its interpolation, the real pi turns above completed.
