Closes #1253 — AI-3392

What & why

When an agent dies without the server hearing about it, its session stays active in the app for about 30 minutes, until the server's cleanup sweep closes it. This happens to a private daemon agent, or to a daemon that can't reach its server. AgentSessions.Reap already kept a local exit record when a claimed agent process was gone. That record now also drives a SessionEnd.

  • A claim now records the vendor and the server the session's hooks report to. It goes in a sidecar next to the claim note (agent-sessions/<pid>.origin), so the two-line note other readers parse is unchanged.
  • During the spool drain, each newly gone session gets a session-end/<vendor> entry with reason: parent_exited. This happens only if it was recorded against the server this process posts to. The drain then delivers it with the usual retries.

The issue's open problem, a session recorded under another profile or server, is answered by not posting there at all. That server's own sweep still closes it.

Where to look

A session the server already ended gets this end too, for example one the watcher or daemon closed. The server's session-end handler treats an already-ended session as a no-op.

Verification

  • AgentSessionsTests (20): a reaped claim returns its recorded vendor and server and drops the sidecar; a dead claim without an origin returns its session alone.
  • SessionEndSpoolTests (3):
    • only this server's sessions are queued, with trailing-slash and case variations of the URL treated as the same server;
    • the drained entry is the parent_exited end on the vendor's route;
    • nothing is queued without a URL.
  • GitHookTests, PreviousSessionTests, UnusableUrlGuardTests and LiveWatchHardeningAcceptanceTests pass.

Summary by CodeRabbit

  • New Features

    • Automatically reports agent session endings when the agent process exits.
    • Session-end events include the session ID, exit reason, transcript path, working directory, and UTC end time.
    • Events are routed to the appropriate vendor and server based on session origin.
    • Sessions without matching server information are not reported.
  • Bug Fixes

    • Preserves session origin information when claims are reused or updated.
    • Avoids reporting sessions that have resumed or belong to another server.
    • Allows a resumed session’s subsequent ending to be reported correctly.
    • Cleans up processed session exit records and associated origin data.

The claim records which server the session reports to, so the end is only ever
sent there; a session recorded under another profile is left to that server.

📝 Walkthrough

Agent session claims now store origin metadata. ReapExited returns exited sessions with origin data. AgentHookPoster queues vendor-specific SessionEnd events for the recorded server.

The server check protects initial queueing, but queued events do not retain their destination server. If delivery is delayed and a later invocation uses another profile, session identifiers and local paths can be sent to that other configured server.

Retained concerns

  • Medium · security · inferred: The new origin-matched report loses its server binding when appended to the shared lifecycle spool. An end queued for server A can remain after an authentication lapse or transient failure; a later invocation for server B can replay it to B, including its session identifier, transcript path, and working directory. This requires profiles or invocations sharing a configuration root, not access to A's credentials. The existing spool design predates the PR, but this new producer inherits its cross-server exposure.
Security review details

Security Blast Radius

  • inferred — The supported exposure is metadata from eligible exited sessions sharing one configuration root: session identifier, vendor route, transcript path, working directory, and exit timestamp. A different configured receiving server can observe that metadata if it drains the retained event. This path does not itself read or transmit transcript contents.

Security Findings and Attack Paths

  • inferred — A report can pass the server-A check, remain queued because delivery is unavailable, and later be sent during a server-B hook invocation using the same configuration root. An operator of configured server B can then receive A-associated metadata without possessing A's credentials. Server-side rejection cannot prevent receipt of the request body.

Trust Boundaries and Controls

  • observed — Credential resolution is bound to the current profile and target server, and the drain checks authentication status before delivery. This protects credential selection, but it does not establish that queued session data belongs to that authenticated destination.

Hardening Proposals

  • proposed — Preserve canonical destination identity in the durable queue or partition queues by destination, and enforce that identity again during replay. Validate the transition from queueing under A through failed delivery to a subsequent drain under B.

Explanation

Issue #1253 requires a SessionEnd for a locally claimed agent process that exits. AgentSessions.ReapExited returns the exited session and its claim origin. AgentHookPoster.SpoolEnds queues session-end/<vendor> with reason: parent_exited only when the origin server matches the current server. Other-server sessions are deferred for that server. AgentSessionsTests and SessionEndSpoolTests cover origin recovery, server matching, vendor routing, resumed sessions, and missing server data.

Full details: Out of Scope Changes check

Explanation

The changes remain within issue #1253. Origin sidecars identify the session server and payload data. Deferred ends handle claims recorded for another server. ClearEnded prevents a prior end marker from suppressing a later end after resume. The added tests support these behaviors. No unrelated product change is demonstrated.

High-Level Assessment

Using the existing lifecycle spool is appropriate: it preserves retry behavior and avoids sending an end to a different configured server. Direct posting was considered but would lose that delivery path; keeping origin in a sidecar also preserves readers of the two-line claim note.

1. Codex exit ends return 404 ✗ Dismissed
Description
SpoolEnds queues Codex exits to session-end/codex, but kcap-server registers no matching
endpoint: its generic session-end route is constrained to claude. When a claimed Codex process
exits, the drain posts to an unmatched route instead of reaching the server's session-end handler.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[326]

+            if (spool.Append(session.Session.Value, $"session-end/{vendor}", body)) spooled++;
Relevance

●●● Strong

Accepted: team fixes missing vendor hook contracts and deployed-route 404s; Codex routing is
explicit PR intent.

PR-#767
PR-#262

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The new CLI path emits a vendor-specific route, and its test explicitly expects Codex. The server's
session-end registrations have no Codex route; the only generic registration is constrained to
Claude, although Codex is a recognized vendor.

2. Resumed starts vanish from the spool ✗ Dismissed
Description
SpoolEnds treats every pending lifecycle entry behind an old ended marker as a straggler and calls
DiscardRemainder before appending the new end. If a resumed run has already spooled its
session-start during a failed post, that call deletes the new start along with any old entries.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[R322-324]

+            if (spool.IsMarkedEnded(session.Session.Value)) {
+                spool.DiscardRemainder(session.Session.Value);
+                spool.ClearEnded(session.Session.Value);
Relevance

●●● Strong

Historical spool findings accept preserving resumed-session data and preventing discard operations
from stranding lifecycle events.

PR-#1348
PR-#1550

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
Ordered draining creates the persistent marker after an earlier end; Claude can spool a resumed
session-start; and DiscardRemainder deletes the live file containing that start. The new exit path
invokes that deletion before it appends the fallback end.

3. Resumed sessions stay active ✗ Dismissed
Description
SpoolEnds appends a fallback end without accounting for the spool’s persistent ended marker, which
causes the drain to discard the new entry before posting it to kcap-server. If a previously ended
session resumes on the server and its new agent process then exits silently, the server has marked
that session active again but never receives this end.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[320]

+            if (spool.Append(session.Session.Value, $"session-end/{vendor}", body)) spooled++;
Relevance

●●● Strong

Recent lifecycle precedents accept preserving session-end delivery across retries and
resumed-session state.

PR-#1550
PR-#1492

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The PR appends the new terminal entry; the CLI drain discards all entries for an ID with an earlier
ended marker, while the server explicitly makes that ID active again on a subsequent session start.

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A prior spooled end leaves a persistent ended marker. When the same session resumes, the new fallback end is appended but the drain discards it, even though kcap-server has reactivated the session.

- src/Capacitor.Cli.Core/LifecycleSpoolDrain.cs[25-35]
- src/Capacitor.Cli.Core/HookSpool.cs[363-397]

## Recommended Fix
Update the CLI’s lifecycle-spool handling so a genuine new run can clear or supersede its prior ended marker before its fallback end is drained, while retaining protection against late entries from the previous run. Add a test covering a delivered end, a resumed session, and a silent exit.

4. Dead agents stay live without another hook ✗ Dismissed
Description
DrainSpoolsCoreAsync is the only path that calls ReapExited and queues the new end. When an
agent dies after its last hook and no other hook runs, the daemon drains existing spool entries but
never discovers the death, so the session remains active until the server's sweep.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[266]

lifecycle recovery gaps are accepted.

PR-#1254
PR-#1424

ⓘ Recommendations generated based on similar findings in past PRs

5. Claude and Codex endings fail ✓ Resolved
Description
SpoolEnds sends only session_id, reason, and ended_at, but kcap-server binds Claude and
Codex session ends to SessionEndHook, which also requires transcript_path, cwd, and
hook_event_name. When either vendor’s exit is reaped, request binding rejects the payload before
the handler runs, and the CLI drops the rejected spool entry instead of retrying it.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[R311-314]

+            var body = new JsonObject {
+ ["session_id"] = session.Session.Value,

6. Queued ends can reach another server ✗ Dismissed
Description
SpoolEnds checks the recorded server before appending an end, but the shared spool retains only
its route and body, not its destination. If delivery is delayed until the active profile changes, a
CLI or daemon drain posts the end to the newly configured server, whose authenticated handler uses
the supplied session ID without checking its originating server and can end a same-ID session there.

PR-#1599

Accepted patterns prioritize preserving session-end metadata across repeated claims and preventing
missing transcript fields.

PR-#1550
PR-#1422

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
Muse makes a pathless claim before resolving its log, and EnsureWatcherRunning makes another claim
with those values. The repeat-claim condition excludes an origin whose server is already set, while
SpoolEnds serializes the missing values as empty strings.

## Issue description
An early Muse claim with a server but no paths prevents a subsequent claim from saving its known log and working directory.
## Fix Focus Areas

- src/Capacitor.Cli/Commands/Harness/MuseHookCommand.cs[186-200]
## Recommended Fix
Allow repeat claims to fill missing transcript and working-directory fields while preserving the claim's first known server. Test Muse's rearm sequence with a log resolved after its initial claim.

9. Concurrent drains can erase a new end ✗ Dismissed
Description
SpoolEnds clears the marker and appends the new end without coordinating with a drain that has
already observed the old marker. If the daemon drain observes that marker first and calls
DiscardRemainder after the append, it deletes the live spool file containing the new end.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[R320-322]

+ // An earlier end of a session since resumed left a marker that would discard this one.
+            spool.ClearEnded(session.Session.Value);

Recent accepted reviews require synchronization when concurrent operations can erase newer state or
lose queued work.

PR-#1673
PR-#831

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The daemon runs an independent lifecycle drain. Its marker check precedes a separate
DiscardRemainder call, which deletes the session’s live .jsonl file; the new code can append to
that file between those operations.

10. Late events replay after a session ends ✗ Dismissed
Description
SpoolEnds clears the ended marker before separating any leftover entries from the previously ended
session. If a late non-terminal event was appended after the earlier end, the next lifecycle drain
posts that event instead of discarding it, even though the earlier end was already delivered.
Code

src/Capacitor.Cli/Commands/AgentHookPoster.cs[R320-322]

PR-#1348

ⓘ Recommendations generated based on similar findings in past PRs

11. Newlines in paths prevent automatic ends ✗ Dismissed
Description
WriteOrigin joins unescaped origin fields with newlines, while ReadOrigin accepts exactly five
newline-separated fields. On a Unix system, a valid transcript or working-directory path containing
a newline makes the sidecar unreadable, so reaping loses the server and vendor needed to spool the
session end.
Code

src/Capacitor.Cli/AgentSessions.cs[R266-267]

+        File.WriteAllText(temp, string.Join('\n', token, origin.Vendor, origin.Server, origin.TranscriptPath, origin.Cwd));

Relevance

●●● Strong

Delimiter parsing with unescaped newlines has been accepted as a data-integrity bug.

PR-#408

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The hook accepts path strings without excluding newlines, and WatcherManager persists them in

12. Sessions with dotted IDs miss deferred ends ✗ Dismissed
Description
TakeDeferredEnds skips every filename containing a dot as an unfinished write, but
SessionId.Parse accepts dots within session IDs. If such a session exits while another server is
configured, its deferred end is never returned when the original server is configured again.
Code

src/Capacitor.Cli/AgentSessions.cs[137]

13. Child sessions outlive a reaped parent ✗ Dismissed
Description
SpoolEnds queues the parent's session end without running the subagent teardown that
PostSessionEndOnParentExitAsync performs before posting that end. If the fallback reaps a parent
before its watcher completes teardown, or after that watcher fails, the parent end can be delivered
while its child sessions remain unfinished.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence

14. Later server posts cannot end the session ✗ Dismissed
Description
Claim keeps an existing origin even when its Server is null, so a later claim with a valid URL
cannot record where the session reports. A Claude session-start can claim its process on the
degraded, no-URL path and subsequently post to a valid server; when that process exits, SpoolEnds
skips its end because the retained origin has no server.

15. Concurrent claims can misroute an old end ✗ Dismissed
Description
ReadOrigin associates a reaped note with its sidecar using only the process start token, although
Claim writes the note and sidecar separately. If a delayed hook for a different session on the

16. A resumed session can receive an end ✗ Dismissed
Description

17. Failed spool writes lose session ends ✗ Dismissed
Description
DrainSpoolsCoreAsync calls ReapExited before SpoolEnds attempts to persist the returned end,

Qodo item 12 (path casing): fixed in 8087cec. AgentSessions.SameServer now delegates to ServerIdentity.SameServer, which folds scheme and host case, ignores a trailing slash and compares the path ordinally. So /Tenant and /tenant are different servers. Pinned by Servers_match_as_server_identities.

🟡 Minor · Use a case-preserving deferred-end filename. · AgentSessions.cs:248-249

src/Capacitor.Cli/AgentSessions.cs:248-249
🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Use a case-preserving deferred-end filename.

When two valid opaque session IDs differ only by case, DeferEnd maps both to the same lowercased path and overwrites the first entry. TakeDeferredEnds can then return only the ID from the last write. The original ID on the first line preserves casing for the surviving entry, but it cannot recover the overwritten session.

Update the DeferredEnd local function to generate a case-preserving,
collision-free filename from the full session.Value, such as a UTF-8 byte
encoding rendered as hexadecimal, so opaque session IDs differing only by case
remain distinct and TakeDeferredEnds can recover each original ID.
🟡 Minor · Preserve the exit record when the session-end append fails. · AgentHookPoster.cs:266

src/Capacitor.Cli/Commands/AgentHookPoster.cs:266
🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Preserve the exit record when the session-end append fails.

ReapExited removes the claim note and origin before SpoolEnds calls HookSpool.Append. Append returns false for any persistence failure, and SpoolEnds ignores that result. The session-end entry is then lost, so a later drain cannot retry it. Requeue the exited session, or defer claim cleanup, until the append is persisted.

Preserve exited sessions when persisting their session-end records fails: update
the flow around AgentSessions.ReapExited, SpoolEnds, and HookSpool.Append so a
failed append requeues the corresponding ExitedSession (or otherwise defers
claim cleanup) for a later retry, while successful appends retain the current
behavior.
