Part of #1040 — AI-2989

What & why

Every kcap MCP server read one request, awaited its handler and wrote the answer before reading the next. A long poll such as get_review_flow_status(wait: true) therefore held up ping and every other call for minutes, which looks to the harness like a hung server.

All 13 servers now run one shared stdio loop, McpStdioLoop. Each tools/call runs as its own task, other methods are answered at once, and only the writer is serialised, one response per line. A call that throws is answered with a JSON-RPC internal error instead of leaving its id unanswered. However the loop ends, at end of input or on a failed read or write, it waits for calls in flight before returning.

Concurrent calls needed their shared state made safe:

  • Lazily created clients. Each server created its client lazily on the first call. That now goes through OnDemand<T>: concurrent first callers share one creation, and a failed creation is retried.

  • CwdRepository. It shares one resolution between concurrent first callers.

  • Servers kept in arrival order (serialCalls). Three servers keep their calls in arrival order, while pings and other methods are still answered during a call:

    • flow-result: a message must not land after the result that closes its round.
    • plans: a task call addresses the plan the call before it declared.
    • judge: an evidence run's ledger, budgets and page handles are one sequence.
  • Flow rounds. In flows, submit_review_round and send_to_participant still run one at a time per flow run and participant, because the server refuses a round while that participant's previous one is running.

  • Telemetry flushes. TelemetryClient runs flushes one at a time, so an overlapping flush can no longer clear events another one has just spilled to the spool.

  • Malformed lines. A line with a duplicated key is skipped inside the parse guard instead of ending the server.

One known overlap remains in flows. A get_flow_status running alongside a submit poll on the same run can show the same pending message twice, because acks follow rendering. The tool descriptions already tell agents to react to each message_id once.

This is part 3 of the issue. Part 4 (patch-based context-only) follows in its own PR.

Verification

  • New McpStdioLoopTests:
    • A long call holds up neither a later call nor a ping.
    • Serial mode runs calls in arrival order while still answering a ping. It fails without serialCalls, which I checked.
    • A throwing call gets -32603 and the loop keeps serving.
    • Notifications and unreadable lines get no answer.
    • A failing read still waits for the call in flight before failing.
  • New FlowRoundGatesTests: a second round for the same participant waits; another participant, flow or tool does not.
  • TelemetryClientTests: an overlapping flush never clears events another flush spilled. It fails without the change, which I checked.
  • New OnDemandTests: concurrent first callers share one creation, a failure is retried, and dispose disposes the value.
  • CwdRepositoryTests: concurrent first callers run the git probes once.
  • All Mcp* unit classes pass: 855 of 856. All telemetry classes: 101 pass. The one failure, McpEvidenceJudgeToolsTests.An_http_timeout_is_a_retryable_unreachable_error, is a 500 ms timeout test on untouched code. It passes alone and in 3 of 3 class reruns.
  • All Mcp* integration classes: 72 pass.

Summary by CodeRabbit

  • Improvements

    • MCP tool requests can run concurrently when they do not need ordering. Related requests remain in arrival order, while pings can still be answered promptly.
    • MCP servers continue handling requests after a tool call fails, returning an error for the failed call.
    • Concurrent repository lookups now share a single resolution, and failed lookups can be retried.
  • Bug Fixes

    • Telemetry flushes now respect their time budgets when queued behind another flush, preserving events for a later attempt when time runs out.

The flow-result, plans and judge servers keep calls in arrival order: their
calls depend on the one before, or share an evidence run's ledger and budgets.

📝 Walkthrough

MCP servers now delegate stdio request processing to a shared loop and use on-demand resource providers. The loop supports concurrent tool calls and ordering by key. Repository detection shares concurrent resolution. Telemetry flushes are serialized, and time spent waiting for an earlier flush counts against the current flush budget.

src/Capacitor.Cli/Commands/OnDemand.cs, src/Capacitor.Cli/CwdRepository.cs, test/Capacitor.Cli.Tests.Unit/Commands/OnDemandTests.cs, test/Capacitor.Cli.Tests.Unit/CwdRepositoryTests.cs
OnDemand<T> shares first-time creation, retries failed creation, and disposes its cached value. CwdRepository.GetAsync shares in-progress detection and retries faulted or canceled tasks. Tests cover these behaviors.
Shared MCP stdio request loop
src/Capacitor.Cli/Commands/McpStdioLoop.cs, test/Capacitor.Cli.Tests.Unit/Commands/McpStdioLoopTests.cs
The loop parses requests, routes tool calls, serializes responses, and waits for in-flight calls at EOF or on loop failure. Calls can run concurrently or in order by key. Tests cover ordering, read failures, invalid input, and tool-handler errors.

Merge Risk: 🟡 Moderate · up to a4ec9

A slow telemetry flush can delay command completion beyond its configured budget. Resolve or explicitly accept this behavior before merging.

Security architecture risk: 🟡 Moderate · up to a4ec9

The change removes the previous one-call-at-a-time admission limit without bounding outstanding work. It can also continue performing actions after responses can no longer be delivered. Existing identity and ordering controls limit the demonstrated exposure, but availability and failure-containment risks remain.

Retained concerns

  • Medium · security · inferred: The shared loop accepts and retains arbitrarily many outstanding tool calls. Previously, the flow server awaited each call before reading another request. A caller able to submit MCP requests can now accumulate task and request state behind long-running calls, including calls sharing an ordering key, while unkeyed calls can amplify concurrent authenticated backend work. Ordering and response serialization do not bound admission. Local resource exhaustion is a supported risk; backend-wide impact depends on controls not established here.
  • Medium · reliability · observed: A failed tool-response write faults its background call task without immediately stopping request admission. While stdin remains open, subsequent tool calls can continue invoking authenticated handlers even though their responses cannot be delivered. The base loop awaited the write directly and unwound on failure. EOF, a read failure, or a later failed inline response eventually exposes the failure, but those conditions do not promptly contain an ongoing stream of tool calls.
Security review details

Security Blast Radius

  • inferred — The demonstrated attack scope requires access to the existing MCP request stream. It includes the CLI process and concurrent operations permitted by its existing session. The evidence does not establish unauthenticated network reachability, cross-tenant authority, or backend-wide exhaustion.

Security Findings and Attack Paths

  • inferred — A request-stream controller can repeatedly submit calls while earlier calls remain incomplete. Each accepted request retains task/request state, and unkeyed handlers may start additional backend work. This is newly enabled by asynchronous admission rather than a newly exposed tool or credential.

Trust Boundaries and Controls

  • observed — Ordering protects selected stateful sequences, malformed ordering-key requests are refused, and uncaught handler failures produce generic protocol errors. These controls preserve sequencing and reduce error-detail exposure, but do not limit outstanding work.

Resilience and Maintainability Implications

  • observed — Telemetry's production facade uses a common 1.5-second flush budget and a standard HTTP handler, with HTTP cancellation based on remaining time. This limits the demonstrated deadline concern, although predecessor waiting and synchronous filesystem work are not independently interruptible. Cross-process spool drain/clear remains explicitly best-effort rather than becoming transactional through this PR.

Hardening Proposals

  • proposed — Introduce explicit active-call and queued-call limits with an overload response, while preserving a responsive path for protocol control methods. Treat failed response delivery as a terminal admission signal and define how already-admitted side effects drain or cancel before resource disposal.

High-Level Assessment

The following are alternative approaches to this PR:

1. Queue calls behind a worker per server
  • ➕ Makes ordered execution explicit for stateful servers.
  • ➖ Adds queue lifecycle management and duplicates scheduling across servers.
2. Keep separate server loops with background calls
  • ➕ Limits the immediate scope of each server change.
  • ➖ Repeats protocol, error-handling, and writer-synchronization logic.

Recommendation: Use the shared loop with opt-in serial calls. It fixes protocol-level blocking once while retaining arrival-order semantics for flow-result, plans, and judge. Reviewers should pay particular attention to concurrent shared state and shutdown; flows can still surface a pending message twice during overlapping status and submit polls.

Action required

1. Flow messages can arrive out of order ✓ Resolved
Description
FlowRoundGates.EnterAsync serializes calls that acquire a participant gate but does not preserve
their input order, because the stdio loop schedules each call independently and client
initialization happens before gate acquisition. When two round messages for the same flow and
participant arrive together, the later task can acquire the gate first and POST its message before
the earlier one.
Code

src/Capacitor.Cli/Commands/McpFlowsServer.cs[R79-81]

+                var client = await sessionClient.GetAsync();
+ using var round = await rounds.EnterAsync(
+                    McpTelemetry.SafeToolName(callRequest), (callRequest["params"] as JsonObject)?["arguments"] as JsonObject);
Relevance

●●● Strong

Accepted concurrency precedent requires serializing read-and-submit sequences to prevent later
messages overtaking earlier ones.

Evidence
The loop launches separate tasks without an execution-order guarantee; each flow task awaits shared
client creation before attempting the participant semaphore, and the round handler sends the
supplied context in a POST. The semaphore prevents overlap, not overtaking.

2. A broken error stream stops the server ✓ Resolved
Description
RunAsync writes the order-key failure to Console.Error before sending the invalid-request
response, without protecting that logging call. If stderr is closed or its write fails, the
exception escapes the read loop, leaving the refused call unanswered and preventing later requests
from being served.

3. Malformed calls bypass serial ordering ✓ Resolved
Description
OrderKeyOf converts an exception while reading an order key into null, causing the request to
use the unkeyed execution path instead of waiting for the previous call. When a serial server
receives a malformed nested property on a stateful call, that call can overlap the prior flow, plan,
or judge operation and violate arrival-order protection.
Code

src/Capacitor.Cli/Commands/McpStdioLoop.cs[R113-116]

+        try {
+            return orderKey?.Invoke(request);

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The loop catches order-key extraction failures and returns null; the subsequent dispatch logic
treats null as an unkeyed call. The added test confirms that a duplicated nested key follows this

4. Completed flow gates accumulate indefinitely ✓ Resolved
Description
FlowRoundGates.EnterAsync inserts a semaphore for each flow-and-participant key, but releasing a
completed round never removes that entry. A long-lived flows server retains every distinct key it
encounters, including keys supplied by unsuccessful calls, so its memory use grows with the number
of flows or identifiers processed.
Code

src/Capacitor.Cli/Commands/FlowRoundGates.cs[R23-25]

+        var gate = _gates.GetOrAdd($"{flowRunId}\n{participant}", _ => new SemaphoreSlim(1, 1));

Timing begins before the new wait; spool draining and payload construction happen before the
existing remaining-budget calculation. The command shutdown path synchronously waits for flushing.

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
An overlapping telemetry flush can spend its entire budget waiting for the previous flush yet still drain and serialize a large spool before checking the deadline.
## Fix Focus Areas
- src/Capacitor.Cli.Core/Telemetry/TelemetryClient.cs[55-60]
- src/Capacitor.Cli.Core/Telemetry/TelemetryClient.cs[75-89]
## Recommended Fix
Check the remaining budget immediately after awaiting the preceding flush. If it has expired, safely spill any queued events without doing the drain or payload build, preserving the existing no-loss and no-throw behavior.

6. Active calls outlive a failed input loop ✓ Resolved
Description
McpStdioLoop.RunAsync awaits inFlight only after the read loop, so a read error or an exception
while answering another method skips that wait. If either occurs during a tool call, the server
unwinds and disposes its shared client and writer while the call is still using them.
Code

src/Capacitor.Cli/Commands/McpStdioLoop.cs[R83-84]

+ // A call already running still answers, and nothing it uses is disposed under it.
+        await Task.WhenAll(inFlight);
Relevance

PR-#1135
PR-#187

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The loop tracks call tasks but reaches its sole wait only after the read loop. A failure at the read
or immediate-answer site bypasses that wait; the server's scoped client is then disposed on unwind.

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
An input or non-tool response failure exits the shared stdio loop without waiting for active tool calls, allowing server-owned resources to be disposed beneath them.
## Fix Focus Areas
- src/Capacitor.Cli/Commands/McpStdioLoop.cs[56-84]
## Recommended Fix
Put the in-flight task drain in a finally path that runs after both normal EOF and loop failures. Preserve the original loop failure after the calls have settled, and add a test with an active call and a failing reader or non-tool answer.

7. Concurrent calls can lose telemetry events ✓ Resolved
Description
McpStdioLoop now permits tool completions to start overlapping telemetry flushes, while
TelemetryClient.FlushAsync protects only its queue snapshot and not the subsequent spool
operations. When one flush clears the spool after another has appended events from a failed send,
those events are deleted without being sent.
Code

src/Capacitor.Cli/Commands/McpStdioLoop.cs[R73-75]

+ var call = serialCalls
+                    ? Task.Run(() => CallAfterAsync(prior, id, request))
+                    : Task.Run(() => CallAsync(id, request));
Relevance

●●● Strong

Telemetry race and concurrent shared-state loss findings are accepted when synchronization does not
cover downstream operations.

PR-#501
PR-#1318

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
Each completed call records through the server's shared telemetry instance, which starts an

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Concurrent MCP tool completions can overlap telemetry flushes that read, append to, and clear the same spool, allowing one flush to delete another's unsent events.
## Fix Focus Areas
- src/Capacitor.Cli/Commands/McpStdioLoop.cs[70-77]
- src/Capacitor.Cli.Core/Telemetry/TelemetryClient.cs[35-91]
## Recommended Fix
Serialize the complete drain, send, and spool-update sequence per telemetry sink rather than only the queue snapshot. Add a test that overlaps a successful flush with a failed flush and verifies that the failed batch remains available for retry.

8. Overlapping flow rounds fail ✓ Resolved
Description
McpFlowsServer now dispatches round calls concurrently, so a second call for the same participant
can reach kcap-server while the first is still polling for completion. The server rejects a
participant with an in-flight round as an uncoded 409, which SendWithSettlementRetryAsync does not
retry; two queued round calls that previously ran in order can therefore return a tool error.
Code

src/Capacitor.Cli/Commands/McpFlowsServer.cs[115]

+        await McpStdioLoop.RunAsync(reader, writer, TimedDispatchToolCallAsync, (id, method, request) => method switch {
Relevance

●●● Strong

Recent flow retry precedent accepts findings exposing unsupported server error contracts and unsafe
concurrency behavior.

PR-#1051
PR-#1302

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The new loop defaults to parallel tool calls; the CLI polls a running round before completing its
call, while the server rejects another round for that participant with an uncoded conflict that the
CLI retry gate excludes.

9. A comment relies on old server behavior ✓ Resolved
Description
McpFlowsServer's timeout comment contrasts a current server with an older blocking one instead
of stating the invariant in terms of the request's possible server-side wait. Because the comment's
truth depends on which server version is deployed, it becomes stale as compatibility changes and
misleads maintainers about why the client-wide timeout is disabled.
Code

src/Capacitor.Cli/Commands/McpFlowsServer.cs[R61-64]

+ // A start or round POST can be held open server-side: an admission wait on a
+ // current server, the whole round on an older blocking one. The default 100s
+ // timeout would abort it, which the server sees as a cancel and tears the
+ // reviewer down — so no client-wide deadline; each lane bounds its own requests.

The timeout comment relies on historical wording about a `current server` and an `older blocking one`, which can become stale as deployed server behavior changes.

## Fix Focus Areas
- src/Capacitor.Cli/Commands/McpFlowsServer.cs[61-64]

## Recommended Fix
Rewrite the comment to describe the current compatibility invariant directly: start and round requests may remain open while waiting for server-side admission or round completion, so the client-wide timeout must be disabled and individual request lanes must enforce their own bounds. Remove the version-based historical comparison.

10. Refused calls leave no trace for operators ✓ Resolved
Description
TryOrderKey swallows the exception from the order-key read with a bare catch, and the refusal
branch writes only a generic -32600 Invalid request with nothing on stderr, unlike CallAsync,
which logs its failure with the server name and exception. In flows, RoundOrderKey reads
params.arguments for every tool, so any call with a bad nested argument is turned away before
dispatch and before telemetry, with no detail for whoever has to debug the failure.
Code

src/Capacitor.Cli/Commands/McpStdioLoop.cs[R71-74]

+                    if (!TryOrderKey(orderKey, request, out var key)) {
+                        await WriteAsync(Error(id, -32600, "Invalid request"));
+                        continue;

PR-#226

PR-#526

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
Lines 118-126 catch every exception and drop it; lines 71-74 write only a fixed error message. By
contrast, CallAsync (lines 41-44) logs `kcap mcp {serverName}: unexpected error handling tools/call:
{ex}` before it answers. RoundOrderKey reads the arguments of every flows tool call, so the refusal
applies beyond round calls.

src/Capacitor.Cli/Commands/McpStdioLoop.cs[115-126]
src/Capacitor.Cli/Commands/McpFlowsServer.cs[126-140]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
When reading a tools/call's order key throws, McpStdioLoop refuses the call with -32600 and drops the exception, so stderr shows nothing about why the call was refused.

## Fix Focus Areas
- src/Capacitor.Cli/Commands/McpStdioLoop.cs[71-74]
- src/Capacitor.Cli/Commands/McpStdioLoop.cs[118-126]

## Recommended Fix
Change TryOrderKey so it returns the caught exception (for example through an `out Exception? error` parameter). In the refusal branch, write `kcap mcp {serverName}: unreadable order key on tools/call: {error}` to Console.Error before answering. Consider using -32602 (Invalid params) instead of -32600, because the request envelope is valid and only the arguments are malformed.

The server refuses a participant's round while its previous one runs, and
overlapping flushes could clear events another flush had just spilled.
