Reported from a downstream V2 fork.

Problem

When a background subagent ends, OpenCode 2 runs the parent's reply on its own. The adapter holds that reply and offers the orchestrator a continuation turn for it. The continuation turn always replayed the oldest held reply (state.wakes.shift()), because the run carried nothing that said which reply it was for.

Suppose reply A's continuation is cancelled while queued, or dropped before it starts (a failed dispatch, a cancelled queued run). A stays at the head of the queue. Reply B's continuation then shows A's output under B's notification, and B's reply stays held forever. That pins the session as having background work.

Reproduction

New adapter replay test: replays only its own background reply into a continuation turn, in OpenCode2AdapterV2.test.ts, using the 2.0.18 frame shapes.

  1. A turn starts two background subagents.
  2. Each one's report wakes the parent into a reply: REPLY_A, then REPLY_B. Two continuations are offered.
  3. Only B's continuation runs.

On main, B's turn shows [ 'REPLY_A' ]. With the fix it shows [ 'REPLY_B' ] and nothing is left pending.

Fix

  • Contract (minimal): ProviderContinuationRequest gains an optional messageId. When an adapter names one, ProviderContinuationService dispatches the continuation message under it instead of allocating one. Requests without it behave as before.
  • OpenCode 2 adapter:
    • Each held reply (Wake) gets its own message id, allocated when OpenCode starts it, and offers it with the continuation.
    • runWake replays the wake whose id matches the turn's message.messageId.
    • Wakes held before it are dropped: their continuations were cancelled or failed before starting, so nothing will ever replay them. These stale replies are not re-offered, because a later continuation already answered the same session.
    • A continuation can't be reordered ahead of another: the orchestrator refuses to reorder a queued message that carries a notification, and every continuation with a reply carries one. So an earlier wake still held here is one whose continuation was cancelled or failed to dispatch, and dropping it keeps it from pinning the session as background work.
    • A turn whose wake is already gone (a user turn joined it, or a Stop or reconnect dropped it) still ends at once.
  • Three existing tests built continuation turns with a made-up message id. They now start them from the offered id, as the real service does.

Verification

  • New tests fail without the fix (actual: [ 'REPLY_A' ]; the service test gets an allocated id) and pass with it.
  • The new adapter test waits on a Deferred the second offer completes, not a poll.
  • Inside unshare -U --map-current-user -p -f --mount-proc:
    • OpenCode2AdapterV2.test.ts + ProviderContinuationService.test.ts: 103/103.
    • testkit/OrchestratorReplayFixtures.integration.test.ts -t opencode2: 16/16. opencode2_background dispatches a real continuation through the service.
  • vp exec tsc --noEmit -p . in apps/server: exit 0. vp lint and vp fmt --check on touched files: clean.
  • Not run: live OpenCode. A cancelled queued continuation with a second reply pending can't be produced on demand.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — The PR changes production continuation routing and adds asynchronous tracking and interruption of nested OpenCode sessions across stops and reconnects. The behavior spans multiple lifecycle paths and carries enough orchestration complexity to require human review.

📝 Walkthrough

Walkthrough

Continuation requests can identify the message associated with a wake. The OpenCode adapter uses that ID to select a wake for replay and tracks subagent sessions announced by held wakes for cleanup when those wakes are discarded.

Changes

OpenCode continuation handling

Layer / File(s) Summary
Continuation request IDs
packages/provider-core/src/server/ProviderContinuationRequests.ts, apps/server/src/orchestration-v2/ProviderContinuationService.ts, apps/server/src/orchestration-v2/ProviderContinuationService.test.ts, packages/provider-opencode/src/server/v2/adapter.ts
Continuation requests accept an optional messageId. The service dispatches the supplied ID or allocates one when absent. The adapter assigns IDs to routed wakes and includes them in continuation offers.
Wake identification and replay
packages/provider-opencode/src/server/v2/adapter.ts, apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts
The adapter selects the wake that matches a continuation’s message ID and discards earlier queued wakes. Tests use captured continuation offers and cover replay ordering, joined turns, and replies from multiple background wakes.
Discarded wake and child-session cleanup
packages/provider-opencode/src/server/v2/adapter.ts, apps/server/src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts
The adapter tracks child sessions announced by held wakes. When a wake is discarded or background work is stopped, it records those sessions and descendants as strays, handles their requests, reconciles inactive sessions, and attempts to stop them. Tests cover nested subagents and Stop outcomes.

Continuations now replay their own background reply, which fixes the reported bug. In one narrow case, Stop does not stop subagent sessions left over from a dropped reply: this happens after the thread has been unloaded and reloaded. Those sessions are only cleaned up when the connection reconciles. The change is mergeable, with a small follow-up recommended.

…d reply

OpenCode runs the parent's reply to a background subagent on its own, and the
adapter holds it for the continuation turn it offers. That turn replayed
whichever held reply was oldest. When a continuation was cancelled while
queued, or dropped before it started, its reply stayed at the head of the
queue, and the next continuation showed it under its own notification while
its own reply stayed held.

Each held reply now gets the message id its continuation run starts from. The
continuation request carries it, and `ProviderContinuationService` dispatches
under it instead of allocating one. The continuation turn replays the reply
its message names, and drops older ones whose continuation will never run.

A continuation drops every reply held before its own, so cancelled

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Match strays by session ID, not by ThreadState identity.

stopStrays picks strays with rootOf(caller) === thread, and strays keeps the ThreadState object from when the wake was dropped. unloadThread does not check strays: hasBackground and owesWork ignore them. So a thread can unload while its strays still run. When the thread loads again, register creates a new ThreadState. After that:

  • A user Stop calls stopStrays(newState). The identity check never matches, so T3 never interrupts the stray sessions. Only a reconnect reconcile can still reach them.
  • discardWakes put the stoppedChildren marker on the old object. A stray's later report to the reloaded thread therefore does not stop the wake it causes. The adapter offers a continuation for a reply the user already stopped.

Compare by native session ID. Also consider clearing or re-homing a thread's strays in unloadThread.
