fix(pi): explain when an untrusted folder makes Pi skip project resources - #27539
T3 launches Pi in RPC mode, which has no startup trust prompt. In a folder Pi hasn't trusted, Pi quietly skips the project's .pi settings, extensions, skills, prompts, MCP servers and system prompt files, so a project skill just isn't there and nothing says why.
The generated T3 Pi extension now checks ctx.isProjectTrusted() on the first turn of every Pi session. If the folder isn't trusted, it posts one notice through the existing notify path. The notice names the folder, says what Pi skipped, and explains how to trust the folder or its parent with /trust in terminal Pi. It also mentions --approve for trusting every project and --no-approve for keeping project files off without the notice. Nothing is trusted automatically, T3 never adds --approve, and an explicit --no-approve stays quiet.
The check runs per session, not on Pi's one-shot project_trust event. Pi resolves trust once per folder per process, and every session replacement (T3 resumes threads with switch_session) builds fresh extension instances that never see the event again. A version keyed on that event lost the notice on every resumed thread. The notice waits for a turn because T3 drops Pi notices that arrive outside one. The Pi user guide's troubleshooting entry now describes the notice and the fix.
Proof
Unit test: the new Pi project trust table in mcpExtensionSource.test.ts fails on main's extension and passes here. It covers a trusted folder, an untrusted folder (one notice per session across two sessions and two turns each) and --approve -na.
Real Pi 1.4.0, RPC mode, isolated agent dir, project with a .pi/skills skill:
Notice shown: an undecided folder, the same folder after new_session, and a saved decline before and after new_session.
No notice:--approve (the skill loads), --no-approve, --approve -na, saved trust before and after new_session (the skill loads), and a folder with no protected resources.
Before the fix: the round-one version lost the notice after new_session.
Pi 0.80.5 (oldest supported): the notice appears for an untrusted folder and not with --approve.
Through T3 (isolated dev server): a new Pi thread in the untrusted project persisted a system_notice turn item with the final text. An existing thread resumed after a server restart (new Pi process, switch_session) persisted the notice on its next run.
Not covered: a visual capture of the web UI. The browser panel was unavailable. The persisted system_notice item is what the work log renders.
vp run check:changed: format, lint, knip and typecheck pass. The test step fails only on ServerBrowserPage.test.ts (no Playwright browser installed) and browser-secret-native.test.mjs (no libsecret dev package). Both fail the same way on main on this host.

Pi resolves project trust once per folder per process, and session
replacement reruns extension factories without project_trust, so the
notice now reads Pi's resolved trust on each session's first turn.

On the first before_agent_start event, the extension displays a notice for an untrusted folder unless --no-approve or -na is present. Tests cover trusted and untrusted folders and launch arguments. The documentation describes trusting the folder or its parent with /trust, starting a new thread in the same worktree, and refreshing the provider.
pingdotgg/t3code#15140: This issue concerns missing trust guidance when Codex skips project-local MCP configuration. The change adds a trust notice for Pi, but does not change Codex behavior.
