- Around line 81-84: Update the fallback refusal status description in the
guide: state that a cooling combo target and blocked main account return 429,
while an unavailable subagent model returns 503. Preserve the surrounding
explanation that refusal occurs before sending and the fallback target is not
contacted.
Review comments at @docs-site/src/content/docs/guides/remote-link.md:
- Line 98: Update the “home_unreachable” guidance to limit the no-forwarding
claim to cases where the tunnel gate rejects the request before forwarding
begins. Clarify that a post-forwarding tunnel failure may still return
home_unreachable after the Home received the request, without changing the
existing fallback-permission references.
Review comments at @docs-site/src/content/docs/reference/cli/agents.md:
- Line 337: Remove the stray leading plus character from the “Fallback off”
heading so it is recognized as a level-two Markdown heading and restores the
correct section hierarchy and table-of-contents entry.
Review comments at @gui/src/components/FallbackPanel.tsx:
- Line 29: Update FallbackPanel’s load and write flows to use a load sequence
ref: increment it when each load starts, when a write begins, and after the
latest PUT result is applied. Require both savingRef.current to be false and the
load sequence to match before applying load success or failure state, preventing
stale in-flight GET responses from overwriting newer write results.
Review comments at @gui/src/i18n/ru.ts:
- Line 4095: Update the “fallback.last” translation to “Последняя попытка
резервного переключения” so it accurately labels both taken and refused fallback
attempts.
Review comments at @gui/src/pages/dashboard-overview-section.tsx:
- Line 13: Propagate Remote Link Child read-only state from App through
Dashboard and DashboardOverviewSection to the routing FallbackPanel, using
targets.link !== undefined; pass it as readOnly and ensure FallbackPanel’s Clear
buttons use locked for disabled state so all controls are non-writable.
Review comments at @src/cli/status-fallback.ts:
- Around line 20-22: Propagate the existing status timeout signal through the
Child path: update collectStatusFallback’s readLinkHomeJson call,
readLinkHomeJson, and its fetchLinkHomeManagement request to accept and reuse
the signal for both relay requests and readBoundedResponseBytes, retaining the
30-second fallback only when no signal is provided.
Review comments at @src/client/link-relay.ts:
- Line 405: The link relay error handling at the return using homeUnreachable
must preserve LinkRelayAuthenticationError as an authentication failure rather
than converting it to a Home-unreachable response. Reserve homeUnreachable for
unavailable connections and keep authentication failures distinct so callers do
not trigger Home-outage retries or device-local fallback.
Review comments at @src/config/diagnostics.ts:
- Line 149: Update validateConfigCandidate to call fallbackPolicyError on the
candidate value before accepting a whole-config write; return { ok: false,
error: fallbackError } when it reports an error, preserving the existing
validation flow otherwise.
Review comments at @src/server/management/fallback-routes.ts:
- Around line 53-59: Update the write-validation flow around the alias lookup
and existence check so writes with value null bypass device existence
validation, allowing stale or revoked entries to be cleared while preserving
validation for non-null writes. Add a test covering a revoked link ID whose
clear operation returns 200.
Review comments at @src/server/responses/core-combo.ts:
- Around line 645-646: Update the proactive failover guard around
pickComboTargetWithWait so every transition from primaryTarget to a later target
is handled, including disabled, quota-exhausted, and otherwise ineligible
primary targets, not only cooldowns. Reuse the exclusion result when available,
and use an appropriate unavailable or quota_exhausted trigger for non-cooldown
exclusions so fallback.provider=false prevents dispatch to later providers.
Review comments at @src/types/fallback.ts:
- Line 28: Update the providers documentation comment and the fallback-off guide
wording to describe scopes as matching hops that leave a provider, specifically
the target that just failed or is unavailable, rather than requests that started
on a provider. Keep the existing provider-scope behavior and the wording in
structure/fallback-policy.md unchanged.
Review comments at @structure/manifest.json:
- Around line 732-739: Update the documents array for fallback-policy.md to
include the src/codex/ area, then regenerate the structure index and run the
structure check so the mapping is synchronized and validated.

…on Children
Add one policy resolver (src/routing/fallback-policy.ts) that every model or
provider hop asks before it switches targets: combo failover (reactive, and
proactive for a cooling first failover target), routing-profile candidates,
subagent model fallback and the main-account hard-lock fallback. A refused hop
answers fallback_refused with the triggering failure's status, never contacts
the fallback target and names the ocx fallback command that allows it.
Config: fallback.{model,provider}, providers/routes/devices maps, and
devices.<id>.localFallback (a permission GOAT reads). A device set to off always
refuses; otherwise route > provider > global; absent keeps today's behavior.
Intra-pool account and key rotation is not a fallback and stays on.
A Remote Link Child and a Hub client still never serve local providers; their
Home-down 503/502 bodies gain code home_unreachable. State is exposed as
ocx.fallback/1 through GET|PUT /api/fallback (also over the link relay),
ocx fallback, ocx status --json, /readyz fallback-policy/1, and a dashboard
panel on Dashboard and Remote Link.
The request-path resolver imported the link store, which put src/link/ on the always-on core graph (tests/lab/core-link-boundary.test.ts). The link listener lifecycle now registers the key-to-link resolver at creation and releases it on stop; with nothing registered a key id is its own device.
PUT /api/fallback accepts { writes: [...] } (up to 8) applied in one config mutation, so the dashboard chains switch and ocx fallback --kind all write both kinds atomically; a refused entry writes nothing. The device list is a table (react-doctor prefer-tag-over-role, async-await-in-loop).
… two Remote Link request assertions A Home or Child without /api/fallback shows no second error on Remote Link. The two Remote Link tests that asserted the page-wide last request and GET count now scope that accounting to /api/link, since the page also hosts the fallback panel.
…ter admission; address #7482 review - combo and routing-profile hops compare resolved destinations (provider alias, blockedModelRedirects); the failed child's resolved target is the hop's origin and a profile's current advances to the redirected candidate - every primary-to-later failover start is guarded (disabled, quota spent, ineligible, cooldown) with cooldown/quota_exhausted 429 and unavailable 503 triggers - guardFallbackHop records refusals at once and taken hops only via commit() after the site admits the dispatch (send-budget reservation, profile candidate not skipped) - Link relay authentication failures answer link_relay_auth_failed, never home_unreachable - validateConfigCandidate rejects a malformed fallback section; clearing a removed device's override is allowed; ocx status keeps its 2 s bound on the Child path - GUI: stale poll cannot overwrite a newer save; routing panel read-only on a Child - docs: per-site refusal statuses, pre-send home_unreachable scope, providers key meaning, stray heading, ru label, manifest src/codex/

…cy evaluator hardLockTargetOf ran previewRouteModel on routing-profile fallbacks, which runs the policy evaluator and opens routing-history.sqlite under OPENCODEX_HOME just to classify the hop. On Windows that handle outlived the request's home: the hard-lock routing test took ~15s and its temp-home cleanup failed with EBUSY. A routing profile is now classified from its declared candidates (any candidate on another provider makes a provider hop). Combos, account namespaces and concrete targets still resolve through previewRouteModel, which is config-only.

Summary
OpenCodex can now be told not to fall back. When a model or provider fails and a combo, routing profile, subagent chain or the main-account hard lock would serve the request from a different model or provider, an operator can refuse that hop per scope. A refused request gets a clear
fallback_refusederror instead of an answer from a target it did not pick. Nothing changes unlessfallbackis configured.src/routing/fallback-policy.ts. Each hop site callsguardFallbackHopat the point it would switch targets. Sites: combo failover (reactive at the dispatch loop, plus proactive whenever afailovercombo starts past its first target: cooling down, quota spent, disabled or ineligible), routing-profile candidates, subagent model fallback, and the hard-lock reroute. Combo and routing-profile hops are classified by resolved destinations (provider aliases andblockedModelRedirectsapplied), and an allowed hop is recorded as taken only after its dispatch is admitted. Nothing else readsconfig.fallback.src/types/config.tsthat 429 rotation cannot be switched off stands. Classifier fallbacks are a static mapping, so they never hop.error.type/codefallback_refused, pluserror.fallbackwith{kind, scope, scopeKey, from, to, trigger}. The status andRetry-Afterare those of the triggering failure; a proactive refusal answers 429 (quota/cooldown/hard lock) or 503. The response carriesx-opencodex-fallback: refused, and the fallback target is never contacted.errorstring and addcode: "home_unreachable", which means an unavailable connection only: a failed Link relay authentication answerscode: "link_relay_auth_failed", so it never reads as Home down.fallback.devices.<id>.localFallbackis a per-device permission GOAT reads (docs: goatclaw hub design on OpenCodex Remote Link goat-si/goat#684 ADR-0021 §1.6, "local fallback opt-in per device").
