fix(advisor): apply the caller's paid-combo consent and device fallback policy - #7894
An Advisor consultation runs as an internal loopback request. #7718 made that hop carry the originating caller's provider/model admission scope, but two other caller-level policies were still evaluated against the internal hop's own admission. As a result, a key with allowMeteredComboTargets: false, or a device with fallback.devices.<id>.provider: false, could reach a metered combo target or a provider fallback through Advisor that the same caller could not reach directly.
The caller's paid-combo consent and device fallback policy are captured once, as an immutable credential-free snapshot (src/server/capture-admission-policy.ts). They are intersected with the host's own limits for every combo pick (including router-collapsed picks), nested JEV decision turn, child invocation and fallback hop inside the consultation. If no destination remains, the worker receives the existing "advisor unavailable" notice.
The snapshot is bound to the consultation through a process-local registry (src/server/advisor-admission-scope.ts). Its random handle is honored only beside the current process-owned internal capability; unknown or missing handles are refused, external callers cannot forge or widen it, and completion, failure or abort releases it.
Capture happens at HTTP authentication in src/server/index/serve-options.ts, before the inbound-body capacity wait and before any body read. The snapshot is then carried, rather than re-read from mutable config, through:
Claude Messages replay
detached JEV decision turns
routed vision helpers (authenticated loopback, intersected with host limits)
policy-profile retries and WebSocket turns
compact Responses and JEV handoffs
Internal Advisor hops on Chat Completions keep inheriting their registration.
Docs: reference/configuration/advisor.md; structure: structure/advisor.md and the owning fallback, protocol and transport contracts.
Verification
Bun 1.4.4. Five independent security review rounds. Rounds 1–4 each found a path that re-captured policy after a config change (Messages body read, HTTP capacity wait, detached JEV turn, then vision/WS retry/compact), and each is now a regression. Round 5: PASS, 0 blocking, including an independent audit of every Advisor-reachable call site.
Routing regressions (tests/advisor/advisor-caller-policy-routing.test.ts, advisor-caller-policy.test.ts) fail on dev (5 pass / 11 fail) and pass here. The ingress, HTTP-capacity, JEV and R4 regressions (advisor-caller-policy-ingress/-http/-jev plus the vision, WS-retry and compact cases) each fail on the head before their fix and pass after.
Advisor, consent and scope suites: 281 pass / 0 fail. JEV suites: 423 pass / 0 fail. Transport, vision, compact and WS suites: 1574 pass / 1 skip (an existing runtime-capability skip), using --isolate for a process-global mock collision that reproduces unchanged on dev.
Layout, layout tooling and file-size guards 38 pass / 0 fail. bun run typecheck, bun run structure:check, bun run privacy:scan and the docs build pass.
Full-suite exception: the full local suite and a completed bun run test:changed were not run because several concurrent worktrees share this host's test runner. The focused coverage above spans every touched path. Live paid-provider traffic was not exercised.
Checklist
Scope stays focused and avoids unrelated cleanup.
Docs or release notes were updated when needed.
Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.
Advisor consultations enforce the caller’s admission-time model access, paid-combo consent, and device fallback limits alongside host restrictions.
These limits persist across retries, nested consultations, routed vision, compact requests, and WebSocket turns. Policies are captured before request-body handling, and later configuration changes cannot broaden access.
Consultations blocked by applicable limits return an unavailable notice; internal requests without valid authorization are rejected.
Documentation
Clarified which credentials determine Advisor host access and when API-key order affects availability.
Recover the caller-policy implementation carried in c7e80fa and separated from
the admission-scope change by f77b3d0. Capture immutable caller policy before
async request parsing and register a private per-consultation handle behind the
existing process capability. Intersect caller and host limits at shared model,
metered-combo and fallback admission guards, including nested decision turns.
Reject missing or unknown internal policy; ignore external policy metadata.
Revoke registry handles immediately on abort and release on every settled exit.
Add routing, immutability, forgery, host-limit and lifecycle regressions and
document the policy contract.

Claude Messages read the body before replaying through handleResponses,
which then captured the Advisor caller policy from the replay config. A
config change during the body read could widen paid-combo consent or drop
the device fallback policy. Capture the admitted policy synchronously at
Messages ingress, as Chat already does, and pass it into the replay.
With maxInboundBodyBytes above the default ceiling, runAdmittedHttpTurn
waits for inbound-body capacity before calling the handler, so the
Responses, Chat and Messages handlers could capture the Advisor caller
policy from a config widened during that wait. Capture it in
serve-options right after authentication and hand the snapshot to each
handler; an internal Advisor hop still inherits its registration.
The JEV decision invoker forwarded admission and scope but not the
Advisor caller policy, so the detached decision turn re-captured it from
mutable config while the parent was suspended. Forward the parent's
snapshot into the child handleResponses call.

Walkthrough
This change captures caller admission policy before request-body handling and carries it through Advisor consultations, retries, nested calls, and routed vision. Routing checks caller policy together with host limits for model scope, metered combo targets, and fallback hops.
Adds immutable captured policies and applies model-scope, metered-target, and fallback checks alongside host limits.

Explanation
Direct issue [#7859] has coding requirements. src/server/capture-admission-policy.ts:15-45 creates an immutable caller-policy snapshot and intersects model scope, metered consent, and fallback policy with host inputs. src/server/admission-model-scope.ts:80-112,127-139 enforces intersected scopes and metered-combo consent. src/server/fallback-policy.ts applies captured device policy at fallback-hop checks. src/server/index/serve-options.ts captures the policy after authentication and before body capacity or body reads. The snapshot propagates through Advisor consultation, Messages replay, JEV turns, child invocations, compact Requests, routed vision, policy retries, and WebSocket turns. src/advisor/consult.ts:175-181 registers the snapshot, and src/server/chat-completions.ts:159-188 validates the capability and applies the snapshot to internal hops. When policy intersection removes all destinations, the existing unavailable response remains used. The focused tests cover combo picks, nested decisions, child calls, fallback hops, ingress timing, scope intersection, capability validation, self-fetch credentials, and unavailable behavior. The reported full-suite exception and absence of live paid-provider traffic do not establish an unmet coding requirement.

Explanation
The changes remain within [#7859]. Source changes implement caller-policy capture, immutable process-local registration, host-limit intersection, metered-combo enforcement, device-fallback enforcement, and propagation through Advisor-reachable paths. The self-fetch credential changes in src/lib/local-destinations.ts:151-194, src/advisor/consult.ts:141-181, and src/server/chat-completions.ts:159-188 prevent internal loopback admission from dropping or widening the required caller and host policy. Added tests directly verify these security and routing paths. Documentation, structure contracts, layout mappings, and translated Advisor pages document or support the same behavior. No unrelated feature or cleanup is demonstrated.

Key order only matters when neither OPENCODEX_API_AUTH_TOKEN nor a service token is set. Add the caller-scope, paid-combo, device fallback and policy-capture guidance to all seven locales.
A loopback listener (loopback bind or the secondary loopback listener) admits
Advisor's internal self-fetch before reading any token, so the host snapshot
carried neither the selected admission key's model scope nor its paid-combo
consent or fallback device identity. A caller whose own scope allowed the
Advisor model got a consultation that Advisor's own key forbids.
After the internal capability and policy registration are validated, Chat now
resolves the presented x-opencodex-api-key and intersects that key's captured
policy with the listener snapshot and the caller registration. An unknown
presented credential is refused. Other loopback callers are unchanged.

Coordination note on the two new CodeRabbit Major findings against src/server/capture-admission-policy.ts (r4408838184, r4408838188).
Both concern the self-fetch credential binding added in 9947e13 (now at b311ba459), not the earlier lane commits. I am not pushing to this branch while you are iterating on it, to avoid racing your commits.
r4408838188 (service-token file on a loopback bind) looks like a real availability regression. When no environment token is set, localAdmissionToken returns the service-token file. A loopback bind can leave that file out of this proxy's admission credentials. Before 9947e13 the presented key was ignored on this path. Now captureSelfFetchCredentialPolicy returns null and every Advisor consultation gets a 403. The suggested gate, sending x-opencodex-api-key from consultAdvisor and postLocalChatCompletion only when destination(config).requiresAdmissionToken, keeps the fail-closed refusal for a credential that is explicitly presented.
r4408838184 (metered consent on the secondary loopback listener) only narrows: a consultation can be denied a metered target that the same key reaches directly. It is not a widening. Restoring consent needs both the merged policy and the loopback branch of admissionMayUseMeteredComboTargets to account for the validated key, while still requiring meteredRequiresComboOptIn.
Happy to take either as a follow-up commit once you say the branch is free.
…ted credentials (#7894)
On a public bind, the secondary loopback listener admits an Advisor or
routed-helper self-fetch as loopback, and loopback on a public proxy never
consents to metered combo targets. A consenting caller and consenting host
key therefore could not reach a metered target the combo opted into, unlike
a direct request with that key. When a loopback listener admitted the hop,
selfFetchHostPolicy now takes paid-combo consent from the validated key, and
the final metered check defers to the captured caller policy on a
loopback-admitted internal hop. The combo opt-in and the caller's consent
still bind; unkeyed loopback callers are still refused; transport identity
is unchanged.
On a loopback bind, startup can leave the installed service-token file (a
hub client key) out of this proxy's admission credentials, but both
self-fetch callers presented it anyway and Chat refused every consult as an
unrecognized credential. selfFetchAdmissionToken keeps the ladder unchanged
where the destination requires admission and otherwise sends only the first
ladder credential this data plane recognizes, so a restricted configured key
still binds. An unknown credential that is presented is still refused.

Summary
Closes #7859. Follow-up to #7718.
An Advisor consultation runs as an internal loopback request. #7718 made that hop carry the originating caller's provider/model admission scope, but two other caller-level policies were still evaluated against the internal hop's own admission. As a result, a key with
allowMeteredComboTargets: false, or a device withfallback.devices.<id>.provider: false, could reach a metered combo target or a provider fallback through Advisor that the same caller could not reach directly. The caller's paid-combo consent and device fallback policy are captured once, as an immutable credential-free snapshot (
