fix(kiro): emulate structured output instead of refusing it - #8071
Kiro now emulates structured output instead of refusing it. Before this change, any request carrying Responses text.format json_schema/json_object, or the Messages output_config.format that maps to it, was rejected locally with HTTP 400 Kiro does not support Responses structured output (src/adapters/kiro/conversation.ts). That broke every caller that only wants a small JSON answer:
T3 Code titles every Claude thread with a Messages request carrying output_config.format (a {title: string} schema) on the thread's model. On a Kiro model the title request failed twice per thread, and T3 Code kept the first line of the message as the title.
Claude Code's own structured-output calls and Codex text.format requests routed to Kiro failed the same way.
The refusal protected a real promise: a caller expecting parseable JSON must never receive prose. The emulation keeps that promise.
Terminal tool: a turn with a structured-output format advertises a private tool, codex_kiro_structured_output. Its input schema is the caller's schema, cleaned the way kiro-tools cleans every Kiro tool schema. The original schema is also restated in the injected instructions when it fits the instruction budget, because the cleaning strips keywords such as additionalProperties and pattern.
Composed or non-object roots: an array, scalar, or root anyOf/oneOf/allOf/$ref is wrapped as {value: <schema>}, with $defs/definitions hoisted so references still resolve. The flattening kiro-tools applies to ordinary tools is not reused, because it would change the meaning of an output contract. json_object becomes {type: "object"}.
Terminal channel: the new tool replaces codex_kiro_final_answer as the way the turn ends, and the turn always runs in required mode, even with no client tools. Client tools are still offered ahead of it.
Stream: a valid call's input is sent to the client as one final-answer text block, and no tool call is surfaced. The check is deliberately light: the JSON must parse, the root must match the schema's declared type (including the branch types of a root anyOf/oneOf), and the root's required keys must be present.
Bounded fallback:
A plain-text reply that already is the requested JSON (optionally in one whole-message code fence) is accepted with no retry, if the model stopped cleanly. A MAX_TOKENS cut is never accepted.
Prose, or a tool input that fails the check, gets exactly one retry with a dedicated instruction.
If the retry fails too, the turn ends with a non-retryable invalid_kiro_structured_output error. Held prose is never sent to a structured-output caller on any path.
Reserved names: both private tool names are reserved against client tools. A call to the one the turn didn't advertise is a protocol error.
The decision is recorded in structure/decisions/ADR-8071-kiro-structured-output-emulation.md, with behaviour in structure/providers/kiro.md and docs-site/src/content/docs/reference/adapters.md. The Cursor adapter's emulation (src/adapters/cursor/structured-output.ts) is prompt-only; this one adds a tool channel and a check because of the promise above.
Combined with ##8072: the two PRs merge cleanly in either order (all four open Kiro/T3 PRs, #8070–#8073, were checked pairwise and merged together). On the combined tree, bun run typecheck, bun run structure:check, ./tests/providers/kiro (737 pass) and the Kiro server end-to-end set (114 pass) all pass. On that tree a refused structured-output turn ends as the non-retryable content_filter refusal, ahead of any structured-output handling. One edge case is left for a follow-up once both have landed: when the bounded structured-output retry is itself refused, the retry's partial text is released as commentary before the refusal. The turn still ends as a refusal, so it is never accepted as a JSON answer.
Verification
All results below are for head 5e8aee0d2.
Review follow-ups (CodeRabbit):
A failed structured answer is retried only after a clean stop (no stop reason, END_TURN, STOP_SEQUENCE). Under MAX_TOKENS, a content filter or an exhausted context window, the stop's own terminal applies.
The bounded retry accepts JSON text only after a clean stop, and an empty or reasoning-only retry ends with the non-retryable invalid_kiro_structured_output instead of a retryable empty fallback. Three regression tests cover these; all fail on the previous head.
src/adapters/kiro/ is mapped to structure/providers/kiro.md in structure/manifest.json, and structure/INDEX.md is regenerated.
A malformed structured tool input on the bounded retry ends with the non-retryable invalid_kiro_structured_output instead of the retryable malformed_kiro_completion.
A truncated structured tool input on the first attempt earns the bounded retry, like one that fails to parse. Under MAX_TOKENS it keeps max_output_tokens with no retry. All three new tests fail on the previous heads.
The same applies when the stream ends with the structured call still open (no tool-stop frame). Only that case bypasses the end-of-stream guard; every other open call keeps it.
Not changed: a complete, validated structured answer still takes precedence over a later content-filter stop. That is dev's existing completion precedence for every Kiro turn; the reasoning is in the PR conversation, and CodeRabbit withdrew the request. A change to that precedence should cover ordinary and structured Kiro completions together.
Doc comments on the functions this PR touches, for CodeRabbit's docstring-coverage pre-merge check (it requires 80% of touched functions with branches). Comments only, no behavior change; the results below were re-run on this head.

The new end-to-end file covers T3 Code's exact title request on /v1/messages (streaming and non-streaming), prose twice giving an error that doesn't contain the prose, and /v1/responses json_schema. The composed-root type check has a test that fails without it.
Live checks against the Kiro runtime, using a probe that runs this branch's adapter directly. Each line is a single request:
Light check: deep schema constraints are not validated.
Final failure: it is non-retryable inside the proxy, but on Messages it surfaces as the 502 overloaded_error other Kiro model-output errors use. Claude Code may retry it, at up to two Kiro calls per attempt.
Full-suite scope exception:bun run test was not run locally. The machine (WSL2, 16 GB shared with other work) runs out of memory on the full suite, so only the domains above were run. The rest of the suite is left to CI.

Summary by CodeRabbit
New Features
Kiro supports structured JSON output requests in Responses and Messages, including schema-based formats.
Valid JSON is returned as text. After invalid output on a clean stop, Kiro retries once; if no valid result is produced, the request fails with a structured-output error.
Output-limit, content-filter, and context-window stops retain their specific errors.
Documentation
Updated Kiro guidance to describe supported formats, retry behavior, and validation limits: validation checks JSON syntax, root type, and required root properties, but not every schema keyword.
Kiro has no schema-constrained response mode, so the adapter refused text.format json_schema/json_object. That broke every caller asking for a small JSON answer: T3 Code titles each thread through a Messages output_config.format request, and Claude Code's own structured-output calls take the same path.
A structured-output turn now advertises a private terminal tool whose input schema is the caller's format, and relays the call's input as the assistant's JSON text. Non-object or composed roots are wrapped under value. The client receives JSON that parsed and passed a light root check, or a non-retryable error after one bounded retry; prose never reaches a structured-output caller.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
No actionable comments were generated in the recent review.
Walkthrough
Kiro now emulates structured-output requests with a private terminal tool. It validates tool input and eligible JSON text, retries once after invalid output on a clean stop, and returns a non-retryable error if the retry does not produce valid output.

Schema and request payload src/adapters/kiro/structured-output.ts, src/adapters/kiro/payload.ts, src/adapters/kiro-constants.ts, src/adapters/kiro-tools.ts, src/adapters/kiro-wire.ts
The adapter derives a schema-backed terminal tool, builds its instructions, and reserves its name against client-tool collisions. Validation checks JSON syntax, admitted root type, and required root keys; it does not cover every schema keyword.
Adapter request and response routing src/adapters/kiro/adapter.ts, src/adapters/kiro/conversation.ts, src/server/responses/response-effects.ts, src/types/request.ts
The adapter accepts structured-output requests and carries the specification from request construction into stream and response parsing. Request and compaction comments describe Kiro’s emulation path.
Stream validation and bounded retry src/adapters/kiro/stream.ts
The parser validates tool input or eligible JSON text, drops held prose on structured-output paths, and retries once when invalid output follows a clean stop. Non-clean stops retain their existing terminal behavior. A failed retry returns invalid_kiro_structured_output.
Adapter and server tests cover tool and JSON-text responses, retries, failures, and API output. Provider and reference documentation describe the emulation and its validation limits.

The title "fix(kiro): emulate structured output instead of refusing it" matches the main change in the changeset. The PR replaces Kiro's local HTTP 400 refusal of structured-output requests with a private-tool emulation, which the structured-output module, payload, stream, and docs all implement. The title is concise, specific, and free of file lists or vague terms. It does not mention the retry or the reserved tool name, but the task says it need not cover every detail.

Review follow-up. A failed structured answer under MAX_TOKENS, a content filter or an exhausted context window now keeps that stop's own terminal instead of retrying into the same limit or verdict. The bounded retry accepts JSON text only after a clean stop as well, and an empty or reasoning-only retry ends with the non-retryable invalid_kiro_structured_output failure rather than a retryable empty fallback. Also maps src/adapters/kiro/ to the Kiro structure doc.

Make malformed structured-output retries non-retryable.

Make malformed structured-output retries non-retryable.
If the first private-tool input fails schema validation without emitting text, the adapter starts its bounded retry with priorEmittedOutput false. If that retry sends incomplete JSON such as {"title":, flushOpen calls protocolTerminal with malformedCompletion true. This branch then returns a retryablemalformed_kiro_completion incomplete event. The outer stream preserves that flag, so the client can replay a turn that has already used its bounded retry. Return kiroStructuredOutputFailure(message, usage()) for this structured-output fallback, while retaining the ordinary completion behavior. Add a regression case with invalid first-attempt input and malformed retry input. (raw.githubusercontent.com)

Handle refusal stops before structured-output success paths.
881-885: Major · Functional Correctness · Quick win
Handle refusal stops before structured-output success paths.
For structured-output requests, CONTENT_FILTERED and GUARDRAIL_INTERVENED must produce the non-retryable content_filter terminal. The current completion branches run first, so a valid private-tool answer can emit final_answer and done. On the bounded retry, prose reaches kiroStructuredOutputFailure and returns invalid_kiro_structured_output instead of the refusal.
Move one shared refusal gate before both completionAnswer handling and the text_fallback validation. Keep the existing priorAttempt.drainDeferred(...) call before that gate so held prose remains suppressed.

Rewrite root-relative # references when the schema is wrapped under value.
66-81: Major · Data Integrity & Integration · Heavy lift
Rewrite root-relative # references when the schema is wrapped under value.
When a non-plain root is wrapped, the sanitized schema moves under properties.value, and the wrapper becomes the document root. The sanitizer keeps $ref values unchanged, and no step rewrites "#". A "$ref": "#" inside the caller's schema therefore resolves to the wrapper object, which requires a value key. For example, with {"anyOf":[{"type":"string"},{"type":"array","items":{"$ref":"#"}}]}, a nested array item such as "a" is checked against the wrapper and fails, even though the caller's schema accepts it. The advertised tool schema then contradicts the caller's schema for nested values.
The current $defs/definitions hoist covers only #/$defs/... and #/definitions/... references. It does not cover root-relative #.
Schema-aware correction:
Before wrapping, rewrite only $ref values that equal "#" to "#/properties/value", and only in schema positions. Walk schema-valued keywords (properties, items, prefixItems, additionalItems, additionalProperties, patternProperties, anyOf, allOf, oneOf, not, if, then, else, dependentSchemas, $defs, definitions).
Do not descend into const, enum, default, examples, or other literal values. Instance data such as {"$ref": "#"} must remain unchanged.
Do not rewrite inside a subschema that declares its own $id. Inside that resource, # refers to the resource root.
If the inner root declares $id, # already refers to the inner root. Do not rewrite it.
Leave other $ref values, such as #/$defs/..., unchanged.
Add a regression test in tests/providers/kiro/kiro-structured-output.test.ts. Cover a nested items: {"$ref": "#"} under a wrapped root, a literal const or enum containing {"$ref": "#"}, and a nested $id resource that contains #.

66-67: Minor · Data Integrity & Integration · Quick win
Keep literal schema values opaque during sanitization.
For a json_schema request with const: {"format":"pdf"}, sanitizeKiroSchema removes format from the literal value and advertises const: {} to Kiro. Review follow-up. A bounded retry whose structured tool input is malformed (for example truncated JSON) returned the retryable malformed_kiro_completion, so a client could replay a turn that had already used its one retry. Structured-output turns now end with the non-retryable invalid_kiro_structured_output; ordinary completion behavior is unchanged.
@coderabbitai on the two follow-up comments in your review of 0b3529408:
1. Make malformed structured-output retries non-retryable: fixed in 9e0a839e0. In text_fallback, protocolTerminal(..., true) now returns kiroStructuredOutputFailure(message, usage()) for a structured-output turn, and the ordinary completion path still returns the retryable malformed_kiro_completion. The regression test "a malformed structured tool input on the retry is non-retryable" sends an invalid first input and then truncated JSON ({"title":), and asserts two sends, no text, and a non-retryable invalid_kiro_structured_output. It fails on 0b3529408.
2. Handle refusal stops before structured-output success paths: not changed in this PR. There are two cases:
A complete, validated private-tool answer followed by CONTENT_FILTERED: the validated answer wins. That precedence is dev's existing behavior for every Kiro completion: on dev the completionAnswer branch already runs before the stop-reason mapping. fix(kiro): report content-filter stops as non-retryable refusals #8072 keeps it explicitly (completionAnswer === undefined && !sawRealTool && isKiroContentFilterStop(...)). Making structured output the one case where a validated answer loses to a later stop would split the completion contract. If maintainers want refusal-over-answer precedence, it should change for all Kiro completions together, in a separate change.

@mushikingh Thanks for separating the two cases. The source supports your explanation.
1. Malformed structured-output retry: the fix addresses the finding.
At commit 9e0a839e0, src/adapters/kiro/stream.ts, Line 464–476, routes malformed completion input in text_fallback to kiroStructuredOutputFailure(...) when structuredOutput is set. Ordinary completions retain the malformed_kiro_completion path.
The regression test in tests/providers/kiro/kiro-structured-output.test.ts, Line 342–348, covers the invalid first input and truncated retry. It asserts two sends, no text, and non-retryable invalid_kiro_structured_output. I inspected the test but did not execute it.
2. Refusal handling: my request was too broad.
Filtered prose without an accepted answer:#8072 adds the refusal checks to both the main path and text_fallback. Those checks require completionAnswer === undefined && !sawRealTool. This belongs to #8072; I do not recommend duplicating the checks here.
A validated private-tool answer followed by CONTENT_FILTERED: the current success branches at src/adapters/kiro/stream.ts, Line 890–897 and Line 965–970, accept that answer. The #8072 patch explicitly preserves this precedence. I withdraw the request to introduce refusal-over-answer precedence only for structured output.
A change to that precedence should cover ordinary and structured Kiro completions together. I have not independently reproduced the combined-tree fixture results. Combined-tree CI should retain coverage for refusals on both attempts and for validated-answer precedence.

Retry malformed structured tool input on the first attempt.
For a required structured-output request, a private tool call whose input is incomplete JSON, such as {"title":, fails isCompleteKiroToolInput in flushOpen at src/adapters/kiro/stream.ts:615-616. That check runs before structuredRetry is set. protocolTerminal then returns a non-retryable invalid_kiro_completion after one upstream request, so the documented bounded retry does not run.
Defer this decision to the stop-reason check, as the parse-failure branch already does at lines 624-626. A clean stop then requests the retry at line 985. Non-clean stops keep their existing handling. A MAX_TOKENS stop still returns incomplete with max_output_tokens and no retry.
Add a regression test next to the existing retry tests in tests/providers/kiro/kiro-structured-output.test.ts. Cover a first-attempt malformed input with a clean stop, which must retry once. Keep the existing MAX_TOKENS case, which must return incomplete with reason: "max_output_tokens" and no retry.

Review follow-up. A first-attempt structured tool call whose input is truncated JSON failed the completeness check before structuredRetry was set, so it ended with a non-retryable invalid_kiro_completion after one request instead of the documented bounded retry. It now takes the same path as an input that fails to parse: one retry on a clean stop, and the stop's own handling (for example max_output_tokens) otherwise.
@coderabbitai on the outside-diff comment in your review of 9e0a839e0 ("Retry malformed structured tool input on the first attempt"): fixed in ebbbe7223. In flushOpen, a required-mode structured-output call whose input fails isCompleteKiroToolInput now counts as a completion call and sets structuredRetry, the same path as an input that fails to parse. The retry is then decided with the stop reason: a clean stop retries once, and other stops keep their own handling. Two regression tests were added and both fail on 9e0a839e0:
"a truncated structured tool input on the first attempt earns the bounded retry": {"title": with END_TURN, then a valid call; asserts 2 sends and the JSON answer.
"a truncated structured tool input under MAX_TOKENS is not retried": asserts 1 send and incomplete / max_output_tokens.
A truncated input on the retry itself still ends with the non-retryable invalid_kiro_structured_output (from 9e0a839e0).

@mushikingh The source at ebbbe7223 addresses this finding.
In src/adapters/kiro/stream.ts, Line 615–624, flushOpen now increments completionCalls and sets structuredRetry when incomplete input belongs to a required-mode structured-output completion call. This avoids the previous immediate protocol failure.
At Line 988–995, the retry still requires a clean stop. The new branch does not request a retry directly, so other stop reasons retain their existing handling.
At Line 464–468, malformed structured-tool input in text_fallback still returns kiroStructuredOutputFailure(...). No additional retry is introduced.
The regression tests in tests/providers/kiro/kiro-structured-output.test.ts cover the three outcomes:
Line 342–349: truncated input with END_TURN produces two sends and the valid JSON answer.
Line 351–358: truncated input with MAX_TOKENS produces one send, no text, and incomplete / max_output_tokens.
Line 360–367: malformed input on the retry produces two sends, no text, and non-retryable invalid_kiro_structured_output.
I inspected the code and assertions. I did not execute the tests or independently verify that the new tests fail on 9e0a839e0.

Review follow-up. When the stream ended with the private structured-output call still open and its input incomplete, the end-of-stream guard returned invalid_kiro_completion before flushOpen could schedule the bounded retry. A required-mode structured call now goes through flushOpen there, so it retries on a clean stop and keeps max_output_tokens under MAX_TOKENS. Every other open call keeps the guard's terminal, and the retry itself still ends non-retryably.
CodeRabbit's docstring-coverage pre-merge check (80% of touched functions with branches) reported 52%. Adds doc comments to the touched adapter, stream, wire, conversation and structured-output functions and to the test helpers; no behavior change.

…chema literals
Strict json_schema formats are now validated against the whole schema (OpenAI strict subset plus simple bounds), and a strict schema with an assertion the emulation cannot check is refused before sending. Wrapping re-points root-relative $ref targets at #/properties/value, and Kiro schema sanitization copies const, enum, default and examples verbatim.
Strict schemas are prepared once at the boundary into a validation graph. $ref is limited to root $defs/definitions, empty reference cycles are refused, and one visit budget and depth limit apply per value. Numbers are compared by their original text, multipleOf must be a positive integer, and pattern is compiled at the boundary with backreferences and repeated quantified or alternating groups refused. The wrapped strict form relays the original bytes of value.
Maintainer integration into dev (account lidge-jun) at head 670ddeb653287569f07337139c94aaffd4de255b. Moved out of draft by a maintainer for this integration (the checklist gate re-drafted it after maintainer follow-up pushes).
fix(kiro): emulate structured output instead of refusing it, in a narrowed form. Strict mode validates nested properties and additionalProperties, refuses root $ref: "#", and preserves const literals. Strict mode also refuses constructs whose contract it cannot honour safely: regex pattern/patternProperties (unbounded synchronous matching), and numeric const/enum/multipleOf (the adapter only sees parsed JSON numbers, so a rounded literal could accept a wrong value). Numeric bounds are kept for safe integers and decimals up to 15 significant digits; the docs-site Kiro page states this limit. Non-strict behaviour is unchanged.
The branch contains current dev; exact-head required CI passed: ci, enforce-target, hygiene, test 1/4–4/4.
Fresh final and security reviews (gpt-6.1-terra, high) PASS on this head; earlier review findings (regex DoS, numeric rounding, schema validation gaps) are fixed with regressions. No unresolved threads.

Retry rejected structured calls that stop with TOOL_USE
When a structured-output call fails schema validation and Kiro ends it with TOOL_USE—the tool-call stop exercised by tests/providers/kiro/kiro-completion-retry-wording.test.ts:54—structuredRetry is true but cleanStop is false. The parser skips the bounded retry and falls through to kiro_tool_use_without_call with retryable: false, even though it received the private tool call. A correctable missing property or wrong type therefore fails after its first attempt. Permit TOOL_USE when a rejected structured call was actually observed, while keeping token-limit, filter, and context-limit stops on their existing terminal paths.

Validate integer roots using the original number lexeme
For a non-strict json_schema with an integer root, both parseKiroStructuredToolInput(spec, '{"value":9007199254740992.5}') and the equivalent JSON-text answer are accepted: JSON.parse rounds the value to an integer before this predicate runs, while the relay returns the original fractional lexeme. A root 1e-999 similarly underflows to zero and passes. This bypasses even the advertised light root-type check and the corrective retry, allowing a caller to receive a non-integer for an integer schema. Use the lexeme-preserving reader and isKiroDecimalInteger for non-strict numeric root checks too.

Summary
Kiro now emulates structured output instead of refusing it. Before this change, any request carrying Responses
text.formatjson_schema/json_object, or the Messagesoutput_config.formatthat maps to it, was rejected locally with HTTP 400Kiro does not support Responses structured output(src/adapters/kiro/conversation.ts). That broke every caller that only wants a small JSON answer:output_config.format(a{title: string}schema) on the thread's model. On a Kiro model the title request failed twice per thread, and T3 Code kept the first line of the message as the title.text.formatrequests routed to Kiro failed the same way.The refusal protected a real promise: a caller expecting parseable JSON must never receive prose. The emulation keeps that promise.
codex_kiro_structured_output. Its input schema is the caller's schema, cleaned the waykiro-toolscleans every Kiro tool schema. The original schema is also restated in the injected instructions when it fits the instruction budget, because the cleaning strips keywords such asadditionalPropertiesandpattern.anyOf/oneOf/allOf/$refis wrapped as{value: <schema>}, with$defs/definitionshoisted so references still resolve. The flatteningkiro-toolsapplies to ordinary tools is not reused, because it would change the meaning of an output contract.json_objectbecomes{type: "object"}.codex_kiro_final_answeras the way the turn ends, and the turn always runs inrequiredmode, even with no client tools. Client tools are still offered ahead of it.anyOf/oneOf), and the root'srequiredkeys must be present.MAX_TOKENScut is never accepted.invalid_kiro_structured_outputerror. Held prose is never sent to a structured-output caller on any path. The decision is recorded in
structure/decisions/ADR-8071-kiro-structured-output-emulation.md, with behaviour instructure/providers/kiro.mdanddocs-site/src/content/docs/reference/adapters.md. The Cursor adapter's emulation (src/adapters/cursor/structured-output.ts) is prompt-only; this one adds a tool channel and a check because of the promise above.Combined with ##8072: the two PRs merge cleanly in either order (all four open Kiro/T3 PRs, #8070–#8073, were checked pairwise and merged together). On the combined tree,
bun run typecheck, bun run structure:check,./tests/providers/kiro(737 pass) and the Kiro server end-to-end set (114 pass) all pass. On that tree a refused structured-output turn ends as the non-retryablecontent_filterrefusal, ahead of any structured-output handling. One edge case is left for a follow-up once both have landed: when the bounded structured-output retry is itself refused, the retry's partial text is released as commentary before the refusal. The turn still ends as a refusal, so it is never accepted as a JSON answer.Verification
