fix(anthropic): keep pool session affinity across restarts and transient detours - #7828
One Claude Code session should stay on one Anthropic pool account until that account is really unusable. On current dev two ordinary events moved a session to another account (#7813):
Restart. Session bindings lived only in an in-memory map, so every proxy restart re-placed running sessions by strategy, often on a different account and with a cold prompt cache.
Transient detour. A request-rate 429 already pauses the account briefly and serves one request on a sibling. The original binding survived only while that pause was still active at commit time; if the pause expired while the sibling's credential or commit was awaited, the sibling permanently replaced the binding although there was no exhaustion evidence.
Changes:
commitAnthropicSelectionRouting keeps an established, still-usable binding when a request is served by a temporary alternate (detour or model-route exclusion), whatever the pause timing, and refreshes its idle timestamp. Shared-quota and entitlement refusals, pause, re-authentication, removal and operator resets still invalidate it explicitly, as before. A restored binding also takes precedence over the startup-derived one-dispatch manual preference for its own session; an unbound first dispatch still honours the saved manual choice.
New src/oauth/anthropic-session-affinity-store.ts persists bindings per config home and Anthropic instance. Rows hold only a domain-separated SHA-256 digest of the session key, the opaque account id, lastUsedAt and a hashed persistent loginId fence; no raw session key, bearer, email or label is written. Loads enforce schema, size, TTL, future-time and the existing 24 h / 2,000-entry bounds, and restore a row only while its account exists, is usable and still has the same login. Ordinary token refresh keeps affinity; a replacement login retires it. Writes use the existing atomic publisher after ACL settlement, coalesce timestamp-only touches for one second and flush on shutdown; failures warn without session identifiers.
Local, Windows, Bun 1.4.4, focused files only (shared host under load):
New tests/adapters/anthropic/anthropic-affinity-restart.test.ts: on unpatched dev the restart case and the pause-expiry-before-commit case both fail (expected account A, received B); the manual-selection guard passes on both.
New tests/adapters/anthropic/anthropic-affinity-store.test.ts: digest-only bytes, TTL/cap, malformed and oversized snapshots, instance isolation, removal/re-add, login replacement versus token refresh, failed write keeps the previous snapshot.
Implementation run: 14 files (both new files plus account-pool, 429-policy, sidecar-failover, combo-cooldown, model-routes, weekly-admission, pause, instance-isolation, test-layout, test-layout-tooling, file-size ratchet, core-lab boundary): 314 pass, 0 fail. After merging current dev: 7 files, 149 pass, 0 fail.
bun run typecheck, bun run structure:check, bun run privacy:scan: pass.
NOT RUN: full bun run test locally (left to CI), live Anthropic traffic, real process-restart E2E, docs-site build.

Summary by CodeRabbit
New Features
Anthropic account affinity now persists across proxy restarts for the same configuration and provider, expires after 24 hours of inactivity, and retains up to 2,000 bindings. Stored snapshots contain digests rather than raw session keys or tokens.
Token refreshes and temporary rate-limit detours preserve the original binding. Manual account selection, replacement login, account removal, or reauthentication can invalidate it; shared-quota rejections may result in a new binding.
Documentation
Updated Claude Code guides in multiple languages and added details about account-affinity behavior.
Anthropic session affinity now persists across proxy restarts for the same configuration home and provider instance. The store limits bindings by age and count. Routing preserves or invalidates bindings based on account state and selection events.

Summary
Persist and validate affinity snapshots src/oauth/anthropic-session-affinity-store.ts, tests/adapters/anthropic/anthropic-affinity-store.test.ts
The store persists session and login digests, filters entries by account eligibility and timestamps, and limits snapshots to 2,000 entries with a 24-hour idle lifetime. Tests cover snapshot contents, validation, isolation, write failures, and flush behavior.
Routing looks up affinity by an instance-scoped digest and preserves eligible bindings across transient detours. Replacement login, removal, pause or reauthentication, and runtime manual selection invalidate bindings. Rate-pause tracking supports preservation after a transient pause clears. Tests cover restoration, detours, manual selection, and test setup; documentation describes the behavior.

Explanation
[#7813] Most requirements are implemented at head 7c0b3be.
Implemented:
Restart persistence: src/oauth/anthropic-session-affinity-store.ts stores per-instance digest-only snapshots (anthropicAffinityDigest, anthropicAffinityPath). Bindings load only while the account exists, is usable, and has the same login fence.
Bounds: AFFINITY_IDLE_TTL_MS (24 h) and MAX_AFFINITY_ENTRIES (2,000) are enforced on load, get, invalidate, and trim.
Request-rate 429 detour: in src/oauth/anthropic-routing.ts, preservePausedAffinity (hunk @@ -1309,16 +1291,23 @@) keeps an established binding when an alternate serves the request. It uses anthropicRatePausedRecently, so pause expiry before commit no longer rebinds. Exhaustion of the bound model still rebinds.
Invalidation: shared-quota and entitlement paths still call clearAnthropicSessionAffinityForAccount. onPause, onSelection, and the reconcile path call sessionAffinity.invalidate.
Manual selection: a restored binding takes precedence over the startup manual preference (if (manualPreference && !sessionAffinity.get(...))).
Unmet requirement:
#7813 proposes: "log each rebind with its reason so it can be audited." The head diff of src/oauth/anthropic-routing.ts adds no logging. The commit path binds or replaces the binding at the else bindAnthropicSessionAffinity(options.sessionKey, accountId) branch with no reason. clearAnthropicSessionAffinityForAccount, onPause, onSelection, and the reconcile hunk (@@ -1427,11 +1421,7 @@) also log nothing. The only new log in the store is the identifier-free persistence warning in warn(), which does not record rebind reasons. The PR description does not claim this logging.
Limits of evidence: the incremental diff from the previous reviewed head could not be read (changed blobs missing). The whole-PR diff against the merge base was read and is the basis for this verdict.
Resolution
In src/oauth/anthropic-routing.ts, emit a reason-bearing log for each binding replacement or removal. Cover at least these sites: the else bindAnthropicSessionAffinity(...) branch in the commit path, clearAnthropicSessionAffinityForAccount, onPause, onSelection, and the reconcile invalidation. Use a fixed reason code for each, such as quota-exhausted, entitlement, paused, reauth-required, removed, operator-reset, or replaced. Log the instance and reason only. Do not log raw session keys, digests of session keys, bearer tokens, emails, or labels. If AnthropicSessionAffinityStore.invalidate or set needs to report the reason, have it return the removed reasons so the caller logs them. Add focused tests that spy on the logger and assert one reason-bearing line per applicable rebind path, and that the line contains no session key or account label.

Cache the auth-store snapshot during affinity operations.
getAccountSet() calls uncached loadAuthStore(). valid() calls it twice per row. set() runs prune() across every binding on each affinity hit, and pause and selection handlers call get() for every binding. With 2,000 bindings, these synchronous reads and normalizations can block the request event loop.
Read the account list once per operation and pass it to fence(), valid(), and prune(). Reuse the computed fence in set(). Add a revalidate() method that removes invalid rows with one account snapshot and one change notification, then call it from onPause and onSelection.
Pass the same snapshot through the load loop, prune(), get(), and flushSync(). In set(), compute the fence once and reuse it for both the stored entry and the touch comparison.

… or replaced accounts
Restore the commit-time rule that rebinds unless the bound account was excluded by a model route or detoured around by a transient pause or family rejection, and extend the pause test with a bounded five-minute detour grace so a pause expiring during the alternate's awaits does not rebind (#7813). Measured quota exhaustion and plain recovery candidates rebind again.
…affinity commit
A short throttle that already expired kept the detour grace marker, so committing a replacement for a measured-exhausted bound account still preserved the old binding. Measured exhaustion of the bound account now always rebinds; regressions cover shared and model-specific exhaustion after an expired throttle.
On POSIX the affinity snapshot is written as soon as it changes, so the original home could hold the row before the home switch and the old final assertion failed in CI. Record every publication target instead and assert that rows bound to a home are only ever written under that home.

Rebind when the routed model has exhausted the existing account.
When the bound account is excluded by a model route and anthropicModelExhausted(bound.accountId, options.model) is true, preserveExcludedAffinity still retains the binding. The routed account serves the request, but the session remains bound to the exhausted account instead of the account that served it.
Add the exhaustion guard to preserveExcludedAffinity. This preserves the existing route-only exception for accounts that remain usable for the requested model.

Preserve affinity when the credential store is unavailable.
loadAuthStoreInternal() converts read and normalization failures to an empty store. getAccountSet() then returns null, the same value used for a provider with no accounts. Affinity hydration treats every persisted row as invalid, marks the scope dirty, and can replace the snapshot with an empty file.
Return an explicit unavailable state from the credential-store read and thread it through affinity hydration and invalidation. Preserve the existing snapshot for that state. Keep invalidation for a successful read that confirms account removal.
This affects the persisted bindings for one config home and Anthropic instance. The next request can create a new binding, so the loss is normally recoverable, but established sessions can select a different account until they are rebound. This supports a minor data-integrity issue, not a broad outage.
