Fixes the intermittent Windows lane=all failure in tests/claude-integration/claude-desktop-first-party.test.ts. In run 40389603119, windows 7/9 returned 500 ca_unavailable ("The local intercept certificate could not be created"). The post-failure icacls listing showed the fixture root with normal grants, while its claude-intercept child had an empty DACL. The test file described this as a known hosted-runner flake. It is an in-process race in product code.
Cause.loadConfig() starts an unawaited, optional asynchronous ACL hardening flight on the config root (src/config/paths.tshardenConfigDir). That flight runs /grant:r with an inheritable ACE, then /inheritance:r. The Claude Desktop routes call loadConfig() and then synchronously create and harden intercept secrets under the same root: the local CA directory, the proxy token and the picker CA. Changing a parent's inheritable DACL makes Windows propagate inheritance to existing children. If a child's fresh explicit grant lands while that propagation is in progress, the grant can be lost. The child's own /inheritance:r then leaves it with an empty DACL. The strict ACL check refuses that, and the bare catch in applyDesktopFirstParty reported only ca_unavailable and discarded the cause.
Fix.
withConfigDirHardeningSettled(dir, mutate) (src/config/paths.ts) awaits the config-dir flight, then every overlapping async ACL operation and retained child (withWindowsSecretAclSettled, which now includes ancestors). It runs the synchronous secret write in the same turn as the final check, so no newly queued hardener can start icacls in between.
Every async entry point that writes intercept secrets under the config dir now goes through the barrier: the native toggle PUT, the agent-settings Desktop apply and first-party reconcile, CLI applyDesktop, desired-state reconciliation, intercept startup (CA and token), and the picker controller, runtime and startup authority. This includes the rollback paths.
The swallowed cause is kept as a sanitized, allowlisted diagnostic: { stage: "ca" | "token", cause } (an error code or class name only; never messages, paths, ACL output or secrets). It is surfaced in refusal bodies and one log line. Public reasons and messages are unchanged.
The "known hosted-runner flake" comment is replaced with an accurate description. Owning structure docs (structure/config.md, structure/runtime.md, structure/clients/claude-desktop.md) state the barrier rule.
No retries, longer timeouts, Windows skips or weakened assertions.
Scope. This PR covers the observed failure class: intercept secrets (local CA, proxy token, picker CA, leaf, trust state and journals) and the Claude settings-home files that carry credentials or ACL-hardened data (settings.json migration and removal, model-default staging and backups). On Windows, atomicWriteFile also hardens every temporary file, so the same overlap between the optional config-root flight (src/config/paths.ts) and required child ACL hardening also exists for unrelated files under the config directory, such as config.json saves, the Desktop 3p library and the remote Desktop store. That broader class predates v2.92.0 (#3257) and is tracked as a separate post-release follow-up. This PR does not change it.
Review rounds: r1 F1 (picker cleanup outside the barrier) and F2 (unbounded retained-child wait); r2 F3 (startup migration and Desktop first-party removals for a nested Claude home); r3 F4 (model-defaults rewrite) and N1 (refusal wording). Each finding was fixed with a regression test that fails without the fix. CodeRabbit nits are applied.
Verification
New tests/claude-integration/claude-intercept-acl-barrier.test.ts (13 cases, registered in layout-clients.json and the expected-layout fixture) runs cross-platform. It uses the platform seam, a synthetic principal and fake sync/async icacls runners, and holds the root async icacls runner on a promise.
Red before: on the unmodified base, the native PUT, the agent-settings POST and offline CLI each ran descendant grant and inheritance-removal commands while the root flight was held (3/3 fail). Removing only the barrier makes all 9 scheduling cases fail; removing only the diagnostics makes the 3 diagnostic cases fail.
Green after: 13/13.
Pinned Bun 1.4.4: the barrier regressions, windows-secret-acl-removal-flight, Lab boundary, file-size ratchet and both layout guards pass, 92 pass / 0 fail. tsc --noEmit, structure:check, privacy:scan, skill:surface:check and git diff --check pass.
claude-desktop-first-party.test.ts on this Windows developer host fails the same 19 cases on dev and on this branch. The cause is a pre-existing local baseline: tests share the per-user client lifecycle lock and a shared ACL timeout memo. The target test passes. With fixture-local lifecycle dependencies, the file passes 38/38. That shared test lock is a separate test-isolation issue and is not changed here; the production lock namespace is untouched.
NOT RUN: full local suite (resource exception), Linux/macOS local execution (covered by CI), a replay of the exact Windows-internal propagation interleaving (not schedulable from user space; the in-process overlap it requires is what this change removes).

Summary by CodeRabbit
Bug Fixes
Claude Desktop interception and picker operations now wait for configuration security checks before creating or publishing certificates, tokens, profiles, and model snapshots.
If those checks time out, affected changes are refused and cleanup can complete safely.
First-party settings changes and removals now wait for security checks to settle.
Improvements
Certificate and token preparation failures include a sanitized stage and cause, without exposing sensitive details.
Documentation
Updated guidance on configuration security and Claude interception behavior.
Claude secret-related operations now wait for config-directory hardening and overlapping Windows ACL work. First-party apply failures can include sanitized stage and cause diagnostics. The change also updates picker and management paths, documentation, and integration coverage for settlement barriers and timeouts.
Adds helpers that wait for relevant hardening and ACL work, including tracked ancestor work, before running a synchronous mutation. The settlement uses one deadline and rejects on timeout.
First-party apply results can include allowlisted CA or token stage and cause details. A settlement timeout maps to a CA-stage ETIMEDOUT diagnostic; settled removal returns unreadable on timeout.
Claude writer integration src/claude/desktop-picker.ts, src/claude/intercept/*, src/cli/*, src/server/management/*, structure/clients/claude-desktop.md, structure/runtime.md
Claude startup, picker, CLI, reconciliation, and management paths use settlement helpers. Picker profile and snapshot publication, cleanup, and related operations also wait at their applicable trust or discovery boundaries.
Adds tests for overlapping ACL work, deadlines, secret publication, picker state, management and CLI paths, and sanitized diagnostics. Registers the new test in the Claude integration layout.

Explanation
The title "fix(claude): drain parent ACL flights before intercept secret writes" matches the main change. The raw summary shows a new withConfigDirHardeningSettled barrier in src/config/paths.ts, plus withWindowsSecretAclSettled in src/lib/windows-secret-acl.ts. Intercept CA and token creation, picker state, and related rollback paths now wait for overlapping config-directory ACL work, including ancestor work, before writing secrets. The title is specific and uses the conventional fix(scope) form. It is 68 characters and one sentence, which is within the concise-title guidance. Handle a settlement timeout in the rollback barrier.
Line 437 awaits withConfigDirHardeningSettled(getConfigDir(), rollback) without a guard. This call can throw WindowsSecretAclSettleTimeoutError. The trigger is retained ACL work that outlives the harden deadline after the apply step already passed the barrier.
The exception then escapes applyFirstPartyDesktop. The function skips the saveDesktopMode call and the structured gateway_profile_active:* or gateway_cleanup_incomplete result. It also skips the "first-party settings rollback did not complete" warning that the code is designed to produce.
src/server/management/agent-settings-routes.ts line 1635 already wraps the same barrier in try/catch and sets settingsRestored = false. Use the same behavior here.

Handle a settlement timeout during settings rollback.
If gateway removal fails after first-party settings change, and overlapping ACL work makes this new rollback barrier time out, withConfigDirHardeningSettled throws. The outer route catch then returns a 400 instead of the gateway-removal response. It also omits the existing warning that first-party settings rollback did not complete. Catch the settlement timeout at Line 1127, set restored to false, and continue through the existing failure response. The rollback helper already checks whether the managed settings still match before it restores them. (raw.githubusercontent.com)

Wait until the POST reaches the settlement barrier.
One macrotask() does not establish that this request has passed body parsing and the route’s dynamic imports. If the ACL flight is released before the POST reaches withConfigDirHardeningSettled, the new test can pass its final write assertions even when the route does not wait for ACL work. Add a barrier-entry handshake, as the management apply tests in this file already do. Check the unchanged setting while the flight is held, then release it. This makes the test exercise the behavior it names. (raw.githubusercontent.com)

Agreed. The handshake is implemented in aa89873 on fix/claude-first-party-rollback-timeout: the test now spies on withConfigDirHardeningSettled, waits until the request enters the barrier for the Claude home, and only then asserts that the settings are unchanged while the parent flight is held. Review round 4 passed this head with zero blocking findings, so to keep the v2.93.0 release gate on its reviewed head (4859387), this test change lands in a follow-up PR after #7777 instead of moving this head.
The reason will be displayed to describe this comment to others. Learn more.
@lidge-jun Understood. The handshake you describe in aa898732 addresses the test-ordering concern. Keeping this non-blocking test improvement in a follow-up PR preserves the reviewed head of #7777.
This finding remains open because the change is deferred from #7777. I have not independently verified aa898732.
Would you like a GitHub issue to track the follow-up, or is the planned PR sufficient?
If you found this review helpful, would you consider giving us a shout-out on X?
Re CodeRabbit outside-diff comment on agent-settings-routes.ts:1127 (review 5660614984): confirmed, this is a real defect. A settle timeout makes withConfigDirHardeningSettled throw, so the rollback skips the existing "first-party settings rollback did not complete" response and lands in the route's generic error path. The same applies at native-integration-routes.ts:771 and cli/claude-desktop.ts:437. The settings left on disk are the same either way; only the reported error is wrong, and it needs an ACL settle timeout during a rollback to trigger.
The fix is ready in aa89873 on fix/claude-first-party-rollback-timeout. It adds rollbackDesktopFirstPartySettled, which returns false on a settle timeout and rethrows anything else, and uses it at all three sites, with a regression test (barrier file 30/30, typecheck clean, pinned Bun 1.4.4). To keep the release gate on the reviewed head, it lands in a separate follow-up PR after #7777 merges.
Maintainer integration decision: MERGE (squash) into dev
Head: 4859387, base dev 9e48f78 (dev unchanged since, so the merge result equals the tested head).
Root cause fixed.loadConfig() starts an unawaited optional ACL flight on the config root, while intercept CA, token and picker secrets under that root were created and hardened synchronously. Parent inheritance propagation could drop the child's fresh grant, which left an empty DACL and caused ca_unavailable (lane=all 40389603119, Windows 7/9). Every intercept secret writer, the startup settings migration, the first-party removals and the model-defaults POST now run their synchronous mutation inside withConfigDirHardeningSettled, which drains overlapping and ancestor ACL work with one bounded deadline and refuses without writing on timeout. Failures now carry a sanitized {stage, cause} diagnostic, and the "known flake" comment was corrected.
Independent review (sol, gpt-6.1-sol high), four rounds:
r1: F1 picker cleanup outside the barrier, F2 unbounded retained-child wait. Fixed in 32720b9.
r2: F3 startup migration and removals over a nested Claude home. Fixed in 7f4056c.
r3: F4 model-defaults POST, N1 refusal wording. Fixed in 4859387.
r4 (rel293-lb-review-7777-r4): PASS, 0 blocking. Each fix has a regression that fails on the prior head and passes here.
lane=all workflow_dispatch: run 40421919065, 30 jobs green (Windows 1–9/9, macOS, Linux).
Earlier head 7f4056c had an unrelated Linux failure in messaging-remote-send.test.ts (run 40419760194, job 121911066526). No rerun was used: the head moved, and every check passed on 4859387. The release captain is root-causing that flake separately.
Scope. Per the release captain, the broader class of optional config-root flights overlapping other required child hardening (config.json saves, Desktop 3p library, remote Desktop store) predates v2.92.0 and is POST_RELEASE; a follow-up issue will be filed after v2.93.0. CodeRabbit's rollback-timeout finding (a settle timeout during a first-party rollback reports a generic error instead of "rollback did not complete") is fixed in aa89873 on fix/claude-first-party-rollback-timeout and lands as a separate PR.
Local (pinned Bun 1.4.4): focused barrier and intercept batches green; 2 picker-restart timeouts reproduce on clean dev 9e48f78 (host baseline). NOT RUN locally: full suite.

Adopted from upstream dd8bf1d. When the Windows config-dir ACL barrier
times out, the rollback is not attempted and the operation reports
failure instead of a false success; other errors still propagate. The

lane=allfailure intests/claude-integration/claude-desktop-first-party.test.ts. In run 40389603119,windows 7/9returned500 ca_unavailable("The local intercept certificate could not be created"). The post-failureicaclslisting showed the fixture root with normal grants, while itsclaude-interceptchild had an empty DACL. The test file described this as a known hosted-runner flake. It is an in-process race in product code.Cause.
loadConfig()starts an unawaited, optional asynchronous ACL hardening flight on the config root (src/config/paths.tshardenConfigDir). That flight runs/grant:rwith an inheritable ACE, then/inheritance:r. The Claude Desktop routes callloadConfig()and then synchronously create and harden intercept secrets under the same root: the local CA directory, the proxy token and the picker CA. Changing a parent's inheritable DACL makes Windows propagate inheritance to existing children. If a child's fresh explicit grant lands while that propagation is in progress, the grant can be lost. The child's own/inheritance:rthen leaves it with an empty DACL. The strict ACL check refuses that, and the bare catch inapplyDesktopFirstPartyreported onlyca_unavailableand discarded the cause.Fix.
withConfigDirHardeningSettled(dir, mutate)(src/config/paths.ts) awaits the config-dir flight, then every overlapping async ACL operation and retained child (withWindowsSecretAclSettled, which now includes ancestors). It runs the synchronous secret write in the same turn as the final check, so no newly queued hardener can starticaclsin between.applyDesktop, desired-state reconciliation, intercept startup (CA and token), and the picker controller, runtime and startup authority. This includes the rollback paths.diagnostic: { stage: "ca" | "token", cause }(an error code or class name only; never messages, paths, ACL output or secrets). It is surfaced in refusal bodies and one log line. Public reasons and messages are unchanged.structure/config.md,structure/runtime.md,structure/clients/claude-desktop.md) state the barrier rule.No retries, longer timeouts, Windows skips or weakened assertions.
Scope. This PR covers the observed failure class: intercept secrets (local CA, proxy token, picker CA, leaf, trust state and journals) and the Claude settings-home files that carry credentials or ACL-hardened data (
settings.jsonmigration and removal, model-default staging and backups). On Windows, atomicWriteFilealso hardens every temporary file, so the same overlap between the optional config-root flight (src/config/paths.ts) and required child ACL hardening also exists for unrelated files under the config directory, such asconfig.jsonsaves, the Desktop 3p library and the remote Desktop store. That broader class predates v2.92.0 (#3257) and is tracked as a separate post-release follow-up. This PR does not change it.Review rounds: r1 F1 (picker cleanup outside the barrier) and F2 (unbounded retained-child wait); r2 F3 (startup migration and Desktop first-party removals for a nested Claude home); r3 F4 (model-defaults rewrite) and N1 (refusal wording). Each finding was fixed with a regression test that fails without the fix. CodeRabbit nits are applied.
Verification
tests/claude-integration/claude-intercept-acl-barrier.test.ts(13 cases, registered inlayout-clients.jsonand the expected-layout fixture) runs cross-platform. It uses the platform seam, a synthetic principal and fake sync/async icacls runners, and holds the root asyncicaclsrunner on a promise.windows-secret-acl-removal-flight, Lab boundary, file-size ratchet and both layout guards pass, 92 pass / 0 fail.tsc --noEmit,structure:check,privacy:scan,skill:surface:checkandgit diff --checkpass.claude-desktop-first-party.test.tson this Windows developer host fails the same 19 cases on dev and on this branch. The cause is a pre-existing local baseline: tests share the per-user client lifecycle lock and a shared ACL timeout memo. The target test passes. With fixture-local lifecycle dependencies, the file passes 38/38. That shared test lock is a separate test-isolation issue and is not changed here; the production lock namespace is untouched.NOT RUN: full local suite (resource exception), Linux/macOS local execution (covered by CI), a replay of the exact Windows-internal propagation interleaving (not schedulable from user space; the in-process overlap it requires is what this change removes).
