Summary

Every 429 from Anthropic on a Claude OAuth account pool was treated as spent quota: the account was cooled and the request walked to the next account. An ordinary per-minute throttle therefore cooled healthy accounts, broke session affinity and dropped the warm prompt cache, and a burst could walk the whole roster. A Fable-only weekly refusal likewise cooled the whole account, so Sonnet and Opus lost an account that still had headroom.

  • Classify 429 before mutating account health (src/oauth/anthropic-rate-limit-policy.ts). A pure decision from trusted anthropic-ratelimit-unified-* headers: shared quota, family quota, transient rate, or request-scoped/unknown. Shared rejection cools exactly the serving credential and recovers on an eligible account as before. A transient throttle keeps affinity: one short same-account replay and at most one sibling detour within the existing send budget. A headerless or unknown refusal gets one request-local retry and never cools the roster or invents a Retry-After. The 403 classifier, credential-generation, abort, committed-output and non-replayable fences are unchanged. Main, continuation and sidecar dispatch all go through it.
  • Per-model weekly admission (src/oauth/anthropic-model-quota.ts, src/providers/quota/anthropic-family-headers.ts). The passive observer now records fixture-confirmed family weekly evidence (7d_oi → Fable) and family rejections. Admission at manual choice, affinity, strategy pick, retry and physical send reads shared 5h + shared weekly + the requested family's weekly. A family-only refusal leaves other families eligible on that account. Absent families are never erased by non-enumerating reads; expired readings become unknown; stale header-only exclusions revalidate through one serialized request instead of a background probe. Thresholds stay soft preferences, including 0 and the all-drained fallback.
  • Single-account users see no behavior change.

This follows the behavior of KarpelesLab/teamclaude (MIT) for the two kinds of 429 and per-model weekly caps; no code was ported.

Verification

  • New tests/adapters/anthropic/anthropic-429-policy.test.ts (20 cases: every header combination, Retry-After date/seconds/invalid/absent, two-account transient refusal, headerless refusal, final-budget recording, concurrent paused requests, no retry after output) with physical-send assertions for main, continuation and sidecar; anthropic-model-weekly-admission.test.ts (28 cases: Fable spent with Sonnet available, shared spent with family headroom, independent resets, absent family headers, malformed fractions/resets, A/B interleaving, credential replacement, stale single-flight). Existing pool, threshold, cooldown, quorum and model-route fixtures now state shared rejection explicitly.
  • bun run typecheck, structure:check, privacy:scan, file-size ratchet, test-layout, core-Lab boundary, docs-site build: pass. Scoped test:changed: 544 pass, 0 fail.
  • Local full bun run test: 35781 pass / 96 skip / 10 fail; the failures are the known injection-model EISDIR, CLI timeouts under worktree contention, and a restart-veto case that exits because a live proxy holds port 10100. Left to CI.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed (provider configuration reference in 8 locales, account-pool, threshold, failover and transport structure docs).
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. Changes account-health and admission decisions only; no token handling or logging changes; independent review recorded on the PR before merge.

Summary by CodeRabbit

  • Documentation
    • Clarified how Anthropic account pools handle shared-quota, temporary-rate-limit, and unclassified 429 responses, including retry and account-switch limits.
    • Documented model-specific quota checks, expiration and revalidation of quota observations, and usage thresholds as preferences rather than hard limits.
    • Confirmed that default single-account behavior remains unchanged.
  • Bug Fixes
    • Prevented unsupported 429 responses from triggering account cooldowns or adding a client-facing Retry-After.
    • Improved quota-aware account selection and recovery while preserving account affinity where appropriate.

📝 Walkthrough

Walkthrough

Anthropic account-pool routing now classifies 429 responses and uses model-specific shared and family quota evidence for account selection, dispatch, retries, and recovery. Passive quota evidence expires and is revalidated. Tests and documentation cover these rules.

Anthropic account pool

Layer / File(s) Summary

Quota windows and cached observations now track family rejection, freshness, credential generation, and authoritative enumeration. Stale family evidence can be revalidated through a serialized request.
Model-aware account selection
src/oauth/anthropic-routing.ts, src/server/responses/request-transport.ts, structure/providers/anthropic-account-thresholds.md, structure/transports/inventory.md
Eligibility, scoring, affinity, selection, dispatch validation, and physical-send admission use the requested model. Existing fallback behavior remains when preferred candidates are exhausted.

Merge Risk: 🟡 Moderate · up to d71ca

Concurrent requests can make the pool retry an account that recently refused a model’s family quota, causing avoidable rate-limit failures. Fix or explicitly accept this bounded routing risk before merging.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to d71ca

Concurrent responses can undo a newer quota exclusion and admit more traffic to an exhausted account. Credential checks and bounded retries limit the impact, but the shared-state ordering defect weakens failure containment.

Retained concerns

  • Medium · reliability · inferred: An earlier in-flight successful request can clear a later same-credential family rejection. Ordinary response publication does not compare family generations, and success clears rejection state even without usable quota headers. This can restore eligibility for a refused family across other sessions using the account, weakening quota failure containment and producing inconsistent cached-versus-live admission state.
Security review details

Security Blast Radius

  • inferred — The demonstrated ordering defect affects shared admission for a configured credential and requested family, not merely the originating request. Other sessions selecting that account can observe the cleared rejection. The inspected scope does not establish deployment-wide or tenant-wide exposure.

Trust Boundaries and Controls

  • observed — Recovery preserves abort and non-replayable fences, current-credential checks and request-local retry bounds. Primary and continuation dispatch apply send-budget gates; sidecar recovery additionally checks committed output and reserves a credential-hop permit. Both inspected sidecar loop callers supply the original physical response, resolving the synthetic-response binding uncertainty.

Resilience and Maintainability Implications

  • inferred — Credential replacement fencing and serialized stale revalidation do not prevent the same-credential out-of-order response case: requests admitted before any rejection can remain concurrently in flight. Cached exhaustion preferences and request budgets limit, but do not eliminate, subsequent refused sends after the rejection map is cleared.

Hardening Proposals

  • proposed — Apply an ordering fence consistently to ordinary-response and probe publication, including headerless successes, and keep cached family windows consistent with live rejection state. Validate the reverse-completion sequence of an older success and newer same-credential family refusal.

P2 Badge Honor rejected family verdicts after restart

When Anthropic returns a family window with status=rejected but utilization below 100% (the new test explicitly covers 30%), the rejection is enforced only through the process-local spentFamilies map. The persisted window retains rejected: true and passiveObservedAt, but after a restart that map is empty and this predicate considers only the percentage, so the rejected account becomes eligible again before the documented 30-minute/reset expiry and can immediately receive another failing Fable request. Treat an unexpired matching rejected window as exhausted or reconstruct the rejection state while hydrating the cache.

Assert account identity in the streamed-refusal regression.

This test disables the Anthropic account pool and checks only the request count. A post-output recovery that changes the bearer while still sending two requests can pass. Enable the pool and assert that the continuation uses the original account.

lines 267 - 276:
Update the streamed-refusal regression test to enable the Anthropic account pool
and verify the continuation retains the original account identity, not just that
two requests were sent. Capture the IDs returned by `seed`, configure the pool
before calling `post`, and assert the second request’s authorization matches the
first; also verify both seeded accounts have no health snapshot changes.

Scoped correctness review of 36488439a98b3fde25acc5ca47ee17332a6371a7.

P2 — Preserve a fresh persisted family rejection across restart (src/oauth/anthropic-model-quota.ts:22, src/providers/quota/account-cache.ts:131).

I independently traced the existing bot finding through the parser, cache hydration, and admission predicates. A response with 7d_oi-status: rejected and 7d_oi-utilization: 0.3 stores a Fable window with percent: 30, rejected: true, and a fresh passiveObservedAt. The live process excludes that account through spentFamilies, without overwriting its reported utilization. After restart, hydrateAccountQuotaCache() restores the still-fresh window but does not reconstruct that rejection; anthropicFamilyRejected() sees an empty process-local map, and anthropicModelExhausted() checks only percentages. A healthy active account can therefore be selected again for Fable despite its retained, unexpired rejection. The final test for a rejection below 100% covers the live process, not reload.

Please retain the verdict at admission or hydrate generation-owned rejection state from valid persisted evidence. Keep the existing age/reset bounds, credential replacement behavior, and Sonnet independence; do not solve this by falsifying the displayed utilization. Add a persist/reload regression with a rejected 30% Fable window, showing Fable remains excluded while Sonnet stays eligible, and that expiry/credential replacement clears the exclusion.

This is a decisive static execution-path review, not a claimed local restart reproduction or a full 43-file approval. I did not contact provider accounts or run a security scan. Remaining authentication/sponsorship and required CI review are separate gates.

Independent correctness/safety review (inherited-model reviewer, read-only) of 3648843: PASS-WITH-NITS. Send budget, committed-output and non-replayable fences, abort/timer cleanup, map bounds, single-account default (no quorum → no pool stamping), credential binding, revalidation race and logging all checked sound; core-Lab boundary passes.

Five edges fixed in a190690 with regression tests: family rejected is recorded only from a 429 (a 2xx served through extra usage keeps the family eligible); an owned 2xx for a family clears its stale entry even without family headers, so revalidation cannot serialize sends forever; unified-status: rejected with no rejected window is shared quota using unified-reset as the cooldown hint; the same-account retry compares against the pause deadline captured before sleeping; family evidence has its own generation fence and no longer cancels shared-cooldown recovery.

Fence successful responses against newer family refusals.

If two Fable sends overlap, a 429 can record a family rejection before an earlier send finishes with a headerless 2xx. Line 283 then clears the newer rejection. Line 285 leaves the cached rejected family window unchanged. Subsequent routing can admit the rejected account while still treating its cached Fable usage as 100% until that window expires.

Capture the family generation at the physical send in src/server/responses/request-transport.ts. Clear a rejection only when the successful send still owns that generation. When a valid success clears a rejection, remove its cached rejected window even if the response has no quota headers. Add a regression test that resolves the overlapping responses in this order.

Maintainer integration decision (dev, admin bypass per MAINTAINERS.md): merging at exact head d71ca1f (rebased on dev after #6442).

  • Required CI on this head: 26 success, 8 skipped (path-filtered), 0 failing.
  • Independent correctness/safety review recorded above (PASS-WITH-NITS); all five findings fixed in a190690 with regression tests.
  • Open follow-up, not blocking: a rejected family window below 100% is not re-applied after a process restart until the next refusal (Codex P2 on anthropic-model-quota.ts:21). CodeRabbit nits left by maintainer decision.
  • No token or account-identifier logging; single-account default unchanged.

Post-integration follow-up: current trusted dev 4b74668 still uses only process-local spentFamilies for the rejected verdict, while anthropicModelExhausted checks percentages. The persisted rejected-at-30% restart case from my review therefore remains unresolved, as the integration comment also records. My correctness objection has not been withdrawn; the owner integration was a separate decision, not my approval. Please track the durable rejection fix in a dedicated follow-up with persist/reload, age/reset expiry, credential replacement and other-model independence coverage. I have not claimed a shipped correction or launched a scan.
