fix(windows): admit config-root cache writes after the optional config-root ACL flight - #7998
Part of #7815. On Windows, loadConfig() starts an optional, unawaited ACL hardening flight on the OpenCodex config root. Its parent inheritance can drop a freshly applied explicit grant on a file written under that root while the flight is still running. #7777, #7826 and #7869 gated the observed writers and the async management/CLI phases behind withConfigDirHardeningSettled. Four timer-driven or async cache writers under the config root were still ungated:
the provider account quota cache (provider-account-quota-cache.json, src/providers/account-quota-disk.ts)
the Codex account quota cache (codex-quota-cache.json, src/codex/quota.ts)
the reasoning support cache (reasoning-support-cache.json, src/providers/reasoning-metadata.ts)
the models.dev reasoning metadata refresh (reasoning-metadata-cache.json, same file)
Each now writes through withConfigDirHardeningSettled, with the same deadline and refusal contract as the earlier writers. The debounced caches build their snapshot inside the admitted synchronous callback, so a late admission still writes the newest rows, and a timed-out admission skips that best-effort write. The refresh admits its publication after the network read; a timed-out admission fails that refresh unpublished, and the in-memory snapshot is not replaced.
On POSIX, or when no ACL work overlaps, admission runs the callback in the same turn, so timing is unchanged. The structure contract in structure/config.md lists the new writers.
Still open on #7815, per the maintainer breakdown there: the synchronous writer APIs that cannot await (saveConfig, persisted mutation, Desktop remote store I/O, management/GUI session tokens, service secrets, the spend-ledger lease, the link store), and the integrations catalog fan-out and Aside sync route.
Verification
Windows x64, Bun 1.4.4, based on dev8063be23e:
New regressions hold a real async icacls runner inside hardenSecretDirAsync on the config root, using the seams from claude-intercept-acl-barrier.test.ts:
tests/providers/provider-account-quota-persistence.test.ts: the scheduled snapshot is not written while the flight is held and lands once it settles.
tests/codex-integration/reasoning-metadata.test.ts: a refresh does not publish while the flight is held and publishes after it settles.
Red before the change: with the two source files reverted, both new cases failed.
tests/codex-integration/main-quota-provenance.test.ts drives the Codex quota timer callback by hand and read the file synchronously. On this Windows host one case failed after the change, because the write now waits for real overlapping ACL work. Its helper now awaits the write through the new settleQuotaPersistenceForTests seam. All 38 cases pass, and the file passed on the unchanged base.
Every test file importing codex/quota, account-quota-disk or reasoning-metadata, 98 files: 3,476 pass / 9 skip / 0 fail.
bun run typecheck, bun run structure:check, bun run privacy:scan and git diff --check passed.
The full local suite was not run because other worktrees share this host. Windows native CI jobs are policy-skipped on PRs.
Checklist
Scope stays focused and avoids unrelated cleanup.
Docs or release notes were updated when needed (structure/config.md).
Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. ACL admission is added, never relaxed; cache contents are unchanged.

Bug Fixes
Improved reliability of quota and reasoning-support cache persistence when configuration-directory security checks are in progress.
Cache writes are skipped if those checks time out, and reasoning metadata is not published if its write fails.
Delayed quota-cache writes use the latest available data, helping persisted information stay current.
…g-root ACL flight
The provider and Codex account quota caches, the reasoning support cache and the models.dev refresh wrote under the config root while an optional config-root ACL flight could still run, so parent inheritance could race the fresh file's explicit grant. Each write now goes through withConfigDirHardeningSettled with its snapshot built inside the callback. Part of #7815.

Walkthrough
Quota and reasoning-metadata cache writes now wait for config-root hardening admission before constructing and writing snapshots. Metadata refresh publishes its in-memory snapshot only after the admitted write completes. Tests now await quota persistence and cover writes during an in-flight Windows ACL operation.
Both quota-cache writers admit persistence after config-root hardening settles and build snapshots inside the admitted callback. The Codex quota module exposes settleQuotaPersistenceForTests(), and persistence tests await completion. The provider-account quota test checks persistence while an ACL operation is in flight.
The support-cache writer waits for hardening admission. Metadata refresh writes its snapshot before publishing it in memory. The test checks that the cache file appears after the ACL operation settles, and the config documentation describes the admission behavior.
Issue 7815 identifies config-root writers that can overlap an optional Windows ACL hardening operation. This change applies hardening admission to quota and reasoning-metadata cache writes.

Merge Risk: Moderate · previous review 3996f
Quota and reasoning-metadata cache writes now wait for the Windows config-root ACL flight, which fixes the earlier ordering problem. As written, repeated quota updates during that wait can queue redundant blocking writes on Windows. A provider cache clear during the wait can also overwrite persisted quota data with an empty snapshot. Both are bounded and recoverable, but should be fixed or consciously accepted before merge.

…fused refreshes to joiners
A fired cache write waiting for config-root ACL admission now checks a generation that cancel, clear and reset bump; the reasoning support flush seam is admitted too; a joined models.dev refresh receives the shared outcome instead of a coalesced success.

Propagate the shared job failure to coalesced refresh callers.
When the shared refresh job rejects, a caller that joins refreshInFlight still receives { ok: true, reason: "coalesced" }. The new admission at src/providers/reasoning-metadata.ts:574 can reject that job on timeout, so the first caller gets { ok: false } and no snapshot is published, while every joiner reports success. Joiners should receive the job's failure outcome.
The same mapping already applies to fetch and HTTP failures before this change, so the semantic is pre-existing. This PR adds a new rejection path that reaches it.
Callers do not depend on ok to mean the snapshot was published. The only result consumer found in the inspected call sites discards it, so the impact is a misreported outcome, not a data fault.
Change the refreshInFlight declaration type from Promise<unknown> | null to Promise<RefreshOutcome> | null. The declaration was not inspected at the reviewed head.
Add a regression case with two callers. Make the first caller's admission time out. Assert that both callers receive ok: false and that no snapshot is published.

Coalesce admissions while the ACL flight is pending.
The debounce timer coalesces updates only before it fires. After the callback sets persistTimer = null, another update can start a second admission while the first admission is waiting.
withWindowsSecretAclSettled waits for the current ACL work, then directly invokes mutate(). It does not serialize or coalesce other admissions. Each waiting callback can therefore run its own synchronous atomicWriteFile after the ACL flight ends. On Windows, each write can run blocking icacls hardening for its temporary file, which can delay request processing.
Keep one pending admission per cache and let it write only the latest snapshot. Apply the same protection to src/providers/account-quota-disk.ts.

Security and correctness review. Fresh gpt-6.1-terra reviews, including a dedicated security review, found these issues on earlier heads. All are fixed on this branch:
The reasoning-support flush seam wrote outside admission. It is now async and admitted.
Cancel, clear or reset did not invalidate a fired write that was still waiting for admission. A generation check now runs inside each callback, with a held-flight cancellation regression.
A joined models.dev refresh reported coalesced success after the shared refresh failed. The shared outcome now propagates, with a regression.
The flush seam also ignored the reset generation. Fixed.
The final review of a51b9d0 returned PASS. A fresh review of the 3533bb7 dev merge confirmed that its structure/config.md conflict resolution keeps both passages, and found no interaction with #7931's selection admission. Every callback stays synchronous, and a timeout writes nothing.
Union.dev (261131d3) overlaps structure/config.md and main-quota-provenance.test.ts, the latter from #8001. The simulated merge with current dev passed typecheck, structure:check, and the quota, reasoning, layout and ratchet focus (90 pass / 0 fail).
Verification. Windows x64 (Bun 1.4.4): the 98 test files importing the three changed modules passed 3,478 / 0 fail. Each new regression failed against the previous source.
Exact-head CI. Cross-platform run 41061308774 on 3533bb7 completed success, including gates and test changed.
Scope. This is part of #7815. The synchronous writer APIs and the integrations fan-out listed on the issue remain open.
Not run: native Windows CI jobs (policy-skipped on PRs) and the full local suite.
#7998 made the provider account quota cache write wait for config-root ACL admission, so the deferred-persistence test read the file before it existed on Windows (ENOENT, windows 9/11 of lane=all run 41068188012). Expose the admitted write like settleQuotaPersistenceForTests and await it.
