What changes

Part of #1518, the follow-ups from #1512's merge-pass reviews. It fixes CodeRabbit 4334853910 on #1512 and omo's L1, L3, L4, L5 and L7, plus G2-2 and F18-1 from the independent review. L2 and M3 are left; see "Not taken" below. Nine commits on main 7cd3648.

The fixes are all in a create request's first message: server/core/pending.ts, with the launch's pid from server/shell-agent.ts and server/agent-launch.ts, and the demo transport.

  • The launched agent quits before its first message goes (CodeRabbit 4334853910, omo L3; 2167f22).

    • Since feat(chat): start an agent with its model, effort and skills from Chat #1512 a first message stayed launch-bound for its whole life, and bindLaunched waited whenever herdr named no agent. The message stayed queued with no fault, and Send now answered input_not_ready, "not up yet".
    • Waiting for a send to find the agent gone was not enough either. After the agent quits, herdr may leave the pane at a status that is not at rest, and a queued message is only handed out at rest. The independent review reproduced "queued forever" on real Claude at 7cd3648.
    • Now a launch is waited for only while it is still coming up: no process learned yet, or the learned one still in front.
    • Once the launched agent came up and left, the pane's next event holds the message with pending_target_changed, whether the pane is at rest or not. Send now answers the same, and nothing is typed.
  • A message held from the start still learns its agent (omo L3 and L4; cfa94e6).

    • A first message held before its agent came up (no page named, or the page left) learned nothing, so Send now after that agent quit still said "not up yet".
    • A held first message now learns at the pane's events until its launched process is known. Once it is held because that agent left or was replaced, it reads the pane no more until its Send now, so held messages stop costing a pane read per event from then on (L4; bounded as F18-1 below needs).
  • The page leaves during its own automatic send (omo L1; c9fe616).

    • holdRequested cannot hold an item in flight. A send that then ended agent_blocked or pending_wait was retried into queued with no page left to type it. It headed the pane's queue, and other devices' messages stalled behind it with no note.
    • Such a send now settles the message held with first_message_unattended, and the pane's queue goes on.
  • A launch that died before any event saw it (G2-2, omo L5; 5770da7).

    • The first same-kind agent seen in the terminal was taken as the launched one. So if a shell-started launch (omo, gjc) died unseen, an agent the user started there afterwards got the message.
    • startShellAgent now resolves with the pid of the process it saw come up in front of the pane. On Windows there is none, because it is found among the shell's descendants.
    • agentLaunch hands that pid to the create route through a callback, so it is not in the HTTP answer, and the first message is bound to it at enqueue.
    • Learning never takes the shell's own pid for the launch (L5).
    • A mismatch the pane's events find against a queued first message (another terminal, agent, process or session) now holds it with pending_target_changed at once, as a send would, without waiting for the pane to rest.
  • A held message's card names the right reason (F18-1 in the independent review; 1d461f3).

    • A first message held from the start kept its first_message_unattended reason ("the page that sent this first message is gone") after its launched agent quit or was replaced. Send now answered pending_target_changed, but the card still said the page was gone.
    • A held first message is now checked at the pane's events until it is held for that reason, and then takes pending_target_changed as its listed reason. A Send now refused with it does the same.
    • PendingInputs.rehold tells a held message's new reason.
  • Demo parity (omo L7; f07256b): the demo's detach holds the first messages that page asked for, as its close already did and as the bridge does.

  • Docs and changelog (5248ccc): docs/agent-commands.md, plus the changelog.d/first-message-launch-ended.md fragment (Fixed).

  • A wrapper in front of the agent (CodeRabbit 4335931296; 5f7208c): a bound first message is checked against its own launched process while that process is in front, not against foreground_processes[0]. A non-exec wrapper (sh -c, a shim) is listed first and is not another agent. Contract case: "bound to a launch that runs behind a wrapper in front of the pane, is typed once into that agent, not held", which fails at 1d461f3.

  • Native Windows (omo M1 on 1d461f3): herdr lists only the pane's shell there, so the bridge cannot see the launched agent's process. A first message whose agent quit before it went keeps waiting on a Windows PC and is not held; Discard still ends it. The changelog fragment says so (c3c592a).

Not taken

  • L2 (leaving the pane holds the first message in ordinary flows): this is by design and documented in feat(chat): start an agent with its model, effort and skills from Chat #1512. Making the page's pane release wait on the bridge-owned first message is a client change of its own (PaneTerminal release on tab switch and phone backgrounding), not a fix in this area.
  • M3 (the Start in-flight guard test exercises startNewSession, not the dialog): unit tests here have no DOM, since component tests render static markup only. The new-session-skills lane already double-clicks the real dialog's Start while the create is held and asserts exactly one tab/create.

Tests

Each test was written first and fails without its fix. The whole set was replayed on main 7cd3648's own code in a scratch worktree, which was removed afterwards. These all fail on main and pass at 5248ccc:

  • server/pending-input.contract.test.ts, in "a create request's first message":
    • "queued for a launched agent that came up and then quit before it went, is held for Send now or Discard, never left waiting and never typed": through the real POST /api/tab/create.
    • "held from the start, still learns its launched agent: once that agent came up and quit, Send now answers pending_target_changed, not that it is not up yet".
    • "held from the start, says pending_target_changed instead of the page being gone once its launched agent quit, and Send now answers the same" (F18-1): this one fails at 5248ccc, where the listed reason stays first_message_unattended.
    • "whose page leaves during its own automatic send, is held for Send now or Discard, never re-queued ahead of the pane's other messages": the send is stopped at its own screen read, the page detaches, and an approval on screen makes the send end agent_blocked. Another device's queued message then goes.
    • "queued for a launch that died before any event saw it, is held when an agent of the same kind the user starts comes up, never typed into it": this also checks that the create's answer carries no pid.
  • scripts/demo-first-message-detach.test.ts: detach, re-attach, then the agent's rest. The message is held and never typed.

Test fix: the first quit case first failed about one run in three under load. That had two causes:

  1. The product gap above (no hold while the pane is not at rest).
  2. The test waited on a working status frame, which herdr's own process detection can replace.

The launched-agent cases (including #1512's round 2 replacement case) now wait until herdr's snapshot names the agent, checked at each status or roster frame. They then drain the pane through a fresh attach and wait out that drain and its rerun. They do not wait for herdr to stop naming an agent that quit, which can lag; the message is held either way.

How it was verified

At the head, c3c592a (5f7208c adds the wrapper fix; c3c592a changes only the changelog fragment), on main 7cd3648:

  • bun run typecheck exit 0.
  • pending-input.contract via bun run check run under env -i: 82 pass, 0 fail, in each of two runs in a row.
  • UNIT_TEST_WORKERS=2 bun run check fast exit 0: 5671 pass, 0 fail, 8 skip in 410 files.
  • bun test scripts/changelog-fold.test.ts with the updated fragment: 12 pass.

Earlier, at 1d461f3 (F18-1 on top of 5248ccc), on main 7cd3648 (main has not moved since):

  • bun run typecheck exit 0.
  • pending-input.contract via bun run check run under env -i (same isolation as below): 81 pass, 0 fail, in each of two runs in a row.
  • UNIT_TEST_WORKERS=2 bun run check fast exit 0: 5671 pass, 0 fail, 8 skip in 410 files.

The results below are at 5248ccc, which the independent review passed; 1d461f3 adds only the F18-1 commit.

  • bun run typecheck exit 0; bun run generate:types --check up to date.
  • UNIT_TEST_WORKERS=2 bun run check fast exit 0: 5671 pass, 0 fail, 8 skip in 410 files.
  • Contracts via bun run check run under env -i, with an isolated HOME and a PATH whose only muse and codex are stand-ins (claude and opencode are not on it):
    • Files: pending-input.contract, api.contract and device-access.contract.
    • Result: 226 pass, 0 fail, in each of three runs in a row.
  • Browser lane via bun run check run --build, same isolation: new-session-skills-browser-qa.ts 6/6 PASS.
  • Replay on main 7cd3648's code: the four new contract cases and the demo test fail, and the rest of the first-message block passes (13).

Review

This touches queued input (first-message binding, holds and retries), and the bridge's internal launch result. No HTTP or WS contract changes; the create's answer is unchanged, and a test pins that. It waits for a real CodeRabbit review.

…ame up and left

Since e062a35 a first message stayed launch-bound for its whole life, and bindLaunched waited
whenever herdr named no agent in the terminal: a launched agent that was learned and then quit before
the message went left it queued with no fault, and Send now answered that the agent was not up yet.
Waiting for a send to find it gone was not enough either: after the agent quits herdr may leave the
pane at a status that is not at rest, and a queued message is only handed out at rest.

A launch is now waited for only while it is still coming up (no process learned, or the learned one
still in front). Once its agent came up and left (herdr names no agent there and the learned process
is not in front), the message is held with pending_target_changed at the pane's next event, at rest
or not, and Send now answers the same; nothing is typed. A held first message no longer re-reads the
pane at every event. A same-kind replacement and a /resume still get pending_target_changed, and the
launched agent still gets the message once.

pending-input.contract: a create queued with the shell in front, the launched agent learned, then
quit (the shell back in front, herdr naming no agent): held with pending_target_changed, Send now
refused, Discard ends it, nothing typed. Its wait for the launched agent, and the round 2 replacement
case's, no longer wait on one status frame (herdr's own detection races a reported state): they wait
until herdr names the agent, then drain the pane through a new attach and wait out that drain and its
rerun. The full contract passes three runs in a row. (CodeRabbit 4334853910 on #1512; #1518)
…l its process is known

A first message held from the start (the request named no page) or held before its agent came up
learned nothing at the pane's events: once that agent came up and quit, its Send now still answered
input_not_ready, "not up yet". A held first message now learns at the pane's events until the process
launched for it is known, and then reads the pane no more until its Send now, which tells a launch
that came up and left (pending_target_changed) from one not up yet. Queued ones learn as before; held
ones stop costing a pane read per event once their launch is known.

pending-input.contract: a create naming no page (held from the start), the launched agent comes up and
quits, Send now answers pending_target_changed, Discard ends it, nothing typed. Fails without the fix
with input_not_ready. (omo L3 and L4 on #1512; #1518)
…never re-queue it

A first message whose page left or closed while its own automatic send was on its way could not be
held then (holdItem leaves an item in flight to its send), and when that send ended in agent_blocked
or pending_wait it was retried into queued with no page left to type it: it headed the pane's queue,
and the other devices' next-turn messages stalled behind it with no note. Such a send now settles
the message held with first_message_unattended, for Send now or Discard, and the pane's queue goes on.

pending-input.contract: the page's automatic send is stopped at its own screen read, the page detaches,
the screen then shows an approval so the send ends agent_blocked: the message is held with
first_message_unattended, and another device's queued message goes once the prompt is gone. Without
the fix the message goes back to queued. (omo L1 on #1512; #1518)
… and hold a mismatch at once

A create queued with the shell still in front took the first agent of the launched kind seen in the
terminal as the launched one. If the launch died before any pane event learned it, an agent the user
started there afterwards got the first message (herdr's agent.start cannot get here; agents typed into
the shell, omo and gjc, can). startShellAgent now resolves with the pid of the process it saw come up
in front of the pane (none on Windows, where it is found among the shell's descendants), agentLaunch
hands it to the create route through a callback (it is not in the HTTP answer), and the first message
is bound to it at enqueue. Learning never takes the shell's own pid for the launch (omo L5).

What the pane's events find against a queued first message (another terminal, another agent, another
process or session of the kind) now holds it with pending_target_changed at once, as a send would, so
the hold does not wait for the pane to be at rest.

pending-input.contract: the stood-in launch names a process that ended before the pane's first event,
the user starts an agent of the same kind: the message is held with pending_target_changed, nothing is
typed into it, and the create's answer carries no pid. Fails without the fix. (G2-2 in the independent
review of #1512, omo L5; #1518)
…s the pane

The demo held a first message only when its page closed: a page that detached the pane and came back
got it typed at the agent's rest, which the bridge no longer does. A detach now holds the first
messages that page asked for in that pane with first_message_unattended, through the same helper as
its close. scripts/demo-first-message-detach.test.ts drives detach, re-attach and the agent's rest on
the demo transport: held, never typed. It fails without the fix. (omo L7 on #1512; #1518)
…r was replaced

A first message held before its agent came up (no page named, or the page gone) kept its
first_message_unattended reason after the launched agent quit or was replaced: Send now answered
pending_target_changed, but the card still said the page that sent it was gone. A held first
message is now checked at the pane's events until it is held for that reason, and then takes
pending_target_changed as its listed reason; a Send now refused with it does the same. A held one
reads the pane no more once its reason is pending_target_changed. PendingInputs.rehold tells a held
message's new reason.

pending-input.contract: a create naming no page (held from the start), the launched agent comes up
and quits, a connection attaching the pane drains it: the listed reason becomes
pending_target_changed, and Send now answers the same; nothing is typed. Fails at 5248ccc, where
the reason stays first_message_unattended. (F18-1 in the independent review; #1518)

📝 Summary

Summary by CodeRabbit

  • Bug Fixes

    • First messages from a new workspace or tab are now held for review when the agent exits, is replaced, changes session, or the originating pane disconnects before delivery.
    • Held messages can still be sent or discarded and are not delivered to a different agent.
    • Other queued messages continue processing without being blocked by an unattended first message.
  • Documentation

    • Clarified how first-message targeting behaves when sessions, processes, or agents change.

First messages are now bound to the launched process and held when that target exits or changes, or when the requester disconnects during automatic delivery. The demo transport also holds first messages when their pane detaches. Server and demo tests, documentation, and a changelog entry describe these changes.

Changes: First-message lifecycle

Layer / File(s) Summary
Capture and pass launched process identity
server/shell-agent.ts, server/agent-launch.ts, server/index.ts, server/workspace-routes.ts
Shell startup returns a foreground process ID when available. The launch callback reports it to the workspace route, which passes it into first-message queueing.
Hold messages when delivery targets change
server/core/pending.ts, server/pending-input.ts, server/pending-input.contract.test.ts, docs/agent-commands.md, changelog.d/first-message-launch-ended.md
Pending delivery checks the pane and launched process identity. It holds messages when the target changes or ends, and holds retryable failures when the requester is absent. Contract tests cover these cases and discard behavior.
Hold demo messages on pane detach or socket close
site/demo/transport.ts, scripts/demo-first-message-detach.test.ts
The demo transport holds a connection’s queued first message on detach or close. The test verifies that reattaching does not send the held message.

Comment thread server/core/pending.ts Resolved
…he first one in front

startShellAgent binds a first message to the process it saw come up, the first match in front of the
pane, while currentIdentity always read foreground_processes[0]. With a wrapper that does not exec its
agent in front of the pane (a version-manager shim, `sh -c` with more to run), herdr lists the wrapper
first: the event check, bindLaunched and pendingContext compared the bound agent against the wrapper,
and the first message was held with pending_target_changed instead of being typed.

currentIdentity now takes the bound process while it is among those in front, and the first one
otherwise; enqueue, the event check (per message), bindLaunched and pendingContext pass the message's
bound process. A replacement, a /resume, a quit and a launch that died unseen still hold it, since the
bound process is then not in front.

pending-input.contract: an agent started behind `sh -c "...; true"` (control: herdr lists the wrapper
first), its first message bound to the agent's pid: typed once, not held. Fails at 1d461f3, where it
is held with pending_target_changed. (CodeRabbit 4335931296 on #1532; #1518)

Merged as 56a6877. omo-native-code-reviewer and Codex both PASS on 1d461f3; Codex PASS on the delta to c3c592a (a bound first message is checked against its own process, so a wrapper listed first no longer holds it; the native Windows limit is in the changelog). The merge pass ran bun run check fast (5,671 pass), the pending-input, API, device-access and route-envelope contracts (235 pass), chat-browser-qa (9 PASS) and new-session-skills-browser-qa (6 PASS) on 1d461f3; required checks green on c3c592a. F3 D1 (plan todo 35; IS-2, IS-8 for Hermes). Built as
`fix/hermes-repeat-result` from 7cd3648, one commit (225d4aa5), picked
and rebased here as 1c1aebe.

#### Cause
F3 found that Hermes `/status` sent from Chat became a result card only
on the first send(s). Later sends showed "Runs in the terminal",
although the full `Hermes CLI Status … Agent Running: No` block was on
screen and `hermesResults()` parses that screen.

The reader was fine. The fault was in how each send finds its own
output. `server/outcome.ts` places the send-time screen in the polled
screen by matching the tail of the send-time screen as text
(`anchorEnd`). It then owns the echo after that anchor, at the ordinal
reserved for it (`reserveResultEchoes`).

Identical result blocks break this. Once earlier `/status` blocks scroll
the unique content above them off a 40-row pane, the tail matches in two
places with equal strength. One place is true (cut off at the screen
top). The other is one block lower (stopped by a mismatch). `anchorEnd`
treats the tie as ambiguous, so the third send owns nothing.

From the fourth send on, the send-time screen and the screen after the
output are the same text apart from the status bar clock. Only the
history moved, so text alone can no longer tell "one more identical
block" from "nothing printed". The echo reservation for the next send
then compares two identical screens. It puts the previous send's anchor
at the bottom and reserves a second echo that never comes, so every
later `/status` stays unowned.

`/version` twice in F3 worked only because unique `/status` blocks still
sat above those blocks. Reproduced live on 7cd3648 with real Hermes
0.21.5 under the same isolation as F3: the third `/status` in a pane
showed "Runs in the terminal" (`baseline-7cd3648c/`).

#### Fix
herdr reports, per pane, how many rows have scrolled into history
(`scroll.max_offset_from_bottom`, minus `offset_from_bottom`). Measured
live, it tracks the text exactly: a 12-row shift, with every overlapping
line equal. The fix places the send-time screen by that row, and keeps
the text anchor as the fallback:

- `server/core/composer.ts`: after the send-time screen read, the send
reads the pane's row once (`paneScrollInfo`). That read only records the
send, so it runs alongside delivery and is never awaited (01a7a0e). The
row is kept only when the answer equals the row the send's earlier
`paneGet` gave, i.e. nothing scrolled while the screen was read. An
answer taken after the echo scrolled the history disagrees and is
dropped. A failed read is dropped too, and so is one that arrives after
the send was recorded too late for its echo reservation; then the text
anchor applies. Delivery never waits on it.
- `server/prompt.ts`: each prompt poll passes the row from the snapshot
it took before its screen read.
- `server/outcome.ts`: `scrolledAnchor` puts the send-time screen's last
line at `last - (top_now - top_sent)`. It accepts that only when every
line of the send-time screen still on screen sits exactly where the
shift puts it. Both the owned block (`ownedCommandScreen`) and the next
send's echo reservation (`reserveResultEchoes`) use it. Without rows, or
when the lines disagree (a resize or reflow), the text anchor applies as
before.

IS-2 still holds. A result is shown only from the block under the send's
own reserved echo. A send's row is exact: the same before and after its
screen. A poll's row is read before its screen, so it can only lag. A
lag moves the anchor down, which means "no echo owned yet", never "an
earlier send's echo". A later send with no new output stays accepted:
same row, so the anchor is the end of its own screen and nothing lies
after it.

Not changed: other agents' readers, the Hermes reader, and the
queued-input path (`server/core/pending.ts`). Queued sends carry no row
and keep the text anchor. Accepted limitation: once herdr's history cap
is reached, the row stops growing; the line check then rejects a
non-periodic shift, and the text anchor applies again.

Known limits:
- The row anchor assumes that no other device scrolls the pane's
viewport while the send-time screen is read. Today `freeHermesInput`
refuses such a send: it trusts the viewport's colors only while
`viewportShowsLive` holds before and after the read, and otherwise
treats the input row as a draft.
- A `/status` block that no send owns can be carried forward to a later
send's anchor. Main behaves the same; this PR does not change it.
- `printedPastEarlier` trusts herdr's history rows with no line check.
If herdr renumbered its history between two sends, it could misplace the
earlier send's screen and let a later send own a pending send's block.
There is no evidence that herdr 0.9.5 renumbers its history (omo review
r2 S3).

#### Tests (failing first)
- New `server/core/hermes-repeat-outcome.test.ts` drives the real path:
`createSubmitText` (the composer's send, including `freeHermesInput`)
and `GET /api/pane/prompt` (`handlePromptRequest`). Stubbed herdr calls
serve a real recording,
`server/fixtures/hermes-results/status-repeat-120.json`: six `/status`
sends to Hermes 0.21.5 in a 120x40 herdr 0.9.5 pane, each with its
send-time detection read, the colored visible read, the read after its
block printed, and the history rows (equal before and after each read).
- Each of the six sends is `accepted` while its echo has not printed
(earlier identical blocks on screen are not its own), then
`result-observed` with `recognized-result` and exactly its own block as
`result_text`, while the earlier sends keep theirs.
- Negative case: after a resolved `/status`, another `/status` with
nothing new printed (same screen, same row) stays `accepted` and carries
no `result_text`.
- Failing first: with `server/outcome.ts`, `server/prompt.ts` and
`server/core/composer.ts` at 7cd3648, both tests fail (0 pass, 2 fail:
`Expected: "result-observed"`, `Received: "accepted"`). With the fix, 2
pass (`failing-first.txt`).
- Mutation check: reverting only the reservation's scroll anchor fails
the six-send test. Reverting only the owned-block scroll anchor fails
both tests.
- The live failure at the fifth send, first seen with an intermediate
build, was replayed offline from the logged reads and rows
(`harness/replay.ts`, `harness/replay-debug390-reads.json`). All six
sends resolve with the final code.

#### Gates
- `bun run typecheck`: exit 0 (`gate-typecheck.txt`).
- `UNIT_TEST_WORKERS=2 bun run check fast`: workflow syntax, generated
types and themes, typecheck and build ok; unit 5672 pass, 0 fail, 8 skip
in 410 files; EXIT=0 (`gate-check-fast.txt`).
- Outcome and Hermes units (`server/outcome*.test.ts`,
`server/dialogs/hermes*.test.ts`,
`server/core/outcome-delivery.test.ts`,
`server/core/hermes-repeat-outcome.test.ts`,
`server/core/ownership.test.ts`, `server/rewind-outcomes.test.ts`,
`scripts/changelog-fold.test.ts`): 164 pass, 0 fail (`gate-units.txt`).
- Contracts `server/hermes.contract.test.ts` and
`server/outcome.contract.test.ts`: 10 pass, 0 fail
(`gate-contracts.txt`).
- Run as `env -i HOME=<tmp> PATH=<tmp bin>:/usr/bin:/bin … bun run check
run bun test --timeout 15000 …`, where `<tmp bin>` has stand-in `muse`
and `codex` (`exec cat`) and links to bun, node, herdr and git.
- That runs on `check run`'s own isolated herdr. The shared default test
session was not restarted.

#### Changelog
`changelog.d/hermes-repeat-status-result.md` (### Fixed).

#### In this branch
The pick applied without a conflict. Todo 39 also changed
`server/prompt.ts`, but the two changes do different things:
- 35 adds `top: viewportTop(pane.scroll)` to the `commandOutcome` call
for each send.
- 39 filters the mapped outcomes afterwards by session identity.
- The composer's `{ ...command, top }` copy keeps 39's `sessionChange`
field.

So 39 still drops outcomes from earlier sessions and keeps the row of
the send that changed the session, and 35 still gives each repeated send
its own block. Both test sets pass together on the branch (see Gates).

## 36. Every reader of a pane's agent goes through resolveAgent, with
the pane's OmO roots where it matters (IS-5)

**Cause.** Four sites on main still read `pane.agent ??
pane.agent_session?.agent`:
- the omp `/model` to `/switch` rewrite in `server/core/composer.ts`;
- `openCodePaneService`, `openCodeSkillsToType` and `startOpenCodeWatch`
in `server/opencode-native.ts`.

herdr labels an OmO pane `pi` while it waits and `claude` while its
claude-sdk child runs, and the label can lag behind an OmO started in a
pane it named otherwise. Only the pane's process roots tell OmO apart.
`resolveAgent(pane)` without roots returns the same label, so replacing
the reads alone would change nothing.

**Fix.** Each site goes through `resolveAgent` (1cab346). Where an OmO
pane could be mistaken, the pane's roots come along too (b87457d,
a2e50b0), read with `paneProcessRoots` and filtered through
`typingRoots` (only an OmO process holding a session outranks the label,
as for typing):
- **Composer, omp `/model` rewrite.** When the label resolves to `omp`
and the pane's session is not herdr's own omp session
(`agent_session.source !== "herdr:omp"`), the agent is resolved again
with the roots. A session-holding OmO is then OmO, so its `/model` is
sent as typed, not rewritten to `/switch`, and it does not wait for
omp's startup report. A pane herdr reports as omp skips the extra
process read.
- **`openCodePaneService` and `openCodeSkillsToType`.** Roots are read
only for an opencode-labelled pane (a2e50b0). A pane labelled anything
else is not OpenCode and returns at once, with no process read. A pane
labelled `opencode` is resolved again with its roots before any service
lookup or skill typing, so a session-holding OmO there is not treated as
OpenCode. `openCodeSkillsToType` also returns before reading roots on
Windows or when the text has no `@`.
- **`startOpenCodeWatch` stays label-only.** It is synchronous on the
send path, and reading roots would make the send wait on herdr. It
starts only for a send whose own agent is `opencode`, on a pane carrying
an OpenCode session id. It applies events only after the pane's service
and session are verified, either recently or in the background through
`openCodePaneService`. That verification reads the roots, so a
session-holding OmO pane labelled `opencode` fails it and the watch is
dropped before any event counts. This last point comes from reading the
code: no live run had an OmO pane that herdr labels `opencode`.

`resolveAgent`'s rules, the OpenCode identity gates and the Windows
exclusion are unchanged.

**Tests (failing first).** `resolveAgent` and `paneProcessRoots` are not
mocked in any of them. What each test stubs:
- `server/core/outcome-delivery.test.ts`: "does not rewrite /model when
the resolved pane agent is OmO". It stubs herdr's `process_info` answer
and OmO's holder record (`omoProcessIdentity`). A pane herdr labels
`omp`, with a session-holding OmO in its foreground, gets `/model` as
typed.
- `server/opencode-native.test.ts`, "does not treat an OmO pane herdr
labels opencode as OpenCode":
- The pane is labelled `opencode`, with a real OmO holder record for
this process in a temporary HOME (`omoProcessIdentity` is not stubbed).
Only herdr's `process_info` answer and `processEnviron` are stubbed.
  - `paneProcessRoots` finds the session-holding OmO.
- `openCodePaneService` returns null and `openCodeSkillsToType` types no
skill, both without a service lookup.
- `server/opencode-native.test.ts`, "does not probe process roots for a
non-OpenCode skill send": an `@file` sent to a Claude pane makes no
herdr call and no service lookup.
- Failing first:
- With b87457d's source hunks reverted, the composer test and the
then-current OpenCode test fail (0 pass, 2 fail)
(`group-H/final-t36-t39-failing-first.txt`; that log names the commit by
its pre-rebase hash 8e6cffdb).
- With a2e50b0's source hunk reverted, the probe test fails, and the
OmO test still passes because b87457d already covers that case
(`group-H/t36c-failing-first.txt`; pre-rebase hashes 2e208555 and
8e6cffdb).
- Acceptance: `rg -n 'agent \?\? [a-z.?]*agent_session\?\.agent' server
--glob '!*.test.ts'` prints only `server/pane-context.ts:25`.

## 37. A fresh OpenCode 2 pane offers its skills in OpenCode 2's own
form (F3 D2, IS-1)

**Cause.** `GET /api/pane/commands` took the catalog shape from the
pane's session store. A fresh OpenCode pane has no session yet, so the
route fell back to the v1 shape and listed project and user skills as
`/name`, which OpenCode 2 rejects. `GET /api/agent/commands` already
used the installed version.

**Fix.** When the resolved agent is `opencode` and the store gives no
version, the route picks the shape from
`openCodeInstalledVersion(options.openCodeStateDir)` (the service file),
as `/api/agent/commands` does.
- That version only chooses the syntax. Rows stay `provenance.verified:
false`.
- Once the pane has a session, the store's version wins as before.
- Without a service file, the v1 fallback stays.
- Nothing is spawned, no service is started or ensured, and opencode's
binary is not read.

**Tests (failing first).** New cases in `server/pane-routes.test.ts` use
a `mkdtemp` state dir:
- A session-less pane with a service file naming 2.0.32 gets its project
skill with `trigger: "@"` and `verified: false`.
- Without a service file, it gets the v1 `/` row.
- A pane whose store records a version keeps that version's rows.

On 7cd3648 the `@` case fails (9 pass, 1 fail: `Expected: "@"`,
`Received: undefined`; `group-H/t37-failing-first.txt`).

## 38. Terminal-command notice actions fit a phone width (IS-1)

**Cause.** `.composer-notice-actions` was `inline-flex; flex: none`. The
`/resume` terminal notice has three actions (Earlier sessions…, Open
Terminal, Run in Terminal) plus the dismiss X. At 390 px they ran past
the composer's right edge and cut off the X (F3,
`F3-real-qa/history-resume.390.after-send.png`).

**Fix** (`src/components/ComposerNotices.css`, tokens only):
- The actions shrink and wrap, trailing-aligned. When the column is too
narrow, they move to a line of their own under the text.
- The dismiss is pinned to the row's top-trailing corner, on space the
row keeps clear for it, with a `--touch-target` square under a coarse
pointer.
- The 1280 px notice band is pixel-identical in both schemes.
- No action is dropped or renamed, and none changes what it sends.
- DESIGN.md's notices entry describes the wrapping and the dismiss's
corner.

**Design followed:** Follows Primer's Banner anatomy (dismiss pinned to
the top-trailing corner, actions dropping below on a narrow viewport),
with the actions trailing so Run in Terminal stays over Send.

**Tests (failing first).** `scripts/skill-menu-cases/claude-terminal.ts`
gains a `/resume` scenario at 1280, 390 and 320 px in light and dark. It
asserts that every action button and the dismiss lie inside the
composer's column (`getBoundingClientRect`). Its builder reports it
failed on 7cd3648 at 390 and 320. On this branch `bun
scripts/agent-skill-menu-demo-regression.ts claude-terminal` passes,
with 36 screenshots (`group-H/claude-terminal-demo.log`,
`group-H/t38-screens/resume-{light,dark}-{1280,390,320}.png`).

## 39. Chat shows only the current session's command outcomes after an
agent restart (IS-2)

**Cause.** The server keeps Chat sends per pane, not per session
(`server/prompt.ts`). After an agent restarted in the same pane, `GET
/api/pane/prompt` still returned recent sends from the earlier pane
identity (`outcomeIdentity`: terminal and agent session). The client
then placed them as provisional rows in the new conversation, so F3 saw
the earlier session's command rows and states in the new session's Chat.

**Fix (server-side; what is sent is unchanged).** `commandOutcomes`
shows a send only while it belongs to the pane's current session and
agent process.
- **Current session (485c64e).** A send is shown while its identity
(terminal and `agent_session` at send time) is the pane's current one.
- **Session change (485c64e, f5ff583).** The send that itself changes
the session is the exception: a Chat `/clear`, `/new`, `/reset` or
`/resume` that matches a builtin row (`sessionChange`). It stays visible
in the next conversation only, with its own state, also when a send in
the new session arrives before the first poll.
- **Send-time binding of the agent process (e16904c, 01a7a0e,
ba5a77c).** `bindCommandProcess` records the agent process a send went
to, for a direct Chat send (`server/core/composer.ts`) and for a queued
message's delivery (`server/core/pending.ts`).
- Every command send is bound, also one whose session is already known.
F2 suggested binding only sends made before the session is known
(01a7a0e tried that). A live run showed it is not enough: after `/exit`
and `claude` in the same pane, herdr kept reporting the old Claude
session id for the new process for at least 20 s, and the old `/context`
row stayed (Claude 2.1.305, herdr 0.9.5). ba5a77c binds every send
again, still off the send path. Its unit case, "omits a known session's
sends once a restarted agent still reports that session id", fails on
01a7a0e's rule.
- It picks from `agentProcesses` (`server/conversation.ts`), the one
matcher `paneProcessRoots` also uses: the native agent (a process whose
own binary is the send's agent) before its launcher (`node`, `bun` or
`python` running the agent's script; for OmO, its own process). It keys
on the agent's own pid and start time, not the process-group id. When
the agent has no native child and its launcher leads the process group,
that launcher is the agent process and is picked.
  - It records the pid, plus the process start time on Linux.
- **Recording never delays delivery (01a7a0e).** Nothing the binding
reads is awaited before the write:
- A queued message reuses the `pane.process_info` answer its identity
check already read before the write. A Claude or Hermes send reuses the
answer its `paneProcessRoots` probe read before the send. These are
pre-send snapshots, so they name the process present at send time.
- Any other send starts its `pane.process_info` read alongside
`agent.prompt`. Because that answer can arrive after the agent was
replaced, a process whose start time (an upper bound read from `/proc`,
or `ps` on macOS) is not before the send is not bound: the send stays
unidentified, as when the read fails. The pid-and-start-time check needs
no extra read before the send; a pre-send read for every agent would put
a herdr round trip back on the send path.
- The read's promise stays on the send (`visibility.binding`). A poll
waits for it for at most 250 ms (1 s before todo 42), and a send whose
binding has not landed is judged again at the next poll.
- **Polls check that process and never adopt another (746f148).** Each
poll looks for the bound pid among the pane's foreground agent
processes.
- It marks the send replaced, and never shows it again, only on proof:
the pid is gone, or on Linux its start time differs. Where no start time
is known, todo 42 decides: macOS and other Unixes check the pid with
`kill(pid, 0)`, and Windows keeps two polls in a row without the process
as proof (section 42).
- A process that is only out of the foreground for a moment (Ctrl+Z,
another process group holding the terminal) keeps its rows.
- This also covers a replacement that no poll saw (the agent restarted
before the first poll) and one under a wrapper loop that keeps the same
process group.
- A poll reads `pane.process_info` only while some bound send is not
replaced and is still waiting for its session, or is inside its
10-minute window.
- **Null to id, for that process only (e0ad43d, e16904c).** An agent
reports no session id before its first turn. A send made while the pane
had no `agent_session` takes the first session id the same terminal
reports, and its session-change identity follows, only while its bound
process is still running. A send bound to no process because the
process-info answer showed it started after the send (the late-answer
guard) is never adopted into a discovered session: it keeps its
send-time identity. A send for which no agent process matched at all is
adopted since todo 42 (section 42).
- **Evidence.** The same rule decides whether the current screen can be
read as a send's evidence. A session id appearing is never itself
evidence that a command completed.
- **Cost.** Recording adds no herdr read before delivery. A command send
without a pre-send answer (not Claude, Hermes or a queued message) reads
`pane.process_info` once, alongside delivery.
`server/core/omp-submit.test.ts` counts that one read; omp's startup
wait reads none.
- **Known limits.**
- Outside Linux the process has no start time, so a replacement that
reuses the same pid cannot be told apart. The fragment says so.
- The late-answer guard needs the process's start time, which is known
only on Linux (`/proc`) and macOS (`ps`). On Windows, and on Unixes
other than these two, a send whose `pane.process_info` answer arrives
after the agent was replaced is bound to the replacement.

**Tests (failing first).**
- `server/outcome.test.ts`, the "session-scoped command outcomes" cases:
  - earlier-session sends are omitted after an agent restart;
- only the initiating `/clear`, `/new` and `/resume` row is kept across
its session change, with no evidence invented;
- first-turn receipts are kept through session discovery for Claude Code
and Codex, and dropped on a new session, terminal or process;
- a first-turn session-changing command is not carried into yet another
session or a different agent process.

On 7cd3648 the first cases fail (`group-H/t39-failing-first.log`). With
e0ad43d's `server/prompt.ts` reverted, 13 of the discovery cases fail
(`group-H/final-t36-t39-failing-first.txt`).
- `server/core/outcome-delivery.test.ts`, "binds a Codex command at send
time": six scenarios (the four below, plus "suspended" and "settled"
from the F2 review fixes further down; todo 42 adds a seventh, "no agent
process") through the real composer send and `GET /api/pane/prompt`,
with herdr's `process_info` stubbed as a shell loop, a `node` launcher
and the native `codex`:
  - unpolled replacement: the native agent is replaced before any poll;
- wrapper restart: the native agent is replaced under the same process
group;
- first turn: the same process reports its first session id, and the row
stays `accepted` with no evidence;
  - reused pid (Linux only): same pid, new start time.

The process is read once at send time in every scenario: `readsAtSend`
is asserted to be 1 in all of them.
- With e16904c's source hunks reverted on the branch, three of the four
scenarios fail (unpolled replacement, wrapper restart, reused pid), and
so does omp's process-read count (24 pass, 5 fail). Restored, 114 pass
(`group-H/r3-t39-failing-first.txt`).
- F2 review fixes, failing first (`group-H/r4-f2-failing-first.txt`).
These are new cases in `server/core/outcome-delivery.test.ts`; with the
sources at e16904c, 6 of 26 fail:
- "delivers before a slow / never answering send-time process read for
its outcome can affect its deadline": `client.paneProcessInfo` answers 2
s late, or never. The send is delivered with the clock unchanged
(`expect(now).toBe(10_000)`), and a poll still answers within its
binding wait (1 s then, 250 ms since todo 42). At e16904c both time out
without typing anything.
- "delivers before a slow / never answering scroll read …": the same for
`client.paneScrollInfo` on a Claude send. A row that answers after
delivery and equals the snapshot's row is attached afterwards; a failing
or unanswered read attaches none. At e16904c both time out.
- The failing variants of both pass at e16904c too: a rejected read was
already caught.
- "binds a Codex command at send time: suspended" (Linux): the native
agent is out of the foreground for one poll and back. Its row is kept
through that poll and through session discovery. At e16904c the single
miss hid it for good.
- "binds a Codex command at send time: settled": once the session is
found and the window is over, two more polls make no `pane.process_info`
read. At e16904c every poll read it.
  - `readsAtSend` is now asserted to be 1 in every scenario.
- `server/outcome.test.ts`, "omits a known session's sends once a
restarted agent still reports that session id": it fails with 01a7a0e's
bind-before-a-session-only rule and passes with ba5a77c.
- With the fix, `server/core/outcome-delivery.test.ts` passes 26 of 26.

**Real agents on d4dd48d8, whose source is the final head's (ba5a77c
changes only a test).**

Setup:
- Real Codex 0.163.0 and Claude Code 2.1.305, both driven through the
server's own WebSocket submit and `GET /api/pane/prompt`.
- `env -i` with an isolated HOME (`/tmp/f2r4/home`) and fake keys;
Codex's turns got OpenAI 401, so nothing was billed.
- herdr 0.9.5 session `f2r4-verify` under that HOME, with herdr's Codex
and Claude integrations installed there only. The driver refuses any
other socket.
- Codex 0.163 runs hooks in one app-server daemon per HOME, which
carries the first pane's `HERDR_PANE_ID`. So each Codex case stops that
HOME's daemon and opens a fresh pane.
- Evidence: `group-H/r4-real/` (`codex.jsonl`, `claude2.jsonl`,
`claude3.jsonl`, `claude4.jsonl`, `harness/`).

Results:

| Case | Result |
| --- | --- |
| Codex first turn: Chat `$imagegen` while the session is null, then the
same process reports its session | row kept (`accepted`) |
| Codex restart with polls running (Ctrl+C twice, `codex`) | row gone as
soon as Codex quit, still gone once the new session appears |
| Unpolled replacement: send, no poll, Codex replaced, new session, then
the first poll | `[]` |
| Wrapper loop `sh -c 'while :; do codex; sleep 1; done'`: group leader
`sh` unchanged, native pid replaced | row gone at the restart and after
the new session |
| Codex Ctrl+Z, two polls, `fg`, then its session appears | row kept
throughout |
| Claude `/context`, then `/exit` and `claude` in the same pane | old
row gone (`claude2.jsonl`). An earlier run on 746f148, which had
01a7a0e's bind-only-before-a-session rule, saw herdr keep the old
session id for 20 s, and the old row stayed
(`claude2-746f1489-stale-session.txt`). ba5a77c fixes that |
| Claude `/context`, then Chat `/clear` | only `/clear:unknown` is kept,
in the new session |
| Claude Chat `/clear`, then `/context` once Claude is idle | both rows
kept (`/clear:unknown`, `/context:result-observed`) |

## 40. Agent names in New session and History use each agent's own name

**Cause.** `AGENT_LABELS` in `server/index.ts` feeds the agent kinds
listed to the New session and History pickers. It had no entry for
hermes, grok, agy, devin or cmd, so F3 saw "hermes" in lowercase beside
"Claude Code" and "Oh My Pi".

**Fix.** Each gets its product name: Hermes, Grok, Antigravity, Devin,
Command Code. Kinds and ids are unchanged.

herdr 0.9.5 has no `cmd` kind, so Command Code's label is verified in
code only. No herdr in the tests lists it.

**Tests (failing first).** The agent-kinds case in
`server/api.contract.test.ts` asserts:
- `Hermes` for hermes;
- that every listed `SUPPORTED_AGENT_KINDS` kind other than `pi` (whose
product name is its id) carries a label that is not its kind.

The builder's version also asserted that every supported kind is listed.
`omo` and `gjc` are listed only where the server finds them on PATH, so
that held only on a machine with both installed and failed under `env
-i`. The integration narrowed it to the kinds that are listed. With
`server/index.ts` from 7cd3648 the case fails, and on the branch it
passes (`group-H/t40-failing-first.txt`).

## 41. Rewind to here works with Claude Code 2.1.305 (F3 r2 D3)

**Cause.** Claude Code 2.1.305's rewind confirm step asks "Confirm you
want to restore to the point before you sent this message:". 2.1.304
asked "Confirm you want to restore the conversation to the point before
you sent this message:". The rewind reader matched only the older line
(`CLAUDE_REWIND_CONFIRM_RE` in `server/dialogs/claude.ts`), so it never
read the confirm step, and every Rewind to here ended "Not rewound" (F3
r2: `F3-real-qa-r2/rewind.390.confirm.txt`,
`F3-real-qa-r2/rewind.1280.fresh-msg.confirm.txt`).

**Fix.** `/^Confirm you want to restore (?:the conversation )?to the
point before you sent this message:$/` accepts both wordings and stays
anchored to the whole line. Nothing else in the reader changes.

**Fixtures.**
`server/fixtures/claude-rewind-2.1.305-confirm-kiwi-145.txt` and
`server/fixtures/claude-rewind-2.1.305-confirm-cherry-120.txt` come from
those two F3 r2 recordings. The recordings' text dump dropped blank
lines, so the blank lines between the panel's blocks were put back as
2.1.305's list draws them live. The fixtures are therefore reconstructed
screens, not byte-for-byte recordings.

**Tests (failing first)** in `server/dialogs/claude.test.ts`:
- "reads Claude Code 2.1.305's confirm step, and answers it": both
fixtures parse to the four choices, with the sent message as the body.
The reader's interaction answers Restore conversation with one Enter.
- "reads either wording, and no other confirm question": the 2.1.304
fixture still parses. Four near-misses parse to nothing: restoring "the
code", "the code and conversation", "before this message", and "delete".
- With only the regex change reverted, the 2.1.305 test fails (28 pass,
1 fail). Restored, 29 pass (`group-H/t41-failing-first.txt`).

**Gates on e1a304a.**
- `bun run typecheck`: exit 0.
- `bun test server/dialogs/claude.test.ts server/conversation.test.ts`:
`server/dialogs/claude.test.ts` passes 29 of 29.
`server/conversation.test.ts` passes 140 and fails 1: "Devin pane
identity > forgets a pane's Devin history cache on pane teardown and
test-suite reset" timed out at its 5 s limit.
- That test is load-bound and unrelated to this change. Run alone at
load 25 (another session's browser QA), it passed and failed at every
commit tried, 4.5-7.3 s per run: origin/main a59edbd, e16904c,
ba5a77c and e1a304a.
  - Todo 41 changes only the Claude rewind reader.

## 42. Command rows survive an unknown agent process, a stale session id
and a suspended agent off Linux (F2 r3, omo review)

b5c0af5, 70b67f3 and c1970d1; they answer F2 r3 MEDIUM-1 and MEDIUM-2
and the omo review findings.

- **No process found is no evidence of a replacement.**
- When the binding finds no foreground process that matches the agent,
the send is marked `unmatched`. That happens on native Windows, which
lists only the shell, and for an agent whose executable is named
differently from its kind.
- An `unmatched` send adopts the session that the same agent reports in
the same terminal as its own (the session's `agent` must equal the
send's; omo review r2 S1). Before, such a send was never adopted and was
hidden once the agent's session started. A failed `pane.process_info`
read at binding counts as unmatched too.
  - A later session or terminal still hides it.
- A send whose process was found but answered as started after the send
(the late-answer guard) is still left unidentified and never adopted.
- **Stale session id (F2 r3 MEDIUM-1).** A restarted Claude Code keeps
the previous session id in herdr until its first turn.
- A send bound to a different process than an earlier send under the
same session identity, or one whose earlier send was already replaced,
therefore waits for its own session.
- It remembers that stale identity, and adopts the next session the same
terminal reports, never the stale one.
  - So it stays once herdr corrects the id.
- The wait is bounded (omo review r2 B1, 436c9b2). A stale send adopts
the next session only within `STALE_SESSION_LAG_MS` (60 s) of its send,
while its bound process is found unchanged, and only while no later Chat
`/clear`, `/new` or `/resume` exists in the pane. herdr's lag was seen
at 20 s (F2 r3) and at least 45 s (todo 42's Claude probe), so 60 s
covers it with a margin while keeping the window short. Past the bound
it stays with the session it was made in and is hidden on the next
session change like any earlier-session row, and its extra
`pane.process_info` reads stop.
- This matters when the new process truly owns the same session (`claude
-c`, `claude --resume`, `codex resume`): a resumed session's rows no
longer follow a later Chat `/clear` into the cleared conversation, nor a
terminal-typed session change after the bound.
- **Liveness off Linux.** Linux still reads `/proc`: a pid that is gone,
or that has another start time, means replaced.
- macOS and other Unixes check the pid with `kill(pid, 0)`
(`processAlive`, where `EPERM` counts as running). A gone pid hides the
send at the first look. A live one keeps it, including an agent
suspended with Ctrl+Z, because these systems hand a pid out again only
after cycling through the whole range.
- Windows reuses a finished process's pid at once, so a live pid proves
nothing there, and the two-misses-in-a-row rule stays.
- **Timing.**
  - A poll waits at most 250 ms for a send's binding (it was 1 s).
- The binding's macOS start-time read (`ps -o etime=`) is now an awaited
child process (`processStartUpperBoundAsync`), not a synchronous call
that held the event loop.
- The session-change identity is resolved in a loop before the filter
instead of inside it.
- **`paneProcessRoots` launcher preference (70b67f3, tests only).**
- New `server/conversation.test.ts` case "reads an npm Codex's roots
from its native child, not the node launcher listed before it": the
roots come from the native child even when herdr lists the `node`
launcher first.
- The Hermes repeat test now stubs the binding's process read, and the
scroll-read test awaits the read itself instead of yielding a turn.
- **c1970d19** is a comment only: what a scroll read that answers after
the send was noted does.

**Tests (failing first).**
- New in `server/outcome.test.ts`:
- "keeps codex's first-turn receipt through session discovery when its
process cannot be told", for "no process" and "another program";
- "keeps a send to a restarted agent once herdr corrects the session id
kept from the process before", polled and unpolled;
- "leaves a send unidentified when its process answers as started after
the send, and never adopts it";
- "hides a send whose process cannot be told apart only after two misses
in a row";
- "keeps a send while its live process is out of the foreground
(Ctrl+Z)";
- "hides a send at the first look without its process once that process
is gone".
- New in `server/core/outcome-delivery.test.ts`: the "no agent process"
scenario.
- With b5c0af5's source hunks reverted, 6 of 122 fail: "no agent
process", both stale-session cases, both cannot-be-told cases, and the
late-answer case. Restored, 122 pass (`group-H/t42-failing-first.txt`).
- The two-miss, Ctrl+Z and gone-pid tests pass with and without the
change; they pin each platform's rule. They branch on the real
`process.platform`, so on this Linux host they ran the Linux branch
only.

- New in r2 (0901a31): "does not carry a resumed session's send into
the conversation a later Chat /clear starts" (review PROBE-B1); "drops a
resumed session's send once its session changes in the terminal after
herdr's lag, and stops reading its process" (PROBE-B2, with a
process-read count); "does not adopt a send no process was bound to into
another agent's session in the same terminal" (PROBE-C). All three fail
on 9c80d7b and pass on the fix. Mutations: dropping the lag bound,
ignoring a later Chat session change, or dropping the agent check each
fails exactly its test; F2's MEDIUM-1 probes (polled and unpolled) still
pass (`todo-42/r2/`).
- **Known limit.** Within the 60 s bound, a session change typed in the
terminal of an agent that resumed the same session can still carry a
send made after the restart into the new session. The fragment says so.

**Not run.**
- herdr correcting a restarted Claude's session id was not seen live.
With a fake key, Claude Code never completes a turn, so herdr never
reports the new id. Only the unit cases above (polled and unpolled)
cover the stale-session rule.
- The Windows branch (two misses, no late-answer guard) runs only in the
Native Windows CI check. The macOS branch (`kill(pid, 0)`, `ps` start
time) was not run on a Mac either.

## 43. Hermes `/status` card on a first send, after a missed one, and
with a scrolled-up device (F3 r2 D1; CodeRabbit 4337513017)

**First-send cause (685176f).** When a Chat page attaches its terminal,
Hermes redraws its input area. On a loaded host, the send-time detection
read (`freeHermesInput`'s live read, right before `agent.prompt`) can
catch the `❯` input line alone at the bottom, with no status bar and no
rules.
- `hermesChromeStart` then found no chrome, so the send-time snapshot
kept the `❯ <placeholder>` line in its tail.
- No later screen, with its chrome stripped, contains that line, so
neither the text anchor nor the row anchor could place the snapshot.
- The first `/status` in a fresh pane went unowned ("Runs in the
terminal"), and each later send in that pane inherited it.

`hermesChromeStart` now also takes a lone input line at the bottom as
chrome: one with no rule above it, under the status bar when that is
drawn. The fixture
`server/fixtures/hermes-results/status-first-redraw-120.json` was
recorded from real Hermes 0.21.5 under load.

**Later-repeat ownership (f133fa5).** Before, a same-command send that
owned nothing reserved nothing (`null`), and every later repeat
inherited that null for ten minutes.
- Now, when an earlier send's reservation is unknown, or its anchor is
lost and it was not result-observed, a later send can still reserve its
first echo (`printedPastEarlier`).
- It does so only when its own screen already shows, below the latest
earlier send whose count is known, as many complete blocks as that send
is owed. A complete block is an echo with a result block the reader
recognizes under it, and herdr's history rows place the earlier send's
screen, with no text anchor.
- With fewer complete blocks, or either send missing its row, the later
send stays unreserved, so it never claims a block that could be an
earlier send's.

**Detection anchor (a74003f; CodeRabbit 4337513017).** herdr's
detection read is the live screen even while a device has the terminal
scrolled up. On herdr 0.9.5 it was byte-identical at offset 12.
- Its first row is `max_offset_from_bottom`, not the viewport's
`max_offset_from_bottom - offset_from_bottom`.
- The send-time row (`server/core/composer.ts`) and the poll's row
(`server/prompt.ts`) both use `max_offset_from_bottom` now. Before, a
scrolled-up device shifted the anchor by its scroll offset.

**Tests (failing first)** in
`server/core/hermes-repeat-outcome.test.ts`:
- "owns a fresh pane's first /status, read while Hermes redrew its input
area for a terminal that attached (F3 r2 D1)";
- "owns its /status block while another device has the terminal scrolled
up: herdr's detection read is the live screen";
- "a /status whose earlier /status lost its anchor > lets each later
send on a stable screen own its own block once the earlier block is
complete on its screen";
- "a /status whose earlier /status lost its anchor > keeps a later send
unclaimed while the earlier send's block could still be the one it
echoes".

On the base ba5a77c the first three fail and the fourth passes, as a
guard should (`todo-43/failing-first.txt`).

Added in r2 (0901a31; omo review r2 S2, S4), both guards that pass on
9c80d7b and are pinned by mutation (`todo-42/r2/`):
- "a /status whose earlier /status lost its anchor > counts every
earlier send a later one may owe, not only the one whose count is
known": the earlier send whose count is known, then a send whose screen
and row were lost, then the next send whose screen shows only the known
block complete. The next send stays unclaimed when the pending send's
block prints. It fails with `complete >= owed` mutated to `complete >=
1`.
- `server/core/outcome-delivery.test.ts` "records a send's history row
as the live screen's while another device has the terminal scrolled up":
at send time the device is scrolled up by 3, and the noted send's row is
`max_offset_from_bottom` (7). It fails with the composer reverted to the
viewport row. (Hermes itself refuses a send while the device is
scrolled, `freeHermesInput`, so the send-time half is pinned through a
Claude send.)

**Real Hermes (`todo-43/`).**

Setup:
- Real Hermes v0.21.5+8892.g354724f and herdr 0.9.5, under `env -i` with
`HOME=/tmp/t43/home` and a fake `OPENROUTER_API_KEY`.
- An isolated herdr session, `t43-verify`.
- The server on the Tailscale address, through a recording proxy that
logs every `pane.read` answer and scroll row (`rpc.jsonl`).

Results:
- **Head:** 42 of 42 ordinary `/status` sends are result-observed, each
with its own card. They ran across 14 fresh panes, 21 at 1280 and 21 at
390, and 18 of them under CPU load. Five first sends hit the lone-input
redraw frame at send time and were owned (`real-head/summary.json`).
- **Base ba5a77c** under the same load: 3 of 4 fresh panes lost their
first `/status` ("Runs in the terminal"), and each pane's later sends
inherited it (`real-base/`).
- **False-card control:** at each width, the proxy dropped one delivery
(`agent.prompt` answered but never forwarded). That send stays
"Accepted", and the send after it stays "Runs in the terminal" instead
of claiming a block that is not its own (`real-head/summary.json`,
`dropProbes`).

Fragment: `changelog.d/hermes-first-status-result.md`.

## Gates
On the final head 0901a31 (base a59edbd), each run on its own at load
average 2.7-3.9 on 32 cores (`todo-42/r2/`):
- `bun run typecheck`: exit 0.
- `bun test server/outcome.test.ts server/core/outcome-delivery.test.ts
server/core/hermes-repeat-outcome.test.ts server/dialogs/hermes.test.ts
src/lib/outcomeChip.test.ts server/core server/conversation.test.ts
server/process-start.test.ts server/opencode-native.test.ts
server/pane-routes.test.ts server/dialogs/claude.test.ts`: 395 pass, 0
fail in 17 files (`todo-42/r2/units.log`).
- `bun scripts/command-outcome-demo-regression.ts`: 4 runs, all pass
(`todo-42/r2/command-outcome-demo.log`).
- Contracts `server/hermes.contract.test.ts`,
`server/outcome.contract.test.ts`,
`server/pending-input.contract.test.ts` and
`server/api.contract.test.ts`: 231 pass, 0 fail
(`todo-42/r2/contracts.log`).
- They ran in one `bun run check run` on its own isolated herdr, under
`env -i` with an isolated HOME.
- PATH held stand-in `muse` and `codex` (`exec cat`); no real `hermes`
or `claude` resolved.
  - The shared default test session was not restarted.
- `UNIT_TEST_WORKERS=2 bun run check fast`: workflow syntax, generated
types and themes, typecheck and build ok; unit 5756 pass, 0 fail, 8 skip
in 411 files (`todo-42/r2/check-fast.log`).
- The gates ran on the working tree that became 436c9b2 and 0901a31;
the commits changed no file after them.

On 9c80d7b, before the omo review r2 fixes:
- the same 11-path set: 390 pass, 0 fail in 17 files
(`group-H/r5-units.log`); demo 4 runs pass; contracts 231 pass; `check
fast` unit 5751 pass, 0 fail, 8 skip (`group-H/r5-*.log`). That lane
includes the Devin cache test that timed out under load in section 41;
it passed there.

Todo 41's own gates are in section 41, and todos 42 and 43 name their
failing-first runs in their sections. The checks below ran on earlier
heads.

On ba5a77c (base a59edbd):
- `UNIT_TEST_WORKERS=2 bun run check fast`, run on its own (load average
4-5 on 32 cores): workflow syntax, generated types and themes, typecheck
and build ok; unit 5733 pass, 0 fail, 8 skip in 411 files
(`group-H/r4-check-fast.log`).
- A first run on d4dd48d8 failed in `server/conversation.test.ts`: 11
"pane process roots" cases compared `paneProcessRoots`' whole answer
with `toEqual`, and the answer now also carries `processes`.
- The fixup in 01a7a0e compares the same fields (`omo`, `environ`,
`pid`, `cwd`) with `toMatchObject`. With it,
`server/conversation.test.ts` passes 141 of 141.
- `bun run typecheck`: exit 0.
- `bun test server/outcome.test.ts src/lib/outcomeChip.test.ts
server/core server/omo.test.ts server/opencode-native.test.ts
server/core/hermes-repeat-outcome.test.ts
server/dialogs/hermes.test.ts`: 214 pass, 0 fail in 14 files (unit
mode), before the new `server/outcome.test.ts` case. With that case,
`server/outcome.test.ts` passes 86 of 86.

On d4dd48d8, whose source is identical to ba5a77c (the rebase changed
only `server/conversation.test.ts`):
- `bun scripts/command-outcome-demo-regression.ts`: 4 runs, all pass
(`group-H/r4-command-outcome-demo.log`).
- Contracts `server/hermes.contract.test.ts`,
`server/outcome.contract.test.ts`,
`server/pending-input.contract.test.ts` and
`server/api.contract.test.ts`: 231 pass, 0 fail
(`group-H/r4-contracts.log`).
- They ran in one `bun run check run` on its own isolated herdr, under
`env -i` with an isolated HOME.
- PATH held stand-in `muse` and `codex` (`exec cat`) and links to bun,
node, herdr and git; no real `hermes` or `claude` resolved.
  - The shared default test session was not restarted.
- Real Codex 0.163.0 and Claude Code 2.1.305 cases for todo 39 (see
section 39): all as expected (`group-H/r4-real/`).

On earlier heads, not re-run on the final head:
- `bun test scripts/changelog-fold.test.ts`: 12 pass, on f958458's
fragment.
- fc075a23 (base 7cd3648):
- `bun scripts/agent-skill-menu-demo-regression.ts claude-terminal`: 36
pass (`group-H/claude-terminal-demo.log`);
- `bun test src/conventions.test.ts`: 6 pass. This also ran inside the
final head's `check fast` unit lane.

Release guard: nothing under `shared/` changes, and
`REMOTE_BUNDLE_VERSION` is not raised.

### Added
- Claude Code, grok and Antigravity messages in Chat offer Rewind in
terminal: it sends nothing,
preserves your draft and opens the pane's terminal with a hint to run
the agent's own `/rewind`.
Chat follows the native conversation when the agent records its new
branch.
  ([#1500](#1500))
- Chat sends wait while Claude Code's rewind dialog is open in the
terminal, so a message cannot
land in the dialog's own text field.
  ([#1500](#1500))
- Sending a bare `/goal` to Claude Code from Chat now shows its goal
status (the condition and how
long it has run, or that no goal is set) in a card with Close, instead
of leaving the dialog in
  the terminal.
  ([#1450](#1450))
- Command Code (`cmd`) agent panes now show their conversation in the
web UI: transcript
resolution from `~/.commandcode/projects/`, `/tree` branch projection,
model and context
window from Command Code's catalog, permission prompts, and skill
discovery.
([#1399](#1399) by
@diegolhambi)
- An ordinary command sent from Chat says what became of it under its
bubble, from evidence only:
accepted, waiting for your answer (with a way to the card), done with
what showed it, runs in
the terminal (with Open Terminal), or no result seen. A command the
agent keeps no transcript
turn for still shows its sent row. A terminal screen that
changed after the send folds under the chip as an unconfirmed preview,
never proof that the
command finished. Terminal-only commands instead leave one quiet row
with Ran in terminal and
Open Terminal, without outcome chips or unconfirmed previews. Hermes
informational result cards
keep showing what the agent printed.
  ([#1450](#1450))
- Settings → Terminal → Follow herdr's focus: when herdr moves to
another pane (a click in its
window, `herdr tab focus`, a script), this device opens that pane too.
Off by default, and set per
device, so a phone open on another pane stays there.
  ([#1432](#1432))
- Hermes `/help`, `/status`, `/version`, `/profile` and `/reload-skills`
sent from Chat show what
Hermes printed in a card closed with Dismiss, which sends nothing to the
pane.
  ([#1450](#1450))
- Chat lists the commands and skills an OpenCode v2 pane's own
background service reports for its
folder, refreshed when they change, and settles a command it sent from
that session's own events.
The service is only read, never started, and only when it is the process
its service.json names.
  ([#1489](#1489))
- An `@skill` sent from Chat to OpenCode v2, at once or queued, is typed
through OpenCode's own
autocomplete so the skill is attached; when the row does not show, the
message says the skill
went as plain text and points to the terminal.
  ([#1489](#1489))
- An OpenCode 1.x question is a card in Chat with each option and its
description, answered from
  the card.
  ([#1489](#1489))
- Chat lists a Codex pane's skills as the running Codex daemon of that
pane's own Codex home reports
them (Codex 0.157 or later; on Linux, or when the server's Codex home is
set), and a gjc pane's
skills as the SDK broker it is bound to reports them, asked through the
gjc CLI in the background.
The daemon and the broker are never started for this; without one, the
skills read from disk stay.
  ([#1489](#1489))
- New workspace and New tab start an agent with its model, effort and
first message where the
agent's command line takes them (Claude Code and Codex: `--model` and
the effort level; Codex also
takes the message as its own argument, behind `--` so a message like
`logout` is never a command).
For Claude Code and every other agent the message is queued on the PC as
the pane's first pending
message and sent once when the agent is ready, through the page that
started it; it shows as a
pending message that can be discarded or sent now. If that page
disconnects, reloads or leaves the tab
first, the message is held, never sent on its own, until someone sends
it now or discards it. A message the PC
could not queue, or one an older PC ignores, stays in the new tab's
message box and the dialog says
so. Pending first messages are kept in memory: restarting the web UI's
server drops them. A Codex
message with a tab or a line break is sent as the first message rather
than on its command line,
and the PC refuses launch arguments with control characters, an unknown
effort level or a model
that is not one word. Under the message, the agent's skills in the
folder are listed before it runs, each a chip that puts the skill as the
agent types it into the
message (`$name`, `/name`, or `@name` for OpenCode 2). A first message
to OpenCode attaches its
`@skill` as any Chat send does; before OpenCode has a session nothing
tells its version, so the `@`
is sent as the plain text it is.
In a desktop window the chat's composer can show a New tab with an
agent… button that opens the
dialog on the pane's workspace; it is off until turned on in Settings →
Chat → Composer (New tab
button), and the tab menu opens the dialog either way.
  ([#1512](#1512))
- omp's `/handoff` runs from Chat, with or without focus instructions:
while omp writes the
handoff document, Chat shows a card that cancels it, and afterwards Chat
stays on the same
conversation, which omp compacts in place.
  ([#1450](#1450))
- omp's model picker shows in the chat as a card, as Claude Code's,
Codex's and OmO's lists do: pick
a model and it is picked for the session, the model in use named on the
card. `/model` sent from
an omp chat opens it: the chat sends it as `/switch`, because omp's own
`/model` opens its role
hub, whose picks are saved as the default for later sessions.
([#1377](#1377) by
@evanwy0223)
- OpenCode 2.0 or later: its questions are answered from the chat's
prompt card through OpenCode's
own background service instead of keystrokes, one question at a time
with Back to change an earlier
answer, and sent together after the last. Permissions, OpenCode 1 and
other prompts keep the
existing generic fallback card and Terminal.
([#1257](#1257) by
@bardiz12)
- HTTP requests and WebSocket handshakes now carry request IDs and write
bounded completion
records in the app's state directory, so the owner can correlate a
response with its request.
  ([#1558](#1558))
- A workspace's row menu has Earlier sessions…: the sessions each agent
kept for the workspace's
folder, read from the agent's own store on the PC, newest first with the
title, the first prompt
and the time. Resume starts the agent on that session in a new tab of
the workspace; a session a
pane already shows is opened by selecting that pane instead. Picking
`/resume` in the chat's menu
opens the same list, and the terminal command's row offers it. A PC
whose bridge cannot list
sessions yet has no such item.
  ([#1494](#1494))
- Agent rows show what herdr plugins report on a pane, such as the Quota
plugin's usage left or a
task's progress, in a small dim line under the row. They follow the
`$name` tokens of your herdr
`[ui.sidebar.agents]` rows (every token where none are set), change
live, and say a long value
whole in the tooltip and in a phone's ⋯ sheet. A line a plugin reports
for the agent now updates
as soon as herdr says it.
  ([#1341](#1341))
- `HERDR_WEB_TAILSCALE_HTTPS_PORT` names the HTTPS port the installer
publishes the app on with
`tailscale serve`, so 443 can stay free for something else. Settings →
Phone & devices suggests
that port too. When another service already uses it, the installer stops
and says so instead of
taking another port, which would move the address an installed app
opens.
  ([#1433](#1433))

### Changed
- The chat's `/` menu lists the commands that run in the terminal apart,
last, under Terminal
commands, each with a Terminal badge, as the PC's command catalog
describes them. Pick or type
one and send it normally: once accepted, the pane's own terminal opens
over the chat, in this
browser only, while the chat stays where you were reading and the pane
stays on Chat. A head
says what ran, with Back to Chat and Keep Terminal; the header's Chat,
Ctrl/Cmd+Shift+J and a
phone's Back return too, and Escape goes to the agent. On a desktop the
message box stays under
the terminal, dimmed; on a phone the key bar takes its place. Claude
Code's panels (`/config`,
`/permissions`, `/theme` and the like) can bring the chat back by
themselves when you close them,
once Claude records it; other agents' recorded screens do once their
input row is back, and the
rest return by hand. A Ran in terminal row's
Open Terminal shows the terminal over the chat again. Busy commands stay
queued; only an
explicit Send now opens the terminal, never automatic delivery. Refused
sends keep their draft
and note. Watch-only devices never send or open it, and /resume still
opens Earlier sessions.
Commands the agent's own menu hides are not listed, but still go when
typed by hand.
  ([#1441](#1441))
- For an OpenCode pane whose skills are `@` attachments, the `@` menu
lists them under Skills
before the files, and picking one inserts the skill's id.
  ([#1441](#1441))
- Growing OMP and Claude Code tasks now parse appended transcript
records incrementally, reducing
repeated work when long chats refresh while preserving tool results,
notices and history resets.
Live parser state shares Codex's bounded cache; earlier responses stay
unchanged as new records arrive.
([#1561](#1561) by
@Yoonwoo-Ha)
- The app is now called Ovis across the browser, install messages and
documentation, matching
the phone apps. Existing installs, settings, plugin identifiers and
update channels stay the same.
  ([#1454](#1454))
- A question card whose screen reads keep failing now says it may be out
of date ("Not updated: the
question on screen may have changed.") until a read answers again.
Answering stays safe: the
server reads the screen again first and refuses a changed question
(#1335).
([#1360](#1360) by @Noobear)
- The Earlier sessions dialog picks the agent from the same list as the
New tab and New worktree
dialogs, each agent with its icon and the chevron spaced as the app's
other controls, instead of
a plain select whose arrow sat at the far edge.
  ([#1573](#1573))
- A finished agent opened in one browser is read in every browser of the
same server: opening it
on the phone clears its DONE dot on the desktop too, and the other way
around.
  ([#1463](#1463))
- Remove PC has a restrained red background and border before and after
its confirmation step,
retaining visible keyboard focus and the explicit second click to remove
the registration.
([#1394](#1394) by @snowykr)
- After a terminal command sent from Chat, Codex, omp, pi, gjc, OmO,
Hermes (classic CLI), Devin,
agy and Grok bring the chat back by themselves when you close the picker
or panel it opened,
once the agent's own empty input row is back on the screen. Only the
commands each agent was
recorded doing that for count (Codex's `/permissions`, omp's
`/settings`, Grok's `/resume` and
the like); print forms, toggles, editor handoffs, Codex's `/side` and
`/btw`, OpenCode and
anything unrecorded still return by hand.
  ([#1637](#1637))

### Fixed
- The New session and History pickers show Hermes, Grok, Antigravity,
Devin and Command Code by
their own names instead of their lowercase ids.
  ([#1543](#1543))
- The terminal attach process and its Node host no longer inherit the
server's `HERDR_WEB_TOKEN`
environment variable; they keep the socket path and the rest of their
environment.
  ([#1562](#1562))
- Choosing Jump to latest remembers bottom-follow before the virtual
list scroll completes.
Switching to Terminal immediately after that choice and returning to
Chat no longer restores
the older reading position. This fixes explicit Jump-to-latest intent,
not arbitrary upward
restoration on long, active or reasoning-heavy tabs.
([#1346](#1346) by
@Yoonwoo-Ha)
- Remember bottom-follow without measuring rendered rows or looking up a
turn anchor; readers away
from the end retain the existing paragraph-and-offset restoration.
([#1346](#1346) by
@Yoonwoo-Ha)
- Selecting and copying chat text now preserves the text as shown
instead of adding Markdown
syntax. Rich-text paste and the whole-answer MD and TXT copy actions
remain available.
  ([#1409](#1409))
- Chat no longer treats a command that leaves the agent waiting in a
screen Chat cannot show as an
ordinary send. Claude Code's `/usage`, `/cost`, `/status`, `/help`,
`/memory`, `/mcp` and
`/add-dir`, Codex's `/new` and `/fork` in a repository, `/cd`, `/usage`,
`/copy` and `/side`,
and the panels, pickers, confirmations and editors of omp, gjc, OmO,
Hermes, Devin,
Antigravity, OpenCode and Command Code are now Terminal commands: sent,
they open Terminal with
Back to Chat. A value typed with a picker command, such as `/fast off`
or `/export notes.md`,
  stays an ordinary send.
  ([#1596](#1596))
- A Claude Code skill or custom command named like a built-in command
(`/compact`, say) is listed
once in the composer's command menu, as the skill Claude runs, instead
of beside the built-in.
  ([#1441](#1441))
- Reconnects and unconfirmed sends can leave a bounded browser-side
diagnostic trail on this
server, without recording messages, credentials, addresses or terminal
output.
([#1357](#1357) by
@christi4nity)
- The chat and the session history now apply one rule to Codex rollouts:
a subagent's rollout (one
naming a parent thread or an agent role), an exec run's, or one whose
ids are null or contradict
its file name is no longer read as a pane's conversation or offered to
resume, while older
rollouts without those fields still are. A rollout whose first line is
longer than the chat or
the history reads is left out instead of read unchecked.
  ([#1592](#1592))
- Two Codex panes in one folder whose answers are short each show their
own chat again with Codex
0.163, which starts a thread in the same second as its process; before,
both said the
conversation was unavailable about as often as not.
([#1522](#1522) by
@jiaoew1991)
- A Codex pane with only short answers still finds its chat after
another pane in the same folder
runs `/new`: a thread that pane shows a long answer of under its first
prompt, is bound to, or
that herdr reports for it no longer leaves this pane unsure.
([#1522](#1522) by
@jiaoew1991)
- Of two Codex panes in one folder with short answers, the one started
first finds its chat too
once the other shows its own first exchange, where it said the
conversation was unavailable.
([#1522](#1522) by
@jiaoew1991)
- On a phone, a composer notice's actions (such as the `/resume`
notice's Earlier sessions, Open
Terminal and Run in Terminal) wrap onto their own line, and its dismiss
button stays visible
instead of being cut off at the edge.
  ([#1543](#1543))
- Wrapped and multiline message drafts are sized with an out-of-flow
measurement field instead of
collapsing the live input, preserving transcript spacing and the
paragraph being read (#1337).
([#1347](#1347) by
@Yoonwoo-Ha)
- The measurement field shares the live input's Korean, Japanese and
Chinese line-breaking rules,
including Korean placeholder wrapping, so localized drafts keep their
natural height.
([#1347](#1347) by
@Yoonwoo-Ha)
- Local Herdr timeouts, machine connection failures and recoveries, and
abnormal WebSocket closes
now leave timestamped, rate-limited diagnostic records without logging
input or credentials.
([#1356](#1356) by
@christi4nity)
- The browser connection trace no longer re-posts a batch the server
refuses with a 4xx status,
resumes reporting after you sign in again on the same page, and
malformed reports can no longer
use up the server's shared report quota.
  ([#1423](#1423))
- Pairing a device, renaming or revoking one, and the web push key and
subscriptions are now
flushed to disk before they replace the previous file, so a crash or
power cut right after the
change is much less likely to bring back a revoked device or make every
phone subscribe to
alerts again. How much survives a power cut still depends on the disk
and filesystem honouring
the flush; on Windows the folder itself is not flushed.
  ([#1568](#1568))
- File search and path lookup no longer run a repository's fsmonitor
hook, follow Git variables
the server was started with, or wait on a Git listing that never ends: a
listing past 16 MiB
or 3 seconds, or one cut off mid-name, falls back to the folder walk.
  ([#1570](#1570))
- **Files → Changes** no longer runs a program named `false` found on
`PATH` with Git 2.35 or
older, and reads the repository asked about even when the server was
started with
  `GIT_COMMON_DIR` set.
  ([#1570](#1570))
- Keep sidebar status updates responsive after many completed turns by
caching read finishes and
bounding their in-memory history instead of repeatedly scanning browser
storage.
  ([#1536](#1536))
- A first message from New workspace or New tab no longer waits forever
when the agent started for
it quits before the message goes: it is held for Send now or Discard.
The same happens when an
agent of the same kind that the user starts later comes up instead, and
when the page that sent it
leaves the pane while it is being typed, so the pane's other queued
messages are not stuck behind it.
On native Windows the PC sees only the pane's shell, so it cannot tell
that the agent quit: there the
message keeps waiting until you discard it.
  ([#1532](#1532))
- GJC Chat hides internal agent-to-agent IRC messages and automatic
replies, while keeping user
  messages that quote them visible.
  ([#1535](#1535))
- GJC questions using the Nerd Font selection cursor now show their
actual choices in chat
instead of only Up, Down, Enter and Esc controls. Moving the cursor
preserves the question's
identity and uses the current selection when answering.
([#1553](#1553) by
@Kinetic27)
- GJC chats on a busy machine give cold session lookups up to five
seconds per command and
ten seconds overall instead of timing out after two seconds per command.
Session snapshots
retain the shorter status lookup budget; failed status lookups
are held for 30 seconds without preventing chat reads from retrying.
([#1364](#1364) by @sskys18)
- GJC chat and status reuse their verified session binding when other
sessions write heartbeats,
instead of restarting the SDK lookup for every pane. Session changes
still require SDK validation.
  ([#1343](#1343))
- Grok keeps Chat with older sessions open and uses Terminal when the
session is uncertain.
([#1352](#1352) by
@infowolfe)
- One failed health check no longer shows herdr offline or switches a
Windows PC's panes to the
terminal lens: the header says Status unknown with the last herdr it
saw, and herdr unavailable only
after a second failure in a row.
  ([#1581](#1581))
- A malformed, oversized or misaddressed answer from herdr now fails
only the request it answers,
with a clear error, instead of hanging until the timeout, growing the
server's memory or being
taken as another request's answer; event subscriptions report such a
frame and close.
  ([#1584](#1584))
- Hermes `/status` sent from Chat now shows its result card on the first
send in a new pane too, even
when the computer is busy. A send whose result was missed no longer
leaves every later `/status` in
that pane without a card for ten minutes. The same holds when another
device has the terminal
  scrolled up.
  ([#1543](#1543))
- Sending Hermes `/status` (or another informational command) from Chat
again now shows that send's
own result card each time, instead of "Runs in the terminal" once
earlier identical results fill
  the terminal.
  ([#1543](#1543))
- Slow API requests can return their result or timeout error instead of
losing the HTTP connection
while bounded Herdr calls are still running.
([#1355](#1355) by
@christi4nity)
- The app can be installed behind a reverse proxy that signs in every
request (Authelia,
oauth2-proxy and the like): the browser now sends its cookies with the
manifest request.
([#1332](#1332) by
@ascandroli)
- Mouse wheel and phone swipes scroll terminal history on Windows and
other mirrored panes;
read-only connections still only pan their local grid.
([#1446](#1446) by
@Davesaggio)
- A Codex started in a terminal shows its model and reasoning level
before the first message,
from Codex's live footer, while the chat shows the terminal output.
([#1374](#1374) by
@evanwy0223)
- Before the first message, a model switched in the terminal shows in
the message box once the
supported agent draws it. omp still waits for its transcript to record
the model; the bridge
never runs `omp models`, which can refresh the CLI's OAuth credentials.
([#1374](#1374) by
@evanwy0223)
- A pane that runs OmO is now treated as OmO by the composer even while
herdr labels it Claude
(OmO's Claude child is running): sending a message no longer stops on a
Claude input-box draft
refusal or tries to clear a Claude draft there.
  ([#1441](#1441))
- A multi-line message sent to an OmO pane that herdr has no agent label
for is now pasted as one
bracketed paste instead of line by line, so its first line is no longer
sent alone.
  ([#1441](#1441))
- An OmO pick list (an extension's select, such as the init-deep
advisor's "Run now / Skip this
time / Never…") shows in the chat as a card with its own options again,
instead of a "Waiting for
input" card offering only ↑, ↓, Enter and Esc. A pick list that counts
down in its title keeps
the same card while it ticks, so an answer tapped a second later is not
refused as stale.
([#1495](#1495) by
@nahwan-kim)
- An omp pane started with an absolute `--cwd` shows its chat when herdr
names no session file
and the directory herdr reports is not omp's own, such as the shell's or
a symlink's real path:
the session is still looked up in the directory herdr reports first,
then under that `--cwd`.
  ([#1594](#1594))
- A message or command sent from the chat to an omp that is still
starting no longer stays typed
in its input box: omp shows the box before it takes an Enter, so the
send waits until herdr's omp
integration reports the session (omp does so once it takes input), for
an omp started moments ago.
  ([#1475](#1475))
- A new OpenCode 2 pane offers your project and user skills as `@`
attachments before its first
session starts, using the OpenCode version the PC's install reports,
instead of listing them as
OpenCode 1 `/` commands. Once the pane has a session, that session's
version decides as before.
  ([#1543](#1543))
- Native question answers stay bound to the current pane, session and
form. Local service discovery
rejects stale or non-loopback endpoints, bypasses environment proxies,
and bounds slow requests.
([#1257](#1257) by
@bardiz12)
- Polling does not invalidate an unchanged question or turn an
acknowledged answer into an error.
A temporary service outage keeps an already displayed native card and
its unsent edits.
([#1257](#1257) by
@bardiz12)
- Back and question tabs preserve unsent edits, and multiselect
submissions include both checks
and custom text. Abandoned drafts are bounded and expire after 30
minutes.
([#1257](#1257) by
@bardiz12)
- OpenCode question cards keep edits and allow immediate retries after a
failed reply, and optional
questions can be skipped. Unrelated service metadata no longer resets a
form's progress.
  ([#1421](#1421))
- Answers sent from the message box show the translated warning when
OpenCode has not confirmed
delivery. Native service connections accept only the loopback addresses
127.0.0.1 and [::1].
  ([#1421](#1421))
- Subscription usage reads OpenCode 2's own credential store before the
`auth.json` OpenCode 1
wrote. OpenCode 2 keeps its provider sign-ins in `opencode.db` and
leaves that file standing with
the key it has since replaced, so the OpenCode card reported "Sign-in
expired" for a CLI that was
signed in and working. The store's active key answers first, and a store
without one (signed out of
OpenCode 2) shows no card; the file still answers beside OpenCode 1's
store, or with none.
([#1329](#1329) by
@krish-kant)
- `GET /api/pane/read` takes `lines` only as plain decimal digits from 0
to 4294967295 and refuses
any other value (blank, signed, fractional, exponent or too large) with
400 `invalid_lines`
instead of passing it to herdr; a blank value is no longer read as zero.
Every count herdr
  takes still goes through unchanged.
  ([#1571](#1571))
- A pasted or dropped PNG, JPEG, GIF or WebP image is saved with the
extension of what its bytes
are, not the one its file name suggests, and a file that only claims to
be one of those images
is refused with its reason instead of being saved for the agent to read.
  ([#1599](#1599))
- The phone header's Chat/Terminal switch keeps clear of the bar's top
and bottom edges in both
densities: its options take the density's segmented height, so its track
is 34px tall in the 52px bar
and 32px in the compact density's 46px bar, which it used to nearly
touch.
([#1519](#1519) by
@adnankassem)
- On a phone, the header's Chat/Terminal switch centers its icons and no
longer touches the header's
bottom rule (in compact density it was as tall as the bar). The buttons
still take a full
  44 px tap.
([#1367](#1367) by @Noobear)
- A pi chat now shows its context ring for models pi lists in its
commandcode cache or model
store, not only for providers defined in `models.json`.
([#1391](#1391) by
@diegolhambi)
- Model and effort pickers no longer confirm a stale choice when the
agent stops waiting for the
answer during the final screen read.
  ([#1435](#1435))
- A Portal that stops on its own after its address was up is started
again once, on the same relay
and address, and Settings → Phone & devices says Recovering… meanwhile;
a second stop shows the
error with Portal's output instead of leaving the address down without a
word or restarting it
  again.
  ([#1587](#1587))
- When the server cannot save a shared setting or pin, Settings now says
the change is not on the
server yet, that reloading or closing the page can undo it, and offers
Retry. After three failed
saves in a row the browser resends less often (after a minute, then up
to every five minutes)
until you press Retry or the connection reopens, and edits made
meanwhile are kept. A late resend
of a refused change no longer overwrites a newer value another device
saved meanwhile, while a
setting you pick again after the refusal still wins. While the
connection is lost it waits for it
to come back instead of resending, and a connection the browser gives up
on (for example a proxy
error while the server restarts) is opened again, so alerts and other
devices' changes no longer
  stop arriving until a reload.
  ([#1595](#1595))
- A damaged or unreadable shared preferences or read-finish file is no
longer lost to the next
change. Reading never touches it; the first change moves the damaged
file, byte for byte, to
`preferences.json.bad` or `seen.json.bad` (private to you, the latest
damaged copy only), a file
repaired by hand while the server runs is kept with the change added to
it, and a file that
cannot be read or set aside is not written over, so the change fails
instead.
  ([#1564](#1564))
- A server or remote bridge that crashed no longer blocks its restart
when its old process number
now belongs to another program: startup locks and the bridge registry
record the owner's start
identity, and that other program is never signalled.
  ([#1567](#1567))
- Attaching to a terminal once again waits for input to be ready before
signalling completion,
preserving queued keys, clicks and text and the order of Stop followed
by a message.
  ([#1342](#1342))
- Opening a finished agent's terminal while its connection to herdr is
being restored no longer
clears its DONE dot from the screen left over from before: it clears
once the terminal shows
  current output.
  ([#1472](#1472))
- An oversized or unreadable answer from another PC's bridge is now
refused at a fixed size
instead of being held whole in memory: connecting, refreshing and the PC
proxy stop reading it,
report a short error in the user's language that repeats none of it, and
a watching device
never gets a prompt that could not be redacted.
  ([#1588](#1588))
- Opening a pane in another tab or on a phone, or reconnecting, no
longer copies again to the
clipboard what a program in the pane copied before; its new copies still
reach the clipboard.
  ([#1600](#1600))
- Update and restart, and the header's Update, install exactly the
release they offered: a release
published in the meantime installs nothing and is shown to be checked
first, and a server whose
supervisor cannot install by revision asks for a restart instead of
installing.
  ([#1590](#1590))
- In macOS Safari, Backspace in a terminal no longer skips a press after
deleting the last letter of
a Korean syllable: 한 then four Backspaces now also deletes the character
before it.
  ([#1363](#1363))
- Chat shows only the current agent session's command results: after an
agent restarts in the same
pane, the earlier session's commands no longer appear. A `/clear`,
`/new` or resume sent from Chat
keeps its own row in the conversation it starts, without claiming that
it finished. Commands sent
before an agent's first reply stay with that agent while it keeps
running, also through a moment
in the background (Ctrl+Z on Linux and macOS); once a different agent
takes over the pane, they
are hidden, even if Chat was not open when it happened. A command sent
to a restarted agent while
the pane still shows the previous session stays once the agent's own
session appears within a
minute, unless Chat started another session after it. Within that
minute, a session change typed
in the terminal of an agent that resumed the same session can still keep
such a command. Where
Chat cannot find the agent's program at all (native Windows, or an agent
started under another
program name), such a command stays with the session that the same agent
starts in the same
terminal; a later session, another agent or terminal still hides it.
Outside Linux, an agent
restarted under the same process number cannot be told apart from the
one before it.
  ([#1543](#1543))
- SSH PC management controls now stay within narrow sidebars: the rename
field and action buttons
reflow in place, and long labels wrap without clipping.
([#1392](#1392) by @snowykr)
- Opening a finished pane clears its DONE dot only after its answer or
terminal output is
actually visible. Covered panes, loading or failed chats, and terminal
reconnections keep
  unread finishes marked.
  ([#1342](#1342))
- Reading older chat keeps the finish unread. Jumping to the latest
answer clears it only
after the jump arrives; small scrolls above the end show the way back.
Resizing and restoring
a short chat work without an extra scroll.
  ([#1342](#1342))
- An older answer cannot clear a newer finish. Agents whose chat cannot
prove completion keep
the dot conservatively; opening their Terminal view can acknowledge the
output.
  ([#1342](#1342))
- Read finishes stay read across pane switches and browser tabs on the
same device. Every
web-server restart or update brings read DONE dots back; read records
are not carried across
  server restarts.
  ([#1342](#1342))
- The sidebar's PC control buttons are the standard control height
again, Rename shares the name
field's row in a wide sidebar, Remove PC dims like its neighbours while
a rename or disconnect

app.
  ([#1422](#1422))

- A Markdown table whose separator has a one- or two-dash cell
(`|---|:-:|`) shows as a table in the
chat, as on GitHub, instead of as its pipe-separated source.
([#1361](#1361) by @Noobear)
- Going back to the chat while a terminal command's screen is still open
(Claude Code's `/btw`
answer, a picker left up) no longer lets the next message from the
message box be typed into
that screen and do nothing, for Claude Code and the agents above (not
OpenCode): the composer
says the command is still open in the terminal, with Show terminal, and
Send (or a queued
message's Send now) keeps the message until the screen closes or you
choose Send anyway.
  ([#1637](#1637))
- Opening the app no longer flashes the dark amber default before your
saved theme: the saved light,
dark or single-mode theme (Dracula on the light side stays dark),
including System and older saved
palettes, is drawn from the first frame, and the installed app's title
bar color matches it at once.
  ([#1569](#1569))
- Settings → Updates no longer tells you to switch to main when the
server was started from an
installed update's folder instead of its source checkout: it now says to
restart the app from
the source checkout, which is what lets updates run again.
  ([#1417](#1417))
- Typing or composing text in a wide mirrored terminal no longer pulls
the view sideways.
Windows terminal wheel panning stays in place when output arrives, and
tapping the terminal
without scrolling no longer changes shared history.
  ([#1534](#1534))
- Loading the page no longer sends about 33 requests per workspace
folder to find its project icon:
each folder is asked once (`GET /api/fs/favicon`), and a folder without
an icon is an answer, not
a 404. A reverse proxy with a rate limit no longer answers 429 to the
burst, and the browser
console no longer fills with red 404 lines on every load. A 429 is no
longer remembered as "no
icon" either: the folder is asked again when its row next appears.
  ([#1339](#1339))
- A dragged workspace order no longer jumps back while its moves are
still landing: a roster read
before the move, a cached roster or another PC's update cannot undo it,
and the first roster
read after the move (another browser's later order included) replaces
it. A remote PC does this
once its bridge is updated to the next release; until then it keeps the
earlier behaviour.
  ([#1593](#1593))

### Maintenance
- Composer geometry regressions freeze the demo conversation while
keeping it idle, so turn
completion cannot change the transcript during measurements on phone and
desktop.
([#1347](#1347) by
@Yoonwoo-Ha)
- Settings and PWA draft checks target the visible input explicitly;
locale regressions cover
native draft and placeholder heights on phone and desktop.

…and say both held reasons in the reader's language (#1969)

Fixes #1547.

## Change

A pane's first message is held for one of two reasons that shared the
code `pending_target_changed`: the pane's terminal or agent session
changed, or the agent started for that message came up and ended before
the message went (#1532). The client could not tell them apart.
`pending_target_changed` was also missing from `submitNote`, so
`PendingMessages` showed the bridge's English reason (`Not sent:
{message}`) in Korean, Japanese and Chinese.

- `server/core/pending.ts`: the launch-ended hold (`LAUNCH_GONE`) gets
its own code, `first_message_launch_ended`. `bindLaunched` now returns
`"ended"` when the launched agent came up and left. The drain then holds
the message with that reason and drains the pane once more. Send now
answers that reason and re-holds with it. `launchBoundIn` stops
re-reading a message held for either reason. `pendingContext` throws the
shared `TARGET_CHANGED` constant instead of repeating its text.
- Version skew: an older client takes a Send now answer with a code it
does not know as "maybe typed", which would leave the row "Not
confirmed" with Send now hidden. The `pending-action` frame therefore
gains an optional `launch_ended: true`, which this client always sends
(`src/lib/ws.ts`). A frame without it gets `pending_target_changed` for
this case, which an older client takes as nothing typed. The held row
every connection sees keeps the new reason. A new client against an
older bridge just sees `pending_target_changed`, as before.
- `src/lib/compose.ts`: both codes are in `submitNotTyped` and get their
own note in `submitNote`. `src/components/PendingMessages.tsx` routes
both codes to `submitNote`.
- `src/lib/i18n/composer.{ko,ja,zh}.ts`: the two notes, next to
`first_message_unattended`'s.
- `docs/agent-commands.md`: the first-message paragraph names both codes
and the frame flag. `DESIGN.md`: the first-message paragraph names the
new hold reason.
- `changelog.d/pending-launch-ended-note.md`.

This changes the WebSocket contract (a new held reason and an optional
`pending-action` field) and touches queued input. Per AGENTS.md it needs
a real review.

A launch also counts as ended while herdr still names its agent for a
moment after the process exits, when the pane's shell itself is back in
front (8efa7f6). An agent exec'd at the shell's pid keeps the shell's
pid, so the process there counts as the shell only under a name other
than the launched agent's, which the message keeps from the process read
while that agent runs in front, from its queueing on (063fd3d,
726b73d). Without a known name, the reason stays
`pending_target_changed`.

Known limits: a launched process named like the shell (an `sh` wrapper
under an `sh` shell) falls back to `pending_target_changed`. A drain
that lands in the instant between a killed agent's exit and an `exec` of
another holds the message as launch-ended, which is what it is at that
moment. In every case the message is held and never typed; only the note
differs.

## Design precedent

None applies. This gives an existing held-message note its own wording,
the way `first_message_unattended` already has one. No new UI.

## Validation

- `server/pending-input.contract.test.ts`, the launched-agent cases: a
launched agent that came up and quit is held with
`first_message_launch_ended`, and Send now answers with it. A frame
without `launch_ended` gets `pending_target_changed` while the row keeps
the new reason. Another agent coming up in the terminal still holds the
message with `pending_target_changed`. These cases were not re-run
locally after the rebase: this PC is at its inotify instance limit, so
watcher-backed contract tests fail with EMFILE here, and the
herdr-backed check run was refused admission (a check slot port was
held). CI's Integration job runs them.
- `src/lib/compose.test.ts`, `src/components/PendingMessages.test.tsx`
(ko, ja and zh rows for both codes, no English server text),
`src/lib/ws.test.ts` (the frame carries `launch_ended`), and
`src/lib/i18n.test.ts`. A mutation check: dropping the two codes from
`PendingMessages.tsx` fails the new row test.
- Browser QA on the demo build (fixture transport, no herdr session), in
Korean at 1440×900 and 390×844 and in Japanese at 390×844, rerun after
the rebase: two held first messages, one per code, each show their own
Korean note and no English server text.
- Reviews before the rebase: `omo-native-code-reviewer` approved with no
HIGH finding. Codex (gpt-6.1-sol, xhigh) found no HIGH finding. Both
raised the old-client skew, which the `launch_ended` flag fixes, and one
test assertion that could not fail, which was replaced. Codex's LOW
about identity-read races is the known limit above.
- Reviews after the rebase onto main: `omo-native-code-reviewer`
approved again with no CRITICAL, HIGH or MEDIUM finding. Its LOWs:
`DESIGN.md` did not name the new reason (fixed here); the drain's
`"ended"` branch is reached only when two quick pane reads disagree and
has no test (correct as written); `launch_ended` is sent on every
pending action, discards included (harmless); a comment above
`targetChanged` on main belongs to `launchGone` (not in this diff).
Codex (gpt-6.1-sol, xhigh) approved the rebased commit with no HIGH,
MEDIUM or LOW finding; it also checked that a held message stays held in
both version-skew directions and that the extra drain runs once.
- `server/core/pending.test.ts` (each process read scripted in turn): a
launched agent learned at an event, or bound at queueing and gone at the
next read, then ended with the shell back under herdr's name for it, is
first held with `first_message_launch_ended` and keeps it once herdr
lets go. An agent of its kind exec'd at the shell's pid is held with
`pending_target_changed`. Send now answers that reason, and
`pending_target_changed` to a frame without `launch_ended`. Nothing
reaches herdr's text, key or screen calls, the pty, or `submitText`.
Each earlier commit of this PR fails its own case. On 726b73d, `-t
'first message'` in `server/pending-input.contract.test.ts` passes 19/0
on this PC.
- Locally after the rebase onto 0.4.18: `bun run typecheck` passes, and
`src/lib/compose.test.ts`, `src/components/PendingMessages.test.tsx`,
`src/lib/ws.test.ts`, `src/lib/i18n.test.ts`, `src/conventions.test.ts`
and `scripts/changelog-fold.test.ts` pass (189 tests, 0 failures). `bun
run check fast` could not run its herdr-backed steps here for the same
host reasons (inotify limit, check-slot admission), none of them in
files this diff touches; per the lead it counts as passed locally, and
GitHub CI runs the full suite.
