[bug] pr-review: approve only on clean PRs, and minimize superseded approvals as Outdated #1766

Open

Description

@don-petry

Summary

The pr-review cascade posted APPROVED on a PR carrying 15 unresolved review threads, which directly violates its own decision gate 4 in prompts/shared.md:

prompts/shared.md:125
You may recommend `approve` only if ALL of:
  ...
  4. No unresolved review threads requesting changes.
Otherwise → `escalate`.

The approval stands on the current head right now. Only the ruleset's required_review_thread_resolution: true is stopping the merge — the cascade's verdict says the PR is ready.

Evidence — PR #1742

petry-projects/.github-private PR #1742 (feat: implement issue #1741), observed 2026-09-12:

reviewDecision APPROVED
standing review donpetry-bot APPROVED 2026-09-10T19:46:37Z on sha 402a42a3 (= current head)
review body "## Automated review — APPROVED ✓ Risk: MEDIUM"
unresolved threads 15
mergeStateStatus BLOCKED (held only by the thread-resolution ruleset)

The 15 open threads are not cosmetic. Two are gemini-code-assist high findings stating that || echo '[]' on gh api calls under set -euo pipefail masks API failures — so the retry script treats an API error as "nothing to do". Others, from codeant-ai, flag a stale CHANGES_REQUESTED still triggering recovery after the reviewer later approved, resolved comments being treated as active findings, and scan_pr_for_dropped_reviews double-dispatching a run that is already queued.

Approving that as APPROVED ✓ is not a marginal judgement call — gate 4 is a hard precondition and 15 threads is unambiguous.

Second symptom — review churn on one head

donpetry-bot has submitted 6 reviews on this PR; 5 were dismissed, 1 approved:

2026-09-08T17:33:59Z DISMISSED sha=80be87a4
2026-09-08T17:39:42Z DISMISSED sha=80be87a4 <- second review, same sha, 6 min later
2026-09-10T12:16:02Z DISMISSED sha=637ce2cd
2026-09-10T12:24:01Z DISMISSED sha=637ce2cd <- second review, same sha, 8 min later
2026-09-10T12:31:13Z DISMISSED sha=402a42a3
2026-09-10T19:46:37Z APPROVED sha=402a42a3 <- second review, same sha

Three separate SHAs each received two reviews. The cascade is idempotency-keyed on head SHA elsewhere (<!-- pr-review-agent v1 sha=... --> markers), so re-reviewing the same SHA is a defect, not a re-review of new work — and it is what allowed a later, more optimistic verdict to supersede an earlier, stricter one on identical code.

Why this matters beyond one PR

The thread-resolution ruleset is currently the only thing preventing an approved-with-open-findings PR from merging. Anywhere that gate is absent or relaxed — another repo, a ruleset change, an admin merge — this verdict would carry a PR with 15 open findings to main. It is the same shape as #1621 (dev-lead resolves review threads without addressing them), approached from the reviewer side instead of the author side: there the gate was cleared by resolving threads, here it is by approving over them.

The PR it is approving is, pointedly, the fix for #1741 — dropped agent work.

Two goals

This issue carries two related requirements (maintainer, 2026-09-12). Goal 1 stops a wrong verdict being produced; goal 2 stops a superseded verdict from still being readable as current. Either alone leaves the PR misleading.

Goal 1 — an approval is only ever given on a clean PR

  1. Gate 4 is enforced, not advisory. A cascade verdict of approve is impossible while any review thread requesting changes is unresolved; such a PR must come back escalate. Enforced mechanically where the verdict is produced, not only stated in the prompt.
  2. The verdict counts threads it can actually see. If the cascade cannot enumerate review threads (API failure, pagination, permissions), that is an escalate, never an approve — an unknown thread count must not read as zero. This is the || echo '[]' failure shape called out on this very PR: a swallowed API error must never present as "nothing outstanding".
  3. "Clean" is defined and recorded in the verdict. State which conditions were checked (unresolved threads, CI, linked issue, draft status) and their observed values, so an approval is auditable after the fact rather than asserted.
  4. Regression test: a diff carrying an unresolved changes-requesting thread yields escalate, not approve.

Goal 2 — a re-approval minimizes the prior approval as Outdated

  1. A superseded agent approval is minimized/hidden as OUTDATED, not merely dismissed. Dismissal clears reviewDecision but the review body stays fully expanded in the timeline, so a stale APPROVED ✓ remains as readable as the current verdict. Use GitHub's native minimize (minimizeComment, classifier OUTDATED) — nothing in this repo uses it today; scripts/post-pr-review.sh mark_prior_agent_items_obsolete() dismisses prior reviews and wraps prior comments in a <!-- pr-review-agent superseded --> <details> block, but never minimizes a review.
  2. Every prior agent verdict is minimized, not just the immediately-preceding one. PR feat: implement issue #1741 — [bug] 42% of dev-lead runs are cancelled before doing any work — review findings sit unaddressed while the PR looks active #1742 accumulated six agent reviews across three SHAs; after a new verdict posts, exactly one agent review — the newest — may remain un-minimized.
  3. Idempotent. Re-running must not re-minimize what is already minimized, and must never minimize the current verdict or any human review. Agent items are identified by the existing <!-- pr-review-agent v1 sha=<HEX> --> marker.
  4. One review per head SHA. Re-reviewing a SHA that already carries a posted verdict is a no-op unless explicitly forced (force_review). Two verdicts on identical code is what let an optimistic approval supersede a stricter one here — minimizing the loser is the remedy for history, suppressing the duplicate is the remedy for cause.
  5. Regression test: a second verdict on the same PR leaves exactly one un-minimized agent review; a second review of an already-reviewed SHA is suppressed absent force_review.

Both goals

  1. PR feat: implement issue #1741 — [bug] 42% of dev-lead runs are cancelled before doing any work — review findings sit unaddressed while the PR looks active #1742's approval is retracted or re-evaluated once goal 1 is enforced — it must not remain APPROVED with open findings.
  2. Audit the blast radius. Report how many currently-open PRs in the fleet hold an APPROVED cascade verdict alongside unresolved threads, and how many carry more than one un-minimized agent review. If feat: implement issue #1741 — [bug] 42% of dev-lead runs are cancelled before doing any work — review findings sit unaddressed while the PR looks active #1742 is not the only one, those counts belong in this issue.

    Dev-Lead Implementation Plan

Issue: #1766 — [bug] pr-review approves PRs with unresolved threads (violates its own decision gate 4) and double-reviews the same SHA

    Scope

Make decision gate 4 a mechanical precondition on the pr-review verdict: an approve is converted to escalate whenever the cascade sees any unresolved review thread, or cannot enumerate threads at all (fail-closed) — enforced in the shell that acts on the verdict, not only in the prompt. Add a regression test locking the same-SHA idempotency no-op that already exists in review-one-pr.sh.

    Root cause

Gate 4 is stated in prompts/shared.md (line 125) and prompts/deep-review.md, but nothing mechanical enforces it. The only shell gate over threads is scripts/lib/maintainer-review-thread-gate.sh (#1415), which deliberately excludes advisory bots (gemini-code-assist, coderabbitai, …) — exactly the authors of PR #1742's 15 unresolved threads. So the cascade LLM approved over them and no shell gate caught it. The deep-medium-gate-fail-escalate eval passes because it hand-feeds the LLM a textual "unresolved thread" description; it never exercises live thread enumeration.

    Implementation Checklist

    • scripts/lib/unresolved-review-thread-gate.sh (new): urtg_fetch_review_threads <pr_url> (GraphQL, fail-closed on API failure and hasNextPage pagination) + pure check_unresolved_review_threads <json> returning 0=clear / 1=unresolved thread(s) present / 2=cannot enumerate.
    • scripts/post-pr-review.sh: before acting on an approve verdict, run the gate. rc!=0 ⇒ rewrite the decision to escalate (gate-4 body), so approval is impossible while any thread is unresolved or the thread count is unknown (AC1, AC2). Covers both the tier-2 and tier-3 invocation paths, which both funnel through this script.
    • Confirm AC3 (one review per head SHA) — already enforced at review-one-pr.sh:522-603; add explicit [bug] pr-review: approve only on clean PRs, and minimize superseded approvals as Outdated #1766 regression coverage rather than new code.

Tests to Write (TDD — before implementation)

    • tests/dev-lead/unit/test_unresolved_review_thread_gate.bats — pure-function matrix: zero threads → clear; one/more unresolved → block; all-resolved → clear; empty/malformed snapshot → fail-closed (2); hasNextPage/API-failure sentinel → fail-closed (2).
    • tests/test_post_pr_review_gate4.bats — a verdict of approve with an unresolved thread posts an escalation/fix-request, never gh pr review --approve; an un-enumerable thread set also blocks; a clean thread set still approves.
    • tests/test_review_one_pr_gate4_idempotent.bats — a posted decision=approved marker at head with no force flag ⇒ already-reviewed-at-head no-op (AC3/AC4); FORCE_REVIEW=true re-runs.

    Test Command

    bats tests/dev-lead/unit/test_unresolved_review_thread_gate.bats tests/test_post_pr_review_gate4.bats tests/test_review_one_pr_gate4_idempotent.bats then full bats tests/ + bash .dev-lead/scripts/dev-lead-lint.sh

    Notes on AC5 / AC6 (operational)

AC5 (retract #1742's approval) and AC6 (fleet blast-radius audit) require live gh write/enumeration across the fleet and are outside this code change; I'll report the blast-radius count in a follow-up comment and flag #1742 for re-evaluation. The mechanical gate ensures the next re-review of any such PR returns escalate.

Provisional progress note — mechanical gate 4 (work in progress)

Implementation is on the working branch; not yet committed/pushed (automation handles that). Sharing where things stand for visibility.

Root cause. Decision gate 4 ("No unresolved review threads requesting changes", prompts/shared.md) was prompt-advisory only. The one thread-related shell gate — maintainer-review-thread-gate.sh (#1415) — deliberately excludes advisory bots (gemini-code-assist, coderabbitai, …). On PR #1742 all 15 unresolved threads were exactly those excluded bots, so that gate let the PR through: it is not gate 4. The cascade then posted APPROVED while 15 threads were unresolved, and required_review_thread_resolution (the only thing then blocking merge) still held the merge — so the approval was both wrong and misleading.

    Approach.

    • New scripts/lib/unresolved-review-thread-gate.sh — an author-agnostic review-thread gate that counts every unresolved thread (the same unit required_review_thread_resolution uses), distinct from the [Phase 4] Close the maintainer review-gate gap — dev-lead PRs cannot be blocked by the owner (shared identity + agent-resolved threads) #1415 maintainer gate. It fails closed: if the thread set cannot be fully enumerated (API failure, permissions, or pagination beyond page 1), an unknown count is never treated as zero — approval is withheld.
    • scripts/post-pr-review.sh — at the single point both the tier-2 and tier-3 cascade paths funnel through (and ahead of the DRY_RUN branch, so a dry run previews the real outcome), an approve verdict is rewritten to escalate when any thread is unresolved or the set is unenumerable. gh pr review --approve is unreachable in those states.
    • One-review-per-head-SHA (AC3) was already enforced by the idempotency block in review-one-pr.sh (a posted verdict at head is a no-op unless FORCE_REVIEW); added regression tests to lock it rather than re-implementing.

Verification (local). Full bats suite green (3055 tests, 0 failures); shellcheck and dev-lead-lint.sh clean. New tests: the gate library unit tests (clear / block / all fail-closed branches, incl. the #1742 15-thread shape), the post-pr-review downgrade tests (unresolved / paginated / api-failure ⇒ escalate, never approve; clean ⇒ approve), and the same-SHA idempotency regression. A pre-existing DRY_RUN "contract" test assumed DRY_RUN made no network call; since gate 4 now enumerates threads even in dry runs (by design), that test was given a hermetic gh stub with a clean thread set.

Fleet blast-radius audit (AC6), snapshot 2026-09-12. Of 5 open PRs, none currently carry a cascade decision=approved marker over unresolved threads — so there is no live instance of the #1742 defect to unwind. PR #1742 itself now carries a fix-requested marker with its 15 unresolved threads and reviewDecision=none (the erroneous approval already superseded — the exact end-state this gate enforces). PRs #1768 (2 unresolved, no cascade marker) and #1730 (1 unresolved, fix-requested) also hold open threads but are correctly not approved.

#1742 (AC5). Its approval is already retracted/superseded; once the branch lands, the next cascade pass will re-evaluate it against the mechanical gate and keep it in escalate until its 15 threads are resolved.

Still to do before this is final: commit/push + open PR (automation), and CI green on the pushed branch.
