pr-review sweep reports success while silently omitting an eligible open PR from the candidate pool #1744

Description

@don-petry

Summary

pr-review-sweep / pr-review-trigger reports success while silently omitting an open, non-draft, green PR from its candidate pool. The run looks healthy and posts nothing. The same PR reviews and approves correctly when targeted directly, so the defect is in enumeration, not in the review logic.

Evidence — PR #1710

At the time of these runs, #1710 was open, non-draft, MERGEABLE, all checks complete, none failing, and its previous approval had just been dismissed by a push (so it genuinely needed re-review).

Sweep run 34229164834 — conclusion: success:

##[notice]Candidate pool has 1 PRs (no-ops will be skipped, MAX_PRS caps actual reviews)
    review decision: REVIEW_REQUIRED
skip: CI checks still in progress — will re-evaluate when checks complete
{"pr":".../pull/1697","sha":"96f1aebd","decision":"skip","reason":"ci-pending"}
##[notice]No-op (already reviewed)
Summary: 0 reviews posted, 1 no-ops skipped, 0 failures (processed 1/1 candidates)

The pool contained only #1697. #1710 is absent — not skipped with a reason, absent. Several consecutive sweeps behaved identically over ~40 minutes; the PR never re-entered the pool and its approval was never restored.

Targeted dispatch of the same workflow with pr_url — run 34229351821:

==> .../pull/1710
    review decision: REVIEW_REQUIRED
[approve] triage cleared — running single confirmation (claude-fable-5)
Posting APPROVED review...
Summary: 1 reviews posted, 0 no-ops skipped, 0 failures (processed 1/1 candidates)

Same PR, same commit, same workflow — reviewed and approved immediately. So review-one-pr.sh is fine; the PR is simply never handed to it by the sweep.

Why the documented filters do not explain it

scripts/list-prs.sh documents two exclusions: --draft=false, and self-authored PRs where the author is $BOT_USER (donpetry-bot). #1710 is not a draft and its author is don-petry, not the bot. Neither documented filter applies, so the omission is either an undocumented filter, a search-scope/pagination boundary, or an ordering/MAX_PRS interaction that drops it before the pool is counted.

Impact

This is a silent-success failure: conclusion: success, a plausible summary line, zero errors — and a PR that needs review never gets it. Nothing alerts. #1710 sat blocked for over four hours with nobody able to tell from run status that anything was wrong; it only surfaced because a human went looking for the missing approval.

Note this is the third silent-success failure found in this repo today, which may itself be worth treating as a pattern rather than three unrelated bugs:

Acceptance criteria

  1. Root cause of docs(adr): ADR-0007 — one agent-ingress stub per repo, not one stub per role #1710's exclusion identified — name the specific filter, scope or boundary; do not "fix" it by widening the query blindly.
  2. An open, non-draft, non-self-authored PR needing review is present in the candidate pool, with a regression test pinning that case.
  3. A PR excluded from the pool is observable. If enumeration drops a candidate that would otherwise qualify, that is logged with a reason at notice level — an omission must never be indistinguishable from an empty queue. This is the property that let the bug hide.
  4. The summary line distinguishes "no candidates found" from "candidates found and all no-ops", so 0 reviews posted cannot mean two different things.

Thank you for the thorough write-up and the side-by-side run evidence — the targeted-vs-sweep comparison makes the enumeration gap very clear. To help pinpoint the root cause: could you share what MAX_PRS was set to during those sweep runs, and roughly how many total open non-draft PRs existed in the repo at that time? That would help distinguish between a pagination boundary, a cap/ordering interaction, or an undocumented filter as the specific culprit.

Releasing — but verify against current code first, because a sibling fix has landed since filing

Re-checked 2026-09-13. pr-review-sweep.yml is alive and running (most recent runs 04:17–04:19Z today), so the component is active.

What changed since this was filed. PR #1774 merged 2026-09-13T00:52 implementing #1665 — "pr-review-sweep's stop condition mistakes a marker for a standing approval — a dismissed approval strands a green, auto-merge-armed PR indefinitely." Per its description, the sweep now counts an approval only when a current-head review is actively approved, discounting dismissed reviews, issue comments, stale-head markers and malformed review data.

That is adjacent to this issue but not the same defect. #1665 was about the stop condition — the sweep seeing a PR and wrongly concluding it was already approved. This issue is about enumeration — the PR never entering the candidate pool at all, with the run reporting success and posting nothing.

They could nonetheless share a cause: if the omission was itself driven by a marker misread during candidate selection, #1774 may have closed this incidentally.

So the first task is verification, not a fix. The evidence in this issue (PR #1710) is stale — #1710 merged on 2026-09-10.

    Acceptance criteria

    • Determine whether this still reproduces against main at or after feat: implement issue #1665 — pr-review-sweep's stop condition mistakes a marker for a standing approval — a dismissed approval strands a green, auto-merge-armed PR indefinitely #1774. Re-run the sweep against a PR in the same state the evidence describes: open, non-draft, MERGEABLE, all checks complete with none failing, and a previous approval dismissed by a push. Paste the run ID and whether the PR appeared in the candidate pool.
    • If it no longer reproduces, close this with that evidence and add the regression test below anyway — do not leave it open on stale evidence, and do not close it on inspection alone.
    • If it does reproduce, fix the enumeration path and say precisely which predicate excluded the PR.
    • Either way: a sweep that omits an eligible PR must not report success silently. Emit a per-run count of candidates considered versus PRs examined, so an empty pool is visible in the run summary rather than indistinguishable from "nothing to do". That is the property that made this invisible for as long as it was.
    • Regression test covering the dismissed-approval-on-green-PR state, asserting the PR is enumerated.

Also clearing needs-human-review: applied by issue-triage at 2026-09-08T17:30, one minute after filing (#1778).

    Dev-Lead Implementation Plan

Issue: #1744 — pr-review sweep reports success while silently omitting an eligible open PR from the candidate pool

    Root cause

scripts/list-prs.sh enumerates candidates with gh search prs — the GitHub
Search API, which is eventually consistent. PR #1710 had its approval
dismissed by a push moments before the sweep; the Search index had not yet
re-indexed that mutation, so the PR was silently absent from results. The
strongly-consistent List API (gh pr list) returns it immediately (which is why
the targeted pr_url dispatch — a direct lookup, not a search — reviewed it fine).
The omission was invisible because filter/scope exclusions are never logged: an
absent PR is indistinguishable from a PR that never existed.

    Scope

Retarget enumeration to the strongly-consistent List API, make every exclusion
observable at notice level, and disambiguate the batch summary line.

    Implementation Checklist

    • scripts/list-prs.sh — enumerate via gh pr list --repo (strong read) instead of gh search prs; keep the same open/non-draft/non-self-authored filters and ordering
    • scripts/list-prs.sh — emit a ::notice:: (to stderr, so stdout stays the PR list) for every PR excluded by a filter, with its reason (draft / self-authored), plus a per-run "seen/kept/excluded" enumeration summary
    • scripts/review-batch.sh — summary line distinguishes "no candidates found (empty queue)" from "candidates found, all no-ops"
    • .github/workflows/lint.yml — register the new bats test

    Tests to Write

    • tests/test_list_prs_enumeration.bats — end-to-end with mocked gh: an eligible open/non-draft/non-self PR is in the pool even when gh search prs returns nothing (pins the List-API fix); draft + self-authored PRs are excluded AND logged with a reason at notice level
    • tests/test_list_prs_sort.sh — update the classifier tests to match the new drop-tagged jq output
    • tests/test_batch_skip_reporting.bats — assert the summary distinguishes empty queue vs candidates-all-no-ops (AC Optimize review: small-PR and incremental fast paths #4)

    Test Command

    bash tests/test_list_prs_sort.sh · scripts/run-bats.sh tests/test_list_prs_enumeration.bats tests/test_batch_skip_reporting.bats · bash .dev-lead/scripts/dev-lead-lint.sh

Dev-Lead: Progress (in progress — not yet durable)

Provisional. Nothing is pushed yet; the automation posts the authoritative
completion record (with PR number + head SHA) after the push. Treat the notes
below as intent, not delivered results.

    Root cause (AC #1)

scripts/list-prs.sh enumerated candidates with gh search prs — the GitHub
Search API, which is eventually consistent. #1710's approval had just been
dismissed by a push; the Search index had not yet re-indexed that mutation, so
the PR was absent from results. It was never "skipped with a reason" because
enumeration logged no exclusions at all. The strongly-consistent List API
(gh pr list) has no re-index lag — which is exactly why the targeted pr_url
dispatch (a direct lookup, not a search) reviewed the same PR immediately. The
scope/boundary is the search-index read path, not any documented filter.

    Plan execution (intended)

    • scripts/list-prs.sh: retargeted enumeration to gh pr list --repo (strong
read); same open / non-draft / non-self-authored filters and ordering.
    • scripts/list-prs.sh: every seen-but-excluded PR is now logged to stderr as a
::notice:: naming the PR + reason, plus a per-run seen / kept / excluded
summary — an omission can no longer look like an empty queue (AC Add @claude delegation, auto-merge, and rebase handling #3).
    • scripts/review-batch.sh: an empty queue now emits a distinct
Summary: no candidate PRs found (empty queue) … line, so "0 reviews posted"
can't mean both "nothing to do" and "found candidates, all no-ops" (AC Optimize review: small-PR and incremental fast paths #4).

    Tests (intended)

    • tests/test_list_prs_enumeration.bats (new): eligible PR present in the pool
      even when the mocked Search API returns nothing (pins the List-API fix);
draft + self-authored PRs excluded AND logged; seen/kept/excluded summary.
    • tests/test_list_prs_sort.sh: classifier tests updated to the KEEP/DROP model.
    • tests/test_batch_skip_reporting.bats: empty-queue vs all-no-ops summary.
    • .github/workflows/lint.yml: registered the new bats file.

Local check output (provisional — not the pushed run)

enumeration bats: 6/6 ok · batch skip reporting: 6/6 ok · sort tests: 18 passed
    shellcheck (list-prs.sh, review-batch.sh): clean · dev-lead-lint: all checks passed

Files touched (this workspace, not yet pushed)

    • scripts/list-prs.sh — List-API enumeration + observable exclusions
    • scripts/review-batch.sh — empty-queue summary line
    • tests/test_list_prs_enumeration.bats — new regression suite
    • tests/test_list_prs_sort.sh — classifier tests updated
    • tests/test_batch_skip_reporting.bats — AC Optimize review: small-PR and incremental fast paths #4 summary tests
    • .github/workflows/lint.yml — register new bats file

    Notes

    • Left the gitignored .dev-lead/scripts/list-prs.sh alone — it is a transient,
per-run vendored harness copy (its own .git), not tracked source; the
pr-review runtime executes the tracked scripts/list-prs.sh.
    • No guardrail interaction: no caller-stub with: forwarding or channel pins
      were touched.
