docs: reconcile PLAN, STAND and KNOWN_ISSUES with live PR state - #258

This section now records an authoritative 04.10 snapshot at dfe459f, but the opening Stand: line still labels the whole plan as the 03.10 PLAN-STATUS-03 snapshot at 6598890. Because docs/PLAN.md is the canonical plan, these competing baselines can make later coordinators treat the newly recorded ARCH merges as stale or unverified; update the opening snapshot metadata in the same change or explicitly mark it as historical.

This adds an eighth known native-tests failure after the explicitly bounded seven-failure sample, but the KI-30 summary still says 7 von 58 and the detailed symptom still says in allen sieben Fällen. That leaves the headline frequency inconsistent with the evidence added here and can skew the E11 cost/benefit decision about continued Windows flake detection; recompute the aggregate through this run or label the old 7/58 figure as a closed historical sample and add the later count separately.

The new E13 status asserts both that this session ran through the replacement starter and that its model was Sonnet 5.5, but it provides no command or captured output establishing either fact. A date and a list of resulting PRs do not show which launcher or model produced this particular session, so this affirmative model/runtime claim is not reproducible; include the observing command and relevant output, or remove the model and launcher attribution.
