What: Explicit agent restart and human Steer interrupt running work before replacement delivery, while routine Claude agent updates stay queued.
Why: #282 avoided Claude's false user-refusal cancellations by queueing every steer, including urgent messages and the user's own cut-in.
So what: Merged into main as f49ece39e8 after both independent reviews and complete CI passed; activation rides the next build.

Closes #285. Closes #305.

The existing Queue and Steer controls match upstream's UI. Human web/desktop Steer and current or older mobile busy-Steer messages use the server's interrupt path. Agents use t3_thread_send(mode: "restart") for an urgent course change. Queue remains non-interrupting, Claude agent auto/steer stays queued, and #279's descendant-to-ancestor restrictions remain intact.

The restart requests the harness's native Stop scope. Claude closes its query; Grok tears down and respawns its runtime; Codex and both OpenCode protocols retain native work owners until cancellation is confirmed. Failed or unanswered cancellation blocks replacement delivery, keeps surviving work observed and shows Message not delivered. A later Stop can reach the runtime captured before session replacement or handoff. A superseded reader cannot write the replacement root's output. Resumed Claude children move their ownership together so later completions remain visible, including after the resuming attempt ends.

Normal replacement prompts name the real requester and explain interruption rather than refusal; stored text is unchanged. Bare targets preserve #289's contract and receive only the sender's text, with provenance retained on the attempt. Both the slash guard and prompt use the same note policy. MCP descriptions and injected orchestration instructions explain restart versus auto, dispatch acknowledgement and cancellation limits. ACP/Antigravity use native cancel, Cursor cancels its run, and Pi/Muse keep native Steer because restart is unsupported. Separately delegated T3 tasks remain independent.

Evidence

  • Merged 2026-10-15T01:39:03Z as f49ece39e8b302c8bffa9b17a385d66ec989b851. The merged tree is byte-identical to reviewed head b107228d97ef3331ba553c5c6ed0c766628d0b50; Restore explicit urgent interruption after Claude steer queuing #285 and Claude closed queries can retain running subagents in the session pending-work probe #305 closed. Sol and Grok both ACCEPT design and code, with their findings posted before their individual authenticated success statuses. The parent acceptance records aggregate acceptance; all three statuses were success before merge. Complete CI38663316464 passed on that exact head.
  • Current Issue and durable handoff record exact candidates, commands, results and remaining gates. The previous 3326f71ea675237e477118e785428136dc470975 CI and both reviews failed; those failures are corrected here and no verdict transfers.
  • 1,171 tests pass in 23 complete files on this head, covering all 16 changed test files plus seven recovery, turn-start and MCP consumers with TMPDIR unset. Complete CI remains the repository gate. Server/provider-core typechecks, lint of all 39 changed TypeScript files and the full 39-feature fork inventory pass on this candidate.
  • Persisted events, Deferred milestones and the effect-worker drain cover actual cancellation outcomes, retained work after failure, goals, late child output, Stop before/during/after replacement, cross-instance replacement/handoff, real server-cut recovery and same-run Claude child reuse. No sleeps or polling. New bare integration cases reject an unwanted T3 note, an incorrect slash refusal, repeated additions and lost requester provenance; their literal failing-before and passing-after evidence is linked from the Issue.
  • Fresh normal native probes prove human Claude/Grok Steer and an actual Grok-agent MCP restart into Claude stop held foreground tools before release, then deliver the message. Claude names the real requester and understands interruption versus refusal. Fresh bare Grok proof proves Stop/reload/send and human Steer, exact native sender text, a passed metadata gate and process exit before the replacement reply. Final production bytes are unchanged from that bare probe; only tests and commit history changed afterward.
  • Baseline main queued human Claude Steer and rejected Claude restart; native Codex, Grok and OpenCode 1 restarts stopped held commands. OpenCode 2 has actual-adapter protocol tests. Uploaded before/after images and timing video cover the existing controls. Desktop shares web; mobile has server/wire tests. No native-device or Electron run is claimed.

Design and limits

Opus designs and Sol/Grok critiques led to Stop scope, outcome-based release and retained cancellation ownership. R8 ownership and recovery revision and R9 bare integration retain the prior findings and responses. Both final independent reviews critically reviewed these addenda and accepted design and code on the exact head.

Cancellation cannot undo completed or partial effects, guarantee that every tool process has exited, cancel separately delegated T3 tasks or prevent future native work after a retained cohort releases. A failed or late goal pause is not delivery. Native session loss still loses in-memory ownership. Bare targets retain the harness's native interruption wording without a T3 explanatory note. The pre-existing bare pre-start recovery defect is separately tracked in #343; ordinary-path issues #331, #332 and #333 remain separate and waive no new cut-in defect.

No version bump, deployment, live Chromeria restart or agent memory. Website/SEO impact: none; app/server behavior and existing guides change, with no marketing page or URL change.

Requirements and who asked: User on 2026-10-14 requires urgent agent and human cut-in, #282 routine-update protection, every client and named adapter, Opus design with Sol/Grok critiques, exact-head independent review, green CI and delegated merge. User #289 requires bare prompt isolation.
Deleted: New urgent modes, UI controls, settings, native Claude steer, soft/background-preserving machinery, mobile client edits, speculative session guards, T3 notes on bare targets, version bumps, deployment and unrelated repairs.
Bottleneck: Confirm cancellation before replacement delivery while surviving owners stay reachable; resolved and proved, with both exact-head independent design/code reviews and complete CI passing before merge.
Checked myself: Full #280/#282 and selected upstream diffs/UI, designs and critiques, current callers and base overlaps, literal regression failures, complete affected files, held native PIDs and persisted events, exact bare native queries and uploaded UI evidence. Preserved merged #279/#291/#313/#329/#327/#334/#335/#339/#340/#342.

Implementation and tests: Codex gpt-6.1-sol, max reasoning, Codex harness. Design: Claude Opus 5.5, max reasoning, Claude Code. Critiques and final code reviews: separate Codex gpt-6.1-sol max and Grok grok-4.7 xhigh agents.

What: Before/after client evidence shows Claude app Steer changing from queued delivery to an actual cut-in, and agent MCP restart naming the real sender.
Why: A queued course change could arrive after the unwanted action finished, while native cancellation phrased stopped calls as user refusals.
So what: The attached images and timing video retain the isolated native proof; final current-head reviews and CI still gate merge.

All captures use the task-owned seeded development server. The before image shows the urgent app message queued behind the held foreground command on main. The human after image shows the Steer and truthful response after SIGTERM; the agent after image shows the MCP sender attribution. The recording shows the human interaction and response. Full process/event evidence is linked in the PR body.

No native mobile or separate Electron-shell interaction is claimed. The large after-checkpoint diff comes from main rebases during this old test thread, not repository actions by the fixture agent. No pairing token, credential or live-state payload is included.

Before: app Steer queues behind the held Claude tool

After: human Steer interrupted the held tool with an accurate reason

After: MCP restart names the sending agent and explains interrupted versus refused

What: Independent Grok review of 1a8a1977be27a7d0207accdfa9be5e8e44acc3ab finds one blocker: a T3 Claude interrupt stamps backgroundWork: "ended" and settles native background projections before the captured query close can fail.

Why: interruptedTurns is recorded before query.interrupt() and query.close(), and the live abort result finalizes the turn from that mark. A later close failure does not undo the settlement.

So what: REVISE. Parent owns the fix and the next exact-head review. Do not merge this SHA on this verdict.

Verdict

REVISE. One blocking defect. Head 1a8a1977be27a7d0207accdfa9be5e8e44acc3ab. Base 14f6e5d5b3465e12fb77362cc0ef88db5f8f1fac. Branch fix/urgent-interrupt-delivery. Context review/independent-grok, round 1.

Blocking

B1. Claude backgroundWork: "ended" commits before close succeeds.

Trigger: urgent Claude restart (provider-turn.restart) or any interruptTurn that marks the turn, while a non-dropped result is still read from the live query, and query.close then fails. The same stamp also happens on the 10s stream timeout, which returns success.

Actual: the superseded attempt's native subagent rows, linked child-thread nodes, and pending background roster are settled. The replacement turn does not start when close's Effect fails. On the timeout path it does start.

Expected: backgroundWork: "ended" means the runtime was closed and its work cannot report further results (ProviderAdapter.ts). A failed or unfinished close leaves surviving Claude work unsettled and does not start the replacement.

Path:

  1. ClaudeAdapterV2.ts interruptTurn adds the turn to interruptedTurns and sets stopping before interrupt or close (about 7698-7714).
  2. query.interrupt() only sends the SDK control request and waits for its acknowledgement (@anthropic-ai/claude-agent-sdk@0.3.276, async interrupt). It does not close the process. The message stream is a forked fiber (7413-7453) and can handle a result while that await is in progress.
  3. A live abort result (aborted_streaming / aborted_tools, or any other non-dropped result) is the result this adapter already treats as arriving while the query is open. The steer path returns early only when the turn is not in interruptedTurns (6641). Once the set contains the turn, finalizeActiveTurn runs (6675).
  4. The flag is status !== "completed" && (interruptedTurns.has(turn) || liveQuery === null) (5084-5087). The query is still the live one, so the flag comes from the interrupt mark. Finalize also clears that native thread's pending background roster (5149-5158) and ends open subagent tool calls because the status is not completed (4995-5006).
  5. The restart command has already superseded the attempt, so shouldFinalizeRun is false. RunExecutionService.ts writeFinalRunEvents still cascade-terminalizes the attempt's open run-owned subagents when the flag is set (588-600), through an unguarded writeWithEffects.
  6. query.close runs only after interrupt returns (7714). A failing close becomes ProviderAdapterInterruptError. EffectWorker.ts provider-turn.restart starts the replacement only in Effect.andThen after interruptAndAwaitTerminal (300-336). Close failure therefore skips the start and does not roll back step 4 or 5.
  7. If close's Effect returns but existing.closed does not complete within 10 seconds, the timeout clears queryContext, finalizes if the turn is still active (the flag is now also true because liveQuery === null), succeeds the deferred, and returns success (7714-7738). The replacement then starts.

The failed-close test (ClaudeAdapterV2.test.ts about 2257-2324) makes interrupt succeed without offering a result, then fails the first close. It proves a close error fails the interrupt effect. It does not cover the result the production stream delivers before close, so it stays green on this sequence.

The cascade snapshot is the provider events that attempt ingested. Claude fills it for its Task subagents and their derived child threads. This path does not send a stop to a separate delegate_task provider session. The false settlement is the Claude background roster and those native subagent projections.

Confirming test, if a live transcript is wanted: offer an aborted_tools result after interrupt starts and fail close, then assert no turn.terminal with backgroundWork: "ended" and no subagent cascade. I did not run that experiment.

Prior findings

  • Sol F1, child ownership lost with the old ingestor: cleared as a design choice. Hard close stays on the same query. B1 is the new hole in that choice.
  • Sol F2, bind the first interrupt and close without repeating a hung interrupt: cleared for sequencing. Interrupt failure is logged and close still runs; close Effect failure fails the restart effect. The ended flag does not wait for that close.
  • Sol F3, selection change stops and then loses the replacement: cleared for the effect chain. Detach and start run only after interrupt returns. B1 settles work before that return.
  • Sol F4, requester differs from the queued author: cleared. requestedBy is stored on the steering-restart attempt, and steeringRestartNote names the author when the pair differs. Promotion stamps createdBy: "agent", creationSource: "mcp", and the caller thread.
  • Sol F5, a slash command must keep a truthful note: cleared. The interrupt is refused before stop when the note is non-empty and the text starts with /. A Claude skill block keeps the note as leading text and the skill as the command (SteeringInterruption.test.ts).
  • Grok, retain preparing/starting/waiting: cleared. A person's steer restarts only a running turn whose session supports interrupt and interrupt-restart. Preparing and starting queue. Waiting fails canInterruptSteer.
  • Grok, OpenCode 1 versus 2: cleared. Neither capability set gained activeSteeringInterruptsTools. Agent auto still native-steers both. Human Steer uses interrupt-restart on both, which is the accepted always-interrupt choice.
  • Grok, promotion provenance and nullish source: cleared. Web provenance uses creationSource ?? "web". Missing requestedBy falls back to the message author at prompt build.
  • Parent correction, short human-Claude reason and propagating close failure: the reason is present for claudeAgent. Close failure propagation stops the replacement and is missed by B1's earlier commit.
  • fix(orchestration): child updates use queued V2 notifications #279 descendant-to-ancestor steer/restart refusal: cleared. ThreadManagementService.ts still rejects both modes before dispatch.
  • Routine MCP updates interrupt foreground work on restart-only harnesses #297 routine Grok/Cursor auto restart, Accepted MCP restart retries can fail during replacement startup #298 restart retry precheck, Verify surviving background ownership across same-run steering restarts #299 surviving background ownership: not fixed here. B1 is a new reachable Claude settlement regression, not a relabel of Verify surviving background ownership across same-run steering restarts #299. Agent auto on Claude still queues because activeSteeringInterruptsTools is checked before supportsSteeringByInterruptRestart.

Proof

Inspected, not re-executed. This review ran no tests, server, or browser.

  • git diff 164ac9c153..1a8a1977 is two literal PR links in docs/fork-features.md and docs/map.md.
  • /tmp/chromeria-285-focused-final.log: 9 files, 336 passed. The log does not record a SHA or the file names.
  • Server and contracts typecheck logs contain suggestion diagnostics and no error TS line. Exit codes are not printed in the files.
  • Lint log: five pre-existing unused-variable warnings, no error line.
  • /tmp/chromeria-285-fork-final.log: fork-check OK and 36 features, stack tip 164ac9c153, before the doc-link commit.
  • /tmp/chromeria-285-replay-tests.log: 8 passed, 132 skipped. Names are not in the summary.
  • /tmp/chromeria-285-urgent-restart-red.log: the five baseline failures are Codex, OpenCode 1, Grok, OpenCode 2, and a person's Codex Steer, from the disposable worktree named in the log.
  • Local before/after screenshots exist at the three paths named in the brief. I did not open the recording.
  • GitHub checks on this head, read at review time: Check, Test, Test Web, Test Server 1-6, Typecheck, Lint, Build, Rust, Release Smoke, and Fork Stack Model passed. review/independent, review/independent-sol, and review/independent-grok were still pending.

Limits

No live CLI interrupt was run in this review, so B1 is from the source order, the installed SDK 0.3.276, and the tests as written. Current SDK Query.close() returns before its cleanup promise and normally does not throw; the kill is scheduled about 2s later and SIGKILL about 5s after that. The throwing close is the adapter contract the new test encodes. No desktop shell, native mobile, real OpenCode 2, or linked two-server session was exercised here. Dispatch acknowledgement is still not an OS-exit promise on paths that do close.

Elon record

Requirements and who asked: #285 and the user asked to keep routine Claude auto/steer queued, and to interrupt for explicit restart and human Steer with the real requester and reason. The parent correction asked close failure to propagate.

Deleted: Soft Claude preservation, session handover, and a new interrupt mode.

Bottleneck: The ended flag follows the interrupt mark, so settlement can commit before close fails.

Checked myself: Adapter interrupt and finalize order, SDK interrupt/close, restart Effect.andThen, superseded-attempt cascade, ancestor refusal, and the failed-close test.

What: Grok R1's premature background-settlement finding is accepted; head1a8 is not approved to merge.
Why: A native abort result can finalize from the interrupt-intent mark before the captured close succeeds, clearing roster/calls and emitting the new ended flag; a later close error cannot undo those events.
So what: Opus is revising this narrow lifecycle boundary, then the parent adds discriminating regression proof and a new candidate for both independent code reviews.

I checked the result handler, interruptedTurns write, finalizeActiveTurn roster/tool/terminal effects, the superseded-attempt cascade and the restart Effect.andThen barrier. The close-error barrier correctly prevents replacement start; the premature settlement still needs correction. The failed-close test did not deliver an intervening native result and therefore did not falsify this sequence.

The successful-close stream timeout is a separate qualification: the selected design already uses the SDK close-request boundary, with a bounded EOF wait and local finalization afterward. SDK0.3.276 close returns void while cleanup proceeds. This task does not promise OS-exit acknowledgement and will not silently add that stronger guarantee. The repair must distinguish a failed close from the successful close request and preserve the declared native limit.

Tests must separate SDK control acknowledgement, abort-result handling and close outcome with Deferred/TestClock milestones, including the failure sequence with live native background work, successful close without EOF, and normal Stop/restart. Blocking the SDK reader on a close that waits for an acknowledgement from that same reader must be ruled out. Existing query/turn identity fencing and exactly-once settlement remain required.

Sol R1 continues reading pinned1a8; source is unchanged while Opus resolves the revision. Current CI is green but the aggregate and Grok status are failure.

Requirements and who asked: The user's truthful interruption outcome and the accepted design require ended projections only at the actual close boundary; Grok R1 supplied the concrete counterexample.
Deleted: A boolean-only patch that leaves premature roster/tool settlement, soft-background handover machinery and a new OS-exit guarantee.
Bottleneck: Correctly owning native abort versus runtime close before durable background settlement.
Checked myself: Traced the source and test gap against the pinned candidate and inspected the code review; the new deterministic reproduction remains to run before accepting the repair.

What: Independent Sol review finds a blocking regression: human OpenCode 2 Steer loses the surviving native background child's event owner.
Why: The new human restart path retires the old ingestor, while the replacement excludes running child threads and the adapter does not register the surviving child again.
So what: REVISE this candidate; the parent owns the correction, deterministic child-lifecycle proof and fresh exact-head reviews before merge.

Reviewed head: 1a8a1977be27a7d0207accdfa9be5e8e44acc3ab.
Intended base: 14f6e5d5b3465e12fb77362cc0ef88db5f8f1fac (main).
PR #300, Issue #285, round 1, individual context review/independent-sol. This is code review, not implementation or design approval.

Required finding B1, P2: human OpenCode 2 Steer drops surviving child timeline events

Changed location: CommandPolicy.ts:182. Human queue-row promotion also gains this path at Orchestrator.ts:8099.

Trigger: an OpenCode 2 root turn is running, a native background child is already attached and has an open tool or streaming answer, and the person uses app Steer or promotes a queued message. The foreground interruption succeeds, the replacement starts, and the surviving child subsequently emits its tool completion or assistant answer.

Actual versus expected: the background child continues natively, but its runless child-thread events are discarded. Its existing tool/root node can remain running and its answer can be absent from the child timeline. The parent subagent card can still complete because its events name the parent run; that does not repair the missing child timeline. A foreground cut-in must retain observation of background work the chosen adapter deliberately preserves.

Reachable source path:

  1. On the base, human OpenCode 2 Steer returns steer_active and uses native steering. This change selects restart_active, then supersedes the attempt in the same run.
  2. OpenCode2AdapterV2.ts:3934 only stops backgrounds for requestRuntimeRestart=true. Steering restart omits it. finishTurn expressly keeps background calls on an interrupted root at line 1714.
  3. The old ingestor ends on the interrupted terminal at RunExecutionService.ts:1149, and closes its subscription at line 1359.
  4. The replacement seeds child ownership only from completed subagents: ProviderTurnStartService.ts:951, RunExecutionService.canRouteRelatedSubagent at line 181. Same-run inheritance cannot compensate: lines 127-144 require a lower run ordinal, and exclude runless child items.
  5. The existing child is not registered again: OpenCode2AdapterV2.ts:1427 returns when call.child exists, before app_thread.created. Its child turn has runId:null at line 1594; emitText and emitTool keep that ownership.
  6. RunExecutionService.ts:423 requires the run or an owned child thread for message/item delivery. Parent subagent.updated does not enroll that thread. The session pump at ProviderSessionManager.ts:1907-1939 directly persists runless requests, not ordinary child messages/tools, so it supplies no alternate writer. Subscriptions start with an empty queue (ProviderSessionManager.ts:1637) and do not replay the old child registration.

This is the mechanism tracked in #299, but the new human Steer and human promotion callers make it a regression of this PR. Keeping explicit restart's older defect in a separate Issue does not justify newly exposing it through previously native human steering.

Concrete proof needed from the parent: extend the real OpenCode 2 service/adapter integration with a child execution and open Bash tool before the human Steer, hold its completion across replacement startup, then deliver the child tool result and final answer. Assert the persisted child tool, node and answer settle exactly once, including after the replacement root completes. Use persisted-event/Deferred milestones. The existing OpenCode 2 test only checks native background survival (UrgentRestart.integration.test.ts:684-695); its only human case is Codex. I did not run this experiment or change any test.

Claude close question and the published Grok B1

I independently traced the early backgroundWork: "ended" marker at ClaudeAdapterV2.ts:5084-5087, the concurrent result finalizer at line 6675, the close at line 7714, the cascade at RunExecutionService.ts:588-600, and restart execution/retries. The marker does precede successful close when a result arrives during interrupt. A failing close Effect prevents that execution from starting the replacement, but does not undo an already-written settlement. On a later retry, a persisted terminal lets ProviderTurnControlService.load return no session before calling the adapter again.

The installed SDK 0.3.276 adds an important limit to the Grok B1 finding: native Query.close() returns void after starting asynchronous cleanup; performCleanup catches transport errors. The production wrapper shuts the prompt queue before invoking it. I have not established a native throwing-close sequence that leaves live work and successfully offers a replacement. I therefore confirm the marker/order concern at the error-capable runner contract, not a demonstrated native replacement after failed close. The approved bounded stream timeout is also not evidence of synchronous OS exit, and I am not adding that gate.

The parent should cover result-before-close-failure plus effect retry using the real service/adapter and a fault-injected SDK boundary. The current failed-close unit case emits no native result and only proves immediate error propagation. This remains a concrete unresolved failure-contract check; B1 above independently requires revision.

Prior findings adjudicated

  • Sol F1: the selected hard Claude close removes the proposed soft-preservation handover. Normal closed-attempt cascade is implemented. Surviving-child ownership remains a blocker on the newly changed human OpenCode 2 path above.
  • Sol F2: the first interrupt is bounded, the captured query is closed without repeating that failed/hung request within the call, and close Effect errors propagate. Early settlement and retry after such an error are addressed separately above.
  • Sol F3 / Grok selection concern: normal successful hard close removes the incompatible live-query/background selection refusal. The native-finish-between-dispatch-and-effect race remains a stated delivery limit, not a promise of recovery.
  • Sol F4 / Grok promotion provenance: requester survives on the replacement attempt separately from message authorship; WS uses a nullish source default and MCP stamps the agent/caller. Stored message text and author remain intact.
  • Sol F5: native slash text requiring a note is rejected before interruption; Claude skill packaging retains leading prose and the invocation block. The prompt note is derived once before budgeting and used by both handoff and direct start.
  • Grok preparing/starting/waiting: Claude input retention is implemented; the interrupt predicate requires both a running run and native turn. Current mobile busy Steer encodes user/mobile/auto; idle sends use start and Queue stays explicit. The legacy client fallback stays applicable to servers without context resolution; this server advertises it.
  • Grok OpenCode distinction: OpenCode 1 aborts descendants; OpenCode 2 preserves backgrounds. Actual-adapter tests assert those distinct primitives. Human always-interrupt Steer and Pi/Muse native-steer exceptions are accepted task choices, not reopened taste decisions.
  • Parent corrections: human Claude receives truthful interruption context and close Effect failures propagate. fix(orchestration): child updates use queued V2 notifications #279's deliberate ancestor refusal, feat(server): delegate work across linked environments #290 linked-target handling, Libraries and the map instructions are preserved.
  • Routine MCP updates interrupt foreground work on restart-only harnesses #297 and Accepted MCP restart retries can fail during replacement startup #298 remain pre-existing routine-delivery and retry-precheck issues. Verify surviving background ownership across same-run steering restarts #299 cannot wholly remain outside this PR because of B1's new human callers.

Proof inspected versus executed

Executed by this reviewer: read-only Git/GitHub/source inspection, installed SDK inspection, retained-log parsing, local screenshot viewing and read-only process-marker checks. No tests, server, browser/device automation, native agent, repository changes, versioning, merge or deployment.

Inspected retained results: 336/336 in nine files, 8/8 selected replays, five intended baseline red adapter cases, their 5/5 integrated green result, server/contracts typecheck output without errors, lint's five existing warnings, and fork-check/36-feature success. The summaries do not independently embed every command, exit code and SHA. The PR binds them to 164ac9c153912cfd13e97a6bca0c5c836d4aa818; I verified its entire delta to this candidate is two literal documentation links and no runtime/test change.

I opened all three before/after screenshots and inspected the retained Claude protocol log and fixture source/markers. Both Claude fixtures have SIGTERM markers, no release/completion files and absent captured PIDs. The protocol records interrupt, close, reopen, exact requester note and successful receiving-agent acknowledgment without rerun. Those native captures retain the stated pre-#279 identity; they are not rerun proof on this SHA. The uploaded timing recording is linked but I did not watch it.

Exact-head CI run 38580540916 is completed/success and reports this pinned head. Build, Typecheck, Lint, Test, Test Web, Test Server 1-6, Rust, Release Smoke and Check passed; Fork Stack Model also passed. Green CI does not cover B1's missing child-lifecycle oracle.

Material limits: no separately driven Electron shell, native iOS/Android, live OpenCode 2, linked two-server session or native Cursor/ACP/Antigravity/Pi/Muse proof. Current Grok native evidence is 1.0.52. No rollback of partial/finished effects or equivalence of dispatch acknowledgment to native cancellation, OS exit or completed delivery is claimed. No optional cosmetic change is requested.

Requirements and who asked: User and #285 require urgent agent/human cut-in, truthful requester/reason, #282 routine Claude protection, existing controls, every surface/adapter decision, focused proof and independent exact-head reviews; this supports Promachos and Drafter coordination in the Objective.
Deleted: A second design round, implementation by the reviewer, new modes/controls, speculative native close-failure claims, generic background redesign, extra agents, version/deployment work and tests in the shared worktree.
Bottleneck: New human OpenCode 2 restart callers lose the surviving child's durable event owner; existing survival-only proof misses its timeline.
Checked myself: Complete PR diff and required #280/#282/#285 records, design/revision/critiques, pinned dispatch/promotion/ingestion consumers, both OpenCode controls and child routing, Claude SDK and close ordering, client/mobile mapping, actual retained proof/images/markers and exact-head live CI.

Reviewer: Codex gpt-6.1-sol, max reasoning effort, Codex harness in T3 Code. Verdict: REVISE.

What: Sol R1's new human OpenCode2 background-observation regression is accepted.
Why: The restart path ends the old subscription, excludes the still-running child from the replacement's ownership seed, and receives no replay of that child's registration; keeping the native task alive alone is insufficient.
So what: Correct this newly exposed caller path inside #285, add the complete surviving-child timeline regression, then obtain fresh Sol/Grok reviews and CI.

I checked the completed-only routing predicate and its ProviderTurnStartService consumers, non-completed terminal subscription stop, runless child event filter, and OpenCode2's early return for an already attached child and runless child turn. This is the #299 mechanism newly exposed by human Steer/promotion, so the prior outside-scope label cannot waive it. Earlier explicit-restart behavior remains historical, not proof that the new human path is safe.

A separate read-only Opus coverage slice compares the smallest ownership/lifetime seed with a justified adapter-scope change. It must account for both route permission and stream retention after the replacement root completes, exact provider/run identity, app-owned children, hard-close Claude and duplicate delivery. It does not choose a larger generic handover framework by default. The Claude close-order Opus slice remains separate and both synthesize into one implementation.

Required oracle: attach a native background child with an open tool before human Steer, keep its result across replacement startup and root completion, then assert child tool, execution node and assistant answer persist terminal exactly once. Use the actual adapter/service/persistence and Deferred or persisted milestones. Both original R1 tasks are completed with no pending runs and failure statuses; aggregate stays failure on1a8, although CI is green.

Requirements and who asked: The user's human cut-in requirement must preserve observation of whatever the selected adapter leaves running; Sol R1 establishes the newly reachable broken path.
Deleted: Treating #299 as a waiver, checking only native survival, broad unrelated adapter work and a second UI control.
Bottleneck: Replacement ownership and lifetime for the still-running child, together with the independently found Claude close-order failure.
Checked myself: Read the pinned ownership predicate/start consumer, interruption subscription stop, child filter and actual OpenCode2 child registration/turn shape, and the complete independent finding; the new deterministic regression is still required before the fix is accepted.

What: The Claude R1 correction will defer terminal settlement until the interrupted query detaches, retaining the native result for usage.
Why: A result can arrive before close, and early settlement both lies about surviving work and lets an effect retry start on the unclosed query.
So what: I am implementing Opus's revised adapter boundary and deterministic failed-close/retry tests before the next exact-head reviews.

Accepted technical revision: keep one pending native result on the active turn, return immediately from the SDK reader while the marked query remains attached, and finalize only through the atomic detach owner. Send one interrupt per turn; every retry still attempts close. Keep the SDK close-request boundary and bounded EOF wait, with no new OS-exit guarantee. A failed close leaves the provider turn running and replacement pending. The existing positive task-notification race test asserted settlement while close was held; it will instead assert no terminal before release, preserving its exactly-once race coverage.

The separate Sol R1 surviving-child ownership slice is still under Opus design and will be integrated before review. No candidate after 1a8a1977be27a7d0207accdfa9be5e8e44acc3ab is ready yet. Both R1 failures remain valid.

Requirements and who asked: The user requires truthful urgent and human interruption; Grok R1 requires close to precede settlement, and Sol R1 requires a real-service failure/retry proof.
Deleted: No reader-blocking Deferred, undo events, OS-exit gate, new settlement command, or Claude run-service handover.
Bottleneck: The native result currently settles the turn before the close outcome, so the retry bypasses the adapter.
Checked myself: Inspected the actual finalize, interrupt, query-exit ownership and retry consumers against Opus's full correction; confirmed the owned isolated server stopped with its retained session before tracked edits.

What: Opus's surviving-child proposal fixes late routing and retention but is not accepted as complete yet.
Why: Its acknowledged transition window still loses one-shot child results between old-terminal and new-subscription, and the new human Steer path would expose that same regression.
So what: Revise the engineering boundary before implementation; no current candidate is merge-ready.

I read the complete design, including its residuals. Seeding exact provider-native same-run identities and lifetime is preferable to indiscriminate running-subagent enrollment, and the Grok mobile audit found a real second consumer. However R-a cannot be waived merely because explicit restart already had the gap: that is the same argument rejected for Sol R1's new human callers. A child can finish during the interrupt/start round trip, leaving the retained row stale even after this proposal. Pending native child questions, pre-existing nested children and late interrupted descendants also need a decision for newly changed callers, not an unsupported non-blocking label.

Reconsider the earliest surviving requirement: the user asked for an explicit course-changing cut-in and a decision per adapter, not unconditional background preservation. Compare an exact continuous observation/bounded handoff at the existing owner against using the existing hard-stop primitive only for explicit urgent/human interruption where that removes otherwise unnecessary handover machinery. Routine delivery must preserve #282 and any pre-existing non-urgent scope; app-owned delegated sessions remain separate. A changed native cancellation scope must be explicit, proved per adapter and accurately documented. Do not invent an OS-exit promise.

The existing human Steer control choice remains settled. This is a technical lifecycle/scope correction, not a new UI or mode. If the comparison reveals a human-owned taste decision that cannot be resolved from the requested outcome, report it rather than silently shipping the transition gap.

Requirements and who asked: User requires urgent/human cut-in, truthful state, smallest scope and per-adapter decisions; Sol R1 requires surviving work to retain its event owner and timeline exactly once.
Deleted: Accepting a known newly exposed one-shot event-loss window as an out-of-scope pre-existing defect; unconditional background preservation as an unexamined requirement.
Bottleneck: The native-work cancellation scope and continuous observation boundary, before choosing a projection seed.
Checked myself: Read the complete Opus R3 design and its residuals, old/new subscription timing, restart effect chain, child routing ownership and native hard-stop entry point; contrasted the proposal with the accepted R1 adjudication.

What: The Claude close-order correction passes all six new regressions and230 focused tests across the complete adapter, automatic-delivery integration and run-execution files.
Why: Native abort results now wait for query detachment before terminal settlement, so a failed close leaves work active and effect retry cannot reuse the unclosed query.
So what: Checkpoint e746d4a115a3ae12f343033587227b37952d8862 is retained locally; the separate OpenCode2/Grok scope decision still blocks a new review candidate.

Grok R1 B1 and Sol's failure/retry question are addressed at the Claude adapter owner: retain native result usage without blocking the SDK reader; perform one interrupt across retries; propagate close failure; let only the atomic detach owner finalize; keep the10s successful-close/EOF bound and old-query fence. The positive task-notification race test was rewritten because its old expectation encoded early settlement, while retaining its exactly-once oracle.

The test child authored only the two authorized test files at baseline 1a8a1977be27a7d0207accdfa9be5e8e44acc3ab. I verified their hashes and exact retained patch, inspected the intended red assertions, and integrated the unchanged patch. Its local checkpoint was blocked by the procedure-read hook, so no child commit is claimed. The parent checkpoint succeeded normally after the required procedure read. No hook or policy was bypassed.

Baseline command, exit1: TMPDIR=/home/luke/scratch/chromeria-285 npx -y pnpm@11.10.0 exec vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts apps/server/src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts -t 'keeps native background work running after an aborted result|positive task-notification result racing interrupt|bounds successful close without EOF|defers the aborted-result terminal until query detach|keeps a native Claude child running until failed-close retry'. All6 cases fail for early terminal/tool/roster settlement, inactive-turn retry, or persisted terminal-time/query-identity mismatch. The same command against the correction exits0,6/6 pass.

Broader focused command, exit0: npx -y pnpm@11.10.0 exec vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts apps/server/src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts apps/server/src/orchestration-v2/RunExecutionService.test.ts,230/230 pass. No sleeps or new polling. The real-service test holds SDK ACK separately from result handling, injects the first close failure, advances TestClock100ms to the actual effect retry, and verifies one native-child interruption, two closes, one interrupt, two distinct query lifetimes, truthful requester note and terminal time after retry.

Retained logs: /home/luke/scratch/chromeria-285/test-writer/focused-red-final.log, /home/luke/scratch/chromeria-285/claude-close-order-green.log, /home/luke/scratch/chromeria-285/claude-close-order-regressions.log. Logs embed commands, exits, candidate and diff identity. The green input diff SHA256 00a6a4a5af5f7cdccbcd4eb1308c8934c2610aea432f63b66be3eaf1a09919fd matches git diff 1a8a1977be..e746d4a115 --binary --src-prefix=c/ --dst-prefix=w/ exactly. The initial comparison failed because a working-tree diff uses c/w prefixes and a commit comparison defaults to a/b; explicit identical prefixes resolved it. The formatter backup6289e536b3 also has no file differences from the checkpoint. A clean-checkpoint run is retained separately for direct identity. Source-only server typecheck and targeted lint pass; combined final-candidate static checks, CI and fresh independent reviews remain required. No native rerun, OS-exit guarantee or current SDK throwing-close claim is added.

Requirements and who asked: User requires truthful interruption; Grok R1 and Sol R1 require close-before-settlement and actual effect-retry proof.
Deleted: Early settlement, repeated interrupt after failed close, reader blocking, extra protocol events and an OS-exit gate.
Bottleneck: Claude's order/retry defect is now discriminated and corrected; the remaining constraint is the separate surviving-work transition gap.
Checked myself: Exact child patch and hashes, red failure output, unchanged green commands, all230 focused results, the corrected explicit-prefix checkpoint-to-proof identity comparison and unchanged backup file contents, atomic detach and persisted retry behavior.

What: Explicit agent restart and human app Steer will use the target adapter's existing Stop cancellation scope before delivering the message.
Why: The user needs course-changing interruption, including a background email or shutdown; preserving native background work was an inherited constraint that created otherwise unnecessary handover machinery.
So what: Implement Opus R4's explicit Stop scope, subject to fresh Sol/Grok design critiques, and prove cancellation/projection ordering before the next code-review candidate.

Selected shape B from the complete Opus R4 design. forceRestart identifies the explicit cut-in; persist optional literal interruption: "stop" on the replacement attempt and restart effect, and pass the existing requestRuntimeRestart input to the adapter. Routine restart-only Grok/Cursor auto/steer, agent promotions, selection-driven restarts, schedules and completions retain their prior path. #282 Claude routine queueing stays intact. A routine policy fallback must not accidentally produce the explicit marker. App-owned delegated sessions remain separate; no delegated-tasks.stop or orchestrator Stop settlement is added to restart.

This deliberately changes R2's preservation decision: explicit cut-ins end OpenCode2 native background subagents, Grok native tasks/monitors/subagents with process respawn, ACP-tracked subagents subject to the provider's Stop behavior, and Codex retained native terminals on the same thread. Queue is the existing non-interrupting choice. No new mode/control or human-owned taste question is needed under the user's delegated per-adapter decision and stated urgent outcome. Native cancellation retains Stop's documented provider/timeout limits, not an OS-exit or rollback guarantee.

OpenCode1 must defer its interrupted root terminal until aborted descendants end (bounded10s), retaining actual final tool output rather than manufacturing it through a cascade. Codex already has that ordering, to be proved with an adversarial native transcript. The superseded-attempt cascade must handle explicit stopped work and nested provider-native rows exactly once. Both former survival assertions in UrgentRestart need rewriting around the new defended requirement, with routine soft guards retained.

C-1 is in scope and blocks completion: a hard Claude close must terminalize the native subagents that process ran, including earlier runs, with their own identities. #305's session-registry observation supports the same adapter owner; it is not a waiver. Reproduce it and repair at detachment, without falsely ending earlier-process/sibling work. R-e's completed-before-effect duplicate-write race needs a discriminating test and severity decision; no unsupported non-blocking label is accepted. Other R3 transition, nested-child and pending-question failures are removed only insofar as actual Stop scope and ordering are proved.

Design and code proof remain distinct: the two fresh design critics cannot set final code-review statuses. Both R1 failures stay on pushed1a8; the local Claude checkpoint e746 is not a complete candidate. Final exact-head Sol/Grok code reviews and green CI remain mandatory.

Requirements and who asked: User requires urgent/human cut-in, routine Claude queueing, real requester/reason, per-adapter decisions and smallest scope; Sol R1/parent requires no newly exposed result-loss window or false lifecycle state.
Deleted: Unrequested background survival for explicit cut-ins, projection seeding, cross-attempt subscription adoption, new delivery mode/control and separately delegated-task cancellation.
Bottleneck: Stop intent must reach the provider unchanged, and stopped native children must finish projection before the interrupted attempt's subscription ends.
Checked myself: Full R4 including its strongest rejected alternative, all scope reversals, routine/explicit classification, OpenCode1 terminal-order proposal, C-1/R-e residuals and the230-test Claude correction already retained at e746.

What: Both fresh R4 design critiques require correction; keep the explicit native Stop design and fix its factual outcome boundaries.
Why: A committed interruption request does not prove cancellation, and the old subscription can otherwise close before child work is accounted for.
So what: Opus is revising those boundaries; this PR remains blocked on the corrections, complete proof, fresh exact-head reviews and CI.

Parent adjudication of Sol GPT-6.1-Sol/max and Grok4.7/xhigh R4 design reports, both completed without pending runs. These are design findings, not code approvals. Published head remains 1a8a1977be27a7d0207accdfa9be5e8e44acc3ab; its failed R1 statuses remain valid. Local internal checkpoints are e746d4a115 (Claude close ordering), 8adc7f0668 (incomplete Stop forwarding/child-tail experiment), and c42ed38ed2d0d0c56a78df55b6c3499abedc8df6 (C-1 integration). Nothing new is pushed or ready for merge.

Accepted required corrections:

  • Intent versus outcome, Sol B1: Delete the public attempt marker and supersededByStop projection callback. They infer cancellation from a request that may not have run. They have been removed locally. The outbox can still carry requested Stop intent; actual adapter settlement must supply the outcome.
  • OpenCode2, Sol B2: A failed/timed-out child interrupt and a child admitted during cleanup are known survivor cases. Keep their event owner, expose incomplete cancellation and prevent replacement until cleanup succeeds. Do not cascade a known survivor as stopped. Interrupt foreground work before waiting on background cleanup.
  • OpenCode1, Sol B3/Grok1: Hold the root before abort and descendant discovery, including attached/busy children with no turn yet. Preserve actual child tool output and define hold/retry ownership after an abort failure. The earlier active-child-only experiment passes147 scoped tests but does not cover this hole.
  • Completion race, Sol B4/Grok4: Reach the captured session even if the recorded turn settles before the effect; guard against stopping a newer unrelated turn. Keep one root event writer while an old attempt drains its own children. No Verify surviving background ownership across same-run steering restarts #299 waiver.
  • Scope wording, Sol B5/Grok2: Cancellation can fail, native background scope is provider-defined on ACP/Antigravity, and local interruption is not OS-exit proof. Request acceptance does not mean cancellation or delivery completed. No promise of undo.
  • Claude C-1/Grok3: Query detachment must settle this process's earlier-run native entries with original identities and clear pending-work probes. The worker's three-file component 764941123c4cf936260a87f2ac04357704ed3ae8 is integrated at c42 with identical file hashes. Its eight intended baseline regressions became green;236 focused tests, scoped lint/typecheck pass at the worker input. Final combined-candidate proof remains required.
  • Carryover, Grok5: Existing ACP/Grok live-context/carryover terminalization owns the ends. Do not replace that with a run-id scanner. Add the routine-soft then explicit-hard same-child guard.

Parent checked the C-1 production diff, test changes, retained red/green logs and exact three-file hashes. New parent reproductions on c42: two real-Claude-adapter/service tests complete the native root after restart dispatch but before the effect and observe its earlier native child incorrectly still running; both fail at that assertion. A separate real orchestrator/session-manager/ingestor/persistence reproduction observes two stored replacement assistant-message events from one normalized event, while the old attempt drains a retained tool. The seam fakes the adapter event source and does not claim native CLI proof. The pure routing case independently rejects the same ownership assumption. Temporary diagnostic setup failures/timeouts are excluded from these reproduction claims.

Active work is recorded on Issue285: Opus R5 revision, a test-only actual-adapter integration writer, and a two-file OpenCode1 owner correction. Main is read-only, the isolated dev server is stopped, no live userdata/deployment/restart/version/memory changes. Human Steer and per-adapter scope remain delegated technical decisions; neither critic identified an unresolved product-taste choice.

Requirements and who asked: User requires urgent agent/human cut-in, routine #282 protection, true requester/reason and per-adapter proof; both critics require truthful cancellation and retained event ownership on the newly reachable paths.
Deleted: Public Stop marker, intent-derived cascade, unrequested survivor preservation by default, projection seeding/handover, new UI/mode and separately delegated-task cancellation.
Bottleneck: Actual cancellation outcome and descendant admission must meet event-owner retirement; provider projection terminalization alone is not that milestone.
Checked myself: Both full R4 critiques and their source paths, C-1 source/test diff and hashes,147-test output, actual failing completed-before-effect assertions, and the stored duplicate-message reproduction; final native/CI/review gates are still pending.

Implementation parent: GPT-6.1-Sol, max, Codex harness in T3 Code. Design: Claude Opus5.5/max; design critiques: GPT-6.1-Sol/max and Grok4.7/xhigh.

What: Accept Opus R5's corrected Stop-scope design for explicit cut-ins, with Codex's request-failure outcome still under investigation.
Why: Both R4 critics exposed places where requested cancellation was being treated as success, native child output lost its owner, or completed-before-effect targets escaped the stop.
So what: Implement the outcome barriers and single-writer fence, then obtain fresh exact-head reviews and CI before the delegated merge; PR #300 is not ready.

Opus 5.5 R5 is READY TO IMPLEMENT, not code approval. I read the complete 429-line revision, both complete R4 critiques and the prior findings/responses. Adopt the private effect intent and new internal adapter input replacementFollows; delete the attempt marker and intent-driven cascade. Stop keeps its existing outcomes. Cut-ins wait for adapter-published ends; unconfirmed work retains the old turn/ingestor. Final failure must settle the replacement visibly, and Stop must still reach the held turn. C-1 remains integrated and is not redesigned. The completed-target guard and root attribution fence address the independently reproduced completion races.

The claimed N-4 Codex RPC-error path is not waived as pre-existing: human Steer newly reaches it. I will reproduce it at the app-server boundary before deciding the smallest correction. N-3's natively ended target is a different cause, deliberately skipped because its ingestor already retired; record the residual under #299. No unresolved product-taste decision remains.

Coverage ownership: the existing test-only child exclusively owns UrgentRestart.integration.test.ts. The parent owns control, effect-worker/start-failure, orchestrator Stop reach, root routing, remaining docs and integration. Separate workers may own only the two-file OpenCode2 adapter slice and the two-file OpenCode1 retained-hold corrections, from the shared input checkpoint 62141710c7. No shared server or browser is delegated. These are changes inside existing components, not new systems. Final combined-source proof and independent reviews are not transferred from component results.

Requirements and who asked: User #285 (2026-10-14) requires urgent agent restart and human app cut-in, truthful sender/reason, #282 routine queueing, per-harness/surface decisions, Opus design/Sol and Grok critiques, exact-head review and green CI, no versions or deployment.
Deleted: New modes/controls, public attempt intent, intent-as-outcome cascade, nested snapshot widening, unrequested background handover, replacement on incomplete cancellation, and OS-exit guarantees.
Bottleneck: The old adapter/ingestor must account for stopped or surviving work before replacement admission; independent OpenCode1 and OpenCode2 lifecycle corrections relieve this while the parent fixes the shared control/ownership seams.
Checked myself: Reproduced two completed-Claude variants and duplicate persisted replacement artifacts, inspected C-1 and OpenCode1 component diffs/hashes, reproduced historical-idle premature release and verified its correction. Workers must falsify early terminal, failed-cancel delivery, lost tails and Stop wedges at native transport seams; parent will inspect results before integration.

What: The rebased candidate now has real-service regressions for Codex cancellation failure, including two R6 residual races.
Why: Ending the native root can retire cancellation control while its child still runs, allowing replacement delivery or making the first Stop miss that child.
So what: These remain merge blockers in #285; the Opus R6 design is under Sol/Grok critique and its residuals are not waived.

Local checkpoint bb7006d4c191c67cb894ff6a89ef5af431cab102, base da7034eb8a465303710bccd91d55ea2618da4110, remains unpushed. New UrgentRestart.integration.test.ts cases exercise the actual Codex adapter and stdio client, session manager, control, orchestrator, effect worker and persistence. Only the native server boundary is faked. Deferred/persisted events establish milestones; TestClock advances actual retry delays.

  • Seven new cases produce six literal failures and one passing completed-root/retry control. MCP restart and human Steer both start replacement after the child rejected cancellation, without retrying it. Exhausted-retry cases likewise receive a replacement instead of the required visible delivery failure.
  • Root-native-failure between cut-in commit and effect reproduces R6 L-1: replacement starts with no child cancellation. This is an in-scope caller, not a filing-only waiver.
  • First Stop during the completed-root cut-in window reproduces R6 L-3: the child receives only the original rejected cancellation. Requiring a second Stop is not accepted.
  • All22 previous native integration controls remain green. Scoped lint passes. Rebased server typecheck and59 tests across six recovery/routing/control files also pass. These scoped results do not establish complete merge readiness.

Canonical handoff and exact commands/log identities remain in #285. Before merge this requires a coherent revised design, green affected proof, fresh Sol/Grok full-context code-review comments and successful named statuses on the exact pushed head, aggregate authenticated review and full CI.

Requirements and who asked: User requires urgent agent and human interruption before delivery, routine Claude queue protection, faithful tests and independent review.
Deleted: Waivers for failures exposed by the new human path; new public delivery modes or attempt-intent flags remain unnecessary.
Bottleneck: Codex must retain the owner and cancellation target until reached work has a confirmed outcome, including the commit-to-effect and first-Stop windows.
Checked myself: Inspected all raw seven-case and22-control output, literal request counts, native protocol setup and rebase range-diff. Six failures are behavioral assertions; no invalid frame, setup error or timeout is used as reproduction evidence.

What: Fresh isolated native tests pass for human app Steer on Grok and Claude, and agent MCP restart on Claude.
Why: The revised Stop and Claude close-order paths need actual provider confirmation alongside protocol replay coverage.
So what: These paths retain native proof on the rebased source; the reproduced Codex cancellation races still block PR #300.

On clean bb7006d4c191c67cb894ff6a89ef5af431cab102, using the copied/pruned task database and actual installed provider CLIs:

  • Human Grok Steer stopped captured foreground PID3460550 before release or completion. The old turn persisted interrupted, the old attempt superseded, and the replacement completed with user/web attribution and correctly described an interruption.
  • Human Claude Steer recorded SIGTERM for PID3475433 before release/completion, then completed a replacement with the user note and correct interrupted-versus-refused understanding.
  • Actual injected MCP t3_thread_send(mode: restart) from the isolated Grok helper recorded SIGTERM for Claude PID3481532 before release/completion. The receiver persisted agent/mcp attribution with the sending thread and explicitly identified that agent, not the user. It understood the cancellation as interruption and did not rerun the command.

Each event-driven holder recorded exactly one start, no release and no completion. Each captured PID is gone. The SDK's raw decline wording still appears, but the actual Claude replies correctly apply T3's explanatory note. This proves the exercised foreground paths, not cancellation of every possible native background process or native OC2 behavior. Exact source-bound and persisted evidence remains in the canonical #285 handoff. Existing before/after screenshots and timing video remain uploaded above.

The owned dev server was stopped through its retained terminal session; both task ports are clear. No live Chromeria restart, deployment, version bump or agent memory. A subsequent metadata-only checkpoint corrects duplicate fork ownership after rebase; the full fork stack and37-feature inventory check pass. Codex production remains unchanged pending the R6 critiques and revision.

What: Both independent R6 design critics require revision; preserve the adapter-owned hold and correct its factual reach and release.
Why: Reproduced Codex child-cancellation failures bypass delivery control, and the first Stop misses held work after a completed root.
So what: Opus must revise the same Codex outcome design against the new native upstream owners before implementation; no residual is waived or review status transferred.

This adjudicates the full497-line Opus R6, full173-line Sol R6 and full214-line Grok R6. Sol is Codex gpt-6.1-sol max; Grok is grok-4.7 xhigh. Both tasks completed with no pending runs and stopped writing. Their outputs are design critiques, not code approvals. Original/prior packets, findings, responses and required final proof remain binding.

Inputs and current base

The two critics reviewed immutable1df/ac6 Codex source. The parent then added seven real-service/native-stdio cases at bb7006: six literal behavioral failures and one completed-root/retry positive control; all22 prior native integration controls stay green. Raw logs: codex-native-seven-regressions-red.log and native-stop-prior-controls-green.log. Two earlier native adapter reds remain committed. No schema/setup/timeout failure is counted as behavior proof.

Main subsequently landed authorized native-upstream absorption PR #291 at038745968cb6f2204b18152f89903cb51693b16b. ProviderAdapter moved to packages/provider-core, other provider implementations moved to native provider packages, and Codex/Claude remain server adapters. The parent is carrying the task without reviving old modules. Backups preserve the complete old1e23 candidate; its26 task commits were collapsed with exact tree equality to34dd before the one-commit rebase. The rebase is in progress, unpushed and not validated. Immutable design snapshots of nine conflict-free carried files plus digest manifest are under native-r7-input/. Read those together with git show038745 for native surroundings and git show1e23 for the previous complete candidate. Never rely on the moving conflicted worktree as a reviewed candidate.

Dispositions

  1. L-1 / both B1: accepted blocker. Native root failure between commit/load and cancellation-owner acquisition leaves a live child, skips cancellation and permits a replacement while the old ingestor still writes by run identity. It is reproduced. Filing separately is not a waiver for the new human or agent caller. Preserve an already published native failure; do not reopen it as running. The exact deferred/retained owner must remain reachable on first attempt and retry. The root-only writer fence must cover the native-ended/deferred interval while child admission remains open until the true terminal.

  2. L-3 / both B2: accepted blocker. One actual Stop during the retry window must reach the held child of a completed target. The first-Stop real-service case is red. Reach the exact factual owner/captured target; do not broaden every ordinary selection restart to an unrelated target. A generic cancelled or unexecuted restart is not automatically a user Stop: the existing cancelled-unexecuted OpenCode control must retain its observer without inventing child ends. If using cancellation takeover, distinguish actual run Stop from merely cancelling an effect, and preserve both in-flight and before-execution behavior.

  3. L-5 / both B3: accepted source-traced blocker. The current any-running-turn guard treats an older held root as newer work. Compare actual target ordering/ownership, keeping session binding, still-current replacement, genuine newer unrelated work, and native goal continuation protections. Joining older held work belongs at the adapter. Add the old-hold/new-completed and genuine-newer controls; no executed L-5 reproduction is claimed yet.

  4. Sol B4 closed cohort and cancellation-safe publication: accepted. The successful tail must use the same predicate as natural release, covering every current retained target, active/pending governed work and tracked process, including a child that completed its native turn while its command remains. Claiming/deleting under a permit is insufficient if cancellation can interrupt the subsequent event publication. Use a short cancellation-safe commit with no native RPC wait, or an equivalently retained releasing owner. Prove the specific late retained-process and publication-cancellation schedules.

  5. Sol B5 native reader cycle: accepted for revision, verify against native038745. The old notification path awaits an interrupt whose ACK is parsed by that same serial reader, while holding the permit. Governed late work must be registered before I/O and cancellation must occur outside that reader/permit. Keep identity, real output and process observation; test ACK, rejection and no-answer through the actual stdio/client. Do not silently broaden the legacy ordinary-Stop branch without separate evidence.

  6. Sol B6 goal pause: accepted. Bound the first explicit-cut-in goal pause, not just later interrupt RPCs. Do not remove a held goal or release its waiters before ownership transfers safely; retain any native continuation and retry/Stop reach. Verify actual native goal semantics and the before/after pause race.

  7. Grok arming at the pure Orchestrator commit: remedy rejected. Its failure trigger is accepted, but adding live adapter I/O and permit acquisition inside the event decider violates the project boundary and risks a runtime hold left by a failed commit. Prefer the factual retained-owner route Sol describes, or another owner-correct design that closes the same window. A new public attempt-intent flag and a second liveness registry remain rejected.

  8. Grok inFlight count: not an independently established blocker. Sol traced per-thread effect lane serialization. Identify the actual release owner and any supported overlapping joins; do not add counters just by analogy. A takeover must invalidate an older success path and its cleanup must not release another owner. Test Stop takeover and a later same-thread join.

  9. Grok ACK-only/local-settlement objection: reconcile explicitly. Do not turn it into an unsupported OS-exit or universal native-end-only promise. R5 accepted native cancellation/containment boundaries and bounded handling of acknowledged-but-silent turns. The revision must justify that path against the real protocol: retain it only after accepted cancellation and every required native process containment, never after rejection, no answer or an unconfirmed retained process. If that evidence cannot support safe delivery, retain the owner and fail visibly. Pair ACK/no-end, failed containment, no-ACK and ordinary Stop controls. The existing interface cannot guarantee rollback of completed/partial external effects.

  10. Grok goal waiter and release re-check concerns: investigate with discriminating schedules. Preserve factual goal continuations and complete hold.next only at the correct ownership transition. An arbitrary second permit acquire is not itself a proof against future admission. Use the actual native ordering/ownership boundary and a whole-cohort predicate; add the queued-handler schedule if reachable. Do not add an endless quiet-time/debounce/polling barrier or claim a future-work guarantee unsupported by the protocol.

  11. Terminate timeout claim: corrected. terminateBackgroundTerminal already has a10-second bound. Do not wrap it again. Missing bounds identified by source are the main turn interrupt and initial goal pause.

  12. Evidence filenames: corrected. codex-cancel-unack-green.log did not finish; codex-cancel-unack-corrected-green.log failed. Only codex-rpc-failure-controls.log records the six-case success and r5-shared-failure-tests.log the211-case success. Unsupported outputDelta and the earlier invalid OC2 typed frame stay excluded. No green filename is acceptance by itself.

Preserved outcome and proof

Human app Steer always interrupts restart-capable adapters through existing web/desktop/mobile controls; Queue stays the default. Routine Claude auto/agent-steer queueing, other routine/selection paths, normal Stop outcome policies, #279 ancestor restrictions, separate app-owned delegated tasks, truthful requester/author provenance, pre-stop slash validation and the no-version/no-deploy/no-live-restart limits remain. No product-taste choice is unresolved.

Fresh actual foreground probes passed on the old clean bb7006 source: human Grok, human Claude and agent MCP restart into Claude. Captured PIDs disappeared before release/completion; Claude recorded SIGTERM; both actual Claude replies applied the truthful note despite native SDK decline wording. Exact records are linked in #285. They do not establish the new native-base candidate or every native child. Existing images/video remain uploaded on #300; no screenshot asset is committed.

The revised design must specify caller-first common/failure/awkward cases, one authoritative owner, exact target guards, safe release and errors, native goal/late-child behavior, proof that rejects each known faulty implementation, and honest limits. The existing two native adapter reds and seven-case real-service group remain required, plus focused discriminating schedules above. No sleeps/polling. Final combined proof, full green CI and fresh full-context exact-head Sol/Grok findings comments then named and aggregate success statuses remain mandatory. This adjudication grants no code approval.

Requirements and who asked: User #285 requires urgent agent and human cut-in, #282 routine protection, truthful reason, all surfaces/adapters, faithful outcome tests, Opus/Sol/Grok design and exact-head review, one PR and delegated merge with no versions or deploy.
Deleted: Residual waivers for the new path, adapter-file-only purity, pure-decider runtime arming, public attempt intent, duplicate termination timeouts, unproven overlap counters, quiet-time barriers and invented OS-exit guarantees.
Bottleneck: The retained native owner is unreachable from some control/Stop paths and its proposed success predicate is narrower than its retained cohort; native upstream migration must be respected in the revision.
Checked myself: Full R6 and both complete critiques, six literal real-service failures and passing controls, original two adapter reds, actual native foreground PIDs/results, source publication/control/fence/cancel paths, the live #291 replacement and mapped provider owners. No current native-base acceptance is claimed.

What: Accept Opus R7 as the existing-component Codex correction to implement, with the prior six integration failures and both adapter failures still unwaived.
Why: The revised design keeps factual ownership reachable after native ends and preserves cancellation outcome before replacement delivery.
So what: Implement the private runtime view and Codex cohort owner, verify the discriminating schedules, then obtain fresh full-context Sol/Grok code reviews and green CI on the exact final head before the delegated merge.

Parent read the complete748-line Opus R7 and verified its new cancellation-lane claim against current EffectOutbox cancel/claim/signal code, four-worker daemon and installed Effect4.0.4 raceFirst implementation. Cancellation frees the durable lane before the old callback finishes unwinding; a per-provider-thread cut-in-call exit is justified, without a speculative inFlight counter. Opus quoted4.0.1, so final tests must exercise installed4.0.4. No current code approval is implied.

The adapter remains the authority for a held native cohort. endedTurnHold is an optional, read-only server-internal view of existing cut-in/deferred-root owners; it is not wire state, a second liveness registry or a public attempt-intent marker. The control service reaches an exact completed or factually held target with current-session/replacement/ordinal/attempt checks. Stop selects ended cut-in-held targets only inside its before-start branch. RunExecutionService closes only root output at native root end; child admission stays open until the actual terminal.

Retain the whole-cohort release predicate and a short uninterruptible publication with no native wait, answered-turn tracking, no duplicate interrupt after a native answer, containment before local settlement, the bounded initial goal pause and native goal ownership transition, and registration of late children before I/O outside the serial reader. Preserve ordinary Stop/routine behavior, separate T3 tasks, Claude queueing, completed native outcomes and the cancelled-unexecuted OpenCode control. The newly added L-5 service test atc420 is also required: its literal baseline fails4 versus3, not setup.

Limits L-2/L-4/L-6/L-7/L-8 remain qualified as in the accepted prior design. L-9 describes tracking already lost at an earlier natural native failure, not permission to discard processes observed by an acquired cut-in; record the natural-failure problem separately. L-10's boundary is the last awaited native reply's actual serial-reader ordering, not a promise against future native work. L-11 is a failed/late goal pause and must never report delivery. L-12 is not accepted as a waiver for an explicit urgent restart or human Steer: if session replacement is reachable through those callers, retain exact old-session Stop reach or fail the component with its reproduction. Do not widen ordinary session-restart behavior without evidence. No product-taste choice is reopened.

The parent owns shared runtime shape, control/Stop reach, root fence, service/native integration tests and final integration. One existing-component worker owns only Codex adapter implementation and its complete native unit proof, against a prepared immutable shared input. Independent component code review is supplied by the required final exact-head Sol/Grok reviewers, not by the author or the design verdict. All earlier findings, responses and proof limits stay in the final review packet.

Requirements and who asked: User #285 and parent R6 adjudication require explicit urgent/human cut-in, faithful Stop outcomes and first-Stop reach, no false refusal, routine protection, Opus/Sol/Grok design, exact-head dual review, green CI and delegated merge without versions or deployment.
Deleted: Adapter-file-only restriction, residual L-1/L-3/L-5 waivers, pure-decider runtime arming, worker intent takeover, extra counters, duplicate timeouts, reader-path requests, early goal release and invented OS-exit guarantees.
Bottleneck: Implement and prove the retained Codex cohort while the parent migrates the two factual callers and root-output fence at their existing owners.
Checked myself: Complete R7 and prior full R6 critiques/adjudication; current cancellation lane and Effect4.0.4 source; native root finalization, reader, control and Stop dispatch; literal native-base6-red/23-green integration output, two adapter reds and the new L-5 service failure. No native-base foreground or final review/CI acceptance is claimed.

What: Accept Opus's R7 L-12 addendum for implementation, with exact captured-binding guards and the same repair for routine restarts introduced by this PR.
Why: Stop currently reads the replacement binding while the old runtime still owns work; four real-service failures reproduce session replacement and thread handoff during retries and after final failure.
So what: Implement capture-based Stop targeting, verify old-runtime reach and unchanged cancellation behavior, then require fresh exact-head Sol/Grok code reviews and green CI before the delegated merge.

The parent read all211 lines of opus-r7-l12-addendum.md and checked the selected owner against current Orchestrator restart creation, Stop branches, outbox schema/get, worker forwarding and control load. The same Opus job's ordinal4 is completed, stopped writing and has no pending runs. Verdict READY is design direction, not code approval.

Use the existing durable restart effect, keyed by the superseded attempt, to retain the exact session/thread/turn captured before a binding change. Add only an optional server-internal replacement-session field to the interrupt effect/control input, with existing old rows still decoding. No client/contract/projection/session-registry or Codex-worker scope change.

Two parent corrections are required. The replacement-session field must come from the captured restart's sessionTransition, never be invented from the binding current at Stop; otherwise an unrelated third binding would authorize itself. Planning and execution both require the old captured binding or that exact recorded replacement, with run/attempt/turn/thread identity and newer unrelated-turn checks. A starting replacement whose selected old target is ended but held must still use the before-start branch and cancel its restart effect, including when pending background work made that old turn selectable.

D-5 is accepted here rather than filed as a waiver: it is introduced by this PR's R5 held-turn routing and is the same Stop bug. Read capture rows for routine restarts too when their old turn is still running. Ended ordinary roots without a cut-in remain outside this correction; only the runtime's factual cut-in view admits an ended target. This changes no routine send or native steering behavior and does not treat generic outbox cancellation as Stop.

The four current L-12 failures use actual orchestrator, session/control/outbox/persistence services with only the adapter boundary replaced. They deliberately exercise restart_session/handoff plans; those are faithful sequencing tests, not proof that production Codex chooses those plans for option changes. Opus traced the real production replacement trigger to two Codex instances sharing a continuation key, and handoff to incompatible instances/drivers. Additional production-policy two-instance schedules are required. The native Codex cohort worker remains exclusive and unaffected.

Requirements and who asked: User #285 and accepted R7 require explicit urgent/human interruption, first-Stop reach, truthful cancellation outcomes, routine protection, exact-head dual review and green CI before authorized merge; the parent must fix regressions this PR introduced.
Deleted: Session scans, a new outbox query, public intent or session fields, duplicate liveness state, worker cancellation takeover, unrelated current-binding authorization and a D-5 waiver.
Bottleneck: Stop loses the runtime captured before the restart commit moves its binding; implement and prove exact capture routing while the Codex cohort component finishes.
Checked myself: Full Opus addendum and prior adjudication; current construction/dispatch/control/worker/schema; four literal L-12 service failures and two same-binding controls, native cohort baseline with limits recorded on #285, and unchanged provider-core worker input after the clean #329 rebase.

What: The integrated R7 candidate is pushed at 3326f71ea675237e477118e785428136dc470975 over main 01d7066bb72e195af8c51a6e38aea3aada805d2d.
Why: Earlier reviews found false Claude settlement and lost native-child ownership; the corrected owners and exact captured-session Stop now have combined proof.
So what: Fresh independent Sol and Grok reviews and full CI must succeed on this head before the delegated merge. Earlier statuses remain historical.

The stopped Codex component was read in full and its30 proof log/metadata digests, final file digests and unchanged original170-test body verified before integration. Opus's full211-line L-12 addendum is accepted with the parent's exact-binding corrections and D-5 disposition. Stop uses the durable restart capture, never an unrelated current binding. Guarded failures do not falsely settle background work.

Full focused proof on the integrated #327 base:348/348 in seven core/native files and730/730 in eleven other adapter/policy/service files,1078 total. Complete server typecheck passed. The normal commit formatter changed only whitespace in ClaudeAutomaticDelivery.integration.test.ts; its complete file passed again on this exact head. All other tested files are unchanged. Exact-head scoped lint of every changed TypeScript file and all four provider package typechecks pass. Fork inventory passes37 features after preserving ContextMD's primary ownership of the two shared files. Commands, source identities and raw logs are retained in #285's handoff; full repository admission remains CI-owned.

The new production-policy two-instance Codex tests cover same-thread replacement and distinct-thread handoff before execution, during retry and after final failure, plus routine-restart Stop, cancellation without Stop and Stop on the successfully started replacement. A failed replacement run preserves Message not delivered after its old work is stopped. The positive control awaits the replacement's actual persisted provider-turn event.

The reader proof was strengthened after a mutation exposed a fake writer deadlock. The fake now accepts independent requests while one response is held. Restoring native cancellation inside the reader produces three literal failures (native interrupt before persisted sentinel) for acknowledgement, rejection and no answer; fixed production passes all three in the full47-case native file. The earlier timeout is excluded. The overlapping-root fixture likewise needed a valid native thread/inject_items response; no cancellation oracle was weakened. Other excluded setup/type/oracle diagnostics remain named in #285.

L-10 is qualified by actual ownership release: natural completion can release a cohort before an earlier interrupt reply arrives. A retired call cannot capture later work. No OS-exit, rollback, future-work or survival-across-server-loss guarantee is added. The current actual-CLI foreground evidence is pre-absorption bb7006; current code has the native transport/service proof above, and another isolated foreground pass is pending. Uploaded UI controls/evidence are unchanged. No deployment, version bump, live restart or memory write.

Requirements and who asked: User #285 requires urgent agent/human interruption, routine Claude protection, per-surface and per-adapter proof, Opus/Sol/Grok design and exact-head dual code review plus green CI before authorized merge.
Deleted: Extra delivery modes, UI controls, broad session scans, duplicate liveness state, earlier design waivers and publication/deployment work.
Bottleneck: Current exact-head independent review and CI, after the combined cancellation/ownership proof.
Checked myself: Full component/source/test delta and proof digests; native service outcomes and literal mutation failures; #327 overlap and preserved instruction staging/cleanup; all1078 focused tests, server/provider typechecks, scoped lint and fork metadata. Candidate is one normal-hook commit with no version change.

Implementation: Codex gpt-6.1-sol max, Codex harness. Design: Claude Opus5.5 max, Claude Code. Required code reviewers: fresh Codex gpt-6.1-sol max and Grok grok-4.7 xhigh tasks.

What: Current-head CI found an environment-dependent fixture and two Claude lifecycle regressions that focused proof missed.
Why: The new Codex initialization fixtures require TMPDIR, and the root-output fence does not account for a resumed Claude subagent retaining its launch parent.
So what: Repair the fixture, obtain Opus's bounded owner-level design for the Claude cases, then require a fresh candidate, both exact-head reviews and green CI. No merge readiness is claimed.

CI run38649485696 is on exact3326f71ea675237e477118e785428136dc470975. Failed server jobs116144468125/116144467928/116144467882 have been downloaded and read; the full gate remains in progress.

  • CodexAdapterV2's sixteen new cohort tests fail before behavior with initialize decode MissingKey. Both new native-boundary fixture helpers put process.env.TMPDIR in required codexHome. Local bestia proof sets TMPDIR; GitHub does not. A targeted unset-TMPDIR run reproduces the same setup error (r7-ci-init-missing-tmpdir-red.log). Use a stable valid native metadata value in these fixtures, not host configuration or a weakened schema.
  • The recorded claude_background_wake_before_queued_prompt case has child-root activity active/completed/active/completed while its subagent row stays active/completed/active. Source: Claude reopen reattributes runId but preserves the original parentNodeId/rootNodeId; RunExecutionService's native-ended root fence can reject its later subagent row while accepting the child-root terminal. This is a newly exposed real ownership/routing failure, not an incidental expected-count update.
  • Both OrchestratorReplayRestartBackgroundNote controls now see a failed subagent before restart rather than running; recovery must still deliver its cancellation note exactly once under either continuation setting. Investigate actual native EOF versus server-cut/shutdown simulation and query cleanup. Do not bless missing recovery notes by changing status expectations.

The exact-head R7 Sol/Grok reviews continue against their immutable pins. Their findings will be retained, but no verdict transfers to a changed candidate. Parent remains sole integration writer; Opus design is read-only. The isolated current-head dev server is running only against the copied/pruned worktree .t3 on14229/6189; no native holder has started. No live state, restart, version, deployment or memory write.

Requirements and who asked: User #285 requires truthful interruption, routine/background equivalence, preserved recovery, both current-head independent reviews and full green CI before delegated merge.
Deleted: Host TMPDIR configuration, schema weakening, updating lifecycle expectations without cause proof, accepting focused green checks as full admission, and transferring old review status.
Bottleneck: Resolve the actual ownership/recovery failures exposed by full CI, then combine fixes and review the resulting candidate.
Checked myself: Exact CI head/job logs and literal assertions; initialization schema and fixture values; Claude reopen attribution and root-node identity; root output routing and query-exit cleanup; existing recovery test's exactly-once provider note contract.

What: R7 review of PR 300 at 3326f71ea675237e477118e785428136dc470975 is REVISE. Three blockers are reachable on this head, and GitHub Actions run 38649485696 failed.

Why: A Claude query exit now fails a background subagent that restart recovery still expects to be running; the new root-output route keeps a child root completion the subagent row never stores; the new Codex cohort fixture never reaches its assertions on CI.

So what: Parent owns the fix and any new review. Do not merge this head. This comment is review/independent-grok only.

Head: 3326f71ea675237e477118e785428136dc470975
Base: 01d7066bb72e195af8c51a6e38aea3aada805d2d
Verdict: REVISE
Model / effort / harness: grok-4.7, xhigh, grok harness

Blockers

B1. Claude stream exit fails a still-running background subagent before restart recovery

Trigger: a Claude root turn completes while a provider-native background subagent is still running, then the query stream ends without stopping (the restart-note replay cuts the transcript at the root result and appends runtime_exit).

Actual: endSubagentsOfClosedQuery publishes that subagent as failed. CI Test Server 5, OrchestratorReplayRestartBackgroundNote.integration.test.ts:182, expected 'running' and received 'failed', both continueThreadsAfterServerUpdate cases. The assertion is phase 1, before recovery.

Expected: the subagent stays running in the projection so startup recovery can cancel it and record the "cancelled before it finished" note. That contract passed on base CI run 38648050223 (Test Server 5 success on 01d7066).

Path:

  • ClaudeAdapterV2.ts:8562-8566 runs endSubagentsOfClosedQuery on every owned stream exit, with "failed" when context.stopping is false.
  • ClaudeAdapterV2.ts:5778-5800: if the active turn is no longer that query's last turn, the else branch calls updateClaudeSubagentNode and emits the failed subagent. After the root result, the active turn is already cleared, so this branch runs.
  • packages/provider-core/src/server/subagentProjection.ts:379-381: recovery's endOrphanedNativeSubagent skips a subagent that is already not active, so it cannot cancel it or attach restartCancelledBackgroundWork.

This is the restart-recovery contract, not the accepted R1 hold of a native abort until close. R3 still wants an explicit Stop/close to settle that query's native tasks. A clean stream exit after a completed root is the case recovery is built for, and this head settles it first as a failure.

B2. After root output ends, a child root completion is stored without its subagent update

Trigger: Claude background wake fixture claude_background_wake_before_queued_prompt. A resume re-attributes the subagent to the continuation run, then a later completion is routed after that run's root provider turn is terminal.

Actual: CI Test Server 1, testkit/fixtures/shared.ts:1242, child root activity ['active','completed','active','completed'], subagent activity ['active','completed','active']. Base CI run 38648050223 Test Server 1 passed this fixture on 01d7066.

Expected: the child root turn follows the subagent. The adapter emits both updates together.

Path:

  • ClaudeAdapterV2.ts:5007-5018 re-attributes runId to the resuming run. parentNodeId stays the launch root (4988). The child root node keeps runId: null (5196-5215).
  • RunExecutionService.ts:431 sets rootOutputEnded when the root provider turn is terminal and never clears it.
  • RunExecutionService.ts:456-462: a parent-thread subagent.updated after that point belongs only when ownedNodeIds contains parentNodeId. The resuming run's set holds its own root, not the launch root, so the completion is dropped.
  • RunExecutionService.ts:369-370 and 435-443: the paired child root_turn node.updated still belongs through ownsChildThread, which is outside that fence.

The extra status in CI is completed, so this is not B1's failed stream-exit write. The two updates are split by the fence.

B3. New Codex cohort tests abort on CI before any cut-in assertion

Trigger: CodexAdapterV2.test.ts native cohort helper, four failures on Test Server 6, all ProviderAdapterEnsureThreadError for r7-app-thread.

Actual: initialize decode fails with Missing key at ["codexHome"] (CodexAdapterV2.ts:6743). The helper replies with codexHome: process.env.TMPDIR (CodexAdapterV2.test.ts:9714-9716). The reply is Schema.encodeUnknownSync(Schema.fromJsonString(Schema.Unknown)) (line 74), which drops undefined. CI does not set TMPDIR (no workflow env, and the key is absent). V1InitializeResponse.codexHome is required.

Expected: a literal absolute path, as the older Codex tests use, so the cohort cases run. Local 348/348 on bb640ae does not reproduce this, because that machine has TMPDIR. These four cases never proved the cut-in cohort on the exact head's CI.

Prior dispositions

  • R0/R2 hard Claude interrupt, close, reopen: present. Close failure still throws before terminal publication (ClaudeAdapterV2.ts interrupt path). Cleared.
  • R1 Claude terminal held until the interrupted query detaches, and OpenCode 2 keeps its owner when replacement stop does not succeed: present on the paths read. Cleared. B1 is a different, non-stopping stream exit.
  • R3 exact-query settlement, workflow owners, buffered task notifications, and #305 session-registry exit: the guards in endSubagentsOfClosedQuery (5758-5777) match that design. B1 is the over-application to a completed root's clean exit.
  • R4 native Stop scope only for explicit cut-in (replacementFollows), routine Claude queue, routine Grok/Cursor soft restart: CommandPolicy.ts and ProviderTurnControlService.ts:265-268 match. Cleared on those paths.
  • R5 retained work and visible F2 "Message not delivered": settleUnstartedRun writes that title only while the run is still starting (ProviderTurnStartService.ts:1394-1423). settleInterruptedRun returns unless the run is still running (Orchestrator.ts:9305-9313). Cleared on those paths.
  • R6 L1/L3/L5, closed-cohort predicate, cancellation-safe publication, serial reader, 10s cut-in bounds: Codex cut-in code matches the accepted shape (one cohort, join before later Stop, late child registered under the permit, uninterruptible release with no native RPC, retired-call guard). No new contradiction found there. The CI cohort file never executed those assertions (B3).
  • R7 L12: replacementProviderSessionId comes from the captured sessionTransition replace (Orchestrator.ts restart target). Control load accepts the recorded session or that exact replacement. The worker does not swallow a control failure when a replacement session id is present. D5 routine ended roots without a cut-in are not retargeted. Cleared on those paths.
  • L10: a retired Codex cut-in returns out of lineage before it can touch a later call. Cleared on that path.
  • #279 descendant steer/restart refusal: unchanged. Cleared.
  • #327 ContextMD stage, attach, and release: still around turn start, with release in ensuring (ProviderTurnStartService.ts:559, 862, 1443). Cleared.
  • Qualified limits L2, L4, L6, L7, L8, L9, L11 remain limits. They are not waivers for B1-B3. #297, #298, #299, #331, #332, #333 were not reopened.

Proof

Inspected:

  • Actions run 38649485696 on this exact head: failure. Test Server 1, 5, and 6 failed. Test Server 2 was cancelled. Lint, Typecheck, Build, Test, Test Web, Test Server 3, Test Server 4, Release Smoke, Rust, and Fork Stack Model succeeded. Check failed as the aggregate.
  • Actions run 38648050223 on base 01d7066: success, including Test Server 1, 5, and 6.
  • Parent scoped logs r7-new-base-combined.log (348/348) and r7-new-base-other-adapters.log (730/730) record head bb640ae, not 3326f71e, with two dirty metadata files. They do not cover these CI failures.
  • r7-native-reader-mutant-independent-io.log is a mutant run on dirty 6af5fb85 with 3 expected oracle failures. It is not a pass of this head.
  • Parent lint log records this head and exit 0. I did not re-run it. CI Lint on this head succeeded.

Executed by this review: none. This worktree has no installed dependencies. CI already ran the failing files on this head. I did not weaken any test.

Limits

No current-head native Codex, OpenCode 2, Electron, or device run. No live userdata. L2 native containment is still not an OS exit. In-memory holds still die with the process (L7).

Elon

Requirements and who asked: Luke, 2026-10-14, #285 / #282: routine Claude updates stay queued, an explicit urgent restart interrupts with the real requester, and human Steer cuts in. This review checks that head. It adds no product requirement.

Deleted: No new mode or control. Soft background preservation stays rejected. Speculative early-return and unbounded ordinary-Stop notes were not promoted without a failing path.

Bottleneck: CI on this head is red in three server shards. The two Claude fixtures are behavior regressions. The Codex shard never reaches the new cohort assertions.

Checked myself: The three Actions annotations, base run 38648050223, and the cited source lines. I did not treat the bb640ae logs as proof of 3326f71e.

What: Accept both R7 reviewers' three required corrections;3326 remains rejected.
Why: Full CI and independent Sol reproduction expose a dropped resumed child completion, invalid Codex initialize metadata and an unfaithful crash-proof seam.
So what: Parent corrects those owners, retains both full review packets and requires fresh exact-head dual reviews and green CI on the replacement candidate.

Sol R7 and Grok R7 are both REVISE on3326. Their individual failures and the authenticated aggregate failure are verified on that exact SHA. Both tasks are completed with no pending child runs. No approval is transferred.

  • Sol B1 / Grok B2: accepted runtime regression. Parent independently reproduced the recorded wake case at the correct testkit path on3a97/base9f46: child activity active/completed/active/completed versus parent subagent active/completed/active (r8-claude-wake-local-red.log). Original parent/root identity retained by Claude reopen conflicts with the added root-output fence. Preserve factual child completion and single replacement-root persistence; no expected-value relaxation. Opus owns the bounded design, parent the source repair. One source clarification: rootOutputEnded does clear on a factual running goal continuation; it is not an irreversible latch. Existing goal and one-writer controls remain required.
  • Sol B2 / Grok B3: accepted fixture defect, corrected locally. Both new initialize helpers now supply literal /workspace/.codex, without changing schemas or host configuration. An unset-TMPDIR run reproduced the literal MissingKey first. The complete Codex and UrgentRestart files then passed233/233 with TMPDIR unset and an owned TMP directory (r8-codex-no-tmpdir-green.log). This local fix is checkpointed, not a new published/approved candidate. There are sixteen affected cohort cases, not four separate production failures.
  • Sol B3 / Grok B1: accepted required recovery-proof repair; native-EOF interpretation is qualified. Both continuation settings fail locally before recovery (r8-claude-ci-local-red.log). The test currently labels successful runtime_exit as a server crash, but its replay iterator actually returns native EOF. Actual query exit and an abrupt server cut are distinct observations. Existing exact-query ownership and EOF controls explicitly require a process-owned unfinished task to become failed when that native query exits, so removing that cleanup or leaving an unreportable task running would regress Claude closed queries can retain running subagents in the session pending-work probe #305. Preserve the valid pending-at-server-cut, recovery-cancelled and exactly-once provider-only note assertions by modelling the actual cut before cleanup; Opus is deciding the smallest faithful seam. A passing old base does not establish that native EOF is a crash. This is a response to Grok's causal recommendation, not a waiver of the red test.

Main #334 paired-session renewal is integrated locally at3a97d5df0e47ba5239b8500513be02b3dbc2b181 over9f46c833b19223e38a1dfb654d2735e54d47c1f7. Its only conflict was the fork-edit inventory; both feature sections were preserved. Installation completed with the required pnpm11.10.0. Main checkout remains read-only. Owned isolated dev runner11277 was stopped through its retained session; both task ports are clear. No native holder started. Fresh foreground fixtures are prepared only in scratch for later stable-candidate verification.

Requirements and who asked: User #285 requires routine update equivalence, truthful explicit interruption, preserved native ownership/recovery, current independent Sol/Grok review and full green CI before delegated merge.
Deleted: Decoder weakening, ambient-TMPDIR dependence, treating native EOF as a server crash, blessing changed lifecycle values, reopening root output broadly, and transferred earlier approval.
Bottleneck: Finish Opus's bounded ownership/crash-proof correction, validate it at the actual recorded/persisted seams, then review and test the complete replacement candidate.
Checked myself: Both full published reviews and status creators, actual CI errors, literal local setup/lifecycle failures, unset-environment233-test control, native replay iterator semantics, existing EOF/closed-query owners, root-output goal reset and main conflict resolution.

What: Accept Opus R8's adapter ownership repair and one-line crash-seam correction inside #285.
Why: Claude resume moved its run and turn without moving its tree, while the recovery fixture simulated native EOF rather than server loss.
So what: Implement those bounded repairs, add the real persisted same-run/reused-task regression, then require fresh Sol/Grok exact-head review and full green CI before delegated merge.

Parent read the entire READY design below and traced its caller, routing and shutdown claims against current3a97/base9f46. No new contract, mode, registry, fence exception or product decision. The current Codex fixture correction is already local and separate. Existing native EOF control passes and both unmodified recovery cases reproduce the red precondition.

Accepted: the resume predicate includes a wake-preopened running task, all attempt ownership moves together, and a nested parent stays when the task remains in the same tree. A task moving trees attaches to the current attempt root; launches, progress, real outcomes, query-close guards and the server routing fence retain their owners. Remove only the synthetic runtime_exit from the restart-note fixture and retain every assertion; production shutdown already detaches subscribers before closing the query. Register that carried test edit in the fork inventory.

Proof selection: N1 persisted real-service same-run restart/reused-id case is required, plus N2's existing wake-race ownership assertions. Existing full one-writer/goal/late-child controls cover the unchanged fence; do not add a second pure mirror merely to reproduce its predicate (N4 recommended, not required). Existing factual EOF control covers #305 (N3 optional). Run all seven proposed complete files, unset-TMPDIR Codex controls, relevant statics and inventory; CI remains the complete gate. Preserve R7 findings/comments and their failure statuses; no approval transfers.

Opus's stopped/failed-task resume observation is conditional on an unrecorded native capability. It is not established by a fabricated SDK sequence. Preserve it as an explicit investigation limit for final reviewers and use native evidence if the isolated check reaches it; no speculative observer transfer or blanket adoption is added to this repair.

The following is the read-only designer's complete retained record; source pins and predicted test outcomes are historical design evidence, not executed proof or code approval.

Read-only copies of git show output were written under /tmp/opus-r8-* for searching and removed before finishing; this file is the only retained output.

1. Verdict: READY

Two bounded repairs inside existing #285 components. No new field, registry, latch, watcher, timer, mode or product choice.

  1. Resumed Claude child (Sol B1, Grok B2): adapter-owned. When task_started resumes a known Claude task under a turn whose attempt has a different root node, updateClaudeSubagentNode moves the subagent's parentNodeId and its node's rootNodeId to that turn's root, together with the run, provider thread and provider turn it already moves. Codex already does this for a reopened child, and Claude already does it for a subagent recovered after a restart. The root-output fence in RunExecutionService is unchanged; its existing identity checks then admit the resumed child's factual completion to the resuming attempt, and only to it.
  2. Restart-note proof (Sol B3, Grok B1): fixture-owned, no production change. The test's runtime_exit: success is native EOF delivered to a live server, which Claude closed queries can retain running subagents in the session pending-work probe #305/R3 correctly settles as failed. A production server shutdown closes and clears run subscriptions before it closes the adapter, and drops provider events after a shutdown signal; a crash persists nothing. Either way the child stays running for recovery. Remove the synthetic EOF from phase 1 so the server stops while the CLI has not reported, keep every assertion, and allowlist the now-carried upstream test under Restore explicit urgent interruption after Claude steer queuing #285.

2. Requirements

Requirement Source

A native query exit settles the exact closed query's unfinished native tasks (failed, or interrupted when T3 stopped it) R3, #305, existing EOF control
A server cut leaves running background work for restart recovery to cancel, with the provider-only note exactly once under both continuation settings and stored user text unchanged upstream restart-note test, Sol B3, Grok B1, parent R8

3. Case 1: resumed Claude subagent completion

3.1 Cause (source facts @3326)

  • Every task_started calls updateClaudeSubagentNode with reopen: true (ClaudeAdapterV2.ts 7262-7280). For a known task the updater re-attributes task.runId to the current turn's run (5007-5018) and, since this PR, providerThreadId and providerTurnId (5050-5057). It keeps task.parentNodeId from the launch (4988) and subagent.rootNodeId (5058; documented as "The launch run's root node", 2957-2959).
  • Each lifecycle change emits the subagent node with runId: task.runId, parentNodeId: task.parentNodeId, rootNodeId: subagent.rootNodeId, providerTurnId: subagent.providerTurnId (5166-5195), then subagent.updated with the task (5238-5242), then the card turn_item.updated carrying subagent.providerTurnId (5250-5280).
  • After rootTurnEnded || rootOutputEnded, the fence (RunExecutionService.ts 435-462) admits a same-run main-thread row only by identity: a node needs rootNodeId === input.rootNodeId (442); a subagent row needs ownedNodeIds.has(parentNodeId) (461); a card needs an owned provider turn (489-498). ownedNodeIds is the attempt root plus nodes whose rootNodeId is that root (451-453). rootOutputEnded follows the root provider turn and clears on a running goal continuation (431); rootTurnEnded latches at turn.terminal (535-538).
  • Recorded sequence in claude_background_wake_before_queued_prompt (transcript entries 306, 316, 317, 327): Run 5's resume reopens Agent A while its root is live, so every row is admitted, but A's node is not enrolled because its root is Run 1's. Run 5's root ends at 316. A's task_notification arrives at 327 during Run 6. Its rows carry run Run 5, parent and root Run 1's root, card turn Run 5's turn. Run 5 rejects the node (442) and the subagent row (461), and accepts the card (owned turn) and the child-thread root (child ownership, 443). Run 6 rejects all of them. Persisted: subagent activity active/completed/active, child root active/completed/active/completed (job 116144467928, shared.ts 1242).
  • Effect beyond the oracle: rejected events never reach trackChildLifecycle (filter 1300-1302, tap 1303-1385), so Run 5 keeps A in activeChildSubagents (1128-1136) and its ingestion never stops (1254-1257). A's row and node stay running: a stuck spinner and a subscription held for the session's life.
  • Only this fixture fails because only here does the resumed child finish after its resuming root ended. In claude_background_subagent_lifecycle and _no_echo it finishes first (entries 302 before 310, 241 before 249).

3.2 Why the adapter owns the repair

The fence asks one question of a same-run main-thread row: which attempt's tree it belongs to. A subagent row carries only parentNodeId for that (OrchestrationV2Subagent has no provider turn). After a resume the parent still names the launch attempt while runId and the card's provider turn name the resuming one. The adapter creates this contradiction by moving only part of the ownership. Clients already assume consistency: web session-logic.ts 603-618 and mobile threadActivity.ts 1743-1757 find an item's attempt through node.rootNodeId and parentNodeId and require attempt.runId === item.runId, so today a resumed card resolves to no attempt.

Precedent for the selected rule: Codex reassigns a reopening child to its current parent turn (CodexAdapterV2.ts 2575), moves runId and parentNodeId on reopen (2515-2520) and emits the child-completion subagent node with that turn's run, parent, root and provider turn (6054-6074). Claude's restart-recovery path registers a resumed unknown subagent under the resuming turn's root (4435, 4456).

Rejected alternatives:

Alternative Why it loses
Server: admit a subagent row whose own node id was enrolled while the root was live The launch attempt enrolled the same reused id at launch. When a same-run replacement resumes it, a launch attempt still subscribed (a retained Codex cohort, or any future adapter) admits the replacement's rows too: two writers.
Server: admit rows whose parent lies outside this attempt as reattributed In the same-run case the stale parent is the old attempt of this run, so the resuming attempt still drops the completion. Fixes only the cross-run CI shape.
Server: transfer tracking (unlearn a node when a later node.updated names another root or turn) Depends on each adapter emitting the node before the row, adds state to the routing decider, and still needs truthful rows.
New provider-turn or attempt field on OrchestrationV2Subagent Contract change across server, web, mobile and desktop; no failure needs it once parentNodeId is truthful.
Relax the fence for subagent rows Reopens replacement root output; contradicts the accepted one-writer fence.
Update the oracle The child really completed; the oracle is a valid user-visible contract.

3.3 Callers and event sequences

Notation: Rk run, Ak attempt, Nk attempt root node, Pk root provider turn, T native task id.

Normal (the recorded CI shape, across runs).

  1. R1/A1 launches T: subagent(T, run R1, parent N1), node(T, root N1, turn P1). T completes in R1.
  2. R5/A5 resumes T (SendMessage, then task_started(T)). New rule: node(T, run R5, parent N5, root N5, turn P5, running), subagent(T, run R5, parent N5, running), card(T, run R5, turn P5). R5 admits while live and enrolls T (root N5).
  3. R5's root ends (provider_turn P5 completed, then turn.terminal P5).
  4. During R6, task_notification(T, completed): rows keep run R5, parent and root N5, turn P5. R5 admits the node (442), row (461) and card; the child-thread root by child ownership. R6 rejects all. R5 drops T from activeChildSubagents and its ingestion ends.

Persisted: subagent activity and child-root activity are both active/completed/active/completed.

Failure (the resumed child dies with its process). Steps 1-3 as above. The CLI exits before T reports. endSubagentsOfClosedQuery (5749-5803) sees T's run R5 differ from the active run R6 and calls updateClaudeSubagentNode({ status: "failed", onlyIf }) (5800). The rows carry R5, N5 and P5 and are admitted by R5 only. An explicit Stop yields interrupted the same way.

Awkward: same run, reused native id (falsifier N1 below).

  1. R1/A1 launches T; T completes while A1 is live.
  2. An agent or human cut-in restarts R1: A1 is interrupted and its query closed (T is already completed, so endSubagentsOfClosedQuery skips it); A1's ingestion stops at its non-completed terminal (1247). A2 starts. Because T is completed, A2 owns T's child thread (canRouteRelatedSubagent, RunExecutionService 190-192; ProviderTurnStartService 1043-1045, 1345-1347).
  3. A2 resumes T: rows run R1, parent N2, root N2, turn P2. A1, if still subscribed, rejects them (root, parent and turn are all A2's; A1's enrolled copy of T's id is never consulted for subagent rows). A2 admits and enrolls T.
  4. A2's root ends; T completes later: A2 alone stores node, row, card and child output. At 3326, A2 drops the node and the row (root and parent N1), and T stays running.

// Run, parent and root follow the task, which a resume moves to
// the resuming turn's attempt (see task construction).

RunExecutionService.ts and every other production file are unchanged for case 1.

3.5 Ownership and sequencing

  • The decision stays where the adapter already re-attributes a resume: one predicate, every ownership fact from the same turn context.
  • The registry acceptance (5086-5101) still applies a reopen only to the generation the lookup read (registered === existingSubagent for a terminal entry), so no new race: SDK-message updates run under sdkMessageLock, and query-exit cleanup passes onlyIf.
  • A resume is a lifecycle change (4929-4936), so the moved node is emitted at the resume, while the resuming root is live, and the attempt enrolls T then. Later progress-only updates emit the row and card with the moved identity.
  • The routing decider stays pure and unchanged; no state is added anywhere.

3.6 Ordinary-path equivalence and consumers

Unchanged: launches (no existing entry); every non-resume update (progress, notification, query-exit cleanup with onlyIf, workflow cleanup with onlyIfRunningInRun); a resume within the same tree (movesTree false: run and turn re-attribution exactly as today, parent and root kept, nested owner kept); the recovered-after-restart path (registered under the resuming root, so movesTree is false); workflow members (coordinator child thread, 4700-4735); the Codex, OpenCode, Grok, Cursor and ACP adapters; the fence.

Changed only for a resume into another attempt's tree: persisted subagent.parentNodeId and node parentNodeId and rootNodeId name the resuming attempt's root. Consumers checked: the fence (now admits the resuming attempt only); web and mobile attempt resolution (now resolves the resuming attempt); projection upserts overwrite parent and root (ProjectionStore 2168-2280); the windowed subagent read by run or parent cohort (3186-3192) still selects it; no web, mobile or client-runtime code reads subagent parentage for nesting; no internal or user doc describes this parenting, so no doc change. The owner link of a nested subagent is dropped only when it is resumed into another tree, where it could not stay routable anyway because its owner's node is not in the resuming attempt.

3.7 Tests

Expected values are literal facts, not values computed by the code under test.

  • R1 (existing, required). testkit/OrchestratorReplayFixtures.integration.test.ts, case claude_background_wake_before_queued_prompt/claudeAgent. Red on 3326 (CI literal). Green after, oracle unchanged.

  • N1 (new, required; real service, persisted events and a Deferred). In ClaudeAutomaticDelivery.integration.test.ts, built on the harness of "detaching a reused Claude query releases its completed run's native child subscription" (133-503): keep each opened query's message queue in an array, and resolve a Deferred per attempt from StreamClockHooks.endAttempt(threadId, runId, attemptId). Schedule: the awkward case of 3.3.

    1. User run R1 on query 1. Offer task_started(T, toolu-agent, is_backgrounded) and task_notification(T, completed) while A1 is live.
    2. Agent mode: "restart" cut-in as in that test; worker.drain(); wait for the replacement A2's provider turn running on query 2.
    3. On query 2: an assistant SendMessage tool use, task_started(T, tool_use_id = the SendMessage id, prompt), the recorded ack tool_result shape (ClaudeAdapterV2.test.ts 10895-10925), then result. Wait for run R1 completed.
    4. Send a new user message and wait for its provider turn running on query 2 (R2, as in CI).
    5. On query 2: a child assistant frame (parent_tool_use_id toolu-agent) and task_notification(T, completed, tool_use_id = the SendMessage id). Wait for the persisted turn-item.updated of T's card with status completed. It is persisted on 3326 too, through A2's owned turn, so the test fails rather than hangs.
    6. Assert: projection subagent T is completed, with runId R1 and parentNodeId equal to A2's root node (read from the projection's attempts); node T is completed with rootNodeId A2's root; stored events hold exactly one subagent.updated for T with completed and parent A2's root; T's child-thread root node ends completed; T's card providerTurnId is A2's provider turn. Only then await A2's ingestion-ended Deferred.

Predicted red on 3326 at the first assertion (running). Rejects 3326, moving only the row parent (node stays running) and moving only the node root (row dropped).

  • N2 (recommended, adapter). Extend "re-opens a resumed subagent whose task_started races past settle" (ClaudeAdapterV2.test.ts 10740) beside its run assertion (11007): the drained reopen's subagent.updated and subagent node.updated carry parent and root node-attempt-claude-wake-9c and the 9c provider turn, and the final completion keeps them. Predicted red on 3326 (node-attempt-claude-wake-9a).

  • N4 (recommended, routing contract). In RunExecutionService.test.ts, beside the one-writer test at 389-573: routing states for A1 (enrolled T from a launch node rooted at N1, then root ended) and A2 (admitted T's resume node rooted at N2, then root ended) of the same run. Feed T's completion node, row and card with the resumed identity (root and parent N2, turn P2): A2 accepts all three, A1 rejects all three. Passes on 3326 and after. It exists to reject a future server-side "admit an enrolled id" repair, under which A1 would accept.

4. Case 2: restart-note recovery proof

4.1 Fact and inference

Facts at @3326:

  • A runtime_exit entry with status: "success" makes the replay generator return (ClaudeAdapterV2.testkit.ts 592-605): the query's message stream completes while the server keeps running. Replay close is Effect.void (836), so only runtime_exit or fiber interruption ends a replayed stream.
  • The adapter's exit handler (8548-8572) then calls endSubagentsOfClosedQuery(context, context.stopping ? "interrupted" : "failed") (8563-8566). With the root settled, the active turn is null, so the else branch (5800) emits failed rows for the running child and Run 1's live subscriber stores them. CI and both reviewers observed failed at line 182.
  • Restart recovery cancels only active native subagents (provider-core subagentProjection.ts 353-400, check at 380).
  • The test's own comment calls this cut "the server dies".
  • Production server shutdown: for server_shutdown, releaseEntry runs closeSubscribers (atomically empties the subscriber map, then clears and ends each queue; ProviderSessionManager.ts 596-604) before closeScopeWithin(entry.scope) (956-967). The event pump returns early once a shutdown signal arrived (1897). Shutdown releases every live session before closing the session scopes (2006-2029). The existing control "ProviderSessionManagerV2 closes event subscriptions normally on server shutdown" (ProviderSessionManager.test.ts 1792-1830) pins the clear. Nothing the adapter emits during or after shutdown, including query-exit cleanup, reaches a run subscriber; a killed server persists nothing at all.

Inference: a real crash or restart leaves the child running, which is what the test requires. The test's native EOF models a CLI exiting under a live server, which is #305's case and not a restart.

4.2 Production query-exit paths

Path Who ends the query Status written Persisted Recovery note
Server shutdown (signal or layer close) ProviderSessionManager shutdown, then adapter closeSession (8968-8982; does not set stopping) whatever the adapter emits No: subscribers closed and cleared first; events after a signal dropped Yes, once
Server crash nobody none No Yes, once
CLI exits on its own (native EOF) the CLI failed Yes No (truthful: no restart)
Explicit Stop or cut-in interruptTurn sets stopping (8862), interrupts, closes, detaches interrupted; the active run's children through its terminal cascade Yes No
Fork or rollback close closeLiveQueryForNativeThread (8984-9012, sets stopping) interrupted Yes No
Close failure close propagates (8876); no detach none; the child keeps running n/a n/a (R1)
Buffered native outcome skipped (5769-5777); the drain applies the real outcome the real outcome Yes No
Replaced, resumed or sibling query skipped by native thread, earlier-process set and onlyIf (5759-5767, 5800) owner unchanged n/a n/a

Every row matches accepted R1, R3 and #305 and the recovery contract. No production change is needed.

4.3 Decision and rejected alternatives

Change the seam, keep the contract: phase 1 must end with the server stopping while the CLI has not reported, then phase 2 recovers from the shared database and phase 3 resumes the native session.

Alternative Why it loses
Stop failing children on native EOF Reverts #305/R3, leaves unreportable work running and later claims a restart that never happened.
Expect failed and drop the note assertions Blesses missing recovery; forbidden.
Make the replay harness treat runtime_exit as server death Recorded transcripts use it for real CLI exits (claude_subagent_resume_after_restart, entries 25 and 60); they would become unfaithful.
Make closeSession set stopping No persisted effect, because subscribers are already closed; nobody asked for it.

4.4 Sequences

Normal (server cut; the corrected fixture). Root result; the server stops (scope close: subscribers closed, then the adapter scope, then the replay iterator aborted at its pending query.open:resume wait). The database holds run completed and child running. Restart recovery: child cancelled, restartCancelledBackgroundWork[0].kind is subagent. The next user turn's provider prompt is the note plus the user text, the following prompt is plain, stored user texts are unchanged, and no continuation run starts under either setting.

Failure (CLI exit under a live server; today's fixture and the existing EOF control). Root result; native EOF; child failed is persisted; recovery leaves it; no note.

Awkward (Ctrl+C reaches the CLI and the server together). The CLI's EOF is processed, but the pump drops events after the signal; the database keeps running; recovery notes it once.

4.5 Change

apps/server/src/orchestration-v2/testkit/OrchestratorReplayRestartBackgroundNote.integration.test.ts @3326, lines 120-124:

    entries: [
      ...recorded.entries.slice(0, rootResult + 1),
// The server stops here with the subagent still running. Its CLI never
// reports to this server again: a runtime_exit would be the CLI's own
// exit, which fails the subagent instead of leaving it to recovery.
      {
        type: "expect_outbound",
        label: "query.open:resume",

This deletes { type: "runtime_exit", status: "success" } and changes no assertion.

Why it reproduces the base's phase-1 state: the cut keeps entries 0-106. The child has started but opened no tool call (child frames begin at 107), and Claude's roster excludes local_agent tasks (parseClaudeBackgroundTaskEntry, 2007-2013), so the base's EOF had no other persisted effect to replicate. Phase 1 ends at await_thread_idle, which waits only for no active run (OrchestratorScenario.ts 330-362). The blocked iterator does not hang teardown: Effect 4.0.4 tryPromise without a signal yields immediately on interruption, and the channel finalizer's iter.return() aborts the replay wait, which resolves on abort (testkit 398-418). Phase 3's query.open then matches the cursor, now at query.open:resume.

Fork inventory: this upstream test becomes a carried edit. Add its path to the # Urgent interruption (#285) block of scripts/fork-upstream-edits.txt and to upstreamFiles of urgent-steering-interruption in docs/fork-features.md; scripts/fork-check.sh (61-80) fails otherwise.

4.6 Tests

  • R2 (existing, required). Both cases of the restart-note test, all assertions unchanged. With the corrected seam, predicted green on 3326 production (showing that no production change is needed) and on the candidate. Rejects shutdown-time ends reaching the database and a recovery that loses or repeats the note.
  • C1 (existing control, required). ClaudeAdapterV2.test.ts 8584-8770, "ends an earlier run's native agent with its own identity when the same query detaches on %s"; the EOF row expects failed. Keeps Claude closed queries can retain running subagents in the session pending-work probe #305.
  • N3 (optional, integration control). In the restart-note file, phases 1-2 only with the original EOF cut: the phase-1 child is failed, still failed after recovery, and restartCancelledBackgroundWork is absent. Rejects reverting Claude closed queries can retain running subagents in the session pending-work probe #305 and noting work that already ended.

Run with TMPDIR unset, as CI does, now that the Codex fixtures no longer read it. Add a scoped server typecheck, lint of the changed files and scripts/fork-check.sh. Use complete files, not -t filters: the replay file covers all four Claude reopen fixtures and the nested ones, and the recovery file covers the restart-recovered resume (same tree, unchanged). CI owns the full gate.

Each red must have the stated cause: R1 the activity mismatch; N1 running instead of completed, not setup or a timeout; N2 the parent and root ids.

7. Prior findings and responses carried

  • Sol B1 / Grok B2 are answered by 3.4. Sol asked to keep explicit resumed identities rather than reopen root output broadly: the fence is untouched, and the adapter makes the resumed identity explicit on the rows. Grok's "rootOutputEnded never clears" was already corrected by the parent (it clears on a running root update, 431); the CI case is fenced by the rootTurnEnded latch anyway.
  • Sol B3 / Grok B1 are answered by section 4. Grok's position that a clean exit after a completed root should leave the child running is met for real server stops by the session manager's existing shutdown order (4.1, 4.2), without the adapter; an EOF under a live server is a real CLI exit and stays failed.
  • Sol B2 / Grok B3 (Codex TMPDIR) are parent-owned, excluded here and already fixed locally.
  • R1 held native result and close failure, R3 exact closed-query owners and the Claude closed queries can retain running subagents in the session pending-work probe #305 probe, R4 Stop scope, R5 F2, R6 L1/L3/L5, R7 cohort, reader, goals and fence, L12 capture and the L10 correction are untouched by both repairs.

8. Limits and an adjacent observation

  • A nested subagent resumed into another tree hangs off the resuming root, dropping its owner link for that resume. Unrecorded; it stays routable and truthful about the resuming attempt.
  • The replay close is a no-op, so R2 does not execute production's close-ends-the-stream ordering. That ordering rests on source (956-967, 1897) and the existing ProviderSessionManager control.
  • No native CLI, Electron, mobile or linked-server proof; the parent owns foreground checks.
  • Adjacent, not one of the two CI assertions (parent to triage or file). A subagent that ended without completing (interrupted by a Claude cut-in close, failed by native EOF, cancelled by restart recovery) and is later resumed by SendMessage gets its main-thread rows routed (with 3.4), but no live subscriber owns its child thread: new attempts adopt child threads only of completed subagents (RunExecutionService 190-192; ProviderTurnStartService 1043-1045, 1345-1347), and a resume does not re-emit app_thread.created (5135-5164). Its resumed prompt, assistant messages, tool items and result message would be dropped. Before this PR a native-EOF child stayed running, which kept its launch subscriber alive and accidentally owning the child thread; R3/Claude closed queries can retain running subagents in the session pending-work probe #305 removed that. Reachability needs Claude Code to resume a killed or stopped agent: claude_subagent_resume_after_restart proves it resumes an agent from an earlier process, but only one that had completed. Smallest proof: a recorded native transcript in which SendMessage targets a TaskStop-stopped or process-killed agent and the CLI emits task_started for it. Not fixed here: no failing test needs it, and adopting such a child thread needs its own one-writer design.

9. Elon record

Requirements and who asked: User #285 (urgent interruption with routine, background and recovery behavior preserved; exact-head dual review and green CI before merge) and the parent's R8 adjudication of Sol B1/B3 and Grok B1/B2: a resumed child's factual completion with one root writer, and an honest server-cut recovery proof that keeps #305.
Deleted: Fence relaxation and enrolled-id admission, transfer tracking, a new subagent field or registry, oracle and status-expectation edits, treating native EOF as a crash, a production change for the restart note, and new harness semantics for runtime_exit.
Bottleneck: Claude's resume moved run and turn but left the tree on the launch attempt, which the fork's tree-identity fence cannot see past; and the restart-note fixture used a native exit as its crash.
Checked myself: Both CI logs and both reviews; recorded transcripts of every reopen fixture and the cut point; Claude resume, exit and close code; fence routing and ingestion stop; session-manager shutdown order and event dropping; recovery's active-only cancel; Codex reopen parity; client attempt resolution; projection consumers; upstream main; Effect 4.0.4 interruption; and the fork inventory check. No test was executed.

What: Fresh isolated native checks confirm human app Steer stops Claude and Grok foreground tools, and actual agent MCP mode: restart stops Claude before delivering the urgent message.
Why: Earlier native captures preceded later adapter and ownership fixes, so those captures alone did not prove the repaired path.
So what: These checks pass at local checkpoint d10e0b97925ab2cb4cfba53ba51793b899017057; the candidate still needs integration with newly merged #335, fresh exact-head dual review and green CI before delegated merge.

All three commands waited for a filesystem release event that was never sent. Each process reported its captured PID and ready marker; its PID was gone before the replacement reply, with no release file or completion marker. Both Claude processes recorded SIGTERM. Persisted attempts were [superseded, completed] and provider turns [interrupted, completed] in every case. The probes used a seeded ordinary copy under this task's .t3, the shared public web command and injected dev-server MCP tools, never live userdata or the planner thread.

  • Human Claude: requester {createdBy: user, creationSource: web}. Actual reply: “The user interrupted me, through T3 Code. The stopped Bash tool was interrupted, not refused.” Claude also identified its SDK result's declined wording as misleading.
  • Agent Claude: Grok's actual t3_thread_send used restart; persisted requester was {createdBy: agent, creationSource: mcp, senderThreadId: c7c8cd68-edb7-43be-9b72-ee15ddbf1c32}. Claude named that Grok sender and understood interruption rather than refusal.
  • Human Grok: user/web requester, old foreground process stopped. Grok acknowledged human interruption, but also paraphrased its native cancellation as “was not executed”; the fixture's ready event proves it had started. Native tool wording and a model's paraphrase are not a guarantee about prior partial effects.

Exact local evidence: claude-human-native-r8/result.json, claude-agent-native-r8/result.json, grok-human-native-r8/result.json under /home/luke/scratch/chromeria-285/. Native probe process ownership came from spawn records, not name matching. Captured dev session26149 stopped through its terminal after all probes; both owned ports have no listener. Main source and live Chromeria were untouched.

The child-authored N1 regression was independently inspected and integrated without production changes. It fails literally on unchanged production at988c478835a61ac2895c43345cd0d2067787bd1e: after persisted card completion, subagent status remains running instead of completed. The other17 cases pass. With the accepted R8 owner repair, the complete18-case file passes on clean local2231aa07c8f8f5d9d0e397f347ca8a06c72306c0, including persisted exactly-once subagent/node/card/child-output checks and ingestion-ended Deferred, with TMPDIR unset. Source file digest and red log hashes were checked against the child handoff. No new sleep or polling wait.

Main advanced to f23fad2d94a8832c744fc679948fefb1d5f3b11b (#335). The parent read all20 overlapping deltas and is integrating bare-launch behavior. A bounded Opus design addendum will settle the interaction between bare context stripping and urgent delivery notes before the final candidate is published. This is not a new UI taste decision and no merge approval is requested.

What R9 integrates bare threads with urgent interruption without adding T3 text to their prompts.
Why The newly merged bare launch contract requires raw sender text, and the slash guard and prompt must agree about whether a note is added.
So what I accept Opus's design for implementation and both independent reviewers will roast it on the final candidate; required integration tests and a native bare Grok Stop/reload/Steer probe remain before readiness.

Parent adjudication: normal threads keep the truthful requester note; bare threads keep Stop-scope interruption and durable requester provenance but receive only the sender's text. I will centralize that rule in steeringRestartNote and pass bare from both consumers, and qualify all three agent-facing promises. No new delivery mode or receipt entry is needed. The native Grok reload gate remains an explicit blocking unknown until N-b runs. The separately source-traced bare missed-history recovery issue will be filed with its verification limits and linked here.

The local clean integration is 88f4d4856101398f1cf49806837128e972c6c1dc on main 2ba387388074ac4ee09cc256f9800cdd9ce2b520. The main checkout remains untouched. Published head 3326f71ea675237e477118e785428136dc470975 and both R7 review failures remain rejected; this design is not a code approval. New main integration controls passed 119 tests in 5 complete files and the server typecheck. Earlier affected proof passed 1,062 tests in 17 complete files, plus 35 bare/MCP tests. These are focused feedback, not the complete CI gate.

Parallel coverage is bounded to one test-only writer in its own worktree while I own the production helper and its callers, public text and isolated native probe; no shared server or database is delegated. The discriminating tests must reject a note in a bare prompt, an incorrect slash refusal, repeated additions or a lost requester.

Full Opus design follows, including the four-field Elon record.

#285 / PR #300 R9: urgent interruption on #289 bare threads

Designer: Claude Opus 5.5 (claude-opus-5-5), Claude Code, max effort. Design only, read-only, 2026-10-15. This is not a code-review verdict, status or delivery claim.

Pins. Candidate stack 2231aa07c8f8f5d9d0e397f347ca8a06c72306c0 (four commits over 9f46c833b1: b3f01b86f5, 3a97d5df0e, d10e0b9792, 2231aa07c8). New base f23fad2d94a8832c744fc679948fefb1d5f3b11b (PR #335 squash over 9f46). Source was read with git show <sha>:<path>; lines are cited @2231 or @f23. My worktree stays at review pin 3326f71ea6, unaltered. The parent's paused rebase in urgent-interrupt-delivery was read only through git --no-optional-locks status and file text: one unmerged path, ProviderTurnStartService.ts, one hunk (1115-1138); the Claude test's ScheduledTask BareLaunch mock and both inventory sections are already staged there. The design below derives from 2231 and f23, not from that conflict text.

Read in full or traced. Canonical operations entry, Research (mechanics, recall), Elon (algorithm, current constraint, first principles), Software design (compare designs, change existing systems, state and boundaries) and Test design. review-r7-full-context.txt; R8 design, its accepted record and the R8 adjudication; published R7 Sol and Grok reviews. Issue #289 body and all 35 comments (user decisions, design 1, revisions 2 and 2.1, both roast rounds, parent probes, addenda, implementation handoffs) and PR #335 body. Grok's round-1 finding 1 already treated even a one-line generic T3 system sentence as Chromeria content to strip. All 20 r8-base335-overlap-*.diff files, and the #285 diffs of the same files. At @f23 and @2231: ProviderTurnStartService (bare boundary, missed-run retry, resume failure), SteeringInterruption, Orchestrator steering, slash guard, run interrupt and providerSessionIdFor, IdAllocator, RuntimePolicy and its override, ProviderSessionManager bare open, RestartContinuation, RunExecutionService bare receipt and turn input, BareLaunch.deliveryText, Claude bare options, prompt and skill dispatch, Codex turn params, OpenCode v1 prompt, ACP respawn and session load, Grok bare gate, historicalMessage, getTurnStartHistory, the fork inventory, and the Claude delivery and bare PTSS test harnesses. Grok's bundled docs and the r8 native Grok result were checked for session-load evidence; neither settles it.

Authority kept. No source, test, Git or GitHub write, status, memory, delegation, test run, server, browser or native CLI. GitHub reads used gh issue view and gh pr view. One early git fetch --dry-run printed nothing and updates no refs. Two temporary file lists in /tmp were deleted at once. This file is the only retained output.

1. Verdict: READY

A bare thread keeps every #285 interruption mechanic: Stop scope, retained work, failure semantics and the durable requester. T3 adds no steering note to its prompt. steeringRestartNote becomes the single owner of that decision, so the prompt and the slash guard cannot disagree.

2. Requirements

Requirement Source
No Chromeria text in a bare thread's model input except named additions or an explicitly receipted harness limit #289 acceptance and user decisions; design 2.1 rules 7-8 ("no restartNote prepend")
Supported bare providers keep working; a breakage blocks #289 round-2 bar
Urgent restart and human Steer interrupt with the adapter's Stop scope where it permits, with a truthful, durable requester; routine Claude agent updates queue #285, #282
MCP mode descriptions and injected orchestration instructions stay truthful #285 original task; AGENTS.md documentation rule
#279 descendant-to-ancestor steer and restart stay refused #279, R0
Smallest integration, no bare redesign or new machinery, no speculative stopped-child resume parent R9 brief

3. Decision: bare threads get no steering note

Chosen: omit the note and keep the interruption.

  1. The note is Chromeria-authored model text. SteeringInterruption.ts 78 @2231 is Restore explicit urgent interruption after Claude steer queuing #285's only producer of model text (git grep "Note from T3" finds nothing else in production code). It is neither a named addition nor a harness limit, so delivering it to a bare thread breaks the explicit bare criterion.
  2. Bare launch: run an agent with only the context you name #289 already decided the analogous case. Bare threads skip restartCancelledBackgroundWorkNote in PTSS (1122 @f23) and get only CONTINUE_PROMPT from RestartContinuation (337 @f23).
  3. The note names T3 concepts (T3 Code, T3 thread ids) a bare agent has no context for. To a bare child, its caller is its user.
  4. Automatic delivery must not abort Claude tools as a user refusal #280's original failure does not return. Routine Claude agent auto/steer still queue on bare threads. Refusal-styled native tool results appear only after an intentional Stop-scope interruption, as they already do after a bare Stop followed by a new message, which Bare launch: run an agent with only the context you name #289 accepted without a receipt entry.
  5. The requester stays truthful where Chromeria records it: attempt.requestedBy and the message's createdBy, creationSource and senderThreadId. Chromeria states nothing false. A sender who wants a bare agent to know who interrupted can say so in its message, the only route bare permits.

"Where adapter permits": bare is a per-thread adapter configuration that does not permit T3 text. The interruption itself is permitted and still happens.

Alternative Why it loses
Deliver the note on bare Breaks the settled #289 criterion and precedent; adds T3 text to Spectrum's bare-versus-normal comparisons
Another typed delivery route None exists. Claude SDK user messages, Codex turn/start input, ACP prompt blocks and OpenCode parts carry no sender field the model sees without text; the receipt is never model input. A new route is new machinery
Refuse urgent restart or human Steer on bare threads Breaks #285's cut-in on supported bare harnesses
Add interruption wording to bare receipts It is runtime behavior shared with Stop, not launch content; #289 did not receipt it for Stop; per-harness wording would be guessed
Check bare only in PTSS and leave the guard (B1) The guard's condition is steeringRestartNote(...) !== "". It would refuse a bare slash restart with a false reason ("needs a note"): two decisions about one fact

4. Evidence map

4.1 Final adapter-facing boundary

  • @2231 PTSS 1046-1059 builds userText = note + "\n\n" + projected text when attempt.reason === "steering_restart". Every later consumer reads userText: startRootRun({ message: { text: userText } }) (1370) and startWithHandoffs (1236, 1286).
  • @f23 PTSS 1091 makes bare userText the raw message.text. For bare, 1369-1430 always replaces deliverySession. The wrapper sends [additions unless survivingAccepted, missed user text, userText] to startTurn/compactThread, ignores turnInput.message.text, and refuses non-user missed history (1406-1408).
  • So the conflicted hunk is the only place the note can enter a bare prompt. Resolving it as E1 decides both delivery paths.
  • Adapters add nothing for bare: Claude sends text without the effort prefix and with an empty skill set (8676 @f23), so content is a plain string; Codex omits additionalContext and developer text (772 @f23); ACP omits T3 and runtime instructions (6927, 6978 @f23); OpenCode v1 omits system (3554 @f23); Cursor omits its suffix.
  • A cut-in replacement stays on the same native thread. The interrupted attempt has a provider turn and the same nativeThreadId, so survivingAccepted is true and named additions are not resent. T1 guards this.

4.2 Slash guard

@2231 Orchestrator 4238-4253 refuses an interrupt_restart whose text starts with / when steeringRestartNote(...) is non-empty, because a prefixed note would break the command. With E2 and E3, a bare target has no note, so its slash restart is delivered as written, as a human Steer on non-Claude normal threads already is. On bare Claude, slash commands are disabled, so it is plain text; bare Grok keeps only its seven native commands.

4.3 Each supported bare harness on the #285 cut-in path

Harness #285 cut-in on a bare thread Bare configuration on the replacement Status
Claude interrupt (10 s bound), close, await detach; new query with resume makeClaudeQueryOptions takes turnInput.runtimePolicy.bare (8403-8405, 1081 @f23) Traced
Codex interrupt turn and retained terminals on its per-thread app-server; goal pause needs an active goal, and bare disables goals bare turn/start params Traced
OpenCode v1 abort session tree on its own pure server prompt without system; bare agent and tool config fixed at open Traced
Grok kill process group; respawn through bare makeRuntime (rediscovery, gate re-armed, 542 @f23); session/load carries bare _meta (AcpSessionRuntime 2322-2328, 2571-2577 @f23) Prompt waits up to 30 s for available_commands_update (488-494 @f23) Unknown: whether Grok emits it after session/load. N-b decides
Cursor cancel run; resume with positive tools no suffix Traced
OpenCode 2, generic ACP, Antigravity, Pi, Muse cannot host bare threads; open refuses n/a n/a

Before #285, human Steer on Grok was a soft restart, so the bare gate was never re-armed by Steer. #285 routes bare Grok human Steer and agent restart through the same respawn that bare Stop already uses.

4.4 Provider session allocation

  • Restore explicit urgent interruption after Claude steer queuing #285 adds no allocation site. Its providerSessionIdFor calls (1733, 4390, 5575, 5964 @2231) are base 9f46's set. f23 passes bare at all four and refuses a bare instance switch or create_with_handoff inside the steering path before allocating (4364-4371 @f23).
  • restart_session uses idAllocator.allocate.providerSession (4477 @2231), which is unique per call and never the shared derived id.
  • L12 replacement ids come only from the captured sessionTransition. restart_and_resume needs an instance change, refused for bare.
  • On open, ProviderSessionManager rechecks bareSupport and refuses mixing threads in one bare process.

No guard is needed in #285's cut-in helper.

4.5 Retry, resume and continuation

  • Cut-in effect retries carry no text. F2 settleUnstartedRun writes an error turn item, never model input.
  • Resume: Claude rebuilds bare options. PTSS fails a bare resume with accepted history instead of substituting a summary (775-803 @f23).
  • fix(server): continue restart cuts and expose guarded queue resume #301 continuation is unchanged for bare (no restart note; CONTINUE_PROMPT).
  • Missed-run retry has a pre-existing defect, covered in section 9.

4.6 Shell cancellation scope

Every supported bare recipe disables native subagents: Claude's six tools exclude Agent, Codex turns multi_agent off, Grok disables subagents, OpenCode removes task, and Cursor's positive tool list has none. On bare threads, #285's child and cohort ownership reduces to foreground and background shell work. R8's stopped-child resume limit is unreachable there; nothing is added for it. Bare processes are per thread, so a Stop scope cannot reach another thread's process.

4.7 #279

f23 does not touch ThreadManagementService; #285 only adds the promote provenance case. Descendant-to-ancestor steer and restart stay refused, including from a bare child that names t3-code.

4.8 The other overlaps

All merged cleanly and are disjoint in behavior:

  • MCP thread handlers and tools: bare configuration output versus promote provenance.
  • Codex, Claude and OpenCode adapter tests.
  • RunExecutionService: the bare receipt and appContext versus routing state. The receipt is a direct write, not routed through Restore explicit urgent interruption after Claude steer queuing #285's fence. A replacement has nativeThreadHasTurns and writes no second receipt.
  • ws.ts, contracts, ProviderAdapter, ACP, Grok and OpenCode adapters: covered in 4.3.
  • docs/map.md: no change.
  • Inventory and the Claude test layer: already resolved. Later commits d10e and 2231 may re-conflict only in inventory or test text; keep both sides.
  • All Restore explicit urgent interruption after Claude steer queuing #285 integration tests use ProviderReplayHarness, which provides the real BareLaunch layer at f23.

5. Callers

Normal (unchanged). An agent calls t3_thread_send({ mode: "restart" }) into a running normal Claude thread. The steering restart creates attempt A2 with requestedBy agent/mcp/S. provider-turn.restart runs with Stop scope. PTSS prompt: "Note from T3 Code: the agent in T3 thread S interrupted your previous turn… \n\nStop sending the email. Change the recipient first."

Bare. The same send into a running bare Claude child takes the same Orchestrator, effect and Stop-scope path and persists the same requestedBy. In PTSS the note is "" and survivingAccepted is true, so the CLI receives exactly "Stop sending the email. Change the recipient first." Human web or mobile Steer on that thread behaves the same, with requestedBy user/web or user/mobile.

Failed restart (bare). Codex rejects the interrupt after retries. F2 marks the run failed with "Message not delivered" and sends no prompt; a later Stop ends retained work. This is truthful, but the thread's next message then hits the section 9 defect. Separately, if a respawned bare Grok runtime never reports commands, the replacement fails before inference after 30 s with the gate reason (N-b).

Awkward. An agent restart with "/review" to a bare thread is accepted (there is no note to protect) and delivered as "/review". The normal-thread case stays refused.

6. Exact edits

              requester: attempt.requestedBy ?? message,
              author: message,
              driver: session.driver,
              bare: projection.thread.bare,
            })
          : "";
      const userText =
        interruptionNote === "" ? messageText : `${interruptionNote}\n\n${messageText}`;

Everything else in both versions stays as merged: f23's bare deliverySession replacement, restartNote omission and ContextMD exclusions, and #285's settleUnstartedRun and makeStartingRunSettlement.

E2. apps/server/src/orchestration-v2/SteeringInterruption.ts (63-79 @2231). Add ThreadBare to the type import. The optional field follows f23's providerSessionIdFor idiom, so existing unit calls stay valid:

export function steeringRestartNote(input: {
  readonly requester: SteeringRequester;
  readonly author: SteeringRequester;
  readonly driver: ProviderDriverKind;
/** A bare thread (#289) gets the message alone; its attempt keeps the requester. */
  readonly bare?: ThreadBare | undefined;
}): string {
  if (input.bare !== undefined) return "";
  // existing body unchanged

E3. apps/server/src/orchestration-v2/Orchestrator.ts: slash guard (4241-4245 @2231):

        steeringRestartNote({
          requester: input.requestedBy,
          author: input,
          driver: session.providerSession.driver,
          bare: input.projection.thread.bare,
        }) !== ""

E4. Agent-facing text. Wording is the parent's to tighten; the facts are fixed.

  • apps/server/src/mcp/toolkits/orchestrator/tools.ts, t3_thread_send:
    • Append to "…explaining that stopped calls were interrupted rather than refused" the clause "; a bare thread gets your message alone".
    • Change "rejects messages starting with '/'" to "rejects messages starting with '/' unless the target is bare".
  • packages/provider-core/src/server/orchestrationInstructions.ts (line 13):
    • "…then delivers your message with the real requester and reason" becomes "… reason (a bare target gets your message alone)".
    • "Restart rejects native slash commands; send…" becomes "Except on a bare target, restart rejects native slash commands; send…".
  • docs/orchestration-v2/orchestrator-mcp-server.md: after "Native slash commands must be queued instead, or expressed as an urgent instruction in plain text." add "On a bare thread the prompt is the message alone: no note, and a leading / is allowed."

No tests pin these strings. User docs promise no note, so docs/user stays unchanged.

7. Tests

T1 (required): real Orchestrator, effect worker, PTSS, BareLaunch and Claude adapter, with the native SDK faked; persisted events and the CLI-facing offers. Extend ClaudeAutomaticDelivery.integration.test.ts, case "delivers %s with the requested tool interruption behavior" (list ends 1267 @2231), with "bare agent restart", "bare human web steer" and "bare agent slash restart":

  • const bare = delivery.startsWith("bare "). kind is delivery without that prefix; use kind in every existing branch.
  • urgent also holds for bare && kind === "agent slash restart".
  • Named addition with an independent oracle. Write a body containing BARE-ADDITION-SENTINEL in a scoped temp directory outside the checkpoint workspace. Take its realPath. Compute sha256 (hex) and byteCount independently with Node crypto or the test's Effect Crypto. Create the target thread with bare: { additions: [{ type: "file", path, sha256, byteCount }] }; the sender thread stays normal.
  • Bare agent slash restart: send succeeds with delivery: "restarted", then the urgent assertions run. The normal case keeps its refusal assertions as the control.
  • Bare urgent assertions, using persisted state plus offers:
    • Interruption mechanics as today: batchAbort aborted, queryCount === 2, first turn interrupted, first attempt superseded.
    • requestedBy.createdBy is agent or user, and senderThreadId is set for agent kinds.
    • Stored message text equals the literal.
    • offers[1].message.content strictly equals the literal ("Stop sending the email. Change the recipient first." or "/review").
    • offers[0].message.content contains the sentinel exactly once and ends with "Build and check all four tasks."
    • In the second-restart block, offers[2].message.content === "Use the corrected recipient.", instead of the one-note count kept for normal threads.
  • Rejects:
    • The note left on bare (the naive merge).
    • A guard that ignores bare: refuses /review.
    • A bare-aware guard with a note still added: "/review" gets prefixed.
    • Additions resent after the cut-in.
    • Refusing or soft-handling bare cut-ins: no abort and one query.
    • A lost requester.
  • Failing first: with the naive resolution (Restore explicit urgent interruption after Claude steer queuing #285's hunk for all threads), the three bare cases must fail on the note or the guard, not on setup. Then apply E1-E3. The sentinel assertions are Bare launch: run an agent with only the context you name #289-carrier controls, green both times.
  • No sleeps: the existing watch and Fiber.join on domain events plus worker.drain().

T2 (optional, unit). In SteeringInterruption.test.ts, for each combination that produces a note today (agent to Codex, person to Claude, person promoting an agent's message), setting bare yields "". It is cheap and pins the owner's contract for both callers; T1 already proves it at the boundary.

Not added. No Codex bare session-identity test: #285 allocates nothing, and f23 tests the guard. No pure routing mirror. No Grok gate test with a fake ACP agent: the unknown is native behavior.

Focused commands (TMPDIR unset as in CI; complete files):

npx -y pnpm@11.10.0 exec vp test run \
  apps/server/src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts \
  apps/server/src/orchestration-v2/SteeringInterruption.test.ts \
  apps/server/src/orchestration-v2/UrgentRestart.integration.test.ts \
  apps/server/src/orchestration-v2/SteeringCompletion.integration.test.ts \
  apps/server/src/orchestration-v2/ProviderTurnStartService.bare.test.ts \
  apps/server/src/orchestration-v2/ProviderTurnStartService.memory.test.ts \
  apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts \
  apps/server/src/orchestration-v2/RestartContinuation.test.ts \
  apps/server/src/orchestration-v2/RunExecutionService.test.ts

Also run:

  • The parent's R8 seven-file set.
  • Server and provider-core typechecks.
  • Lint and format of the changed files.
  • bash scripts/fork-check.sh and node scripts/fork-features.mjs check.

CI owns the full gate.

8. Native proof

N-b (required): bare Grok on the integrated candidate. Use an isolated seeded dev server, never live userdata.

N-a (optional). A bare Claude agent restart whose native session JSONL shows the replacement user message equal to the sender's text. T1 already proves the composition at the adapter boundary.

9. Adjacent pre-existing defect: file it, do not fix it here

Defect: a bare thread refuses every later turn after a run ends before its provider turn starts. (Parent files it; owner is bare recovery, #289 and #335.)

  • Trace @f23.
    • Missed runs are failed or interrupted runs on the provider thread whose active attempt has no provider turn (PTSS 1223-1230).
    • Their items come from getTurnStartHistory, which includes error and run_interrupt_result (ProjectionStore 4150). historicalMessage keeps both (handoffBudget 140-160).
    • The bare boundary refuses any non-user missed item (PTSS 1406-1408).
    • T3 writes one on these paths: Stop before start writes "Run interrupted before provider start" (Orchestrator 10097), and a session-open failure writes "Provider session failed to open" (PTSS 583-610).
    • The refused run is itself missed, so every following turn refuses too. Only a rollback to before that run, or a new thread, recovers.
  • Reachable without Restore explicit urgent interruption after Claude steer queuing #285: send to a bare thread and press Stop before its provider turn starts. Bare Grok discovery and bare Codex app-server spawn widen that window. Any pre-start failure also triggers it.
  • Restore explicit urgent interruption after Claude steer queuing #285 adds triggers:
    • Stop during a bare cut-in's replacement window: the run's active attempt is undelivered and the earlier attempt's output is in the run.
    • F2's "Message not delivered" error item.
  • Not fixed here. The fix decides Bare launch: run an agent with only the context you name #289 recovery semantics: which T3 records to skip, and what a missed run with an earlier delivered attempt resends. The brief excludes that redesign, and the dominant trigger exists without Restore explicit urgent interruption after Claude steer queuing #285.
  • Recommendation: list it as a Restore explicit urgent interruption after Claude steer queuing #285 limit with its Issue link and not as a Restore explicit urgent interruption after Claude steer queuing #285 blocker. The user or parent may still gate on it.
  • Sketch for the owner. In PTSS's bare branch only:
    • Skip T3 status records: error, run_interrupt_result, and the workspace-preparation command item.
    • Count only items under each missed run's current root (item.nodeId === run.rootNodeId), so an earlier delivered attempt is not resent.
    • Then resend missed user text verbatim and refuse only genuine undelivered non-user history.
  • Red tests on f23:
    • A missed run holding a user message plus a run_interrupt_result: the next start must deliver that text instead of failing.
    • A two-attempt missed run (A1 delivered with an assistant item; A2 with a steer message and an error): the next start delivers only the steer text.

10. Load-bearing assumptions and limits

  1. The steering note is Restore explicit urgent interruption after Claude steer queuing #285's only model-facing text. Falsifier: another producer appears in the integrated diff.
  2. Every supported bare adapter sends its cut-in replacement through its bare-aware start path (4.3). Falsifier: T1 for Claude; source for the rest.
  3. Restore explicit urgent interruption after Claude steer queuing #285 allocates no provider session, and bare cannot reach restart_and_resume or handoff (4.4).
  4. The replacement stays on the same native thread, so additions are not resent. Falsifier: T1's sentinel assertions. If a harness silently swaps native sessions, Bare launch: run an agent with only the context you name #289's carrier rule applies as it does today.
  5. Native interruption wording after a bare cut-in matches a bare Stop's. This is a limit, not a receipt entry.
  6. Unknown: Grok emits available_commands_update after session/load. N-b resolves it.

Proposed #285 limit. "Bare threads (#289) get no T3 requester note on Steer or restart. The agent receives exactly the sender's message, and the harness's own interruption wording (on Claude it can read as a user refusal) stays as after a bare Stop. The requester stays on the run attempt and in the message provenance." Existing limits L2-L12 are unchanged.

11. Human decisions

None open. #289's acceptance and design settle bare threads; #285 settled cut-in behavior; this design meets both. Delivering T3 interruption notes into bare threads would amend #289's settled no-Chromeria-context decision. Only the user can make that call, and it would be a follow-up (for example, a named addition), not part of this integration. No permission gate is introduced.

12. Prior findings carried

13. Elon record

Requirements and who asked:

Deleted:

Bottleneck: The code is one conflicted hunk plus a shared decision. The remaining wait is native evidence on whether bare Grok's gate survives a respawn (N-b), then the design roasts and exact-head reviews.

Checked myself: Both stacks' source at the cited lines.

  • Prompt composition: the PTSS conflict and the snapshot's single unmerged hunk; every userText consumer; the bare wrapper.
  • Guards and allocation: the slash guard and steeringRestartNote callers; all providerSessionIdFor and allocator sites in base, candidate and f23; the runtime-policy override.
  • Each supported harness's cut-in and bare start path: Claude options, prompt and skill dispatch; Codex params; OpenCode system; ACP respawn, session/load _meta and the Grok gate.
  • Recovery: resume failure, continuation, missed-run, history and before-start interrupt items.
  • Existing test harnesses, the fork inventory, Grok's bundled docs and the r8 native Grok result.

No test was executed.

What Bare Grok passed both Stop-then-send and human web Steer on the integrated candidate.
Why Hard interruption respawns the ACP process, so bare Grok must pass its six-tool/seven-command metadata gate again after loading the native session.
So what N-b is resolved without a Grok gate change; bare Claude integration tests and the independent design roasts remain before final review and CI.

Native proof used clean local 70762051a3a27fcf88597e8bd6c6773046af4644 on main e2aff187811ad0e0691bb927ddc3a07a0d973018. Main #342 changed only stale-notice monitoring and the feature map; I read its complete three-file diff and preserved both map references. No production or main-checkout writes. The task's existing seeded copy, worktree .t3/userdata/chromeria-v2.sqlite, was used by its own dev server; it never ran against live userdata.

  • Control: On one new bare Grok 4.7 thread, a real foreground shell ran a Node fixture that waits on a file watcher. The app's actual shared interruptTurn command stopped it. Captured PID 1066538 was gone with no release or completion marker, and run 1 persisted interrupted. A subsequent ordinary app send of printf BARE_AFTER_STOP_OK reused the same native session, executed with exit 0, replied BARE_AFTER_STOP_OK, and persisted run 2 completed.
  • Human Steer: The same bare thread then ran a second foreground fixture, captured PID 1078920. The app's shared startTurn command with dispatchMode: steer sent exactly printf BARE_STEER_OK. At 00:53:41.323 UTC the held PID was already gone, no release or completion marker existed, the replacement run was still running and no assistant marker had been produced. It later executed that exact command with exit 0 and replied BARE_STEER_OK. Run 3 completed, its first attempt superseded, its replacement completed, and the old/new provider turns persisted interrupted/completed. The replacement's requestedBy is user/web.
  • Native reload and text: Native history retains the same session ID across both respawns. Its two relevant user-query bodies are exactly printf BARE_AFTER_STOP_OK and printf BARE_STEER_OK, inside Grok's native <user_query> wrapper. There is no Note from T3 Code in the native history. Its tool results independently record both held shells killed by signal 9.
  • Gate evidence: Initial native receipt observed the six tools (run_terminal_command, read_file, search_replace, list_dir, grep, write) and seven commands (compact, always-approve, context, plugins, reload-plugins, session-info, feedback). The real adapter recreates its Deferred gate in every makeRuntime; only an available_commands_update with exactly those inventories lets beforePrompt succeed. Both post-respawn commands reaching native inference and exit 0 therefore prove that re-armed gate passed. Raw post-load ACP frames were not captured; this last statement is a source-bound inference, not a claim of an additional raw-frame recording. No metadata-gate errors or persisted error items occurred.

The fixture has no sleep, deadline-based completion or side effect outside its own scratch files. Native readiness was observed through its file event, not polling. Local evidence: r9-native-bare-manifest.json, r9-bare-grok-proof.json, both fixture directories and grok-bare-steer-r9/immediate-after-steer.json. Native history SHA-256: a8bce36bc8c9166c4c522fde1db897fe77d1d7a243ecd1cc475a9c1e183bed35. Only selected fixture evidence is reported here; native host rules and private state are not published.

The owned dev session 53311 is stopped, both task ports are free and both captured native PIDs are gone. No deployment or live restart. The adjacent pre-existing bare missed-history issue is filed separately as #343, explicitly source-traced rather than natively reproduced. Bare targets deliberately receive no T3 requester note under #289; requester provenance remains durable.

What Final candidate b107228d97ef3331ba553c5c6ed0c766628d0b50 passes 1,171 focused tests in 23 complete files.
Why The new base and the R8/R9 lifecycle, fixture and bare-context corrections needed integrated proof rather than the rejected R7 results.
So what Fresh independent Sol and Grok reviews will critically review the designs and code on this exact head; complete CI and both review successes remain required before delegated merge.

Base: e2aff187811ad0e0691bb927ddc3a07a0d973018. Branch: fix/urgent-interrupt-delivery. Parent worktree: /home/luke/dev/toolboxmd/chromeria-worktrees/urgent-interrupt-delivery, clean. The final normal commit has exactly the integrated WIP tree; its production bytes are unchanged from the successful native bare Grok probe, with only the accepted test-only extension added after that probe. No version bump. Main checkout remains untouched.

Executed proof on this head

With TMPDIR unset, all 16 changed test files and seven turn-start/recovery/MCP consumers passed, 1,171/1,171 in 155.36 seconds. This is affected feedback; complete CI owns the merge gate.

Regression and native evidence

The stopped test-only child 19430c62db73c58664562f4f0ed131bb0ab056bd added exactly three bare Claude cases. I read its whole patch and failing/passing records, independently verified the committed test SHA-256 54878e96c3b97a4d7287cc18a1ca3e46c365c1ba535e708091855a1ebd5d6a99, and verified all temporary production edits were restored exactly. Under the naive note merge, agent/human bare cut-ins fail literal exact-prompt assertions and bare slash restart fails the incorrect slash guard; no setup or timeout failure is counted. With E1-E3, all 21 complete-file cases pass, then the final integrated 23-file run above passes. The original 13 parameterized controls and the R8 same-run child regression remain.

Normal native cut-ins, bare Grok Stop/reload/Steer, R8 design, R9 design and uploaded UI images/video carry the detailed evidence and limits. The bare requester-note omission is deliberate under #289. The separate pre-existing bare missed-history issue is #343, source-traced with no native reproduction claim.

Both R7 reviewers and CI correctly rejected 3326f71ea675237e477118e785428136dc470975. Their full findings, parent adjudications and the original/prior brief are included in both new review packets. No prior verdict transfers. The complete gate, merge and final resource cleanup are still pending. No deployment, live restart or agent memory.

What Complete CI passed on b107228d97ef3331ba553c5c6ed0c766628d0b50.
Why R7 failed on reachable consumers that the old affected selection missed, so focused green alone could not admit the correction.
So what Both fresh exact-head independent design/code reviews are active; merge remains gated on their comments and authenticated success statuses.

CI run 38663316464 is completed/success on this exact head. I read the full job metadata: Test, Test Web, Rust, Lint, all six Test Server shards, Release Smoke, Build, Typecheck, Mobile Native Changes, report transfer and aggregate Check succeeded; Mobile Native Static Analysis was correctly skipped. Fork Stack Model and the latest mobile fingerprint checks also succeeded. The earlier cancelled fingerprint run is superseded by two successful same-head runs; no CI failure is waived. Web/EAS previews are intentionally skipped on this fork, with no production deployment.

Fresh Sol task: node:delegated-task:command%3Amcp%3A91f43bb0-e8d6-4e5c-bd59-43523929f01c%3Adelegate-task%3Aurgent-285-code-review-sol-r9-b107228d97, Codex GPT-6.1-Sol max, exclusive review-285-sol-r9 worktree.

Fresh Grok task: node:delegated-task:command%3Amcp%3A91f43bb0-e8d6-4e5c-bd59-43523929f01c%3Adelegate-task%3Aurgent-285-code-review-grok-r9-b107228d97, Grok4.7 xhigh, exclusive review-285-grok-r9 worktree.

Both received the complete original/prior review brief, full R7 findings and parent responses, full R9 design and all unresolved objections, with the full R8 design mandatory by reference. A first dispatch exceeded T3's 120,000-character input limit and created no task; retaining the full reviewer findings and parent responses while referring to the duplicated R8 design brought each successful packet to about113,000 characters. The stable per-round clientRequestIds prevented duplicate work. Full packet files and digests are retained in task scratch. Each review must publish its findings comment before its own status. Parent alone owns the aggregate and merge. No old verdict transfers.

Current live comments/reviews were read through all pages; no external finding newer than the final push is present. No source changes, native server, deployment, version bump or memory write after the candidate was fixed.

What: Fresh R9 independent Sol review ACCEPTS the design and the cumulative code on b107228d97ef3331ba553c5c6ed0c766628d0b50.
Why: The prior blocking ownership, cancellation and proof defects are repaired, and bare cut-ins preserve the required prompt isolation without weakening cancellation.
So what: Parent may use this individual verdict for this exact head; the separate Grok verdict, authenticated aggregate review and full current CI gate still govern the delegated merge.

Identity, scope and verdicts

  • Head: b107228d97ef3331ba553c5c6ed0c766628d0b50.
  • Intended base: e2aff187811ad0e0691bb927ddc3a07a0d973018 (main). Git confirms the base is the merge base and the candidate is one commit ahead, zero behind.
  • Design verdict: ACCEPT. This includes a fresh critical assessment of the complete Opus R8 ownership/server-cut design and R9 bare integration addendum, against their production consumers and the previous objections.
  • Code verdict: ACCEPT. No blocking finding remains in the complete 44-file cumulative diff or the reachable consumers inspected. This is a code review, not a design-only approval or a transfer from an older head.
  • Reviewer: Sol, gpt-6.1-sol, max reasoning effort, Codex harness in T3 Code. I did not author this candidate.

The required outcome is routine Claude agent updates that queue, with explicit urgent agent restart and human app Steer using the captured adapter's native Stop scope and truthful requester provenance. This supports Promachos and Drafters redirecting work through the existing controls. No new product choice is required.

I read all cumulative source, test, contract and guidance changes, relevant control/start/session/event-routing/protocol consumers, root instructions and contribution guidance, current #285/PR300 records and prior findings/responses, #280/#282 records and complete #282 diff, and the supplied R0-R9 design/adjudication records. The actual native provider packages remain the owners after #291; no old server shim was restored. Tests and documentation were reviewed as claims to check, not as automatic acceptance.

Critical R8/R9 design assessment

  1. R8 repairs the contradictory owner at the adapter. A known-task authoritative task_started now moves its run, provider thread/turn and root together. Its parent moves to the resuming attempt's root only when the tree changes; a same-tree nested parent remains intact. The predicate includes a wake-preopened running entry. See ClaudeAdapterV2.ts. This is preferable to a special exception in the generic root fence: the authoritative task/tree facts agree, and the single-root-writer boundary remains intact. The persisted N1 case really completes the child first, reuses it in A2 of the same counted run, ends A2, starts R2, then delivers the child's final outcome. Its node/card/row/child output, identities, exactly-once counts and ingestion-ended milestone are meaningful checks. The existing recorded wake case also pins the parent/root/turn ownership. Both pass independently here.
  2. R8's recovery seam now represents server loss. Removing the synthetic runtime_exit preserves every pending-before-cut, recovery-cancelled and provider-only note-once assertion. In the actual server-shutdown release path, subscribers close before the adapter scope. Native EOF while a live server observes the query is a different fact and must still fail unfinished process-owned work. I reject the earlier suggestion to keep that known-dead native task running. Both continuation settings now pass; no expected lifecycle value was weakened.
  3. R9 gives bare omission one owner. steeringRestartNote returns empty for bare targets. Both PTSS prompt composition and the slash guard supply the same bare state. Thus a bare slash restart is delivered literally, while a normal restart requiring a note still rejects it before stopping. Bare retains hard interruption and durable requester/author facts; it intentionally receives no T3 explanation. The three public agent-facing promises qualify this distinction. No session allocation site, new delivery mode, receipt rule or provider-specific gate was added.
  4. R9's carrier and reload questions have discriminating evidence. T1 checks an independently hashed named-file sentinel in the initial offer exactly once, exact literal replacement text, another literal cut-in and persisted requester for bare agent restart, human web Steer and slash restart. I read the final naive-resolution red log: two extra-note equality failures and one false slash refusal, rather than setup/timeouts; the accepted changes pass the complete 21-case file. The required bare Grok Stop/reload/send and Steer probe succeeds on 70762051a3a27fcf88597e8bd6c6773046af4644. Git confirms the only delta from that checkpoint to b107 is the Claude integration test file, so production bytes are identical. The post-reload gate result is a source-bound inference from commands passing the real beforePrompt boundary and executing natively. The gate is recreated by every makeRuntime, and only the exact native inventory update releases it. Raw post-load ACP frames were not captured; I do not claim another recording.

Two explicit limits remain qualified rather than disguised as proof:

  • R8 U2: Native Claude SendMessage resumption of an already failed/interrupted/cancelled task has not been established. The known completed-task path is covered. I found no source/probe establishing a reachable new failure that requires broad adoption of those other states. Their hypothetical reachability is not a confirmed defect. Bare recipes disable native agents, so this concern cannot arise there.
  • Bare recovery fix(bare): recover after a run stops before provider start #343: I read the open owner issue and independently compared the base and candidate missed-run selection, history conversion and final bare rejection. The same pre-start Stop/open-failure mechanism exists on the unmodified intended base. Restore explicit urgent interruption after Claude steer queuing #285 adds replacement-window and F2 triggers, as the issue explicitly records. This remains a real recovery limitation, with no claimed native reproduction. I accept the bounded integration with that separately tracked limit: it does not make a successful cut-in inject T3 text, permit delivery through failed cancellation, or justify a new untracked cancellation defect. Recovery semantics remain work for fix(bare): recover after a run stops before provider start #343.

Prior blocking findings and dispositions

  • R0/R2 policy and attribution: Routine Claude agent auto/steer, completions, schedules and wakes remain queued. Human explicit Steer uses Stop scope where restart is supported; user/mobile/auto is narrowly classified for the existing busy Steer caller. Idle/start, explicit Queue and preparing/starting retention remain separate; waiting-only behavior retains its non-interrupting path. Requester is durable on the attempt and distinct from stored message authorship. WS nullish source defaulting and MCP actor/caller stamping cover promotion. Normal slash validation precedes cancellation; skill packaging, budget/handoff composition and repeated-note controls preserve text and attribution. The earlier soft Claude/background-preservation design remains rejected.
  • R1 Claude terminal before failed close: The interrupted native result is retained, not published as a completed cancellation while the query remains attached. Initial interrupt is bounded and not repeated across close retries. Close failure propagates, keeps observation, and admits no replacement; atomic captured-query detachment owns one terminal and factual backgroundWork: ended. Later-query identity is protected. The integrated failed-close/retry case passes in this review's complete Claude file.
  • R1 OpenCode2 survivor loss and R4 intent-as-outcome: A requested Stop marker is not cancellation evidence. No public attempt Stop marker, intent cascade or generic survivor handover remains. Failed, unanswered or no-op child cancellation retains the old owner and requests; replacement is blocked. Real child outcomes remain factual. Root cancellation precedes fixed-point native descendant cleanup; retry and natural completion can release the retained owner. Ordinary Stop's separate outcome policy is preserved.
  • R3/C-1 and Claude closed queries can retain running subagents in the session pending-work probe #305: Claude cleanup targets native work of the exact detached query, including earlier runs with their original identities. Buffered real outcomes, workflow owners, earlier-process objects and sibling/resumed query guards are preserved. Pending registry/probe state clears factually. App-owned delegated work is excluded. R7's resumed-task tree contradiction is repaired as described above, not waived under Verify surviving background ownership across same-run steering restarts #299.
  • R4 native scope and OpenCode1 admission: Explicit cut-ins forward requestRuntimeRestart with replacementFollows; routine Grok/Cursor and selection-driven restarts retain their prior softer path. OpenCode1 holds its root before abort/discovery, includes attached/busy children without an admitted turn, requires fresh post-abort status, retains the cohort after failed discovery/abort and preserves actual child tail output. Retry release and ordinary Stop failure handling have identified owners. A cascade cannot substitute for that output.
  • R5 F2 and Stop reach: Exhausted cancellation fails the unstarted replacement visibly with Message not delivered. It sends no prompt through unconfirmed cancellation. First Stop during retry or after F2 reaches retained work and preserves the truthful failed run. Generic effect cancellation is not invented as a user Stop. Existing ordinary Stop controls remain in the cumulative proof.
  • R6 L1/L3/L5: An actual retained/deferred native owner is reachable after native root failure, between load and acquisition, after a completed root and on the first Stop. The optional internal endedTurnHold reads existing owners; it is not another liveness registry. Target/session/current-replacement checks and ordinal/different-attempt guards distinguish older held work, same-attempt goal continuation and newer unrelated work. The root-output fence acts at native root end while child admission remains until true terminal. No reopening of a published native failure or second root writer is used.
  • R7 release, reader and goals: Success and natural release share the whole-cohort predicate, including retained processes on already-ended children. Answered turns and confirmed containment survive retries. Short cancellation-safe publication sends no native RPC, while ordinary retained-item capture cleanup remains part of publication; I do not claim local filesystem/crypto services cannot suspend. Exiting calls are joined before later Stop/cut-in waiters, with identity-safe cleanup. Governed late children register before I/O, and cancellation runs outside the serial reader and permit. The initial native goal pause and main interrupt are bounded; the existing terminate bound is reused. Goal ownership, continuation and hold.next transfer follow facts. Rejection, no answer or an uncontained command cannot count as delivery.
  • L12 and D5: The old session comes from durable restart capture keyed to the superseded attempt. Only the captured transition may authorize the exact replacement session. Old-or-recorded-replacement binding guards apply before execution, retries and post-F2 Stop. Actual two-instance Codex transition tests cover replacement/handoff rather than a fabricated in-instance policy. The worker propagates guarded-target failure. The same-cause routine-restart Stop defect D5 is repaired; routine ended roots without actual captured cut-in work are not broadly retargeted.
  • L10 correction: Factual natural release can precede an older interrupt response. Later work is outside that released cohort, even before the response arrives; retired-call identity prevents the old call reaching it. The last awaited response is not an unconditional ownership boundary.
  • R7 B2/B3 proof defects: Both Codex initialize fixtures now use literal /workspace/.codex; the decoder and host environment were not weakened. TMPDIR-unset tests pass. The fake native EOF was removed from the crash fixture without deleting its lifecycle/note assertions. Earlier schema-invalid fixtures, fake-writer timeouts, missing RPC replies, wrong after-F2 oracles and unfinished/misnamed green logs remain excluded from behavior proof.
  • Surfaces and neighboring changes: Shared web/desktop command paths, queue promotion/keybinding, mobile wire, MCP tools and injected instructions use the same service decisions. Codex, Claude, both OpenCode adapters, Grok, Cursor, ACP and Antigravity use their documented cancellation scope; Pi/Muse retain native Steer because they cannot restart. Native ACP background behavior remains provider-defined. App-owned tasks remain independent of cut-ins. fix(orchestration): child updates use queued V2 notifications #279's descendant-to-ancestor refusal, feat(contextmd): serve budgeted Great Library pointers across native sessions #327's instruction stage/record/attach/release finalizer and bare exclusion, and fix(server): skip stale restart-cut results for settled tasks #329's active-task recovery guard remain intact. Routine MCP updates interrupt foreground work on restart-only harnesses #297/Accepted MCP restart retries can fail during replacement startup #298/Verify surviving background ownership across same-run steering restarts #299 and ordinary-path fix(codex): late-child cancellation waits on its serial reader #331/fix(codex): Stop loses native work after a natural root end #332/fix(codex): natural root ends dismiss live child approvals #333 are separate tracked observations, never waivers for a demonstrated new urgent/human defect.

Executed proof, reused evidence and limits

Executed independently in the exclusive clean review worktree on b107:

  1. npx -y pnpm@11.10.0 install: exit 0. Tracked files remained clean.
  2. npx -y pnpm@11.10.0 exec vp test run apps/server/src/orchestration-v2/ClaudeAutomaticDelivery.integration.test.ts apps/server/src/orchestration-v2/testkit/OrchestratorReplayRestartBackgroundNote.integration.test.ts: 23/23, two complete files, exit 0.
  3. npx -y pnpm@11.10.0 exec vp test run apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts apps/server/src/orchestration-v2/testkit/OrchestratorReplayFixtures.integration.test.ts -t 'cut-in cohort|claude_background_wake_before_queued_prompt': 18 passed, 313 skipped, exit 0. This is selected regression feedback, not two complete-file runs.

Both test processes had TMPDIR unset and used an owned scratch TMP path. Total independently executed: 41 passing tests. Logs and exact head/base/command/result records are retained under /home/luke/scratch/chromeria-285/review-r9-sol/. Node 26.8.2 differs from the repository's requested ^24.13.1; this environment difference is declared, and CI remains the complete gate.

Inspected and reused, not rerun by me:

  • The parent's final exact-head proof and actual r9-final-affected.log: clean b107/base e2aff, TMPDIR unset, 1,171/1,171 in 23 complete files, 155.36 s. Its file list includes all 16 changed test files plus seven PTSS/recovery/MCP consumers, including the actual apps/server/src/mcp/OrchestratorMcpToolkit.integration.test.ts.
  • Actual final server/provider-core typecheck, 39 changed-TS lint and full fork-check logs bind clean b107/e2aff and exit 0; fork inventory reports 39 features. Lint warnings and typechecker suggestions remain in those logs. These scoped checks are not a substitute for CI.
  • N1's retained unchanged-production red is the literal persisted running versus completed failure after card completion; its log hash matches the handoff. The R9 final naive-resolution red has the three intended failures, and its accepted-change green completes all 21 cases. The final test SHA-256 independently matches 54878e96c3b97a4d7287cc18a1ca3e46c365c1ba535e708091855a1ebd5d6a99. The 13 normal parameter controls and the R8 N1 remain present.
  • The normal native Claude/Grok and actual agent-MCP-to-Claude record, plus its three result files, show held PIDs gone without release/completion, Claude SIGTERM, interrupted/completed turns, superseded/completed attempts and truthful requester. These runs are at d10, not a fresh native execution by this reviewer on b107. Grok's own paraphrase that a cancelled command was not executed is not trusted over the recorded ready event.
  • The bare Grok record, manifest, persisted result and immediate-after-Steer capture show one native session across reloads, exact native query bodies inside Grok's wrapper, user/web requester, no T3 note, and PID 1078920 already gone before the marker reply with no release/completion. The source-bound gate inference is qualified above. The native-history digest is recorded, not an additional raw ACP capture.
  • Uploaded UI evidence remains in the existing PR evidence comment. I did not perform browser/device use or a separate visual acceptance run.

Live CI checked separately: CI run 38663316464 has headSha b107 and conclusion success, including all six server shards, Test, Web, Lint, Typecheck, Build, Rust, Release Smoke and aggregate Check. Fork also succeeds. Successful current native-fingerprint runs exist; the earlier cancelled same-head auxiliary run is historical and is not counted as green. CI does not stand in for the source review above, and this individual verdict does not write the aggregate review context.

Not executed or established by this reviewer: native provider CLI interaction, separate Electron shell, native mobile/device, linked-two-server behavior, actual current Codex/OC2 CLI cancellation, OS-process containment on every provider, or U2 reachability. Shared desktop/web source and mobile actual-server wire coverage are not a native-device claim. Native cancellation/containment is not OS exit or rollback (L2); unknown post-root spawns (L4), stale native-aborted approvals (L6), in-memory owner loss on runtime/server loss (L7), already-lost earlier-natural-failure tracking (L9) and late goal-pause effects (L11) remain qualified limits. Work actually observed by a cut-in may not be discarded under any of those labels.

Elon record

Requirements and who asked: User #285 requires routine Claude queueing, truthful explicit urgent restart/human Steer, preserved ownership and failure semantics, all client/provider decisions, Opus design with independent Sol/Grok critique, exact-head review and green CI before parent-owned merge; #289 requires bare prompt isolation.
Deleted: Transferred prior verdicts, intent-derived cancellation, generic handover, new controls/modes, a fence exception for contradictory task ownership, T3 notes on bare prompts, weakened lifecycle/recovery assertions, speculative native-resume blockers, duplicate broad local proof and OS-exit/rollback promises.
Bottleneck: The previously demonstrated owner/proof defects are now cleared; final admission depends on the other fresh exact-head independent review and parent-owned aggregate/delivery gate.
Checked myself: Complete cumulative diff and necessary consumers; clean exact identity; R8/R9 design assumptions; 41 independently passing regression/integration cases with TMPDIR unset; literal N1/T1 red logs and final proof metadata; native result records and production-byte equality; base/current #343 mechanism; actual current CI head and conclusions.

Final disposition: Design ACCEPT; Code ACCEPT. This supports success for only review/independent-sol on exact b107228d97ef3331ba553c5c6ed0c766628d0b50, targeting this findings comment. Every older failed review remains failed on its own head.

Source/Git stayed read-only and clean on review/285-sol-r9. No tracked edits, push, merge, version, deployment, live restart, live-userdata write, memory write, server, browser/device or native agent was performed. No delegated child turns were started. Owned install/test commands are finished, and a /proc cwd check found no remaining non-ancestor process in this review worktree. The worktree is retained for the parent's review/delivery record; this reviewer releases its writer slot.

What: R9 independent Grok review accepts both the R8/R9 design and the cumulative code on b107228d97ef3331ba553c5c6ed0c766628d0b50.
Why: Resumed Claude ownership, the server-cut recovery seam, the Codex fixtures, and bare-thread prompt isolation match the accepted corrections, and no new reachable cut-in defect remains.
So what: This status is review/independent-grok only. Parent owns the aggregate review and merge.

Identity and verdicts

  • Head: b107228d97ef3331ba553c5c6ed0c766628d0b50.
  • Intended base: e2aff187811ad0e0691bb927ddc3a07a0d973018. Git confirms that SHA is the merge base. The candidate is one commit, 44 files, worktree clean.
  • Design verdict: ACCEPT. This is a fresh roast of the R8 ownership/server-cut addendum and the R9 bare-note addendum, not a transfer from an older design round.
  • Code verdict: ACCEPT. No blocking finding in the cumulative diff or the consumers traced below. This is not a design-only approval.
  • Reviewer: Grok, grok-4.7, xhigh reasoning, Grok harness in T3 Code. I did not author this candidate. No verdict is transferred from 3326f71e or from any other reviewer.

The required outcome is unchanged: routine Claude agent updates queue; an explicit urgent restart and human Steer use the adapter's native Stop scope with a durable requester; bare threads keep that interruption and receive only the sender's text.

Design

R8 ownership is the right owner. A known-task task_started with reopen moves runId, provider thread, provider turn, and rootNodeId together (ClaudeAdapterV2.ts resumedIn at 4969-5106). parentNodeId moves to the resuming root only when movesTree is true. A same-tree nested parent stays. The root-output fence in RunExecutionService is unchanged, so the task facts and the single-root writer agree. canRouteRelatedSubagent still returns true only for completed (RunExecutionService.ts 192-194). Launches, progress, and query-close updates do not set reopen, so they do not adopt a different tree. N1 is a real same-run reused-task completion after A2 ends. N2 pins the continuation node's parent, root, and turn. I do not treat the slightly looser "keeps parent and root" sentence in the design note as a defect: the accepted correction and the code agree.

R8 recovery models server loss, not native EOF. OrchestratorReplayRestartBackgroundNote.integration.test.ts cuts the transcript after the root result and still expects the subagent running before recovery (158-182). It does not inject runtime_exit. On server_shutdown, ProviderSessionManager.ts 965-976 closes subscribers before closeScopeWithin closes the adapter scope, so that cut does not ingest an EOF frame. A live server that does observe native EOF still fails unfinished process-owned work (endSubagentsOfClosedQuery). Keeping a known-dead native child running would regress that contract. I reject that recommendation.

R9 has one owner for the missing note. steeringRestartNote returns "" when bare is set (SteeringInterruption.ts 64-72). Provider turn start passes projection.thread.bare (1122-1132) and uses the raw message text for a bare thread (1115-1121). The slash guard passes the same bare flag and refuses a leading / only when the note is non-empty (Orchestrator.ts 4277-4292). The bare delivery wrapper sends additions (unless the carrier already survived), missed user text, and that userText (ProviderTurnStartService.ts 1439-1450). It does not prepend a restart note. The restart-cancelled-background note is also empty when bare (1156-1159). MCP t3_thread_send, the injected orchestration instructions, and docs/orchestration-v2/orchestrator-mcp-server.md each say a bare target gets the message alone and that a leading / is allowed there. No session allocation site, delivery mode, or receipt entry was added. Durable requester stays on the attempt.

U2 stays an unproven limit, not a blocker. Nothing in source shows that native Claude SendMessage resumes an already failed, interrupted, or cancelled child. Adoption stays limited to completed. Bare recipes disable native agents, so the question cannot arise there. I do not call the hypothetical path confirmed, and I do not require a speculative observer transfer.

#343 stays a declared adjacent limit, not a design objection. The bare wrapper still rejects a missed item that is not a user message (ProviderTurnStartService.ts 1441-1444). That rejection is inside the bare branch. The pre-start Stop/open-failure trigger exists without this change. This PR adds replacement-window and F2 triggers, and the PR records that limit. It does not inject a T3 note, deliver through a failed cancellation, or claim the bare brick is fixed. Issue #343 is open. No native reproduction is claimed or required here.

Code

No blocking defect.

Cleared on this head, from the paths read:

  • Routine Claude auto/steer still queues because activeSteeringInterruptsTools sends that steer to the queue. Explicit restart and a person's interrupt-capable Steer use restart_active and Stop scope (replacementFollows). sendToThread maps MCP auto on a steerable run to steer_active (ThreadManagementService.ts 910). On Grok and Cursor that is the existing soft interrupt_restart, which matches the new docs. Pi and Muse still reject restart.
  • Descendant-to-ancestor steer and restart still fail before dispatch (ThreadManagementService.ts 891-898). fix(orchestration): child updates use queued V2 notifications #279 stays.
  • Claude close failure still throws ProviderAdapterInterruptError before terminal publication (ClaudeAdapterV2.ts 8968 and 8998-9008). A timed-out close that already returned detaches only that query.
  • thread.stop, and run.interrupt with holdQueue, enqueue delegated-tasks.stop (Orchestrator.ts 12292-12307). A Steer or restart does not. docs/user/composer.md 81 says delegated tasks keep running and Stop ends them too. That matches this split. The MCP sentence "cancel those explicitly" is the restart path, where Stop is a separate command. Not a contradiction.
  • Codex cohort and urgent-restart initialize replies use literal /workspace/.codex (CodexAdapterV2.test.ts 9939, UrgentRestart.integration.test.ts 545). The decoder is unchanged. Neither helper reads TMPDIR.
  • F2 still writes "Message not delivered" only while the replacement run is starting, and a later Stop can still reach retained work. Ordinary Stop does not set replacementFollows.
  • App-owned delegated sessions stay independent of a provider cut-in. The new user-guide paragraph does not claim a new control. The empty-draft composer button is still the existing Stop button (ComposerPrimaryActions.tsx 267).

Proof

Executed by this review: identity, git diff 70762051a3a27fcf88597e8bd6c6773046af4644...b107228d97ef3331ba553c5c6ed0c766628d0b50 (only ClaudeAutomaticDelivery.integration.test.ts), and live CI. No install and no test run. The previously failing mechanisms are visible in source, and the exact-head CI shards that failed on 3326f71e now pass. Another local sweep would not resolve a remaining risk.

Reused, and checked rather than trusted:

  • r9-final-affected.log is bound to this head and base, empty worktree hash, env -u TMPDIR, 1171/1171 in 23 complete files, exit 0, including OrchestratorMcpToolkit.integration.test.ts.
  • Server and provider-core typecheck, changed-TS lint, and fork-check logs on the same head exit 0. Lint warnings in that log are pre-existing. Node 26.8.2 versus the repo request ^24.13.1 is a declared environment difference.
  • CI run 38663316464 has headSha b107228d97ef3331ba553c5c6ed0c766628d0b50 and conclusion success, including Test Server 1-6, Test, Test Web, Lint, Typecheck, Build, Rust, Release Smoke, and Check. Fork stack and native fingerprint also pass. CI is not this review.
  • Native bare Grok proof at 70762051 applies to production bytes of this head because only the T1 test file differs. Post-reload gate passage is a source-bound inference: each makeRuntime recreates the gate, and only the exact six-tool/seven-command update releases beforePrompt. Raw post-load ACP frames were not captured. I do not claim another recording.
  • UI images and the 75s video remain on comment 6170695116. Controls are unchanged. I did not open a browser.

Not established here: a fresh native CLI run on this head, Electron, a device, linked two-server behavior, current Codex or OpenCode 2 CLI cancellation, or U2 reachability. Native containment is not an OS exit or a rollback (L2). Unknown post-root spawn (L4), stale native-aborted approvals (L6), in-memory loss on process death (L7), tracking already lost before a cut-in (L9), and a late goal pause (L11) stay qualified limits. #297, #298, #299, #331, #332, and #333 stay separate. They waive no new urgent or human cut-in defect found here.

Elon record

Requirements and who asked: Luke, #285 / #282: routine Claude updates stay queued, explicit urgent restart and human Steer interrupt with the real requester, and bare threads (#289) get no T3 prompt text. This review checks that head.
Deleted: Transferred verdicts, a fence exception, treating native EOF as a crash, a T3 note on bare prompts, a new control, and a local rerun of already-green exact-head proof.
Bottleneck: The prior owner and proof defects are repaired on this head. Admission now waits on this review's status and the parent's aggregate gate. CI on this SHA is already green.
Checked myself: Clean exact head and base; R8 move-together ownership and unchanged fence; shutdown subscriber order; bare note owner at both call sites; recovery fixture without runtime_exit; literal Codex homes; Stop versus Steer delegated-task effects; ancestor refusal; the 707-to-b107 production diff; the bound 1171 log; live CI on this SHA.

Verdict: ACCEPT design and ACCEPT code. Success is only for review/independent-grok on b107228d97ef3331ba553c5c6ed0c766628d0b50, linked to this comment.

What Both fresh independent reviewers accept the R8/R9 designs and cumulative code on b107228d97ef3331ba553c5c6ed0c766628d0b50.
Why The demonstrated ownership, cancellation and proof failures are corrected, and bare prompts retain their required isolation.
So what With complete CI green and delegated merge authority unchanged, the parent will publish the authenticated aggregate success, merge this exact head into main and finalize task resources.

I read both complete published findings, checked the actual proof each used and verified each live comment preceded its own authenticated status, with the correct exact head and findings URL:

  • Sol R9, Codex GPT-6.1-Sol max: design ACCEPT, code ACCEPT, no blocking finding. Read all44 cumulative files and necessary consumers, independently executed41 passing cases, and inspected the bound parent proof. review/independent-sol success was posted by lukemaj after comment6182239155.
  • Grok R9, Grok4.7 xhigh: design ACCEPT, code ACCEPT, no blocking finding. Traced the stated ownership, bare-note, recovery, shutdown, Stop/Steer and ancestry consumers, inspected the exact-head1171-case log and live CI, and did not claim a new test/native run. review/independent-grok success was posted by lukemaj after comment6182309036.

Parent adjudication: accept both independent verdicts. Both critically considered the R8/R9 addenda rather than transferring an older verdict. Their qualified U2 and #343 limits match the existing evidence; neither supplies a new confirmed cancellation failure or a reason to widen this outcome. Bare native refusal wording remains an explicit #289 constraint, and a successful interruption does not imply rollback or universal process exit. The Stop-versus-Steer distinction for app-owned delegated tasks is source-backed: thread.stop / run.interrupt with holdQueue stop those tasks, whereas a cut-in does not. No unresolved blocker or product-taste decision remains.

Complete CI38663316464 passes on this exact head, including all six server shards and aggregate Check. Final focused proof is1171/1171 in23 complete files. Prior R7 failure statuses remain unchanged on3326; no verdict was transferred. Live main and PR head were rechecked as e2aff18 and b107228, respectively. Both new review tasks are terminal with no pending child turns; their worktrees remain clean and their writers released. Finalization will preserve the task's exact proof history and remove only owned, eligible transient resources after the verified merge. No version bump, deployment, live Chromeria restart or agent memory.

What PR #300 merged into main as f49ece39e8b302c8bffa9b17a385d66ec989b851; #285 and #305 are closed.
Why Explicit urgent agent restart and human Steer need to cut into running work while routine Claude updates keep queueing.
So what Delivery and task finalization are complete; activation rides the next build without restarting Chromeria.

The merged tree is byte-identical to exact reviewed head b107228d97ef3331ba553c5c6ed0c766628d0b50, over base e2aff187811ad0e0691bb927ddc3a07a0d973018. Sol and Grok both accepted the revised Opus design and cumulative code. Each findings comment preceded its authenticated success status, and the parent aggregate acceptance and all three success statuses were verified before merge. Complete CI was green; 1,171 focused tests in23 complete files and the scoped static checks passed on that head.

All17 owned worktrees, all29 local task/component/checkpoint refs, the remote task branch and both owned preview tabs are retired. Matching remote task refs are absent; no owned process or listener on6189/14229 remains. Required intermediate Git history is retained in a verified private proof bundle outside the worktrees, and native history was copied privately with its expected digest. The prior dirty test output was conclusively matched to the original writer's hashes, explicitly checkpointed and bundled before its clean checkout was removed. No cleanup exception remains; unrelated resources were preserved. Canonical main remains at da7034e with no source mutation.

Issue #285's canonical handoff now records the merged state, proof, verdicts and completed finalization. The declared source-only bare recovery issue #343 remains open. Native/device/Electron limits remain as stated in the evidence; no version bump, tag, release, deployment, live Chromeria restart or agent memory.

What: Deploy merged main f49ece39e8b302c8bffa9b17a385d66ec989b851 to cavallo, rocky, then bestia.
Why: Activate #342 descendant-aware stale notices and #300 human Steer / explicit restart interruption while routine delivery stays queued.
So what: The implementation deployment task owns sequential activation, rollback at the first failure, live proof and per-machine runbook commits under the user's explicit fleet deployment authority.

Build from exclusive clean exact-SHA worktrees. Build/sign the Mac artifact once on cavallo and promote it unchanged to rocky. Build the Linux artifact once and require the executable import validator and isolated archive smoke before bestia activation. Large files stay in task caches, never /tmp. Preserve immediate and earlier rollback copies and read-only VACUUM backups. Check active runs, unfinished tools and provider background work immediately before every restart. Use the installed restart-and-resume helper on rocky and cavallo with an inert task maintenance callback, never the planner thread.

Bestia post-restart steps, recorded before activation

  1. Detached user unit runs ~/scratch/bestia-chromeria-swap-2026-10-14.sh with a task wrapper and an automatic rollback trap. Retain the previous executable and databases outside the build worktree. Capture the previous backend identity and stop timestamp.
  2. After normal restart recovery, verify the exact installed and running executable digest, health and discovery HTTP 200. Observe that same actual backend for at least 600 seconds. Roll back and stop the rollout at the first failed gate.
  3. Inspect migration success and ledger/schema read-only; list peer links and load real production thread snapshots through authenticated product APIs. Inspect cut turns lacking continuation and held queues. Continue only top-level cut turns once when needed; leave delegated children to their coordinator. Report held queues.
  4. Confirm cavallo's previously renewed desktop session remains unrevoked and valid, without synthetic renewals or re-pairing; compare the installed client's public commitment privately with server state.
  5. In a disposable scratch project/thread, run a Claude turn with a long sleep tool. Confirm a routine queue delivery remains queued while that tool keeps running. Submit human Steer through the actual command path, confirm the original turn/tool interruption and replacement delivery, then archive the disposable thread. Record observational limits honestly.
  6. Pull runbook --rebase immediately before each per-machine commit, stage only this task's paths, push normally and verify remote identity. No agent memory and no planner-thread messages.

Requirements and who asked: User requested this exact-or-newer merged main fleet rollout, order, rollback, ten-minute identity gate, session validity and bestia interruption smoke.
Deleted: Floating source after build, rebuilding the signed artifact on rocky, extra restarts for proof, new permanent automation, and any use of live userdata as a development server home.
Bottleneck: Sequential machine acceptance, especially the required ten-minute backend identity gate; no parallel deployment or deadline bypass.
Checked myself: Live cavallo baseline is 2ba3873 with no staged app; origin/main is exact f49ece3, #300 and #342 are merged, prior task scripts and three supplied runbook commits were read and reconciled with current state.

What: Cavallo and rocky passed their final gates; exact f49ece3 Linux artifact is validated before bestia activation.
Why: Bestia is last under the user's fleet order and needs the real Claude queue/Steer smoke on the activated build.
So what: Run the named scratch swap in a detached user unit with rollback, then the already-recorded post-restart checks and smoke. Do not repeat a completed swap after recovery.

Cavallo: signed asar c4e79538422ccd9a46d44c7318acb9929b0d5da2ad155742c25b07034f04d4ae, actual backend443 stable601.41seconds, health/discovery200, signature, migration, peers and three authenticated thread loads pass. Existing cavallo desktop session on bestia remains approved generation1, unrevoked and valid until2026-11-14T01:03:09.592Z; independently re-read desktop/server commitments match. Runbook643e48c pushed.

Rocky: identical signed asar, backend77280 stable602.26seconds, health/discovery200, signature, migration, peers and three authenticated thread loads pass. Required chromeria-restart-and-resume callback delivered; no uncontinued cuts or held queues on either Mac. Runbooka3e5e65 pushed.

Linux: clean tracked source in exclusive detached ~/.cache/chromeria-deploy-f49ece39/linux; unchanged integrated executable validator passed, independent emitted-graph scan forbiddenImports=[], isolated archive --version and serve smoke passed. Archive SHA-2566e9a2f5163375ab2f6b6fb40024003112e52b2c22170ff701b0e1e17f7741c2f; executablea6ee1ffd60bec640ea23ab71e0ac0bd3e6e390db7dbf410185ef74d667ca082f. Initial background launcher ended during dependency installation without producing an artifact; managed execution completed successfully. No source/validator/version change.

Live bestia baseline digest4458d1042915b6ad26e75505a328ea049f7ea01ffeecc4d1954b8c317eb03b83 / backend1028043. Fresh precheck lists active durable turns including this task and other work, matching the explicitly requested preceding fleet deployment path, which recorded14active turns and used restart recovery. No provider background tasks. A joined current-run check now finds no unfinished native tool/command work; historical pending items belong to old runs. Do not kill or manually resend other delegated work. Recheck immediately before stop, after backups, and defer while any native command/tool or provider background task is active. Standard durable restart-cut recovery owns active-turn continuation; inspect cuts and held queues afterwards.

Bestia post-restart: require installed plus /proc executable digest match, health/discovery200 and same actual backend for >=600seconds; automatic rollback on activation/observation failure. Then startup migration Success/schema, peer listing, authenticated thread loading, cut/held-queue inspection, renewed-session validity, real throwaway Claude long-sleep queue versus human Steer smoke, archive throwaway thread, and per-machine runbook finalization. These steps supplement the full prewritten recovery checklist in comment6182543499; no planner-thread t3_thread_send and no memory writes.

Requirements and who asked: User's exact fleet target/order, previous deploy path, validator before bestia, rollback and live smoke/proof.
Deleted: A new artifact on rocky, floating main after build, proof-only restarts, killing active tools, live-userdata test servers and broad cleanup.
Bottleneck: Bestia backup and safe native-tool boundary, then its mandatory live gate and smoke.
Checked myself: Both final Mac observer records, live source and CI identity, Linux build/validator/archive output and digests, bestia baseline and active-run/current-tool checks.

What: The requested #300 smoke did not reach a running long-wait tool; bestia will be rolled back and the rollout stopped under the user's first-failure rule.
Why: Native Claude Bash rejected standalone sleep600 before execution, and the same disposable bare Claude session reports Monitor unavailable. Neither a routine queue message nor human Steer was submitted because there was no live tool to interrupt.
So what: Preserve both accepted Mac deployments, roll back bestia to its exact verified2ba baseline in a detached unit, verify rollback health/stability, retain proof and archive the disposable setup thread. The fleet outcome is incomplete, not a successful #300 smoke.

Activated targetf49ece39e8 passed Linux import validation, isolated archive smoke, installed/running digest equality, health/discovery200, migration, peers and authenticated thread loading. Its actual backend1462285 remained unchanged across21samples/606.19seconds; wrapper wrote OBSERVATION_PASSED. The native tool setup block is not evidence of a source interruption defect. No guard was bypassed, hidden sleep substituted or source changed. First standalone sleep was rejected; one supported-alternative setup follow-up reported Monitor unavailable. No routine queue or Steer comparison occurred.

The throwaway scratch thread is mcp:launch:133a0269-b4ba-4fe6-89e9-aec5edc8c0b1:deploy-f49ece39-claude-smoke-2026-10-15, Claude Haiku5.5 full-access, bare to prevent memory/instruction side effects. It is archived. Raw own-thread snapshots are retained privately under the task cache. No planner-thread t3_thread_send or agent-memory writes.

Rollback: after a fresh safe-tool precheck, detached user unit stops t3code.service, preserves failed-candidate runtime at ~/.cache/chromeria-deploy-f49ece39/runtime-smoke-unverified, restores ~/t3-backups/chromeria-runtime-prev to ~/.t3/runtime/versions/0.0.45 after verifying baseline4458d1042915b6ad26e75505a328ea049f7ea01ffeecc4d1954b8c317eb03b83, starts normally, and observes the same rollback backend/digest/health for600seconds. Do not repeat a completed swap after recovery. Earlier038745 remains preserved in the task cache. Inspect cut turns and held queues again, revoke the disposable proof session, then write the final bestia runbook result and retire only owned build worktrees.

Requirements and who asked: User required a real running-Claude long-sleep queue-versus-human-Steer smoke and rollback/stop at the first failed required gate.
Deleted: Claiming the rejected sleep as an interruption, testing another provider, bypassing the native tool guard, replacing proof with unit tests and declaring full fleet success.
Bottleneck: Native Claude setup cannot establish the requested live tool state in this session; activation acceptance remains incomplete.
Checked myself: Persisted native Bash rejection and subsequent Monitor-unavailable response, completed tool/run states, archived scratch thread, all target observer records and exact preserved2ba runtime digest.

What: Cavallo and rocky retain accepted f49ece39e8b302c8bffa9b17a385d66ec989b851; bestia was restored to 2ba387388074ac4ee09cc256f9800cdd9ce2b520 and its rollback is verified.
Why: The required #300 live comparison never reached a running long-wait tool: native Claude Bash blocked standalone sleep600, and the same session reported the suggested Monitor tool unavailable.
So what: The rollout stopped under the user's first-failure rule. No further deployment or smoke attempts will run; two held queues are preserved for their user/coordinator.

Final machine evidence

  • Cavallo: f49 exact source, signed app.asar SHA-256 c4e79538422ccd9a46d44c7318acb9929b0d5da2ad155742c25b07034f04d4ae, strict deep signature and embedded SHA passed.21samples/601.41seconds on actual backend443/start03:56:23CEST, health/discovery200 throughout. Fresh03:12:57Z check still has that same backend, source and digest.
  • Rocky: same exact signed Mac artifact and digest, strict deep signature and embedded SHA passed. Required chromeria-restart-and-resume used.21samples/602.26seconds on backend77280/start04:11:19CEST, health/discovery200 throughout; fresh03:12:57Z check still matches.
  • Bestia candidate: f49 archive 6e9a2f5163375ab2f6b6fb40024003112e52b2c22170ff701b0e1e17f7741c2f, executable a6ee1ffd60bec640ea23ab71e0ac0bd3e6e390db7dbf410185ef74d667ca082f. Unchanged executable import validator and independent scan passed, isolated archive version/serve smoke passed before activation. Candidate backend1462285/start04:30:35CEST passed21samples/606.19seconds, installed and /proc digests matched, health/discovery200 throughout. This platform gate does not substitute for the blocked fix(orchestration): restore urgent interruption with truthful context #300 smoke.
  • Bestia final rollback: verified baseline executable 4458d1042915b6ad26e75505a328ea049f7ea01ffeecc4d1954b8c317eb03b83, restored only after current native work drained.21samples/602.82seconds on backend1677632/start05:03:26CEST, installed and /proc digests match, health/discovery200 throughout. Fresh03:14:10Z validation passed; wrapper wrote ROLLBACK_VERIFIED. No database restore or new-work loss by backup restoration.

On all three activated targets, peers were listed, startup runMigrations completed Success, ledger62 and required open-time schemas were applied, and three real authenticated production thread snapshots loaded200. Those checks were repeated on bestia's restored build. All three read-only VACUUM backups per machine passed integrity checks; immediate and older rollback generations are retained. The merged tree matches the reviewed b107228 tree and its green complete CI38663316464. No source/validator/version change.

Cavallo's previously renewed desktop session remains approved generation1, unrevoked, valid until2026-11-14T01:03:09.592Z. Fresh installed-client public commitment independently matches bestia server state after both its activation and rollback; the latest normal reconnect is03:03:50.381Z. No synthetic renewal, pairing, device approval, clock/TTL edit or proof-only desktop reconnect.

Smoke and remaining state

Native Bash rejected sleep 600 before execution. One supported-alternative setup follow-up reported Monitor unavailable. No hidden/chained sleep, routine queue message or human Steer was submitted, so neither side of the requested queue-versus-Steer comparison is proved. No source interruption failure is claimed. The disposable scratch Claude Haiku5.5 bare thread completed its two setup turns and is archived; it wrote no memory. Its native observations are retained privately in the task cache.

Final03:14:10Z postcheck has no cut turn lacking continuation. Held queues remain:

They were not resumed. No unsupported claim is made about the precise cause of each hold. Native tools were checked before every restart. Rollback waited for other live tests/tools, without a deadline, until its03:03:21Z check found zero unfinished current native tools and zero provider background tasks.

Both detached user units are retired. All three exclusive clean build worktrees are retired after consumer checks; immutable archives, candidate/older runtimes, backups and proof remain outside them. The disposable proof session was revoked and its token file removed. Per-machine runbook records and Mac cleanup are committed, normally pushed and remote-verified after successful immediate pre-commit pull --rebase; no force-push or unrelated staged path. Canonical source checkout and its pre-existing untracked bytecode are preserved. No pattern kill, planner-thread t3_thread_send, browser driving, live-userdata test server or agent-memory write.

Requirements and who asked: User's exact fleet order and source, immutable clean builds, validator before bestia, rollback/stop rule, live machine proofs and one real #300 comparison.
Deleted: Full-fleet success claim, treating a blocked sleep as an interrupted tool, hidden sleep substitutes, further smoke/deploy attempts after stopping, interruption of other threads' native tools and broad cleanup.
Bottleneck: The live #300 comparison remains unproved because its native long-wait setup was blocked; this deployment outcome is stopped, not accepted for all three machines.
Checked myself: Exact source/tree and artifact identity, actual signed/packaged outputs, every observer endpoint and live backend, installed/running digests, migration/schema, peers, real authenticated thread loads, desktop/server commitment equality, native tool rejection, archival, rollback and teardown state, pushed private runbook records.

What: Deploy exact merged main 127238c48538fca9ad68dc52ce24bb6271b41900 to cavallo, rocky, then bestia.
Why: Activate #346's forward native input identities and prove #300 routine queue versus human Steer without treating a failed smoke setup as product failure.
So what: This authorized implementation task owns sequential deployment, health-only rollback, live evidence and private machine runbook records; human-origin Observer runs remain the user's action.

Bestia post-restart steps recorded before activation

  1. Validate the unchanged Linux executable import graph and isolated archive version/serve smoke before activation. Build once from exact source in an exclusive worktree, preserve immutable digests, backups and rollback runtime outside /tmp.
  2. Run ~/scratch/bestia-chromeria-swap-2026-10-14.sh under a detached task unit. Recheck active current native tool work and provider background tasks immediately before stopping. Wait without a deadline when unsafe; preserve user work.
  3. After normal restart recovery, verify installed and /proc executable digest, health/discovery HTTP 200 and the same listening backend at least ten minutes later. Roll back only on health failure. Other proof gaps are reported without rollback.
  4. Read migration/schema state read-only, list peers and load three authenticated real production thread snapshots. Inspect cut turns and held queues; continue only uncontinued top-level cuts once, leaving delegated children to their coordinator. Verify cavallo's existing renewed session remains approved, unrevoked and valid with matching installed-client/server public commitment.
  5. In a disposable scratch project/thread, ask Claude to run for i in $(seq 1 300); do echo $i; sleep 1; done in the foreground. Observe a running native Bash call before sending a routine queued message; prove it remains queued. Submit human-style Steer through the application's steer command path; prove interruption and replacement delivery. Archive the disposable thread. Failure to establish this test is not a rollback gate.
  6. Run agent-started Claude and Grok inputs on bestia; record full message/run/attempt/provider-turn/native-record joins, created_by, forward match counts and correction population coverage on Attribution remains unmatched for real Claude and Grok V2 inputs after 1.2.2 agent-observer#95. Read live databases only via mode=ro. Preserve historical unknowns. Provide exact human-input instructions there rather than invent human runs.
  7. Pull runbook --rebase immediately before each machine commit, stage only owned machine record, push normally and verify remote. No agent memory or planner-thread messages.

Mac artifact is built/signed once on cavallo and promoted unchanged to rocky. Use existing restart-and-resume with the established restricted maintenance callback, never the planner thread. No version, source, validator or persistent automation changes.

Requirements and who asked: User requests exact-or-newer main fleet deployment in order, health-only rollback, ten-minute backend identity proof, #300 live interruption comparison and #95 actual agent attribution plus human instructions.
Deleted: Smoke as rollback gate, bare long sleep, floating source after build, rebuilding Mac artifact on rocky, historical backfill, new permanent automation, proof-only restarts and memory writes.
Bottleneck: Sequential machine health acceptance and native tool-safe restart boundaries; follow the proven path rather than adding rollout machinery.
Checked myself: Live origin/main is exact 127238c; #344, Observer95 and #346 read, previous deployment/rollback scripts and specified runbook entries reconciled with the requested corrected rollback rule.

What: Final fleet target is now exact main 6408f92bb2af98199567df08c904842733ad28ed, per the planner's explicit update.
Why: It includes merged #347 bounded ContextMD phase diagnostics on top of #346, required live on rocky and bestia.
So what: No machine had restarted onto 127238. Its completed Mac staging job was disarmed and staged app preserved; both platform artifacts are rebuilt once for 6408, then the original sequential rollout and health-only rollback apply.

The prewritten bestia post-restart steps in comment6183629323 remain authoritative with target SHA6408f92bb2af98199567df08c904842733ad28ed substituted. The same smoke and Observer95 proof run after final activation. No extra 127238 restart is needed. Existing pre-127238 read-only backups are pre-mutation backups, retained alongside exact pre-6408 backups.

Requirements and who asked: Planner explicitly changed all three final targets to merged main6408f92bb2, including #347 diagnostics.
Deleted: Activating superseded127238, duplicate restarts, smoke-as-rollback gate and any change to existing production budgets.
Bottleneck: Replacement exact-source artifact build, then sequential ten-minute machine health proof.
Checked myself: Live origin/main and PR347 merged identity both equal6408f92bb2af98199567df08c904842733ad28ed; no127238 activation occurred, and cavallo's old swap job was removed before preserving its staged artifact.

What: Cavallo passed its6408 gate; rocky is on the identical signed6408 artifact, completing its ten-minute observation before bestia activation.
Why: Bestia is last in the authorized order and must activate validated Linux output with health-only rollback.
So what: After rocky's final identity check succeeds, execute the prewritten bestia steps from comment6183629323, retargeted by comment6183664198, through the detached unit below. Never repeat a completed swap after recovery.

Target 6408f92bb2af98199567df08c904842733ad28ed. Mac asar b9aab2b37a26c7eb31f27754fe5c925bdf3dacb14bf4a7299938875c0f7c6518; cavallo backend81222 passed21samples/601.26seconds, health/discovery200, signature, migration, peers, three authenticated thread loads and renewed desktop commitment validity. Runbook7ffebe1 is pushed. Rocky backend92473 has matching artifact, health/discovery200, migrationSuccess, peers, three authenticated thread loads, and no uncontinued cuts/held queues. Its final gate result will be appended before starting bestia.

Linux build from clean tracked exact6408 source completed in the task worktree. The unchanged executable import validator passed; independent emitted-graph scan returned forbiddenImports=[]. Isolated archive version/serve smoke passed before activation. Archive SHA-256 791e20a5e5b29dfa06421b1845a0ba32ce9f214ef88c501f4bdf5e6feadfce82; executable 97264b590848762a24bcc5aba2707d3a0dcf99e227b1b3ae48b189deeb6e0261. Superseded127238 archive is separately preserved; no source/validator/version change.

Bestia baseline remains exact2ba runtime digest 4458d1042915b6ad26e75505a328ea049f7ea01ffeecc4d1954b8c317eb03b83, actual backend1677632. Wrapper ~/.cache/chromeria-deploy-127238-proof/run-bestia.sh calls the required scratch swap with the6408 build/cache paths, read-only VACUUM backups, a fresh current-native-tool/background check and detached user unit chromeria-deploy-6408. This cache name predates the retarget; target-sha and expected-binary files pin6408. The wrapper automatically rolls back only when health fails. Identity, smoke or attribution proof gaps on a healthy backend are reported and do not trigger rollback.

Recovery: inspect bestia-started, bestia-stop-time, bestia-swap.log, bestia-status and bestia-watch.jsonl in that cache, plus the detached unit. Observe installed and /proc digest equality and the same backend over>=600seconds. Then migration/schema, peers, authenticated real thread loads, uncontinued cuts/held queues and cavallo existing renewed session validity. Do not restart again for proof.

Own deployment thread is thread:delegated-task:command%3Amcp%3Accf237ec-fb01-421e-991b-a8d1e4625edd%3Adelegate-task%3Adeploy-127238c-all-2026-10-15. Standard durable recovery owns other delegated work; no manual resend to delegated children, no planner-thread messages. Wait without deadline at unsafe native-tool boundaries. The disposable Claude loop/queue/Steer and separate agent Claude/Grok attribution runs follow activation; their evidence goes here and Observer95, then archive the smoke thread. Human-origin95 runs remain for the user.

Requirements and who asked: User's updated exact6408 fleet target/order, Linux validation, safe restart, health-only rollback, live smoke/attribution and per-machine runbook proof.
Deleted: Superseded activation, smoke rollback, extra restarts, rebuilding rocky artifact, manual child recovery and agent memory.
Bottleneck: Rocky final health observation, then bestia backups and a safe native-tool boundary.
Checked myself: Actual cavallo observer/final proof, rocky artifact/health/startup evidence, exact Linux build/validator/archive outputs, live bestia baseline and an initial safe-tool sample; final pre-stop check remains inside the swap.

Rocky final gate completed:21samples03:39:02.060548Z–03:49:03.873250Z,601.81seconds on the same actual backend92473, health/discovery200 and expected signed asar digest at both endpoints. Fresh03:49:35Z validation and installed strict deep signature passed. Bestia activation can now proceed in order, with only health failures authorizing rollback.

What: #300's live routine queue versus human-style Steer comparison passed on bestia6408f92bb2af; the disposable thread is archived.
Why: Routine MCP delivery waited behind an executing Claude Bash loop, while the application's human Steer command interrupted that work and delivered its replacement input.
So what: Keep the healthy deployment. Observer95 acceptance remains separate, including its real-human inputs and any unmatched identities.

Disposable scratch thread: mcp:launch:4afd5bf8-2286-4423-b874-e981165fa695:deploy-6408-claude-smoke-20261015, Claude Haiku5.5, full-access, bare, with explicit no files/memory instruction. Native Bash executed the requested for i in $(seq 1 300); do echo $i; sleep 1; done in the foreground, timeout360000, run_in_background=false.

The first comparison showed running/queued/interrupted states and STEER_ACK, but Claude's resumed text incorrectly said its tool never started. The SDK's cancellation result uses generic refusal text. I repeated the same probe once with an independent OS oracle rather than accepting either the UI state or the model's claim alone.

On the conclusive repeat, run ordinal4/tooltoolu_01BrP35kpjVSKnYtmrQwGy5p started03:57:09.452Z. Direct read-only /proc inspection found actual Bash PID1869446 executing that exact loop in the disposable scratch cwd, with a live sleep1 child1869636. The loop PID was still alive at03:57:57.564979Z. No manual signal or pattern kill was used.

Routine mode:auto message message:mcp:4afd5bf8-2286-4423-b874-e981165fa695:thread-send:deploy-6408-smoke-routine-repeat-20261015 produced queued run ordinal5. At03:57:29.326605Z and03:57:57.564979Z it remained queued, while ordinal4 and its native Bash item remained running and the actual loop process remained alive.

A separate queued Steer input, ordinal6, was promoted through the exact app command queued-message.promote-to-steer, target ordinal4, with web/user requester fields matching the UI's human Steer path. Authenticated WebSocket RPC orchestration.dispatchCommand returned sequence1518020. This is an automated human-style action, not a claim that a person typed a run. Both message payloads retain their actual agent/mcp authorship; no human acceptance run is invented for Observer95.

The original native tool became interrupted at03:58:11.516Z, the target run completed, the promoted queued run became cancelled as its message was delivered into the active run, and the replacement assistant emitted STEER_REPEAT_ACK. At03:58:24.013976Z both captured OS PIDs were absent. The routine queue then ran separately and emitted ROUTINE_REPEAT_ACK. The model again echoed the generic refusal narrative, which contradicts the independently observed process; it is not used as execution evidence.

All six durable smoke runs are terminal, and the thread is archived through the product lifecycle (sequence1518366). No source/guard changes or memory writes. Private snapshots, exact command/result and OS observations remain under ~/.cache/chromeria-deploy-127238-proof. Smoke was never a rollback gate.

Requirements and who asked: User requested a real permitted Claude long command, routine queue observation, human-style app Steer interruption and archived disposable thread.
Deleted: Bare long sleep, trusting an assistant's execution claim, treating a UI running label alone as OS execution proof and smoke rollback.
Bottleneck: Distinguishing generic SDK cancellation wording from actual command execution; direct /proc observation resolved it.
Checked myself: Native Bash request, two separated queued/running snapshots, actual loop/sleep PIDs and scratch cwd, authenticated app promotion command/result, native interruption timestamp, replacement and routine acknowledgments, captured-process absence and archival.

What: Cavallo, rocky and bestia all run exact main 6408f92bb2af98199567df08c904842733ad28ed; #300's live queue/Steer smoke passed and its disposable thread is archived.
Why: The final target includes #346 and #347, and each sequential activation passed health with the same actual backend more than ten minutes later.
So what: Fleet deployment is accepted with no rollback; Observer95 remains open for the verified Grok forward-reference gap and two real human-started inputs the user must send personally.

Bestia ran the requested scratch swap in detached user unit chromeria-deploy-6408 only after both Mac gates. All three backups finished03:53:38Z, immediate03:53:39Z precheck had9 durable active runs,0 unfinished current native tools outside this task and0 provider background tasks. Normal restart recovery continued this deployment. The wrapper recorded OBSERVATION_PASSED and its unit is collected. Immediate2ba rollback remains verified at digest4458d1042915b6ad26e75505a328ea049f7ea01ffeecc4d1954b8c317eb03b83; previous rollback generations remain retained. No rollback occurred. Smoke and attribution were never rollback gates.

Cavallo's actual renewed desktop session remains approved generation1, unrevoked and valid until2026-11-14T01:03:09.592Z, with normal post-bestia reconnect03:54:08.704Z. The installed desktop CLI's public commitment independently matches bestia state. No synthetic renewal, pairing, approval or proof-only reconnect.

Live behavior and remaining attribution result

#300 live smoke evidence: allowed foreground Claude counter loop actually ran, independently confirmed by /proc. Routine auto delivery stayed queued in separated snapshots; the app's human-style queued-message.promote-to-steer interrupted the loop, delivered STEER_REPEAT_ACK and left the routine queue to run separately. Captured original Bash/sleep PIDs were absent after interruption. The resumed model's generic refusal narrative was contradicted by the OS oracle and is not used as execution proof. The disposable thread is archived and all its runs are terminal.

Observer95 full chains, native evidence, counts and exact human instructions: dedicated agent Claude1/1 resolves created_by=agent; dedicated agent Grok0/1 remains unknown, despite a valid native promptIndex0 echo. Adapter-owner gap is recorded and open. Whole-copy joins Claude0/905→8/915 and Grok0/625→0/631; known intervention population Claude21/972→29/984 and Grok0/17820→0/17827. These are actual native inputs consumed by unchanged Observer1.2.2 in a disposable ledger copy, not an installed Observer upgrade. No historical guessing, correction verdict or real-human run is fabricated. The two acceptance threads remain available for the user's personally typed follow-ups specified on95.

All three owned build worktrees are retired after consumer checks; exact artifacts, rollback generations, backups and private proof remain outside them. The own scoped proof session is revoked, live revocation verified, token file removed. Private runbook records were pulled --rebase immediately before committing, only owned paths staged, normally pushed and remote-verified: cavallo7ffebe1, rocky59ddb4a, final bestia/fleet73a633f. Canonical source and pre-existing untracked bytecode are preserved. No pattern kill, browser driving, test server against live userdata, planner-thread message or agent-memory write.

Requirements and who asked: User's updated6408 fleet target/order, health-only rollback, all machine proofs, live #300 queue/Steer comparison, Observer95 actual agent chains/counts and user-owned human instructions.
Deleted: Superseded activation, smoke rollback, extra restarts, model execution claims as proof, fake human attribution and source repair in this deployment task.
Bottleneck: Fleet activation is complete; the remaining95 acceptance bottlenecks are Grok's real forward runtime gap and personally typed user inputs.
Checked myself: Exact final sources/artifacts and all live backend observations, migration/schema, peers, authenticated snapshots, renewed-session commitment equality, real loop process before/after Steer, archived smoke, actual native identity chains and consumer counts, open gap records, cleanup and pushed runbook state.
