fix(server): steering no longer cancels a pending question or approval - #26842
Fixes#26841. With Steer as the default follow-up (#24718), a message sent while the agent waits on the user steers the running turn. That cancels the pending AskUserQuestion or approval, so the user never gets to answer it. Claude steers with priority: "now", and the SDK aborts the pending permission callback ("User cancelled tool execution."). The message can come from the composer, a promoted queued message, or another agent's t3_thread_send in auto mode.
Change
CommandPolicy.resolveMessageDispatchIntent turns any steer_active into queue_after_active while the thread has a pending runtime request. It already does this behind /compact and /logout. Every message.dispatch passes through it, so the composer, the auto delivery intent and sendToThread are all covered in one place.
queued-message.promote-to-steer checks the same predicate. The promoted message moves to the front of the queue instead of steering, so it runs right after the answer.
restart is unchanged.
The composer guide says that a steer queues while a question or approval waits.
Scope and approval
A focused fix for one bug, reported in #26841, which is not triaged yet. The question is cancelled without the user ever seeing it, which is clearly a bug and not a product choice. The fix only adds this one condition to the queueing rule that steers already follow.
Verification
Failing first. On main @ 8ff9ae1, with only the two source changes reverted:
the new CommandPolicy.test.ts cases fail for a pending user_input and a pending command request;
the new SteeringCompletion.integration.test.ts case fails. That case sends a steer and promotes a queued message while a user_input request is pending, then checks that nothing steered, the request stays pending, and the queue order is promoted, queued, steered.
With the fix:vp test run src/orchestration-v2/CommandPolicy.test.ts src/orchestration-v2/SteeringCompletion.integration.test.ts: 35 passed.
vp run check:changed: format, lint, knip and typecheck (36 packages) pass. The full test run had 4 failures in 6 files, all outside the server and unrelated to this change:
two desktop files: Electron is not installed in this checkout;
scripts/build-desktop-artifact.test.ts, the cross-architecture Windows probe: also fails on clean main here;
GuestPage.browser.test.tsx: also fails on clean main here;
two web browser tests that pass when rerun in isolation.
Not checked: a live Claude turn on this exact commit. The same guard has run since 2026-10-28 in a downstream fork's preview build. Live checks of the guard on that build are still owed.

With Steer as the default follow-up (pingdotgg#24718), a message sent while the
active turn waits on a runtime request steers it. Claude steers with
priority "now", the SDK aborts the pending permission callback, and
AskUserQuestion resolves as "User cancelled tool execution." (an approval
as cancelled), so the user never answers it.
resolveMessageDispatchIntent now queues any steer behind the turn while
the thread has a pending runtime request, the same way it already queues
behind /compact and /logout. This covers the composer, the auto delivery
intent and sendToThread. Promoting a queued message to a steer moves it
to the front of the queue instead, so it runs right after the answer.
Restart is unchanged.

Macroscope's review found this PR approvable — Steering queues behind pending questions or approvals instead of cancelling them; promoted messages move to the front without interrupting the active run. The change preserves pending user decisions and leaves ordinary steering unchanged.

Steering messages now queue while a runtime request is pending. Promoting a queued message also leaves it queued and does not stop the active run. Tests cover dispatch policy and queue order, and the composer documentation describes the behavior.

Summary
Dispatch policy for pending requests apps/server/src/orchestration-v2/CommandPolicy.ts, apps/server/src/orchestration-v2/CommandPolicy.test.ts
The dispatch policy changes steering to queueing when a runtime request is pending. Tests cover pending user_input and command requests, restart delivery, and steering after resolution.
Queued-message promotion and behavior coverage apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/SteeringCompletion.integration.test.ts, docs/user/composer.md
When a request is pending, promotion puts the queued message first without stopping the active run. The integration test checks that messages remain queued and the request remains pending. The documentation describes the queueing behavior.
The change keeps ordinary steered or promoted messages queued while a question or approval is pending. A /goal set or /goal resume message can still interrupt the run and cancel the pending question or approval. Add the pending-request guard to both /goal stop decisions before merging.

Full details: Description check
Explanation
The description follows the repository template. It has a Problem section that explains the bug and its reproduction context (Claude steering with priority now aborts the pending permission callback). It has a Change section that lists the fix in CommandPolicy, promote-to-steer, and the composer guide. It has a Scope and approval section that links issue #26841 and explains why the fix qualifies as a focused fix. It has a Verification section with the failing-first evidence, passing test counts, and unrelated failures. It also states what was not checked. The description is complete. One note: the template asks for a link to a triaged issue or maintainer approval, and the author states #26841 is not triaged yet, so the approval basis rests on the small-fix exemption the author argues.

Explanation
Issue #26841 requires pending questions and approvals to remain pending while new messages wait and run after the answer. waitsOnPendingRequest detects pending runtimeRequests, and resolveMessageDispatchIntent changes steering to queue_after_active. The policy covers direct dispatch, composer delivery intents, and auto delivery. Orchestrator applies the same guard when a queued message is promoted to steer, and places that message first in the queue. Tests cover pending user_input and command requests, restart behavior, resolved requests, direct steering, automatic delivery, promotion ordering, and request preservation. The documented composer behavior matches the implementation.

Composer documentation explains the new queue behavior. No unrelated product behavior or unrelated files appear in the reviewed pull-request changes.

Do not interrupt a run with a pending runtime request.
A queued /goal promotion can reach this branch while a runtime request is pending. The /goal branch runs before waitsOnPendingRequest(projection). For /goal set or /goal resume, it can pass the live target to stopRunForQueuedCommand, which dispatches an interrupt. Interrupt settlement cancels pending requests owned by that run.
This is not separate from dispatchMessage. Its /goal handling can also set goalStopRunId after resolveMessageDispatchIntent changes pending steering to queue_after_active. Apply the pending-request guard to both stop decisions while preserving queue position.
