fix(server): artifact comment auto-replies arm in Claude threads - #24962
In a Claude thread, asking Claude to watch a claude.ai artifact connects the watch but never arms comment auto-replies, so "Send to Claude" on a comment never wakes the thread. After one stop, Claude Code refuses every later watch in that session. The same request in a terminal claude session works. Fixes https://git.cafe/pingdotgg/t3code/issues/24131.
Claude Code arms auto-replies only on a turn it can verify a person started. The Agent SDK leaves that to the host: its SDKMessageOrigin type says a host wrapping keyboard input must stamp { kind: "human" }, and a message without one fails closed. T3 sent every prompt without an origin, so Claude Code recorded composer messages as turnOrigin: "sdk", the same as automated input.
Change
ClaudeAdapterV2 now stamps origin: { kind: "human" } on sends and steers a user made from a client. It reuses the rule the orchestrator already applies for this (sentByUserFromClient): createdBy: "user", creationSource of web or mobile, and no scheduledTaskId. Web and desktop sends arrive as web.
These stay unstamped and keep turnOrigin: "sdk":
scheduled task runs
server-created messages, such as usage-limit resumes and the plan feedback and async answers the server relays
MCP and agent sends, including t3_thread_send and delegated children
idle compaction
The Claude replay fixtures record each prompt T3 offers, so their prompt.offer frames gained the same field. A one-off script added it to those frames only, and each fixture header now has a handEdited note saying so. Provider frames are untouched. The testkit recorders stamp the same origin, so a re-record matches.
Scope and approval
Linked issue: https://git.cafe/pingdotgg/t3code/issues/24131, open, with no maintainer comment yet. In a follow-up, the reporter (rolfilho) found that Claude Desktop records typed messages with origin: {"kind":"human"}, which puts the fix in the host.
For maintainers to confirm:
Which sends count as typed. Plan feedback and async answers start as user text but reach the adapter as creationSource: "server", so they stay unstamped. For a trust gate that is the conservative choice; widening it is a one-line change.
Queued message edits.t3_queue edits stamp agent/mcp provenance. Web and mobile edits stamp user provenance and their client source. Both the message and turn item use the latest editor. The websocket boundary overrides submitted authorship and limits queued edits to web/mobile sources; older clients without an editor stamp default to user/web. Internal edits without metadata preserve existing attribution.
Hand-edited fixtures instead of a live re-record. The edit adds one T3-side field to frames the recorder writes itself and changes no provider output.
Other providers: Codex turn/start and ACP session/prompt have no provenance field, so only the Claude adapter changes.
This awaits maintainer review.
Verification
vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts -t "human only when a user typed": the new test sends web, mobile, scheduled, server and MCP messages, each as a start and a steer. With main's adapter, web and mobile fail (expected undefined to deeply equal { kind: 'human' }) and the other three pass. On this branch all five pass.
vp test run on ClaudeAdapterV2.test.ts and the five replay integration files that read Claude fixtures (ClaudeReplayFixtures, OrchestratorReplayRecovery, OrchestratorReplayRestartBackgroundNote, ThreadFork, ThreadMergeBack): 6 files, 267 tests passed.
vp fmt --check and vp lint on changed files: clean; the only two warnings are unused-variable warnings that main already has (layerClaudeAgentSdkReplayQueryRunner in the testkit and layer in the adapter).
vp run typecheck in apps/server: no errors.
Real Claude CLI (Claude Code 2.1.310): a script drove ClaudeAdapterV2.startTurn through the real query runner and read the prompt line Claude Code wrote to its session transcript.
main, web send: turnOrigin: "sdk", origin: null
branch, web send: turnOrigin: "human", origin: {"kind":"human"}
branch, MCP send: turnOrigin: "sdk", origin: null
The Claude fix changes provider input. Queued editor provenance also drives the existing agent label; the real-client before/after captures and recording are in the verification comment below.

Takeover fixes and review: gpt-6.1-sol through the Codex harness in T3 Code.
Claude Code trusts a prompt as human only when the host stamps
origin {kind: "human"}. T3 sent every prompt unstamped, so Claude treated
composer messages as SDK input and never armed artifact comment
auto-replies. Stamp sends and steers a user made from a web or mobile
client; scheduled tasks, server continuations and MCP sends stay
unstamped.

Recorded prompt.offer frames now include origin {kind: "human"}, which
the adapter stamps on prompts a user typed. The frames were edited by
hand and each fixture header says so; provider frames are unchanged.
The recorders stamp the same origin so a re-record matches.

Macroscope's review found this PR approvable — This is a narrowly scoped Claude integration bug fix that adds human-origin metadata only to web/mobile user messages, while leaving scheduled, server, agent, and MCP messages unchanged. The remaining changes update tests and replay fixtures, with no product-default, schema, infrastructure, or static-analysis changes.

Walkthrough
The change adds conditional human-origin metadata to Claude prompts and updates replay records to include it. It also adds editor provenance to queued-run edit commands and applies that provenance to updated queued messages and user turn items.

Summary
Derive and pass Claude message origin apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter marks unscheduled user messages from web or mobile as human-originated. It passes origin metadata through message construction, including attachment and steering paths. Tests cover eligible and excluded message types.
Replay helpers, the rollback probe, and the restart integration test record human origin on user prompts. The replay expectation includes the origin field.
Update Claude transcript fixtures apps/server/src/orchestration-v2/testkit/fixtures/*/claude_transcript.ndjson
Claude transcript fixtures add human origin to outbound prompt offers. Their metadata records the manual origin edits.
The edit command accepts optional editor metadata. Client and MCP entry points identify the editor and source, and dispatch assigns web or mobile provenance.
Apply and verify queued-run editor metadata apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/ThreadManagementService.test.ts, apps/server/src/orchestration-v2/runtimeLayer.test.ts
The orchestrator applies editor metadata to the queued message and user turn item. Tests cover agent and user provenance and preservation when editor metadata is omitted.

The Claude human-origin change itself looks sound. Queued-message edit provenance has two gaps: edits from older clients can change who a queued message is attributed to, and an agent edit to a held user message can let it be released without review. Resolve both before merging.

Explanation
Issue #24131 requires T3 Claude threads to mark a typed watch request as user-started so Claude artifact comment auto-replies can arm. ClaudeAdapterV2 now computes { kind: "human" } when createdBy is user, creationSource is web or mobile, and no scheduledTaskId exists. It passes this origin to both start and steer user messages, including attachment messages. The focused test covers web, mobile, scheduled, server, and MCP messages for both paths. Scheduled, server, and agent messages remain unstamped. The live artifact flow remains unverified, but the reviewed code and regression test support the required provider input.
Full details: Out of Scope Changes check
Explanation
The changes stay within issue #24131. Adapter changes implement the Claude origin gate. Adapter tests, testkit recorder updates, replay fixture annotations, and the probe script support the new prompt field without changing provider frames. Queued-run editor provenance is connected because the latest editor determines whether a queued message is a human web/mobile message or an agent/MCP message before the adapter computes its origin. The contracts, client operations, MCP handler, websocket boundary, orchestrator projection, and provenance tests implement that attribution path. No unrelated provider behavior is changed.

at 9d733cde638d6d7c46fd8ec9bf4a21311517be0e, queued edits also carry editor provenance. t3_queue_edit marks its replacement as agent/mcp input, so it cannot inherit a human stamp. web/desktop and mobile edits identify the human editor. older clients preserve existing provenance when they omit the new optional field. this uses the existing message update events and attribution fields, with no new service or storage migration.

Keep queue ownership separate from editor provenance.
releaseUnattendedHolds uses message.createdBy, not creationSource. An MCP edit can change a held user message to createdBy: "agent" while the run remains queued. A later qualifying scheduled-run completion can then release that message without user review.
Preserve the original queue owner separately from the provenance used for Claude origin.

Preserve the existing provenance when editor is absent.
Explicit WebSocket edits should receive human provenance. However, an older WebSocket client sends no editor, so the server must preserve the message's existing provenance. The current unconditional replacement changes an MCP-authored queued message to user/web. The orchestrator applies command.editor over the stored message, and the runtime test requires an editor-less edit to keep agent provenance.
