fix(claude): honor effective bypass restrictions - #21806
Conversation
Co-authored-by: shivam <91240327+shivamhwp@users.noreply.github.com>
Resolve policy at the turn cwd and configured Claude home, including the managed-policy disk cache. Keep the effective query policy through reuse and approval callbacks, and announce actual downgrades once per thread. Co-authored-by: shivam <91240327+shivamhwp@users.noreply.github.com>
Ask for ordinary tool permissions forwarded by a bypass query regardless of queued init/status updates. Restore only plan mode on query reuse, and describe unavailable bypass neutrally. Answer forwarded MCP approvals in the existing metadata fixture. Co-authored-by: shivam <91240327+shivamhwp@users.noreply.github.com>
Explain that CLI-forwarded requests still need a person's decision under bypass, including ask rules, safety checks, and settings fallback, before queued mode updates arrive. Document the settings caveat for Full access. Co-authored-by: shivam <91240327+shivamhwp@users.noreply.github.com>
ApprovabilityVerdict: Needs Human Approval Macroscope's review found this PR requires human approval — The PR changes Claude’s production permission pipeline so Full access can downgrade to approval-enforcing modes and reused sessions can recover from restricted bypass settings. Because this changes access-control behavior across managed settings, worktrees, and session reuse, human review is warranted. You can add or adjust custom eligibility rules. Learn more. |

The two session-recovery findings are confirmed: a temporary resolution failure should be retried at the same cwd, and a rejected plan-to-bypass restoration must recover to approval mode. I am extending the existing regression scenarios before implementing those fixes; no new test cases are planned. The description/title warning used the initial generated stack template. The current body now contains the problem, implementation, original-author credit, test results, actual Linux CLI/browser proof, and explicit missing human approval. The optional failure-logging suggestion is deferred; the existing neutral user notice and focused failure tests remain, without adding log machinery to this fix. |

Cache only successfully resolved bypass availability so a transient failure only downgrades the failing turn. If Claude rejects restoring bypass after plan mode, restore default and keep requiring approval on forwarded asks. Co-authored-by: shivam <91240327+shivamhwp@users.noreply.github.com>

The current head passes all validation jobs that ran and CodeRabbit reports no actionable code comments. The body now uses the required problem/change/scope/verification structure and links the original scope discussion. Human approval is explicitly still missing; the takeover authorization does not claim product approval or an exemption. The linked-issue warning describes broader provider-state and runtime-picker UI behavior. This replacement preserves the original adapter bug-fix scope and does not claim to close that broader request. Expanding into cross-client picker behavior would need a separate product decision. The existing Full access label remains visible while the shared adapter enforces the restricted effective policy, as disclosed in the body. |

Problem
Claude can disable bypass while T3 still treats Full access as unconditional approval. That silently approves permission requests Claude intentionally forwards to the client, and a reused session can try to restore a bypass mode that Claude rejects.
Change
Resolve effective user/project/local/managed settings at the turn's working directory, including an isolated custom Claude home and the remote policy cache. Use the lower layer's effective permission policy for query options, identity, callbacks, plan restoration, and continuations. Cache successful resolutions for the latest working directory per session and recheck worktree handoff. Missing files are absent; unreadable or malformed policy downgrades the current turn conservatively and retries on the next turn. A preflight downgrade emits one existing system notice per thread.
Any ordinary tool permission request Claude forwards for a bypass query now asks the T3 user, including when init/status is queued behind the SDK callback. Ordinary unrestricted actions can execute inside Claude without calling this callback. Claude can still forward requests while bypass is enabled, including explicit ask rules and safety checks; these now prompt in Full access, so unattended agent or scheduled runs can wait for approval. Query reuse restores from plan mode while preserving Auto/default policy fallback. If Claude rejects restoring bypass after a live restriction, switch to default and retain approval handling; subsequent turns reuse default without another bypass attempt. A live restriction missed by cached preflight can prompt without a proactive notice.
Stack top: #21805 is the policy layer below this PR.
Rebuilt from #15964 by @shivamhwp. The 1,858-line original is replaced by two review layers. Its 13 net-new tests plus four rewritten cases become five net-new behavior cases in the existing adapter file; generic LRU machinery and duplicate per-round tests were discarded.
Verification
Verification: pinned main has 286 adapter cases; the integrated head passes 291/291. The initial regressions, explicit remote-cache handling, callbacks before init/while a downgrade status is withheld, and invalid bypass restoration each failed before the corresponding fix. Scoped lint/format and server-only typecheck pass. Independent parallel adversarial reviews re-read both layers and the combined diff; round one exposed the callback race and forbidden bypass restoration, now fixed without growing the case count. Post-publication review exposed cached transient resolver failures and rejected bypass restoration after plan mode. Both are fixed by extending existing scenarios: 289 pass / 2 assertion failures before, 291/291 after, preserving all 795 assertions. Final round four returned no actionable findings from either reviewer; CI and configured bot reviews are tracked on the published head.
Live Linux proof on 640e856acc, after restarting the isolated dev server: a real Claude CLI 2.1.303 query starts in unrestricted bypass and enters plan mode. After project settings disable bypass and Auto, Decline leaves a marker absent. The same query enters plan again; the following turn recovers to default and presents approval instead of failing the turn. Approve creates the recovery marker. The next turn asks again; Decline leaves its marker absent. All five turns complete with one query open. The runner logs successful mode changes only; the rejected bypass call is established by the guarded recovery path, the default restore, and the separately captured real-CLI rejection. Failed settings-read retry is proven by focused tests rather than live fault injection. Sanitized protocol and test evidence.

Scope and approval
This implements the server adapter safety/backstop portion of #15964, following its scope discussion. It does not claim to complete the broader provider-state or runtime-picker UI requirements of #4927; those need a separate product decision. No approval or contribution-policy exemption is claimed.
Human approval of the Full access downgrade, forwarded ask-rule/safety prompts (including unattended runs), and conservative fail-closed and successful-resolution cache policy remains outstanding. No enterprise account, policyHelper, native MDM, macOS, Windows, iOS, Android, Electron desktop, or remote/relay/tunnel execution was verified. The SDK resolver does not run policyHelper; native/enterprise parity is not claimed. Replay suites outside the focused adapter file can depend on host Claude/managed policy. Shared server adapter behavior covers existing clients and agent/scheduled paths; other providers and contracts are unchanged.
Published-head validation: lint, build/typecheck, packages, all eight server shards, aggregate Check, Macroscope correctness, and CodeRabbit pass. Scoped web/native/Rust/release jobs skipped. All inline review threads are resolved; the approvability check is neutral because human policy approval is still missing. CodeRabbit’s linked-issue warning concerns the broader UI scope above.
Models/harness: tests gpt-6-astra / Codex; implementation gpt-6.1-sol / Codex; review gpt-6-sol / Codex and claude-opus-5-5 / Claude.
