ApprovabilityVerdict: Needs Human Approval Macroscope's review found this PR requires human approval — The PR adds a narrowly scoped, tested fallback for Claude permission modes while preserving existing behavior for current callers. Because it governs permission bypass and approval enforcement, the change warrants human review even though its current production call sites use the compatibility default. You can add or adjust custom eligibility rules. Learn more. |

Problem
Full access currently derives Claude permission mode only from T3's requested runtime policy. When Claude disables bypass, its fallback still needs approval even if T3's policy says never ask.
Change
This first layer lets the existing query-policy compiler accept effective bypass/Auto availability. Restricted Full access becomes Auto, or default when Auto is also disabled, installs approval handling, and omits dangerous bypass flags. Other requested modes keep their existing behavior. The next layer resolves settings and uses this policy throughout the adapter.
Stack base: #21806 is the dependent integration layer.
Rebuilt from #15964 by @shivamhwp. The original 1,858-line change is replaced by policy and integration layers; generic bounded-cache machinery and its tests were discarded.
Verification
Verification: one focused case in the existing adapter suite covers downgrade, forced approval, and unchanged explicit Auto. The complete integrated stack passes 291/291 adapter cases, scoped lint/format, and server-only typecheck. The initial behavior regressions failed before implementation. Independent adversarial reviews cover both layer diffs and the combined change; final round four returned no actionable findings from either reviewer. CI for this published head is tracked separately.
Scope and approval
Original scope discussion. Human approval of the Full access downgrade and fail-closed policy remains outstanding. This is unmerged review work; enterprise/MDM and native platforms have not been verified. Integrated proof belongs to the next layer.
