docs: correct rollout tracker — provisioning already onboarded - #34

Verifying live state (not the tracker) showed kyle-sexton/provisioning already runs the full ci-workflows pipeline (all PowerShell-appropriate lanes by SHA → ci-status), with standards configs at root and a per-repo ci-gate ruleset. The tracker still listed it Bare/next. Mark it Integrated, advance the sequence (next: claude-code-plugins, the only genuinely greenfield repo left), and tick the plan.md Phase 6 box. Also refine the org github-iac note: the requires-ci tag is merged (PR #16); only the live `pulumi up` apply remains. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>
