fix(combo): fail over zero-output max_output_tokens terminal - #8222
When a combo target reaches its output token limit before producing any visible output or running side effects, combo routing previously treated the terminal response as final and returned an empty incomplete response to the client instead of attempting the next candidate target.
On remote compaction tasks, Codex requests were additionally blocked from failing over because classifyZeroOutputTokenLimit was gated on !routedCompaction.
Non-runTurn streaming combo targets were also committed because preflightComboStreamResponse explicitly treated max_output_tokens as a non-retryable semantic reason.
Fixes applied:
In src/server/responses/run-turn-execution.ts, removed the !routedCompaction guard so remote compaction tasks can safely fail over when a provider hits max_output_tokens before producing the summary.
In src/server/responses/combo-stream-preflight.ts, updated retryableZeroOutputTerminal to classify response.incomplete with reason max_output_tokens as retryable whenever output is empty. failedTerminalResponse maps it to a 502 with code zero_output_token_limit, preserving token usage.
Replay remains strictly prevented whenever output deltas or items were already emitted before the limit.
Verification
bun test tests/routing/combo-stream-preflight.test.ts tests/adapters/run-turn-queue.test.ts tests/server/server-combo-zero-output-failover.test.ts: 113 pass / 0 fail.
Verified remote compaction failover on zero-output max_output_tokens.
Verified stream preflight failover on zero-output max_output_tokens.
Verified that max_output_tokens after visible output does not fail over.

A provider could hit its output limit before emitting any text or tool
calls. Combo routing previously treated that terminal response as final
instead of trying another target.
Introduce classifyZeroOutputTokenLimit to check the first meaningful event:
when an incomplete response (with reason max_output_tokens) or done response
(with stopReason max_tokens/length/max_output_tokens) arrives before any
output or side effects, classify it as a 502 zero_output_token_limit error.
This allows combo routing to fail over to the next candidate target while
preserving usage data and avoiding replay after visible output, tool side
effects, or compaction.

Walkthrough
Eligible zero-output token-limit events are classified as upstream errors. Combo routing uses this classification to dispatch a backup target, while preserving usage data from errors.
Eligible incomplete and done events produce a 502 zero_output_token_limit error. The classifier includes usage when present. Tests cover qualifying events and exclusion conditions.
Apply classification in combo routing src/server/responses/run-turn-execution.ts, tests/server/server-combo-zero-output-failover.test.ts
Combo routing checks the classification before existing tool checks, except during routed compaction. Streaming preflight and buffered failure paths bind usage from error events. Tests cover backup dispatch with no output and no backup dispatch after visible text.

Zero-output limits can trigger backup routing, but exhausted attempts return an error instead of the original incomplete response and usage. This bounded compatibility issue should be fixed or explicitly accepted.

Preserve the limit terminal when failover is exhausted.
If the last combo target reaches max_output_tokens before emitting an event, the classifier converts the terminal to a 502. The client loses the incomplete reason and usage. Preserve the original terminal and return it when no eligible fallback remains.

Avoid cooling targets for request-specific output caps
In a multi-target combo, when this classifies a candidate’s zero-output token-limit terminal, the resulting 502 zero_output_token_limit falls through comboFailureCooldownScope to target scope, so advanceComboAfterFailure records the configured cooldown (60 seconds by default). A later request with a larger output budget then skips this otherwise healthy target even though only the prior request’s cap failed; treat this code as request-local for cooldown purposes while retaining the hop for the current request.

Document zero-output token-limit combo failover
This first-event classification turns incomplete/max_output_tokens into a 502 so a combo can hop, but docs-site/src/content/docs/guides/combos.md:796-802 still says explicit max_output_tokens incompletes remain terminal. Users choosing a combo will therefore be told the wrong retry behavior; document the zero-output exception and keep translated copies consistent.
…ht and compaction
Support zero-output max_output_tokens failover in preflightComboStreamResponse
when no output items or text deltas were emitted before the terminal.
Additionally allow compaction turns to fail over zero-output token limit
terminals by removing the !routedCompaction exclusion in run-turn execution.
