Summary

When an upstream model hits max_output_tokens or max_tokens, downstream consumers may prefer to treat a clean, truncated text-only response as completed. Following maintainer review, this PR provides an opt-in, narrowed completion policy strictly for plain-text Responses delivery:

  1. Opt-in & Configuration:

    • completeOnMaxOutputTokens: optional boolean in config schema and types, default false (absent, false, and malformed values leave it off).
    • Scoped request-locally and passed only to translated Responses delivery in adapter-delivery.ts and run-turn-execution.ts.
    • Chat Completions (length), Anthropic Messages (max_tokens), and native Responses passthrough (including native WebSocket framing) are completely unaffected and retain their existing stop semantics.
  2. Plain-Text Allowlist & Exclusions (src/bridge/max-output-completion.ts):

    • Only nonempty plain text, optional message boundaries, and heartbeats qualify.
    • Raw max_tokens, length, and max_output_tokens stops qualify for conversion to completed (omitting incomplete_details).
    • Exclusions: Any tool call or hosted operation (even if completed or closed by bridge cleanup), reasoning (hidden or signed progress), structured textFormat (json_schema / json_object), compaction, explicit endTurn: false, errors, pause_turn, and context-window exhaustion are strictly excluded and remain incomplete/failed.
    • Model text is untouched: no Markdown repair or synthesis; valid empty fences and open Markdown blocks remain exact.
    • Provider continuation state and usage survive both buffered and streamed delivery.
  3. Documentation & SSOT:

    • Documented in docs-site, structure/INDEX.md, structure/config.md, structure/manifest.json, and structure/transports/max-output-completion.md.
    • Layout registration in scripts/test-layout/layout-responses.json and tests/fixtures/test-layout-expected.json.

Verification

Validated final head 8b26d105da9ca3f1a7597004c55e79adb1d58051 on Windows with Bun 1.4.4 and OCX_TEST_NO_QUEUE=1. Ordinary merge of origin/dev at b16bc0978b2eef1799f38042084809b5a0cd11bf; no rebase or force-push. The branch contains that dev tip (0 behind at validation).

  • bun test --no-env-file --isolate tests/responses/graceful-max-output-tokens.test.ts tests/responses/graceful-max-output-tokens-exclusions.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/file-size-ratchet.test.ts --timeout 60000: 167 pass / 0 fail.
  • bun run typecheck, bun run structure:check, bun run privacy:scan, bun run --cwd docs-site build and git diff --check origin/dev...HEAD: exit 0. Frozen dependency installation completed before validation.
  • Documentation follow-ups: 8b26d10 adds the server-fields table row and maps the bridge builders to the structure contract; bun run structure:index regenerated INDEX.md. The runtime policy remains the preserved 52d2abd rework; its earlier maintainer receipt records 111 pass / 18 fail before and 129 pass / 0 fail after. Native WebSocket files match dev exactly.
  • Full-suite exception: this shared Windows host has concurrent carry lanes. Per the owner's instruction, only the listed focused regression files ran, with one Bun test process at a time. bun run test and unrestricted test:changed were not run; broader coverage remains with PR CI.
  • This refresh adds no runtime behavior fix. Earlier red-to-green results remain historical evidence in the linked maintainer carry/review comments; this lane did not rerun a pre-fix baseline. No failed checks were rerun and no workflows were dispatched or approved.
  • NOT RUN: full local suite, live providers, native-client/platform acceptance. Current-head hosted CI must finish successfully before an owner-authorized merge.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. Existing maintainer review is retained; this refresh makes no runtime security-boundary change.

condition from gracefulComplete. This keeps responses with a tool call open at
truncation marked incomplete while preserving graceful completion for other

closeOpenMarkdownBlocks only to the final message that was still open at
truncation, leaving earlier completed messages unchanged. Use a local type guard

- Line 1268: Update both gracefulComplete computations in the SSE handling and
buffered response builder to allow graceful completion only when no

Maintainer review of head 22f95b51: not merging in this form. The current head already defaults completeOnMaxOutputTokens to off (the description still says on), but the core concern is that a token-limit stop is reported as a completed turn. Codex distinguishes response.incomplete from completion, and its compaction collector requires a completed response with exactly one compaction item, so hiding truncation changes client behavior.

Blocking findings from an independent review, each reproduced locally:

  1. High: compaction history is fabricated or truncated. src/bridge/response-json.ts:638 and src/bridge/sse.ts:1292 substitute an empty-summary placeholder even with the setting off. A real compact-handler probe returns 502 on dev and 200 with replacement history on this head. With the setting on, truncated summaries are published as compaction items.
  2. High: WebSocket terminal framing contradicts itself. src/server/responses/codex-ws-exchange.ts:652 encodes before rewriting and :781 sends the stale bytes, producing event: response.completed with data: {"type":"response.incomplete",...}.
  3. High: eligibility hides truncation. sse.ts:1268 and response-json.ts:621 can declare completion with unfinished tool calls, and the Chat (length -> stop) and Messages (max_tokens -> end_turn) handlers change too.
  4. Medium: response-json.ts:646 rewrites earlier completed messages (unlike SSE), and the helper changes a valid empty fenced block.
  5. Medium: sse.ts:1370 drops event.providerState; the Cursor state callback receives undefined.
  6. Medium: ws-upstream.ts:256 calls loadConfig() after the WS reservation, bypassing request-local policy.
  7. Medium: tests/responses/graceful-max-output-tokens.test.ts is not registered in the layout maps, so the layout guard fails.

A version that could land would drop every compaction exception and the native WS rewrite, leave model output unmodified, keep provider state, and apply only to explicitly enabled Responses text-only output with no unfinished operations or structured output, with tests for each exclusion. Leaving this open for that rework; it is deferred from the current integration batch.

omar391 and others added 5 commits October 28, 2026 15:36
When an upstream model hits max_output_tokens / max_tokens, Codex CLI and
consumers can fail (e.g. 'Error running remote compact task: Incomplete
response returned, reason: max_output_tokens', retry loop exhaustion, or
parsing errors on open markdown code fences).

- Adds closeOpenMarkdownBlocks to close dangling fences and paired tags.
- Introduces completeOnMaxOutputTokens config option (defaults to true).
- In bridge SSE, response JSON, and codex-ws exchange, gracefully completes
truncated turns with status completed, repairs markdown fences, and
synthesizes compaction items when compaction was requested.
Preserve the contributor history while removing compaction exceptions, native
WebSocket rewrites, and output repair. Exclude structured output and all non-text
operations before bridge cleanup; retain provider continuation state and usage.
Register bridge and real delivery regressions in the existing layout shards.

Maintainer-side rework for review (no merge), following the 2026-10-27 review. The branch is rebased onto dev fdc6dd82d and now points at 6ddde6dac4adcad25c12fa23448ad4ccdbc42d60. Your three commits keep your authorship; two commits on top narrow the change to the landing conditions in that review.

How each blocking finding is resolved:

  1. Compaction: compaction is excluded (src/bridge/max-output-completion.ts:12). A real compact-handler probe returns the same 502 status and body as dev with the setting absent, false and true; no replacement history is produced.
  2. Native WS framing: the WS rewrite is removed; codex-ws-exchange.ts and ws-upstream.ts are identical to dev.
  3. Eligibility: an allowlist accepts only plain text events (max-output-completion.ts:35), gated to Responses delivery at all four sites. Chat keeps length and Messages keeps max_tokens.
  4. Output modification: markdown-block-repair.ts is deleted; model text, earlier messages, open Markdown and empty fences are left untouched.
  5. Provider state: converted SSE terminals pass event.providerState (src/bridge/sse.ts:1345); the Cursor callback tests cover both terminal forms.
  6. Config after reservation: removed with the WS change.
  7. Layout: both test files are registered in scripts/test-layout/layout-responses.json and the expected fixture.

completeOnMaxOutputTokens stays default off (malformed values read as off) and is request-local. Structured output, compaction, tools and hosted search, reasoning, unfinished operations, pause_turn, context exhaustion, errors and a missing terminal are all excluded, each with a test.

Verification (Bun 1.4.4): the feature matrix is 111 pass / 18 fail on dev and 129 pass / 0 fail here. An independent review of this head found 0 blocking issues; its run of the feature, compaction, terminal-refusal and protocol encoder suites was 295 pass / 0 fail, layout guards 29 pass / 0 fail, and typecheck, structure check and privacy scan pass.

Not run: full local suite, live providers or native clients. Current-head PR CI has been approved. Please update the PR description to match the narrowed scope (the earlier text still describes compaction handling and Markdown repair).

Merged dev b16bc0978b2eef1799f38042084809b5a0cd11bf with an ordinary merge; original author history and Co-authored-by credit are preserved. Focused verification: bun test --no-env-file --isolate tests/responses/graceful-max-output-tokens.test.ts tests/responses/graceful-max-output-tokens-exclusions.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/file-size-ratchet.test.ts --timeout 60000 — 167 pass / 0 fail. Typecheck, structure check, privacy scan, docs build and diff check: exit 0.

  • Documentation follow-ups: 8b26d10 adds the server-fields table row and maps the bridge builders to the structure contract; bun run structure:index regenerated INDEX.md. The runtime policy remains the preserved 52d2abd rework; its earlier maintainer receipt records 111 pass / 18 fail before and 129 pass / 0 fail after. Native WebSocket files match dev exactly.

Review threads: 11 replied to and resolved with fixing-commit or source evidence. No new runtime behavior fix was needed, so this lane has no new red-to-green claim; the existing maintainer comments retain the prior regression evidence.

NOT RUN: full local suite (shared-host exception; focused files only), unrestricted test:changed, live providers, native clients and cross-platform acceptance. Hosted CI is checked once after publication and remains an owner gate; this lane cannot approve fork CI or merge. Spanish language/render acceptance, where applicable, remains separate.

CI note for head 8b26d105d: the test 3/4 failure, and the ci aggregate that depends on it, is unrelated to this change.

The only failing case is compiled bundled CLI verifies real native recovery and ignores a protocol-speaking renamed outsider (tests/service/service-desktop-recovery.test.ts:333). Its fixture compiles a native helper with rustc under a 30-second spawnSync timeout. On this runner the compile was killed at the limit: the exit code was null and the test ran for 30017 ms. Every other test in the shard passed.

This PR touches only the Responses bridge token-limit completion path, its config field, docs and tests. It does not touch the desktop recovery service, its fixture or the CLI bundle. The previous head 6ddde6dac passed all 30 checks.

The fix belongs with the desktop-recovery test owners: give the fixture a precompiled helper, or keep the compile out of the per-test timeout. I have not rerun the job to turn it green. All review threads on this PR are resolved.

Maintainer-side conflict refresh (no merge): a9a99cc2e merges origin/dev into this branch on top of 8b26d10, as a merge commit with no rebase or force push. The conflicts came from #7882 (LM Studio single leading system message) landing on dev:

  • structure/config.md: this PR's completeOnMaxOutputTokens link and dev's LM Studio link shared one summary line. Both links are kept.
  • tests/fixtures/test-layout-expected.json: dev's first line, with the new LM Studio registrations, is kept, and this PR's two graceful-max-output-tokens* registrations follow it.

The PR's diff against dev is otherwise unchanged. Local checks on the merge (Bun 1.4.4): test-layout guards, both graceful max-output suites and the LM Studio policy suite pass (163 / 0), and typecheck, structure check and privacy scan pass.

dev currently fails the repository file-size ratchet on scripts/test-layout/layout.json (2000 lines); #7943 addresses that. A full shard on this head may show that same dev baseline failure until #7943 lands.

The push reset the readiness checklist; please re-tick it for this head once CI completes.

Maintainer decision (lidge-jun merge lane): merging at exact head 1fee7a6. the carry lane marked it ready; the maintainer lane merged dev to resolve a test-layout registration conflict. The earlier test 3/4 failure was the inherited flaky test; all 30 checks pass on this head. That push reset the readiness checklist; this decision stands in for it. Required PR CI is green on this head, there are no unresolved review threads, and the dev lane=all batch 40831805734 is green. A local union simulation of this merge set on today's dev passes typecheck, structure, file-size and test-layout gates. Thank you, @omar391.
