fix: compare Codex and remote workspace file identity with bigint stat - #8023
Continues #8019 (fix: compare file identity with bigint stat). NTFS file IDs can exceed 2^53, where adjacent IDs are equal as JavaScript numbers. This PR moves the Codex-side and remote-workspace runtime identity checks to bigint stat results. Each observation is still a single stat call made with { bigint: true }, and identity is compared with sameFileIdentity.
Shim runtime checks:shim-fingerprint (sameFingerprint compares a bigint identity, and a fingerprint without one compares unequal), shim-state-file (read guard), shim-restore-lock, shim-wrapper-write (the staged wrapper's identity is captured from a bigint lstat and drives ownedWrapperFingerprint and wrapperInodeIsOurs) and shim-inspect.
Remote workspace:remote-control/workspace-executor (opened file and approved root).
Millisecond values: new statTimeMs(ns) in src/lib/file-identity.ts derives *Ms from bigint *Ns as whole seconds × 1000 plus the nanosecond remainder ÷ 1e6, which is how the runtime computes its numeric value. Plain Number(ns) / 1e6 differed from the numeric mtimeMs for about a quarter of files sampled on Windows, and some of these values are compared exactly against numeric or persisted stats. Every such derivation now uses the helper, including two from fix: compare file identity with bigint stat #8019 (oauth/store, service/state-lock).
Unchanged: the persisted shim state keeps its numeric dev/ino/*Ms fields; changing that stored format needs a versioned migration.
Verification
Behavioral regressions inject a replacement whose ino differs only above 2^53. Each fails against the previous source and passes with this change, in the existing suites for:
shim fingerprint and state read, shim restore lock, shim wrapper ownership (2 cases)
remote workspace file and root
tests/lib/file-identity.test.ts: statTimeMs equals the numeric lstatmtimeMs and ctimeMs for fixed fractional utimes values, plus a deterministic case where whole-number division would round differently.
tests/lib/file-identity-callers.test.ts: the tripwire now covers these modules. The shim files are excluded with a comment because they keep numeric persisted fields. Substituting the previous versions of main-account, history-provider, workspace-executor and coordinator-doctor fails those four scopes.
Windows focused suites: history provider 105, Windows shim revision 20 (+2 skip), native residue 86 (+2 skip), transition state 27 (+1 skip), coordinator doctor 8, main-account refresh 14, native-main owner lifetime 11, remote workspace 18 (+1 skip), service ownership state 47, xai refresh lock 10. All pass with 0 fail.
bun run typecheck, bun run structure:check, bun run privacy:scan, file-size ratchet and test-layout guards: pass.
NOT RUN: full local suite (shared host). Unix-only shim and overlay cases are skipped on Windows; Linux runs in this PR's CI, and macOS in the batched lane=all on dev.

Bug Fixes
Improved detection of file changes during reads and access checks, including when file identity values exceed JavaScript’s precise-number range.
File and lock validation now more reliably distinguishes files with closely matching identities, helping prevent actions on files that have changed unexpectedly.
File timestamps are handled more accurately when checking whether files have changed.
NTFS file IDs can exceed 2^53, where adjacent IDs are equal as JS numbers. Take dev/ino from bigint stats in the Codex home, coordinator, main-account, native-main, residue, history, shim runtime and remote-workspace identity checks. Persisted shim state keeps its numeric fields.
…tity as bigint
Derive *Ms from bigint *Ns with the runtime's own formula so values still match numeric and persisted stats; carry bigint identity through the shim wrapper ownership and inspection checks; fingerprints without identity compare unequal.

Walkthrough
The change updates Codex and remote-workspace file checks to use bigint filesystem identity values and shared timestamp conversion. It adds regression tests for file identities above JavaScript’s safe-integer limit, where distinct device or inode values can otherwise convert to the same number.
The new statTimeMs helper converts bigint nanosecond timestamps to milliseconds. Lock snapshots use the helper. Utility tests cover timestamp conversion and file-identity callers.
Journal, config, credential, and auth-file checks use bigint metadata and shared identity comparisons. Tests simulate adjacent large inode values and verify the relevant result or error.
Coordinator, history, residue, and transition-state checks use bigint metadata to detect identity changes. Tests simulate large adjacent inode values and check the resulting classifications or errors.
Shim fingerprints, wrapper ownership, state reads, and lock checks use bigint identities. Tests cover identity changes during path probes, reads, ownership checks, and lock cleanup.
Approved-root and opened-file checks use bigint identities and normalized birthtimes. Tests cover changes to file and root identities above the safe-integer limit.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Move the scratch directory to tmpdir() and clean it with removeTreeWithRetry.
This test creates its fixture under repoPath(".tmp", "ino-audit"). The cleanup on Line 70 uses fs.rmSync and removes only the identity-* child. The .tmp/ino-audit parent stays in the working tree after every run.
The first test in this file already uses mkdtempSync(join(tmpdir(), ...)) with removeTreeWithRetry. That helper retries removal on Windows, which is the platform this PR targets. If a failure occurs before the try on Line 69, the fixture is also leaked.
Remove the unused repoPath import on Line 9 if nothing else uses it.

Consolidate the duplicate timestamp conversion as an optional cleanup.
milliseconds in src/codex/coordinator-doctor.ts:40-42 and statTimeMs in src/lib/file-identity.ts:5-7 have equivalent behavior. The 1000 and 1e3 literals are equal, so no reachable value differs. The repository instructions do not require this helper.

Compare size and mtimeNs as exact bigints in sameStat.
sameStat compares two observations of the same file, but it compares lossy projections of the values instead of the exact values:
Number(left.size) === Number(right.size) (line 108) rounds sizes above Number.MAX_SAFE_INTEGER. Sizes 2^53 and 2^53 + 1 both become 9007199254740992, so sameStat can return true after the file size changed. classifyHistoryDatabase calls sameStat after the query (line 599), so the result can be clean where it should be indeterminate. SQLite ignores bytes after the header's page count, so trailing data does not block the open or the query. The case still needs a file above 2^53 bytes, which needs a filesystem that supports such sizes, plus a same-millisecond rewrite. The risk is narrow.
milliseconds (lines 99-101) duplicates statTimeMs in src/lib/file-identity.ts:5-7. Both compute the same value. milliseconds also compares floating-point milliseconds when the exact nanosecond values are available.
The sibling checks compare size and mtimeNs exactly, for example src/codex/codex-home-owner.ts:73, src/codex/orca-auth-source.ts:47, and src/codex/inject/bounded-config-reader.ts:55. The millisecond contract in statTimeMs exists for comparisons between a bigint observation and a numeric or persisted value. sameStat compares two bigint observations, so that contract does not apply here. Exact comparison is stricter. A sub-millisecond change that the millisecond check accepted can now return indeterminate.

Maintainer integration (lane L1). Squash-merging into dev under the maintainer-integration exception in MAINTAINERS.md. This records the decision; it is not a self-approval.

Security review (dedicated, independent of the author; touches lock, credential-reader and shim ownership checks):
Runtime identity checks in these modules and in shim-wrapper-write, shim-inspect and shim.ts use bigint identity end to end.
When sameFingerprint finds a missing identity it compares unequal, which leads callers to defer or preserve, never to unlink.
No bigint reaches JSON serialization on the traced paths.
Residual security concern for this scope: none. Out of scope and tracked for follow-up: the persisted shim-overlay numeric identity (needs a versioned format migration), plus cli-update-*, cli-install-provenance, account-store, catalog/heal-observation and lab/*.
Code review: three independent rounds.
Round 1 (MUSTFIX, from both reviewers): Number(ns)/1e6 disagreed with numeric *Ms, and shim ownership and inspection still compared numeric identity. Fixed with statTimeMs and bigint shim identity, with red→green tests.
Round 2 (NEAR-PASS): a tautological assertion, now a deterministic case.
Final delta review (NEAR-PASS): confirmed the constants independently. Its residual is that macOS exactness has not been observed yet; the batched lane=all covers it.
Batch evidence:lane=all 41130108315 on dev 86b72a689 (#8019) succeeded. Union check: dev is still at base 86b72a689. shim.ts is 1,165 of its 1,246-line cap, and the ratchet and layout guards pass.
Regression tests from #8019/#8023 matched the spied path by raw string; code under test stats canonical paths (/private/var on macOS, long names for an 8.3 short TEMP on hosted Windows), so the injection never applied (lane=all run 41136081084). Add tests/helpers/same-path.ts and use it for every target selection.
