Summary

Refs #7456.

A Responses request translated to Chat can forward tool_choice: required yet receive only text and incorrectly end as completed. Wrap the existing Chat parser with the shared mandatory-choice predicate: a clean terminal without a complete client tool call becomes non-retryable required_tool_not_called, preserving usage and previously delivered output. The existing response bridge retains selected-name authorization and namespace/code-mode alias restoration; the adapter does not introduce a second name resolver.

The guard does not resend, buffer an entire stream, add a second runTurn preflight, or invent a provider/model allowlist. JSON and SSE, direct and combo routes share the same verdict. Already admitted buffered Responses retain their existing HTTP 200 envelope convention with status: failed; streaming ends with response.failed. Auto/none and existing incomplete/error outcomes remain unchanged.

Scope: translated Chat client-function/custom/tool-search catalogs. Catalogs containing proxy sidecars stay with their existing execution owner. Native passthrough, runTurn adapters and a global capability/fallback policy remain outside this patch. This is a scoped fix, not the reporter's staging allowlist or a claim that every #7456 route is covered. The issue remains open.

  • Failure before: new adapter regression 16 passed, 4 expected failures (required/named requests with no call, JSON and SSE).
  • bun test tests/adapters/openai/openai-chat-required-tool.test.ts tests/responses/responses-required-chat-tool.test.ts tests/adapters/openai/openai-chat-parallel-stream.test.ts: 71 passed, 0 failed. Includes a complete synthetic 151-function call/result cycle, direct/combo JSON/SSE, exact one-send/no-fallback-after-text, upstream selection preservation and usage, valid dotted/bare namespace echoes, and wrong named calls rejected by the existing bridge before executable output.
  • GOMAXPROCS=2 bun test tests/adapters/translator-budget.test.ts: 21 passed, 0 failed, including the real compiler positive/negative adapter-budget contract. The initial bare test environment lacked a usable Bun child; a later run hit the native compiler thread cap. The final bounded child environment fixes PATH and lowers compiler concurrency without raising any cgroup cap.
  • bun test tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/file-size-ratchet.test.ts: 36 passed, 0 failed. Both maps registered; no size cap raised. The initial layout-map line overflow was corrected.

  • bun scripts/structure-ssot.ts, bun scripts/privacy-scan.ts, git diff --cached --check: passed.
  • Docs build via bun node_modules/astro/bin/astro.mjs build in docs-site: 585 pages, 83,068 internal links, complete. Reused an existing isolated frozen dependency installation after verifying identical docs manifest and lockfile; no new install claimed.
  • Unverified locally: whole-project bun x tsc --noEmit hit the unchanged 300-second local cap, including a retry with lower GOMAXPROCS=2. This is not a passing typecheck. Current-head hosted typecheck remains required.
  • Full suite/import-connected broad run deferred under the documented shared-host resource exception. Focused behavior, parallel parser, translator budget, layout and ratchet checks above are executed; the selected Ubuntu hosted CI has since passed as recorded below, while unselected macOS/Windows paths remain unverified. This PR is ready for independent review; no self-approval, merge or release is recorded.

The implementation updates the mapped adapter contract, public wire reference and ADR-7456. Protected local runtime file existence/ownership/mode/hash checks remain unchanged.

Hosted follow-up on final head cd7dce1513775d7bcde9dac1702912e7ac24279c: Cross-platform CI, Catalog contracts, and React Doctor completed successfully. The gates job executed Typecheck successfully; all four selected Linux test shards and the docs build passed. Unselected OS jobs are not universal platform validation. Ready for independent review; no self-approval or integration.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.
    • No auth, credential, dependency, release or workflow behavior is changed. Ordinary privacy checks passed; independent review remains pending. No Codex Security scan was run.

Summary by CodeRabbit

  • New Features
    • Translated Chat requests that require a client-side tool call now fail with required_tool_not_called if the upstream response completes without the required call. Usage and already-streamed text are preserved.
    • Streaming Responses end with response.failed; buffered Responses return status: "failed". This failure does not trigger fallback replay.
    • Existing error and truncation outcomes remain unchanged, as do requests that do not require a tool call.
  • Documentation
    • Documented required tool-call behavior and cases where existing behavior remains unchanged.

Walkthrough

The OpenAI Chat adapter now detects clean completions that omit a required client tool call. It replaces those completions with a non-retryable required_tool_not_called error. Combo stream preflight does not retry that error. Tests and documentation cover the behavior and its exceptions.

Changes: Required Chat tool results

Layer / File(s) Summary
Detect and enforce required tool calls
src/adapters/openai-chat/required-tool-result.ts, src/adapters/openai-chat.ts
The adapter wrapper checks whether a request requires a client tool call. If a clean, non-truncated completion omits that call, the adapter emits a non-retryable 502 error. It applies the guard to streaming and buffered responses.
Keep required-tool failures out of fallback
src/server/responses/combo-stream-preflight.ts, tests/responses/responses-required-chat-tool.test.ts
Combo stream preflight treats required_tool_not_called as non-retryable. Tests check that a missing required call does not trigger fallback or replay.
Validate and document the behavior
tests/adapters/openai/openai-chat-required-tool.test.ts, tests/responses/responses-required-chat-tool.test.ts, tests/fixtures/test-layout-expected.json, scripts/test-layout/layout-chat.json, docs-site/src/content/docs/reference/proxy-formats.md, structure/decisions/ADR-7456-mandatory-chat-tool-results.md, structure/providers/chat-required-tool-results.md, structure/INDEX.md, structure/manifest.json
Tests cover required, missing, and mismatched calls; streaming and buffered responses; continuation; and preserved outcomes. Documentation describes the contract, exclusions, and response statuses. Test-layout mappings and structure entries include the new tests and documentation.

49-59: Approve the buffered terminal replacement.

The wrapper releases the original done event and retains the replacement. This keeps the budget retention accounting balanced. events[i] is replaced in place, so the event order is preserved.

35-42: 🩺 Stability & Availability

The reported race is not present in the repository request paths. Native Chat creates a fresh adapter for each request. The Responses path calls resolveAdapter, which directly constructs a new registered adapter without caching. Request B therefore cannot overwrite request A's required flag through these paths.

The filename matches the enforced pattern ADR-NNNN-[a-z0-9-]+.md. Decision records are also explicitly excluded from the manifest parity check, so this ADR does not require a manifest.json entry. Its owning document links it from structure/providers-and-adapters.md:597.

593-597: 📐 Maintainability & Code Quality

The document is 597 lines, below the configured 600-line budget. The manifest maps providers-and-adapters.md to src/adapters/, and the index maps src/adapters/openai-chat/ to providers/mimo-bare-calls.md. No missing map entry or failed structure contract is established. This is a validation reminder, not an actionable finding.

⚠️ Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Fix: a tool call that spans an error or truncation is handled correctly, but a stray tool_call_start without tool_call_end can never satisfy the guard.

The fulfilled flag is set only on tool_call_end. The Chat adapter emits start, delta and end together in flushToolCalls, so this is consistent today. The guard depends on that ordering, though. Document the assumption in a short comment so a future adapter change does not break it silently.

The code is otherwise correct. failed is latched, so a later done cannot override an earlier error or incomplete. isTruncatedStopReason keeps the length and content_filter outcomes.

Independent 2.89.0 backport validation of the pinned guard

RHODIZ operator follow-up for #7456. I tested the exact required-tool-result.ts from commit cd7dce1513775d7bcde9dac1702912e7ac24279c against a clean, isolated copy of the released 2.89.0 npm source, adding only the adapter import/wrapper. This was not a wholesale replacement with current dev, and the production service was not patched.

Reproduction and regression results:

  • The two new PR test files against unpatched 2.89.0: 27 pass / 8 fail. The failures reproduce missing mandatory-call false success through both adapter and Responses routes, buffered and SSE.
  • The same tests after the narrow backport: 35 pass / 0 fail.
  • A broader set of 15 adjacent adapter/Responses files, including those 35 cases: 406 pass / 0 fail. Coverage includes bounded tool names, dangling calls, parallel streaming, EOF/error handling, reminted call IDs, tool-result images, allowed-tool choices, and undeclared-tool rejection.
  • An additional, disjoint synthetic 11-target acceptance file: 24 pass / 0 fail. Twenty-two cases select each possible winning position 1–11 in JSON and SSE after earlier targets return pre-output 503, preserving all 151 declared functions and a complete call/result continuation. Two negative controls verify that visible text without the required call fails and does not replay into any of the ten remaining targets.

That is 430 distinct passing cases across the broader and additional suites; the initial 35-case run is not counted twice. The 11-target checks use injected synthetic upstreams, not live provider/account certification.

Strict source typecheck (tsc --noEmit, TypeScript 7.0.2) passed. Bun 1.4.2 bundled the patched CLI successfully; an isolated executable smoke reported opencodex 2.89.0. Guard SHA-256: 4ef8291e751342e0538bc052d62a3cb10d30a2253d4f99479c24e718ec6f1cfa; narrow backport patch SHA-256: 60438900851c014c39bc4270d12600f419250cbd1b6160da255d87686c26d48e.

The unchanged production 2.89.0 service was rechecked separately: NVIDIA required with one inert function still returns HTTP 200 / completed with zero function calls in both JSON and SSE. Thus this is positive isolated backport evidence, not a claim that production is fixed, that the complete upstream CI passed, or that missing mandatory calls can safely be replayed after output. No credentials or user request contents are included.

Thanks for the carefully bounded backport evidence. The 35-case before/after reproduction and the distinct 430-case synthetic coverage support the narrow mandatory-call guard; I am treating these as your independently reported results, not as tests run in this checkout or proof of a production rollout.

This PR remains open at cd7dce1513775d7bcde9dac1702912e7ac24279c, awaiting independent owner review. The no-replay-after-output boundary and the separation from provider/tool-count compatibility claims remain unchanged. #7456 should stay open until the fix actually lands.

Triage note (2026-10-24, against dev at 2494848).

Needs fixes before this can land: resolve conflicts; preserve the adapter cap; relocate/rewrite the six added contract-document lines to stay within 600 lines; validate the resulting head and obtain independent owner review.

Still needed on dev: yes. The parser still emits clean done without this mandatory-call guard (src/adapters/openai-chat.ts:661).

Exact-head CI at triage time: cd7dce1 — Ubuntu PR CI passes, including four test shards and aggregate ci, run 39552739266. lane=all on exact head: n.a.; no manual run found.

Please refresh onto current dev when addressing these; heads older than dev need a fresh exact-head CI run before review.

Resolve layout conflicts by registering both tests in the layout-chat shard, and move the contract section from the capped structure/providers-and-adapters.md into structure/providers/chat-required-tool-results.md (manifest + regenerated INDEX).
Combo stream preflight treated every zero-output response.failed as replayable, so a required_tool_not_called verdict from the Chat adapter still sent the turn to the next target. Treat that code as final and add a zero-output streamed combo regression (fails before: one fallback send).

Maintainer integration decision for v2.93.0 (lane L-D1): merging into dev by squash.

  • Exact head 5303bafed52f306fbeb58fc00c350d1b72c84293: PR CI 40281600743 passed (aggregate ci, four test shards, gates, docker smoke) and Catalog async contracts passed. This change is adapter and preflight logic only with nothing platform-specific; the CI change filter selected no Windows shards and no lane=all run was dispatched. The hygiene and target-branch runs on this head were cancelled when newer runs replaced them in the saturated queue; they are metadata checks, not code tests.
  • I merged current dev into this branch before review. Merge conflicts in the layout maps were resolved by registering both tests in the existing layout-chat shard. Because structure/providers-and-adapters.md is at its 600-line cap, the contract moved to the new structure/providers/chat-required-tool-results.md.
  • Independent review round 1 found one blocker: a zero-output streamed combo child that skipped a required call still moved on to the next target, because combo-stream-preflight treated every response.failed as replayable. Fixed in 536c03fc with a regression test that fails before the fix (one fallback send) and passes after. Round 2 passed with no blockers; its end-of-file nit was fixed in 5303bafe.
  • Regression first: the two new test files give 27 pass / 8 fail on the old adapter and pass after the fix.
  • Merged with current dev bada676a6 locally on Bun 1.4.4: tsc --noEmit exit 0, structure check passed, 165 focused tests passed with 0 failures (required-tool, parallel stream, combo preflight/refusal/zero-output failover, layout, ratchet).
  • scripts/ci/assert-mergeable-review.sh --maintainer-integration 7463 returned OK for this head. No outstanding CHANGES_REQUESTED.
  • Coexists with fix(combo): reject buffered required-tool false successes #7741 (native Responses combo contract), which covers a different route.

Refs #7456; that issue stays open for the routes this does not cover.
