fix(responses): display hosted image results in local Codex clients (carry #6233) - #6394
Conversation
- Project hosted image results into final assistant messages backed by validated local artifacts. - Preserve upstream continuation state and add bounded delivery, synthetic tests, and documentation.
- Replace generated image paths with opaque artifact references before request dispatch. - Cover full-history replay and pruned artifacts with regression tests and document the display boundary.
…udget Moves the hosted-image SSE detail into data-planes/images.md and leaves a one-line pointer; dev already sat at the cap.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Unindexed SSE frames all go to output_index 0.
Line 160 uses 0 as the index whenever event.output_index is missing or invalid. Line 162 still accepts the event when event.item.id is a string. doneEvents then emits output_index: 0 for the synthesized lifecycle frames (Lines 114, 117, 124).
Example: an upstream sends an image output_item.done with a string id and no output_index. The image sits at real position 2. The client receives content_part.added, output_text.delta, and output_item.done frames tagged output_index: 0. Those frames can collide with the message actually at index 0. The state key is still correct because it comes from the id. Only the emitted index is wrong.
The terminal snapshot path does not have this problem because it uses the real array index.
Fix: when hasIndex is false, omit output_index from the synthesized events instead of writing 0.
Proposed fix
- function doneEvents(item: Row, index: number): Row[] {
+ function doneEvents(item: Row, index: number | undefined): Row[] {
const state = complete(item, index);
...
- const common = { item_id: message.id, output_index: index, content_index: 0 };
+ const at = index === undefined ? {} : { output_index: index };
+ const common = { item_id: message.id, ...at, content_index: 0 };Apply the same at spread to the added and done events. Pass hasIndex ? event.output_index : undefined from rewrite. In stateFor, use the key "index:" + index only when index is defined.

Include the output index in image display state keys
When an upstream reuses the same string item.id for image calls at different output indices, both calls resolve to one state entry: the second output_item.added is suppressed, its completion reuses the first artifact, and the terminal snapshot contains duplicate copies of the first synthetic message instead of the second image. Key indexed events by both output_index and item.id (falling back to the ID alone only when no valid index exists), and add a regression case with duplicate upstream IDs at distinct indices.

Summary
Carries #6233 by @chiperman onto current
devas a maintainer PR. The three commits are replayed with their author. Two maintainer commits follow: a security fix found in review, and a structure-doc line-budget fix.A completed hosted
image_generation_callfrom a routed Responses provider can carry valid base64 image data and still show nothing in Codex App (#6231). For loopback-admitted, recognized Codex Responses clients, this projects hosted image items into an assistant message withphase: final_answerand a Markdown link to a validated local artifact. The projection applies the same way to SSE, JSON, and JSON synthesized into SSE.output_indexand no stringitem.idpass through untouched, so id-less items cannot share display state (CodeRabbit's finding, fixed by the author).Maintainer security fix
d6ff960a3d"fix(responses): redact hosted image paths on compaction and normalize matching". An independent security review off457ecee0dfound two gaps. P2: the remote compaction handler (src/server/responses/compact.ts) forwarded the client's history without the path redaction. After a hosted image turn, compaction therefore sent the assistant's local artifact link, which includes the home path, upstream. It now applies the same assistant-only redaction right after the body is parsed. P3: redaction matched only one exact spelling of the artifacts-directory prefix. It now matches the strictimg-codex-<uuid>.<ext>name directly inside the artifacts directory, across/or\\separators,file://URLs and./..segments, and ignores case on macOS and Windows. Unrelated paths and non-assistant content stay untouched. Regressions: a real compact-handler request asserts that no local path reaches the upstream body (tests/responses/responses-hosted-image-delivery.test.ts), and path-variant cases cover redaction (tests/responses/responses-hosted-image-display.test.ts). The review's remaining P3, that client recognition trusts loopback admission plus the originator header, is the existing loopback trust model. Artifact writes stay bounded (128 items, byte budgets, pruning).Maintainer commit
