Summary

A Kiro account that ran out of its monthly allowance or was suspended took the whole request down with it. Generic OAuth account failover only moved on HTTP 429, so a monthly-quota refusal sent as a 400 (MONTHLY_REQUEST_COUNT) or a suspension 403 reached the client even when another logged-in Kiro account was healthy, and the pool had no memory that the account was spent or suspended.

After this change:

  • Kiro refusals are classified before the pool reacts: a rate 429 gets a short cooldown; a monthly-quota refusal (429 or 400) marks the account exhausted until its reset; a confirmed suspension 403 quarantines it. Unrecognised 400/403 bodies stay ordinary failures and never quarantine anyone.
  • Each classified refusal rotates to another eligible account before any client bytes, inside the request's send budget. The original refusal is kept until a replacement is built and admitted; if none can be, the client gets the original status with the normalized Kiro message.
  • A terminal refresh rejection (the existing allowlisted OAuth codes) marks only that credential for re-login and tries a sibling.
  • A served success — including web-search and image sidecar completions — clears an older exhaustion verdict for the account that actually served, fenced by the login identity from fix(kiro): keep usage evidence across restart and refuse ARN-less probes #5967.
  • Gating follows the existing split: rotation after an upstream refusal is reactive and follows presence-is-consent; steering away from a known-dead account before dispatch follows the proactive preference switch (oauthAccountFailover.enabled, narrow over broad).
  • Final Kiro error text is normalized on the dispatch, combo and continuation paths, reusing the fixed 5xx strings from fix(kiro): send through provider egress and rotate hosts on gateway errors #5981.
  • Non-Kiro rotation code in adapter-dispatch.ts and adapter-continuation.ts is unchanged (kept in an else branch).

Verification

  • bun run typecheck — pass
  • One-process run of tests/providers/kiro/, the three server Kiro e2e suites, credential-redirect-guard, web-search-sidecar-429, generic-oauth-failover, oauth-store-multi, images/loop, adapter-inner-send-budget-wiring, layout, ratchet and core–Lab boundary — 863 pass, 0 fail
  • New tests cover each refusal class, unrecognised 400/403, spent send budget (one physical send, original refusal returned), cancel-once on successful rotation, rotated build failure, restart followed by a served success, sidecar success clearing only the serving account, explicit enabled: false keeping reactive rotation but refusing proactive steering, and an unlisted refresh error not marking reauth.
  • An independent review compared the non-Kiro dispatch and continuation blocks byte for byte with the base.
  • privacy:scan, structure:check, docs-site build — pass. Hosted CI: see the follow-up comment.

Checklist

  • Scope stays focused and avoids unrelated cleanup. (One unrelated test fix: web-search-sidecar-429.test.ts now restores globalThis.fetch, which leaked into credential-redirect-guard when both ran in one process.)
  • Docs or release notes were updated when needed. (structure/transports/responses-failover.md, structure/providers/kiro.md, reference/adapters.md and reference/cli/providers-accounts.md en/fr/tr)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (After rotation, bearer, profile ARN, region and routing metadata all come from the new account's snapshot; no upstream refusal text or token is logged.)

Summary by CodeRabbit

  • New Features
    • Kiro can now switch to another account after rate-limit refusals, confirmed monthly quota exhaustion, or confirmed suspension, including during eligible OAuth refresh failures.
    • Exhausted or suspended accounts can be temporarily excluded from rotation. A successful response clears an older quota-exhaustion record for that account.
    • Proactive account selection can avoid accounts already known to be exhausted or suspended when the preference is enabled.
  • Documentation
    • Updated Kiro adapter and account-management guidance in English, French, and Turkish to explain refusal handling, account rotation, and proactive selection.

Kiro refusals are now classified before the pool decides what to do: a rate
429 gets a short cooldown, a monthly-quota refusal (429 or 400) marks the
account exhausted until its reset, and a confirmed suspension (403) is
quarantined. Each of these rotates to another eligible account before any
client bytes, inside the request's send budget, keeping the original refusal
until a replacement is admitted. A terminal refresh rejection marks only that
credential for re-login and tries a sibling. A served success clears an older
exhaustion verdict for the account that actually served. Reactive rotation
follows the existing presence-is-consent rule; steering away from a known-dead
account before dispatch follows the proactive preference switch. Final Kiro
error text is normalized on the dispatch, combo, and continuation paths.
Non-Kiro rotation code is unchanged.

📝 Walkthrough

Walkthrough

Kiro refusal handling now classifies rate limits, monthly quota exhaustion, and confirmed suspension. The Responses and sidecar paths can rotate accounts, track account-specific evidence, and retain the original refusal if alternate-account setup fails. Completed responses can clear older exhaustion evidence for the same credential.

Changes

Kiro account failover

Layer / File(s) Summary
Refusal classification and account evidence
src/adapters/kiro-refusal.ts, src/providers/kiro-usage.ts, src/oauth/generic-account-failover.ts, tests/providers/kiro/*, tests/oauth/generic-oauth-failover.test.ts, docs-site/src/content/docs/{fr,tr,reference}/..., structure/providers/*
Kiro responses are classified as rate, monthly-quota, suspended, or other refusals. Monthly-quota evidence is tied to credential generation and account identity; suspension cooldowns are identity-checked. Account eligibility and cooldowns use these refusal types.
Initial selection and adapter retry
src/adapters/base.ts, src/adapters/kiro-retry.ts, src/adapters/kiro/adapter.ts, src/oauth/kiro-terminal-failover.ts, src/oauth/generic-account-failover.ts, src/server/responses/request-transport.ts, tests/providers/kiro/*
Kiro can select an alternate before the first send when proactive preference applies, and can try an alternate after a terminal refresh failure. A pooled throttle response can return directly to account rotation rather than retrying on the same account.
Refusal recovery across request paths
src/server/responses/adapter-dispatch.ts, src/server/responses/adapter-continuation.ts, src/server/responses/sidecar-execution.ts, src/images/loop.ts, src/web-search/loop.ts, tests/server/server-kiro-refusal-e2e.test.ts, tests/providers/kiro/kiro-auth-context-continuation.test.ts, structure/transports/responses-failover.md, devlog/_plan/...
Responses dispatch, continuations, and sidecars pass Kiro refusals through classification and refusal-aware rotation. Replacement requests use the alternate credential snapshot. If replacement setup fails before dispatch, the original refusal is retained; final Kiro errors are normalized.
Completed-response evidence
src/server/responses/adapter-delivery.ts, src/server/responses/sidecar-execution.ts, tests/server/server-kiro-refusal-e2e.test.ts, devlog/_plan/...
Completed streaming, buffered, and sidecar responses record success for the sent Kiro credential. Account state is persisted when success clears an older matching exhaustion verdict.

Priority: ➖ Normal

Change: Bug fix

Merge Risk: 🟡 Moderate · up to 66e75

A transient rate limit can reach the client instead of being retried when the other stored account cannot serve. Gate the retry bypass on an eligible alternate before merging.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to 66e75

Account failover is bounded and generally tied to the credential that received the refusal. One sidecar failure path may replace a provider refusal with a gateway error before a replacement request is sent, obscuring the reason recovery failed.

Retained concerns

  • Medium · reliability · inferred: After selecting a replacement credential, the image and web-search loops cancel the original Kiro refusal before building and sending the replacement. A pre-send replacement failure can therefore surface as a gateway error instead of the original 400 or 403, obscuring an account suspension or quota refusal during recovery.
Security review details

Security Blast Radius

  • inferred — A mistaken account verdict can affect subsequent requests using that Kiro account, while rotation can reach another stored Kiro credential. The observed selectors exclude cooled, exhausted, and reauthentication-required candidates; deployment-wide scope cannot be established without runtime topology.

Trust Boundaries and Controls

  • observed — Upstream HTTP status and bounded response-body fields provide refusal evidence. Before using another credential, account selection checks eligibility; admission rebinds the bearer together with Kiro routing metadata rather than swapping a token alone.

Resilience and Maintainability Implications

  • inferred — Unlike main Responses dispatch, sidecar recovery does not retain its original refusal through a selected replacement's pre-send failure. Losing the 400 or 403 can impede diagnosis of quota or suspension state even though it does not establish a credential-boundary bypass.

Hardening Proposals

  • proposed — Carry the original sidecar refusal until a replacement reaches its physical-send boundary, and restore its status and normalized message if replacement preparation fails earlier.

P2 Badge Pass the Kiro failover preference into sidecar fetches

When a pooled Kiro web-search or image iteration receives a transient 429, this disables only the bridge's generic retry policy; both loops still call fetchResponse without kiroPreferAccountFailover, so fetchKiroWithRetry performs its three-attempt same-account throttle ladder before on429 can rotate. This can add multiple backoffs and send three requests to the already-throttled account before trying the healthy sibling; propagate the Kiro failover preference through the sidecar fetch context as the main and continuation paths do.

P2 Badge Keep same-account retries when no alternate is usable

When two Kiro accounts are stored but every sibling is currently needsReauth, cooled, suspended, or monthly-exhausted, isGenericOAuthFailoverEnabled is still true because Kiro's quorum now counts all stored rows. This flag therefore makes the adapter return the first transient 429 immediately, but the outer rotator then finds no eligible alternate and surfaces that 429, removing the bounded same-account recovery that remains available with one stored account. Set this preference only when a usable alternate exists for the serving account, or fall back to the adapter's transient retry when rotation cannot select one.

Useful? React with 👍 / 👎.

The CLI reference must still say reactive rotation cannot be turned off; the
refusal-class wording now keeps that sentence (docs-429-failover-claims).

🩺 Stability & Availability | 🟠 Major | ⚡ Quick win

Kiro's pooled-429 bypass depends on presence quorum, not on whether an alternate account is eligible.

kiroPreferAccountFailover is set from isGenericOAuthFailoverEnabled(config, "kiro"). For Kiro, that predicate counts every stored account, including needsReauth rows, and ignores cooldowns. When the flag is true, fetchKiroWithRetry returns the first 429 without the same-account throttle retry. If account B needs reauthentication, is cooled, or is quarantined, account A's transient 429 reaches the client, while a one-account install would retry it. Gate the flag on live eligibility instead.

  • src/server/responses/adapter-dispatch.ts#L308-L308: add && !!transportState.genericFailoverAccountId && hasEligibleGenericOAuthFailoverTarget("kiro", transportState.genericFailoverAccountId, Date.now(), route.modelId). Apply the same change at Line 481.
  • src/server/responses/adapter-continuation.ts#L212-L212: apply the same eligibility condition to the continuation fetchResponse context.
  • src/oauth/generic-account-failover.ts#L146-L149: keep the widened count for terminal-refresh consent only. Do not use it as proof that an alternate can serve.

라인 - src/providers/kiro-usage.ts 284줄.

라인 - src/server/responses/adapter-dispatch.ts 308줄, 481줄. src/server/responses/adapter-continuation.ts 212줄. kiroPreferAccountFailover는 저장된 계정이 두 개면 켜집니다. src/oauth/generic-account-failover.ts 148줄은 다시 로그인이 필요한 계정도 그 숫자에 넣습니다.

라인 - src/web-search/loop.ts 499줄. src/images/loop.ts 603줄.
