What & why

On a long session the legacy-route judge answered insufficient_evidence because get_session_errors and search_session returned more than Claude Code shows inline. Claude Code saved the results to files, and the judge has no tool that reads files. Every kcap mcp judge result is now capped below that limit, with a note on how to narrow the call. get_session_errors returns a paged summary that groups failures by repeated operation, search_session cuts long snippets to a window around the query term, and get_tool_result pages a long body by offset.

Where to look

JudgeErrorDigest: the grouping key (tool plus the command, path or pattern acted on) is what the repeated-failures question reads. The tools prompt is unchanged because it is byte-pinned, so the new arguments are described only in the tool schemas. A row's call_id appears once the server sends it (kurrent-io/kcap-server companion PR).

Verification

Unit tests cover grouping, paging, head-and-tail error excerpts, refs across a chain, the cap and the search window. Reverting the grouping threshold or the snippet windowing makes its test fail. A Release AOT publish reports no IL warnings.

Summary by CodeRabbit

  • New Features
    • Session error results now provide a compact, paginated summary with failure totals, grouped errors, and references.
    • Search results show excerpts around matching terms and report when results are omitted.
    • Long tool results, session summaries, and recaps can be retrieved in pages using offsets.
    • Tool-result requests can specify an agent to retrieve agent-specific results.
  • Improvements
    • Oversized responses are capped with guidance for retrieving additional content.
    • Invalid pagination values return clear tool errors.

Claude Code spills an oversized MCP result to a file, and the judge has no tool that reads files.

Merge Risk: 🔵 Low · up to a8120

Long judge responses can exceed the stated character cap slightly. The change is mergeable with that bounded limitation understood.

Security architecture risk: 🔵 Low · up to a8120

The change alters how session evidence is delivered without adding write permissions. Risk appears limited, but subagent access and complete evidence retrieval depend partly on behavior outside the reviewed change.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The demonstrated exposure is the session evidence returned to a judge through the existing read path, including already-requested continuation errors and newly selectable subagent output. Broader tenant, datastore or infrastructure exposure is not demonstrated; verifying the subagent boundary requires the upstream authorization implementation.

Trust Boundaries and Controls

  • observed — Caller-controlled session identifiers are checked against the bound session before requests are sent. Caller-controlled call and agent identifiers then select upstream evidence. Escaping protects URL structure, but does not itself establish that the selected agent belongs to the authorized session.

Resilience and Maintainability Implications

  • observed — Malformed digest or search JSON falls back to capped raw text, and malformed tool-output JSON falls back to text paging. The complete body is still downloaded before shaping, as at the base, so the new output budget is not an upstream download or memory-consumption boundary.

Hardening Proposals

  • proposed — Make the cross-service contract explicit that agent_id only selects evidence within an already-authorized session, with mismatched agent/session ownership rejected upstream. This is an expected control to verify, not an observed missing authorization check.

Keep judge tool results readable in long sessions

🐞 Bug fix ✨ Enhancement 🧪 Tests 🕐 40+ Minutes

Grey Divider

AI Description

• Bound judge tool responses so oversized results remain readable inline rather than spilling to
 files.
• Summarize and page failures, window search snippets, and page full tool-result bodies.
• Add tests for grouping, pagination, excerpts, argument validation, and response limits.

High-Level Assessment

The following are alternative approaches to this PR:

1. Paginate at the session API
  • ➕ Avoids fetching and processing the entire error list or tool-result body for each page.
  • ➖ Requires a companion server API change and coordination across deployments.

Recommendation: Keep the client-side shaping for this fix: it makes existing session API responses usable by the judge without changing their contracts. Consider server-side pagination if long-session transfer or processing costs become significant.

P1 Badge Compute next_offset from rows that survive the cap

When get_session_errors is called with the supported limit: 100 and the failures contain long operations or errors, the rendered digest can exceed 40,000 characters. This cap then cuts the JSON partway through the failure array, but JudgeErrorDigest.Render has already emitted next_offset = offset + page.Count, so following that offset skips every row removed by the character cap. Limit the page to rows that actually fit, or calculate next_offset from the last delivered row.

Fixed in 2576c57. The digest is assembled from pre-serialized fragments against the cap, so a page is always one complete document and next_offset follows the last row it holds.

P1 Badge Exclude failures without an operation from repeat groups

When /errors entries omit tool_input, as the existing ErrorEntry contract and server-backed fixture currently do, Operation returns an empty string. Consequently, any two failures from the same tool are reported as one repeated operation with operation: "", even if they came from unrelated commands; the tool description then presents this as direct evidence of repeated failed attempts. Unknown operations should not be grouped together.

P2 Badge Group by the full operation before clipping it

For commands or paths longer than 200 characters, two distinct operations with the same prefix are reduced to the same string here and subsequently grouped as repeated attempts. This can happen with long test or build commands whose distinguishing filter or target is near the end, producing false evidence for the judge. Preserve the normalized full value as the grouping key and clip only the displayed operation field.

Remediation recommended

1. Search excerpts can miss the match ✓ Resolved
Description
JudgeSearchExcerpts.Terms removes OR but retains other FTS5 operators such as AND and NOT,
which Excerpt then searches as ordinary terms. For a query such as foo AND bar, an earlier
incidental “and” in a long snippet can determine the window and cut out the actual matching terms.
Code

src/Capacitor.Cli/Commands/JudgeSearchExcerpts.cs[R39-42]

+        query.Split((char[]?)null, StringSplitOptions.RemoveEmptyEntries)
+            .Select(t => t.Trim('"', '\'', '(', ')').TrimStart('-'))
+            .Where(t => t.Length > 0 && !t.Equals("or", StringComparison.OrdinalIgnoreCase))
+            .ToArray();
Relevance

●●● Strong

Retaining FTS operators causes deterministic incorrect excerpt selection and is a local parsing bug.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The search schema accepts an FTS5 query; the new parser excludes only OR, while the windowing code
chooses the earliest occurrence of any retained token.

3. Large session summaries lose their ending ✓ Resolved
Description
Shape passes get_session_summary through the raw-string cap without a summary-specific paging or
narrowing argument. A summary longer than 40,000 characters loses its remaining fields and is
returned as a partial JSON document followed by a prose truncation marker.
Code

src/Capacitor.Cli/Commands/McpJudgeServer.cs[R243-247]

+        return JudgeResultCap.Cap(shaped, toolName switch {
+ "get_transcript" => "Pass a smaller take, or skip past what you have read.",
+ "get_session_errors" => "Pass a smaller limit, or page with offset.",
+            "search_session"     => "Pass a smaller limit or a narrower query.",
+            _                    => "Use search_session or get_transcript for the detail."

5. Tool-result pages split the envelope ✓ Resolved
Description
Shape pages the entire HTTP response for get_tool_result, while kcap-server returns a JSON
object whose output field contains the requested body. For a long result, offsets count envelope
and JSON-escape characters, so subsequent pages start inside serialized JSON rather than at the
promised character offset in the tool output.

Description
JudgeErrorDigest.Render includes up to 30 repeated-operation groups with 20 references each on

- src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[23-25]
- src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[80-80]
- src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[99-104]
- src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[120-120]

## Recommended Fix
Replace each direct `ValueKind` comparison or pattern with the equivalent `JsonElementExtensions` method, such as `IsArray()`, `IsObject()`, `IsString()`, `IsNumber()`, or `IsNull()`, while preserving the existing control flow and fallback behavior.

Informational

9. Error rows lack tool-result links 🔗 Cross-repo conflict ≡ Correctness
Description
JudgeErrorDigest.ToFailure emits call_id only when an error entry contains it, but the pinned
kcap-server errors response omits that field. Until the server supplies it, the judge cannot use an
error row to retrieve full output through get_tool_result as the tool description instructs.
Code

src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[R91-92]

+        return new(reference.ToString(), Str(e, "timestamp"), Str(e, "tool_name") ?? "(unknown)", Operation(input),
+            Excerpt(Str(e, "error") ?? ""), Str(e, "call_id"));
Relevance

● Weak

The PR explicitly coordinates a companion server change, matching rejected cross-repository contract
findings.

PR-#1538

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The server identifies results by call ID internally, but its error-entry response model and fixture
omit call_id. The digest writes the field only when it is present, while the tool description
directs the judge to use it for get_tool_result, so current response rows cannot support that
follow-up.

## Issue description
The digest advertises full-output lookup from error rows, but the pinned server's errors response does not supply the required `call_id`.

## Recommended Fix
Add and populate `call_id` in the server's error response and coordinate its deployment with the client. Until that response is available, gate or revise the CLI instruction so it does not promise row-based full-output lookup for entries without a call ID.

10. Judges mistake unrelated errors for retries ✓ Resolved
Description
JudgeErrorDigest.ToFailure derives the grouping operation from tool_input, but the current
errors-response contract does not include that field. For those responses, every failure from the
same tool has an empty operation, so different failed commands are presented as repeated attempts at
one operation.
Code

src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[R89-92]

+        var input = e.TryGetProperty("tool_input", out var i) ? i : default;
+
+        return new(reference.ToString(), Str(e, "timestamp"), Str(e, "tool_name") ?? "(unknown)", Operation(input),
+            Excerpt(Str(e, "error") ?? ""), Str(e, "call_id"));
Relevance

● Weak

Cross-repository contract mismatches are rejected when companion server support is expected.

PR-#1538

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The existing response model and server-backed response fixture have no tool_input; the new
fallback converts its absence to an empty operation, which the grouping key then shares across
failures from one tool.

Agent prompt

## Fix Focus Areas
- src/Capacitor.Cli/Commands/JudgeErrorDigest.cs[89-104]
- src/Capacitor.Cli.Core/Models.cs[53-61]

11. Comments depend on changing tool limits 📘 Rule violation ⚙ Maintainability
Description
The summary comment cites Claude Code's default 25,000-token output limit as part of the
implementation rationale. That external limit can change independently of this code, leaving the
comment's stated behavior and rationale stale for later maintainers.
Code

src/Capacitor.Cli/Commands/JudgeResultCap.cs[R3-4]

+/// <summary>Bounds every judge tool result. Claude Code spills an MCP result past its output limit (25,000 tokens by default)
+/// to a file and tells the model to read it, and the judge has no tool that reads files, so an oversized result is lost.</summary>
Relevance

● Weak

A similar stale vendor-specific measurement comment was rejected as acceptable implementation
rationale.

PR-#978

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The added summary explicitly attributes the cap to Claude Code's output limit and states that the
limit is 25,000 tokens by default. This is an external, time-sensitive process constraint rather
than a stable description of the current code behavior.

Rule 2897915: Avoid time-sensitive or process-reference metadata in code comments
src/Capacitor.Cli/Commands/JudgeResultCap.cs[3-4]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The XML summary references Claude Code and its current default output limit, which is external and time-sensitive metadata.

## Fix Focus Areas
- src/Capacitor.Cli/Commands/JudgeResultCap.cs[3-4]

## Recommended Fix
Rewrite the summary to describe the stable behavior and rationale of the code itself, such as bounding judge tool results so they remain readable inline and provide paging guidance, without naming an external tool's current limit or default.

Remaining Qodo items from the summary (2576c57):

  • 6 Search excerpts can miss the match: fixed; operators and negated words are skipped and the query is split on punctuation.
  • 7 Page boundaries can corrupt Unicode: fixed; every cut steps back off a surrogate pair.
  • 8 Tool-result pages split the envelope: fixed; the output field is paged inside the envelope, with offset, next_offset and output_chars.
  • 9 Comments depend on changing tool limits: kept; the 25,000-token default is the measurement that justifies the 40,000-character cap.
  • 10 Judges mistake unrelated errors for retries: fixed; failures with no operation never group.
  • 11 Error rows lack tool-result links: the field arrives with kurrent-io/kcap-server#2473; until then rows simply omit it.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Reserve space for the paging notice.

When a non-envelope body exceeds 40,000 characters, PageOutput falls back to Page. Page selects up to MaxChars characters and then appends a notice, so the returned page exceeds the cap. Cap has the same budgeting error at Line 19. Subtract the notice length before selecting either text slice.
