• Reframes the Continuum README around the no-privileged-graph doctrine and stack responsibilities.
• Adds the Think/Echo north-star documentation already present on this branch.
• Registers Graft explicitly as the structural observer and review engine in the Continuum stack and contract family registry.

Why

Graft now has a root NORTHSTAR that positions it as the code-aware structural reading layer over witnessed causal history. Continuum should mirror that role in its coordination truth so Graft is not hidden under a generic app/tool bucket and does not drift into runtime, debugger, or semantic-owner territory.
