What is failing

The MeshWeaver.AccessContext PostPipeline never-null tripwire is firing in production: a portal reads hub (address portal/reads-*, hosted under the per-circuit portal hub) is posting a non-exempt application message (GetDataRequest, target AgenticEngineering) with no AccessContext — neither the ambient AccessService.Context nor CircuitContext was set at post time. The PostPipeline correctly fails the delivery (fail-closed, per design), so the requesting user's read of AgenticEngineering data never completes.

Probable cause

Medium confidence. Per Doc/Architecture/AccessContextPropagation ("the portal is the worked example"), the per-circuit portal hub is supposed to carry the circuit user via ICircuitContextAccessor.UserContext → a per-hub PostPipeline step that stamps the circuit user when a post has no ambient context. This post reached the pipeline with neither source set, which points at one of:

  • the portal/reads-* hosted sub-hub posting from a thread where the circuit user was never established (e.g. a background/hydration read initiated outside the inbound-activity SwitchAccessContext wrap), or
  • the circuit-user stamping step not being applied to this hosted sub-hub's posts, or
  • the post occurring after the circuit's context was torn down.

I cannot pin the exact call site from the log alone — the sample carries no stack — so the first investigation step is to reproduce with the PostPipeline error and capture the posting stack for GetDataRequest originating from portal/reads-*.

Impact

Low volume: 2 occurrences within an 18-second window on two portal pods in namespace memex-cloud, first seen 2026-09-22. A user-facing read against AgenticEngineering fails closed (OnError to the awaiting observer) — intermittent, not an outage. No security exposure: the invariant held and the delivery was refused rather than delivered with a null context.
