fix(contextmd): acknowledge full Codex reads before retrying a gated action #501

Open

Description

@lukemaj

What: ContextMD repeatedly rejects a Codex WIP commit after the requested procedure was successfully read in full.
Why: The live gate does not acknowledge the completed read; a shell-input or projected-result mismatch is a possible cause, still unconfirmed.
So what: Repair read acknowledgement so agents can complete authorized work without changing the rule or disabling its gate.

Observed during toolboxmd/agentsmd#164, T3 Code, Codex harness, gpt-6.1-sol, medium effort, 2026-10-19 on bestia. The authorized task explicitly requires an internal component WIP commit, push, and PR. Delivery is currently blocked.

Reproduction and evidence

  1. The gate requests a full read of the canonical AgentsMD version-control procedure before committing.
  2. Run a standalone cat /home/luke/dev/toolboxmd/agentsmd/skills/operations/procedures/version-control/index.md through the Codex shell tool. The tool returns exit 0 and the complete 987-token file, without truncation. Repeated this full read after rejection.
  3. Retry the authorized WIP commit. Three attempts have been rejected with the same request, including after the standalone canonical read.

Exact gate output:

ContextMD, the user's own rule store, stopped this call: follow this rule from the user, then run the same call again.

Read /home/luke/dev/toolboxmd/agentsmd/skills/operations/procedures/version-control/index.md in full, then run the same call again.
AgentsMD: follow the version-control procedure before committing.

Relevant inspected source, not a proven live root cause:

  • apps/server/src/contextmd/Reads.ts recognizes shell reads only when input.command is a string.
  • This Codex tool's public input uses cmd, via functions.exec calling tools.exec_command({cmd: ...}).
  • Harnesses.ts lists exec_command as a Codex shell tool.
  • ServingState.recordReads also requires a tool-use ID and waits for a projected successful exit when Codex reports no failure signal.

Inspect the actual normalized hook event and projected tool/result identities before choosing a fix. This report does not establish whether the live failure is input normalization, missing post-tool data, or the projected-read proof path. No rules, hook state, server, or installation were changed to force the action through.

Outcome and acceptance

  • A successful, complete read through the real Codex code-mode shell path acknowledges the pointed file and lifts the relevant rule for the same session.
  • A failed or truncated read remains insufficient.
  • Regression proof covers the actual incoming tool-input shape and projected result identity.
  • The existing before-commit rule remains enforced; changing the deny cap is not the requested repair.

Non-goals: changing AgentsMD rules, the #164 component, deployments, or disabling serving.

Objective contribution: reliable serving supports Drafters and persistent Wight work by allowing authorized tool actions after their required context is loaded.

Elon record

Requirements and who asked: The user's AgentsMD rules require filing observed tool problems at their owner; the current implementation request requires a WIP commit and PR after reading version-control.
Deleted: Gate-policy changes, live-install changes, repeated attempts to exhaust the deny cap, and speculative implementation changes without the raw normalized event.
Bottleneck: A completed required read is not acknowledged, so the authorized component cannot be committed or published as a PR.
Checked myself: Read the exact canonical procedure in standalone successful shell calls; observed three repeated denials; inspected Reads.ts, Harnesses.ts, ServingState read acknowledgement, and searched open Chromeria issues for a matching report.

What: #444 independently reproduced the same repeated Codex full-read acknowledgement failure while committing its reviewed-train component.
Why: Three commit attempts were denied despite successful standalone whole-file reads; the live normalization/projection cause remains unconfirmed.
So what: Keep this as owner #501 evidence, separate from #444's TinyFish implementation; no serving rule, hook state, cap or installed app has been changed.

Observed on bestia, 2026-10-19 Europe/Warsaw, T3 Code/Codex, gpt-6.1-sol/max. In exclusive /home/luke/.t3/worktrees/chromeria/feat-444-tinyfish-fallback, source commit 7a59d884b13a0eddc74336166d42e4af9cccc00c already exists, with a proved two-file response-scope follow-up pending. The requested final follow-up commit is ordinary in-scope delivery, with no version bump per project policy.

Three retries of git commit -m 'fix(research): close TinyFish response scopes' were rejected by PreToolUse. Between retries, standalone cat /home/luke/dev/toolboxmd/agentsmd/skills/operations/procedures/version-control/index.md through functions.exec -> tools.exec_command({cmd: ...}) returned exit0 and the full 987-token file, with no truncation. The unchanged deny names that same exact file and says AgentsMD: follow the version-control procedure before committing.

Inspected current task-base Reads.ts and #501's existing evidence. Its input.command versus the public cmd shape is a plausible seam, not a demonstrated live root cause. No synthetic read/hook event, argument obfuscation, live serving-state write, hook/rule change, install or restart was attempted. ContextMD's README documents a three-denial fail-open policy, but reaching that policy would not prove the read was acknowledged or fix #501.

Requirements and who asked: The user's rules require reading the procedure, authorized component delivery and reporting found tool defects at their owner.
Deleted: An unrelated source fix in #444, bypassing/disabling the guard and inferring acknowledgement from a later allowed action.
Bottleneck: Correct acknowledgement of the actual successful Codex tool result.
Checked myself: Three exact denials, two new standalone full reads after the original full procedure load, existing #501 and current Reads/Select/Harness source paths.

Finding (01:40 Warsaw): the commit gate counts only a read of the canonical clone's path. A worker in a task worktree that read skills/operations/procedures/version-control/index.md (relative, inside its worktree) stayed blocked. After it read /home/luke/dev/toolboxmd/agentsmd/skills/operations/procedures/version-control/index.md, the path the block message names, the commit went through (toolboxmd/agentsmd PR #240). So agents in worktrees are blocked until they read the exact absolute path, even when they have read the same file from their checkout. Either accept the worktree copy, or name the required absolute path in the block message as an instruction.

Another occurrence, 19 Oct 2026 about 01:00 Warsaw on bestia: a Codex (GPT-6.1-Sol) worker on lukemaj/promachos#18 was denied git commit three times with "Read .../version-control/index.md in full, then run the same call again" after full cat reads that exited 0. It was also blocked twice from filing an Issue by the Elon/specify pointers after reading those files. The same commit from a Claude Code thread passed. Worker thread: build-car-gavin-r2.

What: The PR535 integration merge was blocked by ContextMD's read gate immediately after both required references had been read in full successfully.
Why: One exec_command contained both full cat reads; the next merge command was nevertheless told to read delivery.md and finalization.md again. This is additional read-acknowledgement evidence, not a missing delivery approval.
So what: Separate complete standalone reads followed by one identical normal retry succeeded; merge06cd67f65c857e1419e6b51dbf2aace8806cf59d is verified. Existing #501 owns the underlying acknowledgement defect.

Exact blocked action: gh pr merge 535 --repo toolboxmd/chromeria --squash --match-head-commit 93a5f85fd57d8d59ad8ab1e1d1e366fdce374482, followed by read-only PR verification in the same shell call. First preceding full reads were canonical skills/operations/references/delivery.md and references/finalization.md, exit0 with complete output. Gate named those same paths. Re-read each as its own exec_command, exit0/full output, then identical command passed. No hook/rule/config alteration, bypass, alternative merge route or changed source/assertion. Exact-head independent review and CI were already passing and checked separately.

Requirements and who asked: Existing delivery/finalization gate requires actual full procedure reads before authorized merge.
Deleted: Treating successful combined reads as unperformed, permission re-asking or bypassing the gate.
Bottleneck: Supported read acknowledgement at the command boundary, not source review or merge authority.
Checked myself: Complete preceding outputs/exit0, exact denial paths, two standalone full re-reads and successful identical merge/after-state.
