Cherry-pick of the original two commits from #53 onto main. Brings the ACP default-model fix and the release-triggering changeset so the next red-release run publishes a new version.

Drops the squashed mega-merge in dev (which carried upstream changes unrelated to the fix) and the unrelated review server template that has unresolved stub imports.

defaultModelFromConfig was falling back to the opencode provider's top model (big-pickle) whenever the configured provider had not finished loading yet, which made build -> plan -> build mode switches snap the model back to the fallback. The configured model now always wins; fallback is computed from the connected providers.
