fix(opencode): honor configured model in ACP default selection - #56

defaultModelFromConfig was falling back to the opencode provider's top model (big-pickle) whenever the configured provider had not finished loading yet, which made build -> plan -> build mode switches snap the model back to the fallback. The configured model now always wins; fallback is computed from the connected providers.

Cherry-pick of the original two commits from #53 onto main. Brings the ACP default-model fix and the release-triggering changeset so the next
red-releaserun publishes a new version.Drops the squashed mega-merge in
