fix(server): /clear resets the Claude context meter - #29961
In a Claude thread, /clear left the composer's context window meter on the old token count until the next turn that made an API call. Reproduced on main (61c3794c32) in the web dev server with Claude Fable 5.1. One turn brought the meter to 7.9% (79k/1m). After /clear, Claude's own /context reported 29.8k / 1m (3%), but the meter still read 7.9% · 79k/1m. Claude Haiku 5.5 behaved the same (stayed at 6.7%).
The Claude CLI marks a clear with a conversation_reset frame, followed by a zero-usage result. The adapter ignored the reset frame and drops zero-input usage, so the /clear turn reported no usage. The client then kept showing the newest earlier report, from before the clear.
Change
ClaudeAdapterV2 now handles conversation_reset. It clears the turn's cached stream usage and reports the turn's context as 0 used, keeping the model's window and auto-compact threshold. This mirrors what compact_boundary already does with its post-compaction size. Both paths share one small helper. Every reset trigger counts, as the SDK says a consumer should reset on every conversation_reset frame whatever its trigger says. The next real request reports the real size again.
Idle compaction also reads the newest turn usage, so it no longer sees the pre-clear size after a /clear.
ClaudeAdapterV2 background wake turns > reports an empty context when /clear resets the conversation (apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts). It replays the frames the real CLI emits on /clear. It fails on main: the newest report is still the pre-clear usedTokens: 166100. It passes with the fix. All 296 tests in that file pass, and so does vp run check:changed (format, lint, knip, typecheck, tests).

After /clear the Claude CLI emits conversation_reset, system/init and a
zero-usage result. The adapter emits no tokenUsage for that turn, so the
newest provider turn report is still the pre-clear one.

Claude answers /clear with a conversation_reset frame and makes no API
request, so the meter kept the pre-clear token count. Report an empty
context for the current turn on every conversation_reset, sharing the
replaced-context report with the compaction path.

Macroscope's review found this PR approvable — Claude conversation resets now immediately reset the context meter instead of leaving the previous count visible. The change preserves existing window and compaction settings and is supported by targeted regression and real-app verification.

ClaudeAdapterV2 now emits zero context usage when it receives a conversation reset. Token-usage emission is shared with compaction replacement handling. A test checks that the cleared turn reports zero used tokens and retains the prior maximum.
A shared helper emits provider-turn token usage. Existing context usage delegates to it, and replacement-context usage uses the model context limit, auto-compact threshold, and timestamp.
Reset and compaction usage apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
A conversation_reset frame clears current stream usage and emits zero used tokens. Valid post-compaction counts use the replacement-context helper. The test checks the cleared turn's usage and retained maximum.
The reset behavior has no identified merge-blocking defect. Normal checks should precede merging.

Full details: Linked Issues check
Explanation
Issue [#29951] requires the Claude context meter to stop using the pre-/clear usage. ClaudeAdapterV2 now handles every conversation_reset frame, clears cached stream usage, and emits a zero-used replacement usage while preserving maxTokens and the auto-compact threshold. The regression test replays the reset and zero-usage frames and verifies that the cleared turn is the newest context report. The reported test suite and changed checks also pass.
Full details: Out of Scope Changes check
Explanation
The changes stay within issue [#29951]. The adapter refactor shares token-usage emission between normal usage, compaction, and reset handling. The added test covers the /clear reset behavior. No unrelated product behavior or files are changed.

Assert the auto-compact threshold on the /clear report.
Assert the auto-compact threshold on the /clear report.
The /clear test uses the default empty autoCompactWindow, so the reset report has no autoCompactThreshold. Its assertions do not check that field. A change that drops the threshold from the reset path would still pass this test. Configure a populated window and assert the threshold. The helper call and the expected value match the neighboring populated-threshold test.
