fix(server): Claude fork subagents no longer show as done when they start - #28513
When a Claude thread launched subagents with Agent and subagent_type: "fork", each subagent row showed as done with a 0s duration and the result "Fork started — …" as soon as it started, although the fork was still running. A fork inherits the parent conversation, so its sidechain starts with a user frame whose parent_tool_use_id is the launching Agent call T and which repeats a tool_result for T. The adapter treated that echo as the subagent's final result.
Change
The live tool-result loop in ClaudeAdapterV2 now skips a tool_result whose tool_use_id equals its frame's own parent_tool_use_id. That echo no longer settles the subagent or creates a tool call. The fork still settles from its top-level tool_result (parent_tool_use_id: null) or task_notification. Nested Agent results inside a subagent have a different id than their frame's parent, so they still settle their subagents.
New test keeps a fork subagent running through its sidechain launch echo in apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts. It fails on main (expected { status: 'completed', result: 'Fork started — processing in background' } to deeply equal { status: 'running', result: null }) and passes with the fix. It also checks that the top-level result still completes the fork with the real report.
vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts: 281 passed. vp run check:changed: format, lint, knip, typecheck and tests pass.
Runtime transcript: the real Claude CLI 2.1.304 (claude -p --output-format stream-json --verbose), with one Agent call using subagent_type: "fork" and run_in_background: false. Excerpt:

user parent_tool_use_id=null tool_result tool_use_id=toolu_…HVPMJoAN "[Subagent hand-back] …" tool_use_result.status=completed <- settles the fork
Server-only fix: no render path changed, so there are no captures. Not checked: a live T3 desktop run with the Claude provider end to end. The reporter is on macOS desktop; the server-side path is platform independent.

…hain launch echo (#28504)
A fork's sidechain replays the launching Agent call's tool_result
("Fork started — …") with parent_tool_use_id equal to that call. This test
reproduces #28504: the adapter settles the fork subagent as completed on that
echo while the fork is still running.

…ch echo
A fork inherits the parent conversation, so its sidechain opens with the
launching Agent call's tool_result ("Fork started — …") under
parent_tool_use_id equal to that call. The live loop treated it as the
subagent's result and marked the fork completed while it was still running.
Skip a tool_result whose tool_use_id equals the frame's own
parent_tool_use_id. Nested Agent results inside a subagent and the real
top-level result or task_notification still settle subagents as before.

Macroscope's review found this PR approvable — This prevents replayed Claude fork-launch results from marking still-running subagents as completed. The change is narrowly scoped, preserves normal completion handling, and is supported by a reported regression test and runtime transcript.

The Claude tool-result loop now skips results whose tool-use ID matches the message’s parent tool-use ID. A regression test verifies that a fork subagent stays running after the replayed launch result and completes when its final report arrives.

Explanation
The title names the main change: Claude fork subagents no longer show as done when they start. The changeset adds a skip for tool_result entries whose tool_use_id matches the frame's parent_tool_use_id, plus a regression test, which prevents a fork's sidechain echo from settling the subagent. The title is specific and follows conventional commit style, so it passes.
Full details: Description check
Explanation
The description follows the repository template. It has Problem, Change, Scope and approval, and Verification sections. The Problem section explains the expected and actual behavior and the cause. The Change section explains the fix and why nested Agent results still settle. The Scope section links issue #28504 and says it was triaged via the automated issue flow. The Verification section lists a new test with its failure on main, the passing test run, a CLI transcript, and states what was not checked. The model and harness are named at the end. One gap: the scope section cites an automated triage flow rather than a maintainer approval comment, so a reviewer should confirm that the approval requirement is met. The description is mostly complete, so the check passes.

Explanation
[#28504] requires that a fork subagent stay running when its sidechain replays the launching Agent call's tool_result, and that only a top-level tool_result (parent_tool_use_id === null) or task_notification settle it. The diff at head adds one guard in the live tool-result loop of apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts: if (toolResult.tool_use_id === parentToolUseIdFromSdkMessage(message)) continue;. The sidechain echo has tool_use_id equal to its frame's parent_tool_use_id, so it no longer reaches resolveSubagentByToolUseId or updateClaudeSubagentNode, and the fork stays running. The top-level result (parent null) and task_notification paths are unchanged, so they still settle the fork. The PR adds the test keeps a fork subagent running through its sidechain launch echo in ClaudeAdapterV2.test.ts. It asserts status 'running' after the echo and 'completed' with the real report after the top-level result. The PR reports that this test fails on main and passes with the fix. I did not run the test. Nested Agent results have a different id than their frame's parent, so they still settle their own subagents. This is consistent with the PR's stated intent and does not conflict with the issue's fork-level requirement. The helper parentToolUseIdFromSdkMessage is not added by this PR. I did not read its body, but the test result reported in the PR supports the comparison's behavior.

Explanation
The whole-PR diff against the merge base (5732bbe) contains a 2-line source change in ClaudeAdapterV2.ts, which is the fork-echo guard described above, and the new test in ClaudeAdapterV2.test.ts. The PR description and raw summary list no other changed files. The guard applies only to the tool_result whose id matches its own frame's parent, so it is limited to the reported defect. The test directly exercises that path, so it supports the change and is not unrelated. No unrelated change was found.

The test checks only the subagent status. A change that prevents only subagent settlement could still fall through to ensureToolCallStarted and settleToolCall, creating or completing a turn_item.updated for TOOL_USE_ID. The current status assertions would still pass.
Add this assertion before the final-result frames. The Agent launch does not create a tool item, and the final result has not yet been processed.
