fix(server): keep failed threads out of automatic settlement - #27058
A failed latest run was still eligible for automatic settlement. If its linked PR had merged while the agent continued working, a provider failure could immediately hide the unfinished thread. A newer held queued run could also mask that failure in the settlement projection.
Exclude failed runs in the shared settlement eligibility rule, and select the latest unheld run using the same rule as the sidebar. This protects merge, PR-closure, and inactivity settlement across providers and clients. A successful follow-up restores eligibility; manual settlement remains available. Update the sidebar guide to describe the rule.
Observed five expected failures before the eligibility fix: the worker settled a failed thread and the SQLite-backed decision returned its completion timestamp instead of null.
Extended the existing SQLite regression with a failed run followed by a held queued run. Before the query fix it selected queued run 2 instead of failed run 1. Afterward it retains the failed status, blocks settlement, and permits settlement after a successful follow-up.
vp run check:changed passes format, lint, both knip checks, GraphQL validation, all three affected package typechecks, and 82 tests across four files.
The reproduction uses the real settlement worker with controlled dependencies and a real in-memory SQLite projection. No live provider capacity failure was induced and no browser/native clients were launched. CI owns the full suites and build.
Implemented with GPT-6.1 Sol through the Codex harness in T3 Code.
Macroscope's review found this PR requires human approval — Failed threads will no longer automatically settle after inactivity or PR completion, while manual settlement remains available. This changes the default thread lifecycle and requires human confirmation of the policy change.
You can add or adjust custom eligibility rules. Learn more.

Walkthrough
Automatic settlement now excludes threads with failed or queued runs. Projection selection also skips held queued runs. Tests cover merge, closure, and inactivity settlement, plus completed follow-up runs.

Summary
Exclude failed and queued threads from settlement candidates apps/server/src/orchestration-v2/ThreadSettlementService.ts, apps/server/src/orchestration-v2/ThreadSettlementService.test.ts, docs/user/thread-sidebar.md
The candidate check rejects failed and queued threads. Tests cover snoozed threads, merged, closed, and inactivity settlement, and worker settlement. The sidebar documentation lists failed or queued runs as exceptions to automatic settlement.
Projection selection skips held queued runs when selecting the latest run. Tests verify that a failed latest run blocks merge-based settlement until a completed follow-up run occurs.
Failed threads now stay visible after a linked PR merges, and a successful follow-up run restores automatic settlement. No actionable merge-blocking risk remains in the reviewed changes.

Full details: Linked Issues check
Explanation
Issue #27052 requires a failed latest run to remain visible after a linked pull request merges. isAutoSettlementCandidate now rejects failed and queued runs. getSettlementCandidates excludes held queued runs when it selects the latest run. Tests cover merge, closure, inactivity, worker, projection, and successful follow-up behavior. The sidebar guide documents the same exceptions.
Full details: Out of Scope Changes check
Explanation
The changes stay within issue #27052. Production changes update shared automatic-settlement eligibility and latest-run selection. Tests verify the required failure, held-queue, trigger, projection, worker, and follow-up cases. The documentation change describes the same settlement rule. No unrelated production behavior is shown.

verified unchanged head 73e5075a17b5d535f3f89c42f8d906fae186bb37 against github/main at 5fe18d79ac. used separate installs, isolated empty state, one background dev server at a time, repository playwright, /usr/bin/google-chrome, and real codex gpt-6.1-sol turns through the local proxy. created the project, messages, queue, and settings through the web app. no mocks, seeded data, or database writes.
main reproduced the bug: a failed provider run automatically settled after inactivity.
this head kept the failed thread active after the same threshold, including a newer queued message that became held on failure. the successful control settled normally.
the existing queue guard blocked manual settlement until the held message was removed. removing it, manually settling, and un-settling all worked.
restored provider settings and sent a successful follow-up in the same thread. it returned SETTLEMENT_RECOVERY_OK; after inactivity the thread automatically settled again.
failure was induced through provider launch settings pointing at a nonexistent path on the real local proxy, producing an actual terminal http 404. inactivity was exercised with the supported one-day threshold by advancing this disposable vm clock two days and restarting the server. restored the clock, ntp, and provider settings afterward. both dev servers are stopped.

main, failed work incorrectly settled:
pr, failed work and held queue remain visible after inactivity:
queue removal, manual settlement, and un-settlement:
native ios/android, electron, macos/windows, remote/relay connections, and other providers were not exercised on this linux vm. live pr merge/closure transitions were not triggered because this task forbids merging, closing, or touching another pr; those conditions passed the focused existing tests. an actual capacity or usage-limit outage and a stranded unheld queued run were not induced. this verifies the shared failed-run settlement rule through real inactivity, not every possible provider failure.
