fix(relay): concurrent link requests can no longer exceed the managed tunnel limit - #26186
The relay's per-user managed tunnel limit counted only allocations that had an environment link. Links are written only after provisioning, and the check ran as a separate step from reserving the allocation. So N concurrent link requests for N freshly generated environments all passed the check, and each one created a Cloudflare tunnel and DNS record on the shared account. Allocations left pending or abandoned (the client disconnects between provision and link) were never counted either. (Security scan findings https://git.cafe/pingdotgg/t3code/pulls/46/#75.)
Fix
ManagedTunnelLimits.ensureCapacity is now withCapacity(input, reserve). It runs the limit lookup, the count and the caller's reservation in one transaction. A transaction-scoped advisory lock (pg_advisory_xact_lock(hashtextextended('managed-tunnel-limit:<userId>', 0))) makes concurrent reservations for the same user wait for each other. The lock is released on commit, so it is safe behind Hyperdrive's transaction pooling. No schema change.
The count no longer depends on links. Apart from the current environment (left out so relinking at the limit keeps working), an allocation counts when either:
it was touched within MANAGED_TUNNEL_PENDING_WINDOW_MINUTES (15). Its provisioning may be in flight, whatever its tunnel or link state. Between reserving and recording the tunnel there are only two Cloudflare calls, each capped at 8 seconds by the API gate, so 15 minutes is far beyond the longest legitimate flow; or
it holds a live tunnel (tunnel_id set, not released) for a link that is not revoked or publish-only.
So an abandoned reservation that never got a tunnel stops counting after 15 minutes, and a few mid-provision disconnects no longer lock a user out. These rows hold no Cloudflare resources. An allocation left behind by a failed deprovision after a revoke or a publish-only relink stops counting once the pending window has passed since its last touch. Revoking again retries that cleanup, and each retry restarts the window; #26320 removes that delay.
ManagedEndpointAllocations.reserve refreshes updated_at when the row already exists (ON CONFLICT (user_id, environment_id) DO UPDATE). It runs under the per-user lock, so a stale, released or revoked row being provisioned again counts as soon as its reservation commits. Several such environments provisioned at once cannot skip each other in the count. Macroscope flagged the released-row case.
ManagedEndpointProvider.provision reserves through withCapacity. The Cloudflare calls still run after the transaction commits.
Not changed: the reaper still only scans Cloudflare tunnels, so it does not delete tunnel-less rows. The count ignores them once they are stale, which is enough to keep quota correct. A row abandoned after its tunnel was created counts until legacy cleanup releases it, because its tunnel still exists on our account. The exception is reprovisioning a revoked or publish-only environment that is abandoned before its link is rewritten: that row stops counting after the pending window. The gap predates this PR, and #26320 closes it.
Verification
ManagedTunnelLimits.test.ts, concurrent case: 5 concurrent reservations against the default limit of 3 lead to 3 reservations and 2 ManagedTunnelLimitExceeded. With the lock removed it fails ("expected length 3 but got 5").
ManagedTunnelLimits.test.ts, window case: with the clock at 12:00Z, the count includes rows updated after 11:45Z, plus rows that hold a live tunnel for a link that is not revoked.
Probe against real Postgres 17 (Docker, every relay migration applied, a real PgClient and drizzle effect-postgres, a 50 ms delay inside each reservation):
Run 1: 6 concurrent new environments gave 3 reserved and 3 ManagedTunnelLimitExceeded. Relinking an existing environment at the limit returned reserved.
Run 2, seeded with two stale tunnel-less rows, one revoked-link row with a tunnel, and a rejected fourth reservation:
a new environment was reserved, because none of those rows count;
re-provisioning both stale environments concurrently with two new ones reserved exactly 2 (one stale, one new) and rejected 2, ending at 3 live allocations.
Run 3, seeded with three released rows and two revoked-link rows that still have tunnels: re-provisioning all five concurrently plus one new environment reserved exactly 3 and rejected 3.
vp run check:changed passes.
Model: Claude Opus 5.5 via Claude Code in T3 Code.
Taken over from @juliusmarminge. The design is unchanged: one transaction holding a per-user pg_advisory_xact_lock, the count, and the reservation. What changed:
ManagedTunnelLimits builds each ManagedTunnelLimitPersistenceError at its failure point instead of through a constructor-only helper (Macroscope Effect conventions). A BEGIN/COMMIT failure now reports operation: "transaction" instead of "lock-user". Comments trimmed. Merged main twice.
Tests went from 2 to 1. The "window case" test listed under Verification above asserted on generated SQL text and was deleted. The counting rules are proven by the Postgres 17 transcripts below. The concurrency test now fails for all three ways of undoing the fix: no lock, the check outside the transaction, or reserving after the check.
One correction to the description above: an allocation can hold a Cloudflare tunnel that was never recorded, when create succeeds and recordTunnel fails. That failure path predates this PR.
After review
593fc2ad1c (Macroscope High): the pending window now skips released rows unless the row was reserved again after the release, and skips revoked or publish-only links written at or after the allocation. Before this, deleting an environment at the limit and creating a new one could be refused for up to 15 minutes. Its test asserts on the rendered SQL; #26320 replaces that with a PGlite test that evaluates the count per allocation state.
Follow-up #26320: an abandoned reprovision of a withdrawn environment counts again, and a failed deprovision no longer restarts the window.
Evidence
Server-only change, so the captures are runtime transcripts. They run the real relay layers on a throwaway Postgres 17 with every relay migration applied, using synthetic data and the same driver and clock on both sides. Provider rows run the real EnvironmentLinker, the recovery HTTP handler and ManagedEndpointProvider, with only the Cloudflare boundary stubbed. The relay tree at head 487841a is identical to 2102cdb. E01 (hostname collision) was dropped: hostname and tunnel name derive from SHA-256(namespace, userId, environmentId), so two rows cannot collide.
What else changed, beyond the title: allocations count instead of links. A tunnel-less reservation stops counting 15 minutes after its last touch. A live tunnel without a link now counts. As of these captures, a row touched within 15 minutes counted even if released, revoked or publish-only. 593fc2ad1c later excluded released rows and withdrawn links from the pending window as well (see After review). Re-reserving refreshes only updated_at. A failure or interruption after reserving rolls the reservation back.

Review round 1 on 2102cdb. Reviewer A (GPT-6.1 Sol, Codex): MERGE. Reviewer B (Opus 5.5, Claude Code): FIX_FIRST on two points:
Whether pg_advisory_xact_lock works behind Hyperdrive, since Cloudflare lists "Advisory locks" as unsupported without separating session-level from transaction-level. The lock always runs inside an explicit transaction and the relay's Hyperdrive has caching disabled. Earlier Hyperdrive docs recommended pg_advisory_xact_lock as the transaction-scoped alternative, and community tests in transaction pooling mode show it blocks and releases at COMMIT (cloudflare-docs#34640, cloudflare-docs#34666). No code change.
The stale "window case" bullet, corrected under Takeover.
Both reviewers ran their own Postgres 17 probes, and both agree with the transcripts.
Not added: a real-Postgres predicate test, because the relay has no Postgres in CI and the transcripts cover the rules.
Proof audit: pass 1 derived the 33-row matrix (matrix.md). Pass 2 accepted every S, R, C and E row and independently reproduced C01 (5/5: 3 reserved, 3 denied, 3 rows). Its rejected provider rows were recaptured with the real-provider harness above, and a re-check found them SUFFICIENT (23/23 variants replayed independently on each side).
Tests: ManagedTunnelLimits.test.ts 1, ManagedEndpointProvider.test.ts 31, ManagedEndpointAllocations.test.ts 1, all pass. vp run check:changed passes on 487841a.
Unverified: the lock through a deployed Hyperdrive and live Cloudflare provisioning (no Cloudflare account on the verification VM); Clerk authentication in front of the link handler (synthetic principal); the optional P06 digest-failure control.

… tunnel limit
The per-user check counted only allocations joined to a link, and links
are written after provisioning, so concurrent link requests for new
environments all passed and each created a Cloudflare tunnel and DNS record.
Count every unreleased allocation, and run the check and the reservation in
one transaction serialized per user by a transaction-scoped advisory lock.

Macroscope's review found this PR approvable — The change makes tunnel-capacity checks and reservations atomic per user while preserving the three-tunnel default. The supplied quota-enforcement findings remain subject to the separate correctness blocking gate.

…uota
A reservation abandoned before its tunnel was created was never released,
so a few mid-provision disconnects locked a user out of managed tunnels.
Tunnel-less rows now stop counting 15 minutes after their last reserve,
and re-reserving refreshes that timestamp under the per-user lock.
Allocations whose link was revoked or switched to publish-only (left
behind by a failed deprovision) no longer count either.

…t the limit
Released rows and rows with a revoked link stayed excluded from the count
until provisioning finished outside the lock, so several of them
re-provisioned at once could all pass. A row now counts while it was
touched within the pending window (reserve bumps it under the lock) or
while it holds a live tunnel for an active link.

Managed tunnel capacity checks and reservations now run in one per-user transaction. Allocation upserts refresh reservation timestamps. Endpoint provisioning uses the guarded operation and maps tunnel-limit persistence errors to a provisioning failure.
withCapacity acquires a per-user advisory transaction lock, counts qualifying allocations, and runs the reservation effect when capacity remains. The tests cover concurrent reservations and allocation-count conditions.
Endpoint provisioning passes its reservation to withCapacity. Tunnel-limit persistence errors map to a provisioning failure at check-tunnel-limit. The test double runs the supplied effect when no error is configured.

The takeover's tests for #26186. Keep one regression: concurrent
reservations for one user against the default limit yield exactly that
many reservations. The fake reserve now yields like a real insert, so
the test also fails when the reservation runs after the locked check.
Drop the test that asserted on generated count SQL; it mirrored the
query, and which rows count is covered against real Postgres instead.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
The fake database always returns reserved.length, so this test checks lock serialization only. It does not model updatedAt or tunnel-less allocation rows. A predicate that counts stale rows or omits recent rows could pass.
Add a focused test with recent and stale tunnel-less allocations. Assert that only the recent allocation consumes capacity.

Test asserts SQL text, not counting behavior.
The test name says pending reservations stop counting once released or withdrawn. The test never evaluates that rule. makeFakeDb returns reserved.length for any where condition. The test only checks that two SQL fragments exist in the rendered condition.
These checks still pass if the boolean structure is wrong. For example, a misplaced or or and, or a dropped not(...), would not fail the test. The test also breaks on a harmless change, such as a column alias or a reordered predicate.
The rule in docs/internals/testing.md says tests cover behavior at the lowest layer that fails on the original code. The path instructions in effect-services.md say tests exercise behavior through the service and do not mock the logic under test. The count query is the logic under test here, and the fake DB replaces it.
Cover the release and withdrawal rules with rows evaluated by a real Postgres. A PGlite-backed RelayDb is one option. Otherwise, rename the test to say it only checks the query shape. Document that the counting rules are verified elsewhere.
Based on path instructions: "Hold changed tests to the rules in docs/internals/testing.md" and "Tests should exercise behavior through the service rather than mock the logic under test."

Fix
ManagedTunnelLimits.ensureCapacityis nowwithCapacity(input, reserve).
MANAGED_TUNNEL_PENDING_WINDOW_MINUTES(15). Its provisioning may be in flight, whatever its tunnel or link state. Between reserving and recording the tunnel there are only two Cloudflare calls, each capped at 8 seconds by the API gate, so 15 minutes is far beyond the longest legitimate flow; ortunnel_idset, not released) for a link that is not revoked or publish-only.So an abandoned reservation that never got a tunnel stops counting after 15 minutes, and a few mid-provision disconnects no longer lock a user out. These rows hold no Cloudflare resources. An allocation left behind by a failed deprovision after a revoke or a publish-only relink stops counting once the pending window has passed since its last touch. Revoking again retries that cleanup, and each retry restarts the window; #26320 removes that delay.
ManagedEndpointAllocations.reserverefreshesupdated_atwhen the row already exists (ON CONFLICT (user_id, environment_id) DO UPDATE). It runs under the per-user lock, so a stale, released or revoked row being provisioned again counts as soon as its reservation commits. Several such environments provisioned at once cannot skip each other in the count. Macroscope flagged the released-row case.ManagedEndpointProvider.provisionreserves throughwithCapacity.

Re-reserving refreshes only

pg_advisory_xact_lockworks behind Hyperdrive, since Cloudflare lists "Advisory locks" as unsupported without separating session-level from transaction-level. The lock always runs inside an explicit transaction and the relay's Hyperdrive has caching disabled. Earlier Hyperdrive docs recommendedpg_advisory_xact_lockas the transaction-scoped alternative, and community tests in transaction pooling mode show it blocks and releases at COMMIT (cloudflare-docs#34640, cloudflare-docs#34666).
