The active-Claude finding on #16863 exposed a missing boundary: a host operation cannot hold provider admission while it restores files and commits rollback history. Rejecting an active turn only inside native rollback is too late when file restoration runs first.
Change
Add an optional adapter-owned rollback admission operation. Claude and Codex exclude new starts and reject active native work before entering the supplied host operation. ACP uses its runtime transition permit and checks teardown before admission. The callback retains its own error and context types, and receives a rollback function that does not reacquire the same permit.
This is the provider boundary extracted from #16863. Host-service integration is prepared separately; this PR alone does not change checkpoint restoration order or claim to fix the complete host workflow. Existing provider file-restore support is retained.
Scope and approval
The maintainer takeover of #16863 preserved the restoration-order fix and reduced redundant coverage. The contributor requested that the later safety follow-up be split for review. This PR contains only the provider boundary and focused adapter tests; it does not claim separate maintainer approval for that extraction.
Broader provider-initiated wake admission and durable crash/interruption recovery remain outside this change. There are no client, wire-contract, or UI changes.

Current-main focused adapter cases: Claude 5 passed, Codex 1 passed, ACP 3 passed. Coverage includes active-turn rejection, the Claude terminal-before-native-cleanup window, blocked starts, interruption release, callback error preservation, ACP replacement/quarantine, and poisoned teardown rejection.
Commands from apps/server: vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts src/orchestration-v2/Adapters/CodexAdapterV2.test.ts src/orchestration-v2/Adapters/AcpAdapterV2.test.ts with focused name filters. Claude admission cases passed; separate exact-name runs verified the Codex legacy rollback/active-goal case and the three ACP replacement/quarantine/teardown cases.
Typechecks passed in apps/server, packages/provider-core, and packages/provider-acp.
Root changed-file lint, formatting, and diff checks passed on all seven files.
No live native-provider or browser/client run was performed. These tests validate the admission operation; the separately prepared host integration validates its use during checkpoint restoration.

The late-child replay failed before the fix and now checks rejection before entering rollback admission, plus an unrelated-thread control. All 104 Codex adapter tests passed, along with server typecheck, scoped lint, formatting, and diff checks.
Review follow-up at ec38f2c3445d796efd3f1bca89fa6d54e038eaaa: capacity retries hold admission through pending-start registration and release it before the RPC response. Explicitly cleared native goals override stale projected active goals. Both regressions failed with the previous behavior. All 106 Codex adapter tests passed, including early-completion and Stop cases; server typecheck, changed-file lint, formatting, and diff checks passed. Reverse race ordering was not separately added.
Implementation and validation: GPT-6.1 Terra high; integration: GPT-6.1 Sol, using the Codex harness in T3 Code.

Walkthrough
The provider runtime contract adds optional rollback admission. ACP, Claude, and Codex adapters coordinate admission with active work and turn startup. Tests cover rejection conditions, admission timing, and rollback behavior.

Summary
Admission contract and ACP rollback packages/provider-core/src/server/ProviderAdapter.ts, packages/provider-acp/src/server/adapter.ts, apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts
The runtime contract adds optional withRollbackAdmission. ACP waits for teardown, checks for active turns or background work, and runs rollback through the admission hook. Tests cover active-turn rejection, rollback, and poisoned-session rejection.
Claude admission and turn coordination apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
Claude serializes turn starts and compaction with rollback admission. Admission checks active turns, active goals, and pending background work. Tests cover terminal-event timing, turn startup coordination, and error propagation.
Codex admission and operation coordination apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
Codex coordinates turn starts, compaction, capacity retries, and rollback admission. Admission checks active turns, pending startup, background work, and active goals. Background-work lookup follows context ancestry.

This change adds an optional rollback-admission hook to the Claude, Codex, and ACP provider adapters, with adapter tests covering rejection and timing. Nothing in the supplied evidence indicates an outstanding defect. The hook has no production callers yet, so host integration is unverified and will need checking when it is wired in.

Merged current main with a merge commit and preserved both the reused-turn guard and rollback retry coordination. Fixed Claude admission treating an explicitly cleared native goal as a missing entry, matching Codex's existing behavior. Extended the existing goal-clear case: it failed before the fix and passed after.

withRollbackAdmission has no production callers at this head. non-test matches are its interface and the three adapter definitions. CheckpointRollbackService still calls session.rollbackThread directly. therefore no current ui, websocket, or cli flow can enter the new callback to prove rejection of active work or exclusion of new starts during host rollback. main lacks the callback, so the requested before/after admission reproduction cannot be performed through the client. successful rewind is regression evidence, not proof of the new admission guarantee. the pr description explicitly defers host integration; i left that scope unchanged.
real claude/acp admission, provider capacity-retry races, failed-root/running-descendant races, native mobile/desktop, and relay/tunnel behavior were not exercised. the adapter tests passed, but this linux codex run does not establish those live paths. no code changes or pushes were made.
