🎯 Functional Correctness | 🟠 Major | 🏗️ Heavy lift

Preserve queued dispatch when a merge-back transfer is pending.

If a thread has a pending merge-back transfer and its latest run failed with a usage limit, this new queue target makes shouldQueue true. The check at Line 5291 then rejects the message because queued merge-back consumption is unsupported. No queued message is committed, and the command ID receives a rejected receipt. Handle the transfer when the queued run starts, or provide another path that retains the message.

Review comment at @apps/server/src/orchestration-v2/Orchestrator.ts at line
5246:
Update the queueBehindRun dispatch flow so a pending merge-back transfer does
not cause shouldQueue to reject the message; handle the transfer when the queued
run starts or use another path that commits and retains the message.
