…ore its prompt A steer offered with priority "now" before the opening prompt reaches the model can make Claude Code run the steer first and the prompt afterwards as a separate turn. The Claude adapter now holds such steers until the prompt's first model output, then offers them as before. Fixes pingdotgg#22531
ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — This is a focused Claude adapter bug fix that queues startup steers until the opening prompt begins producing model output, with FIFO and edge-case regression tests. Existing normal and continuation paths remain unchanged, and no schema, deployment, security, billing, or configuration surfaces are modified. You can add or adjust custom eligibility rules. If a turn ends without model output, a steer sent during startup can be dropped. This known limitation warrants owner awareness but does not establish a broader merge blocker.
Security architecture risk:
Low · up to 4945a

The change affects message ordering within an existing run without adding access or privileges. The main tradeoff is that messages still waiting when the run ends are discarded rather than retried; termination and interruption behavior have less direct verification than normal delivery.
Retained concerns
No architecture-level concerns identified.Security review details
-
Security Blast Radius
- inferred — The demonstrated exposure remains steering of an already active turn through its existing session. The production delta changes delivery timing, not the available callers, credential scope, runtime permissions, or tool authority.
Trust Boundaries and Controls
- observed — Release requires the current query, a root stream or non-synthetic assistant frame, and compatible prompt identity. Explicitly different prompt identifiers do not release held steers. Untagged output remains acceptable when the session is not known to echo prompt identifiers early, so this is compatibility-based correlation rather than universal cryptographic provenance.
Resilience and Maintainability Implications
- observed — The production offer implementation places messages into a local queue, and close shuts that queue down before closing the runtime. Stream processing captures failures and finalizes the owning active turn as interrupted or failed, rather than transferring ownership to a replacement query.

Issue
#22531coding requirements are met.ClaudeAdapterV2.tsholds startup steers inheldSteersand returns before offering them. Lifecycle frames, init, synthetic assistant frames, child output, and output correlated with another prompt do not release the held steers. The adapter offers held steers in FIFO order after eligible root prompt output, withpriority: "now", and keeps the run active while the prompt is pending. The parameterized adapter test covers startup with and without aninitecho and verifies no premature offer. Tests also cover FIFO ordering and held attachment reservations across compaction.
Full details: Out of Scope Changes check

Explanation
The changes stay within issue
#22531. Production changes implement startup-steer holding, FIFO release, turn cleanup, and related PDF reservation handling. Test harness and attachment-test changes support deterministic coverage of these behaviors. No unrelated product behavior or unrelated files appear in the reviewed changes.

Do not acknowledge a held steer as delivered before release.
ProviderTurnControlService.steer treats a successful session.steerTurn call as success. ClaudeAdapterV2.steerTurn returns successfully after it adds the message to heldSteers. If the turn ends before the first qualifying root model frame, finalization only logs the drop. It does not emit a user-visible notice or retry the steer.
The deliberate-drop intent does not satisfy the existing success path. Either keep the submission pending until release, or propagate the drop so the orchestrator starts the follow-up turn.

Note
Claude writing on behalf of @r3xsean.
Agreed: if the turn ends before the opening prompt produces model output, a held steer is dropped without reaching Claude. Keeping the steer effect pending until release would block Stop and dialog responses behind it in the thread's provider-effect lane. Recovering a drop after the steer has returned needs asynchronous orchestration handling, related to the delivery-recovery work in #22423. I'd suggest leaving that for a follow-up; the limitation is disclosed in the PR and logged as orchestration-v2.claude-held-steers-dropped.

Fixes #22531.
A steer sent while Claude was still starting up on the opening prompt was offered with priority
now, and Claude Code could run it first: the model answered the steer without the prompt's context, and the prompt ran afterwards as a separate turn. The Claude adapter now holds a steer until the opening prompt's first model output, then offers it with
nowas before; held steers go out in the order they were sent. Lifecycle events, init, synthetic notices, and output tagged for another prompt don't release them. Holding returns immediately, so it doesn't block the thread's effect queue. A held steer's PDF budget reservation stays pending until the steer is offered, so a compaction while it waits doesn't drop it from the count. One behaviour change: a turn that ends without any model output (for example a bare
/goalstatus check) now drops a held steer with a warning log, where previously the steer could run as a separate turn.Verification
